Brake Vacuum pump that was removed from vehicle to inspect and diagnose is still available for inspection. Recent findings show that Chevrolet/GM is aware of the issue in their vehicles but refuses to address or replace faulty brake vacuum pumps. It is a common problem amongst 2013-2022 Chevrolet Malibus. Vehicle that is in question was inspected by a certified mechanic shop with failed emissions test. Solid check engine light appeared once the brake had become harder to push and stop the car. OBD scans show DTC codes that were pertaining to MAF and MAP malfunctions in readouts and communications to the ECM. After further inspection, the ABS module had been acting as "backup" for hydrolic brakes system that had zero vacuum to power the brake booster, this could have caused the engine to stall, longer than normal stopping distances (stronger risk of rear ending other drivers), and multiple components and sensors to fail, requiring them to also be replaced.Read more

I am submitting a formal complaint regarding ongoing and severe engine issues with my 2018 Hyundai Sonata SE. I have detailed documentation, receipts, mileage logs, and maintenance history supporting this. In addition to oil consumption, the vehicle has repeatedly misfired, produced diagnostic trouble codes, and fouled spark plugs with oil. I have been forced to replace spark plugs approximately every two weeks, which is clearly abnormal for any modern vehicle. On multiple occasions, the vehicle has experienced violent knocking noises, severe loss of power, hesitation during acceleration, and near-stalling. One incident was so loud and aggressive that it felt like the engine was going to come out of the hood. These symptoms pose a serious safety risk while driving. I brought the car to a Hyundai dealership at approximately 118,000 miles for an oil-consumption test. The vehicle failed that test, clearly confirming excessive oil consumption. Despite this, Hyundai refused to provide repair, internal engine cleaning, or engine replacement, claiming the vehicle was already over 100,000 miles I have maintained this vehicle responsibly and have documentation of regular oil changes, spark-plug replacements, inspections, and diagnostic reports. This is my first Nissan and wasn't aware of issues like this as I typically buy Buick's, so I researched Nissan issues and recalls and recently saw a recall from October 2025 related to the fuel pump injector system reset that describes what ive experienced exactly, but when I took it to the Nissan dealership in Saratoga NY, they said its not a recall, just a warning for my model and year, and they would need to charge me to inspect it, and didnt advise if it was repairable. Anyone else who has this model Nissan I would want warned immediately, and I mean immediately. This failure created an immediate risk of a loss-of-control crash on the highway. Being instructed not to charge the vehicle for months due to safety recalls also indicates a high-voltage battery safety risk that could endanger me, my passengers, and others. Now Dealership did full diagnostics and tells me the car needs PDM (Power Distribution Module) replaced. Nissan previously issued Service Campaign P9351 for this exact issue, but my VIN is not included. I believe my vehicle has the same defective PDM and poses a safety risk due to sudden loss of drive power. Nissan has declined coverage. I am requesting NHTSA investigate whether the recall should be expanded to include additional affected vehicles. Also there is open Recall R24B2 regarding Lithium Battery getting too hot durinig Fast DC charging for which they don't have the Remedy yet. I recall that only 3-4 weeks before above incident, I had my previous Fast DC charging session, after which I got a message on dashboard saying something like "Battery is very hot". I waited for 10 minutes before driving. Each time I would drive from standstill position, it will accelerate very slowly, but eventually drive OK after speeding up. I have now been without transportation for over 48 hours, and the manufacturer is not providing a reasonable remedy or safe transportation while the recall repair remains unavailable. I am filing this complaint so NHTSA can document the delay, the lack of an accessible remedy, and the manufacturer’s failure to provide a safe, timely solution for an undrivable recall-affected vehicle. Internal impeller swelling, seal leakage, and bearing seizure can cause coolant pressure spikes and ultimately head-gasket failure with coolant entering the cylinders. This defect has been reported by thousands of RAV4 Hybrid owners (2019–2021). Toyota has addressed it quietly through internal TSBs such as XXX and XXX but no public recall has been issued. The safety risk is severe: the engine began emitting thick white smoke and ran roughly, risking sudden power loss, overheating, or engine seizure while driving. A stall at highway speed could easily cause a collision. Toyota USA opened Case #XXX but declined assistance despite the defect being well documented. This failure was sudden, with no warning lights prior to the incident. The rear fuel tank mounting strap rusted in half. After filling vehicle with fuel and driving 10 miles home, I noticed the skid plate hanging down that covers the fuel tank. The plastic skid plate started to crack from the weight of the tank. The design of the skid plate to the tank allows moisture and contaminants to reside between the two, rotting the straps. I ordered new straps and hardware, but during the parts search I found many owners of the Toyota FJ, all years, are having the same problem. Apparently the straps are just mild steel and were not treated with any corrosion preventative from the factory. I feel they should have been at least stainless steel. Part numbers for the straps are 77602-35050 and 77601-35110. I'll have photos once the parts come in and I take the vehicle apart. Currently I have a floor jack holding up the tank. Also to clarify, the skid plate is mounted to the fuel tank straps. Once both straps let go, the fuel tank is coming out with the skid plate. The fuel tank is located in front of the rear axle, the vehicle would run over it's own fuel tank, at least until the engine shuts off.Read more

