The throttle lag is a major safety concern. When pulling way from a stop there is a huge delay in throttle response. This is consistent and has caused me to have several close incidents and nearly an accident. This is a programming issue that Toyota needs to address.
Incident date: Oct 15, 2025
When accelerating from a stop, on occasion there is a multi second delay before the vehicle accelerates. I have had a few close calls with oncoming traffic when entering a new road from a stop due to these delays. It is quite worrisome. This has happened since new and after a software flash on the transmission.

Incident date: Nov 26, 2025
When pressing the accelerator, there is an extremely loud and abnormal noise coming from the fuel pump. The noise is so severe that it raises safety concerns while driving. The issue appears similar to fuel-pump–related problems reported in other recalled Honda vehicles.
Incident date: Nov 26, 2025
The truck has a hesitation to start from stop/slow state. This is concerning because when I start to accelerate, sometimes it doesn’t want to go for a 1-2 second time period making it unsafe to pull into traffic. It is sporadic and seems to happen more when I start and stop in quick succession.

While driving on the highway, a strong burning smell began inside the vehicle, similar to a brush or forest fire. At first it was unclear whether the smell was coming from outside, but when traffic slowed, smoke was seen coming from the driver-side wheel well and from under the front hood. No warning lights, alerts, temperature warnings, or malfunction messages appeared before or during the smoke.
... The vehicle was moved off the highway to a side road. Upon opening the hood, light but steady smoke continued from the engine area. The coolant reservoir was full and there were no visible leaks. The vehicle continued to smoke consistently for approximately 50 minutes while waiting for a tow truck. After the vehicle was loaded onto the flatbed and transported back toward the highway, an orange glow became visible inside the cabin, indicating an active fire had started inside the interior. The tow truck driver immediately pulled over, used a fire extinguisher, and unloaded the vehicle from the flatbed for safety. Smoke increased significantly from the cabin and wheels. Both white and black smoke were present. Police and fire department personnel responded to the scene and extinguished the fire. The vehicle was a nearly new plug-in hybrid with low mileage, and there were no dashboard warnings or prior symptoms of failure. The cause of the fire is unknown, but it appears related to a malfunction involving the electrical, battery, thermal-management, or propulsion systems. The event presented a significant safety hazard to occupants, the tow operator, and other motorists.Read more

I am reporting a safety issue with my 2019 Subaru Legacy that hesitates and nearly stalls when accelerating from a stop and is now undriveable due to stalling and shaking. I bought the car new from AutoNation Subaru Carlsbad in 2019. It has about 65,000 miles and I have driven it less than 2,000 miles since its last dealer visit in August 2025. Since around August 2025, the car has intermittently
... lost power under light acceleration from a stop or low speed, especially while pulling into or through intersections. Pressing the accelerator sometimes produces very little power, the engine feels like it may stall, and traffic behind me must brake or go around me, creating a risk of a rear‑end collision in intersections. In August 2025 I took the car to AutoNation Subaru Carlsbad and reported that the symptoms seemed like a fuel pump problem (my independent mechanic agreed). The dealer kept the car for about two weeks, experienced the hard‑starting/turn‑over issue, but did not test or replace the fuel pump. They performed other services, charged me $1,956.21, and returned the vehicle without fixing the safety issue, even though they later acknowledged the fuel pump is covered under an extended warranty. One day after picking the car up, the same hesitation and loss of power recurred in an intersection. On November 26, 2025, the car again became disabled: it cranks, briefly starts roughly, shakes, and then stalls, and will not stay running. I believe this is a defective low‑pressure fuel pump or related fuel system component, similar to known Subaru fuel pump defects that can cause stalling and loss of power. No crash or injuries yet, but there have been near‑misses due to unexpected loss of power in traffic.Read more

At around 47,000 miles (April 27, 2024) my car would not move when I put it in reverse to leave my