The rear seat belt recall involves improperly secured retractors, and the transmission recall is due to a start-stop accumulator endcap with missing bolts, which could cause a leak and a loss of propulsion or a fire risk. The car will only turn back on after putting some gas into it, so I make sure to never let the gas needle go below the middle line or have the gas range reading go below anything around 160-200 miles since the vehicle has shut down at a different amount each time, and there's been multiple times. Also, the fuel level needle never matches with how many miles is shown on the range, displaying inaccurate readings. It has poor acceleration, poor mileage, and now making a loud whining noise that is heard from outside the vehicle near gas tank. I have tried looking up the VIN and it does not show that it falls under the recall, despite it having all the symptoms of a faulty fuel pump/sending unit. I love my 4Runner but I am in fear for my life whenever I drive it and not showing accurate fuel levels. I pray that nobody runs into me if the vehicle stalls out in the roadway, while going 30-80 mph (it has done it on regular roads and on the highway). There are no warning lights or check engine light. It's at about 208,000 miles, with the issue slowly getting worse over time. After waiting a significant amount of time I tried to restart the engine. Exactly the same thing occurred. On the final attempt to start the engine, 15-20 minutes later, it started and ran quietly like nothing had ever happened. On returning to the vehicle my wife asked what all the loud grinding noise was about. She had heard it while she was inside the store. I advised her it was our truck! ... to have this issue at a Chevrolet dealership. The car was a CPO through FORD and was covered. I owned the vehicle for 14 days. This unit has the L87 Motor, and had already undergone a motor replacement under the recall warranty. The injectors apparently were not replaced, only the O-Ring. Unlike many manufacturers GM has removed Fuel Injectors from their PowerTrain warranty. A motor with under 5,000 miles was not covered since the body and bolt-ons were original with 45,000 miles. GM has a very recent history of injector recalls and issues. These injectors should undergo the same scrutiny as previous GM injectors.Read more

... this past summer, Coolant seems to be leaking somewhere as just filled with coolant month ago and needed to refill with about quarter to refill to correct amount. Oil also seems to be having slow leakage problem somewhere within the engine as there is excessive oil on drip pan. Had all 4 spark plugs changed in March of 2025. Car has been going into limp mode and looses power for few seconds to minutes sometimes, almost causing multiple accidents while driving on highways. Pull over to shut off and give few minutes to restart and able to resume driving No warnings before this occurs. Randomly all dashboard lights light up 2 weeks ago and put car into limp mode, was driving on highway at high rate of speed (65mph) safety was compromised several times while this occurs. Engine seems create knocking sounds to known due to oil dilution problem. Cold weather seems to affect issues to onset more often. ... I was transporting livestock in a towed trailer with my three children in the truck. The vehicle experienced an abrupt loss of power in traffic and became unable to move. We had to remain stopped until a tow truck arrived, creating a significant safety hazard for my family and other drivers. The Ford dealer documented the failure as “high pressure pump froze up and cracked high pressure pump,” which caused metal contamination throughout the fuel system and required complete replacement of the pump, injectors, rails, lines, and a fuel tank flush. The total repair cost was $16,574.44, which I paid out of pocket. Ford has stated they cannot process reimbursement until the recall remedy becomes available, despite federal regulations allowing reimbursement for repairs completed after notification of a safety defect. I am requesting NHTSA’s assistance in ensuring Ford complies with the recall reimbursement requirements and continues to monitor this defect due to the sudden and dangerous nature of these failures. When arriving at the high school parking lot to pick our grandson up from band practice, we experienced Sudden Unintended Acceleration of our vehicle as we attempted to pull into a parking spot. This resulted in a collision with another vehicle that had backed into the parking space in front of us. That driver was inside her vehicle and claimed injuries. ... space at a very slow rate of speed when all of a sudden the car lunged forward at a high rate of speed, jumped two parking lot cement barriers and slammed into the rear of her vehicle. The car is equipped with all the latest collision avoidance equipment, but did not stop before hitting the other vehicle ... and no automatic warning sounds went off that I am aware of. A police report was filed and a tow truck had to be called to tow the vehicle. The vehicle had less than 5,ooo miles on it and had only been owned by us for 4 months (it was new when we purchased it). The accident happened on August 27, 2025 and we did not get our vehicle back from the body shop until Monday, November 17. Not only was our safety at risk, but the other driver and several students in the parking lot at the time. This incident could have been so much worse, or could have ended in a fatality. Volvo did an inspection of the vehicle, but the results were inconclusive and our insurance company paid the claim. As soon as we got our vehicle back we took it to our local Volvo service manager to have him check it over. He has indicated the car is safe to drive ... but I am very nervous about driving the vehicle now. The cost to repair the vehicle was $14,145. I was not distracted at the time of the accident and I am certain that I did not hit the accelerator pedal.
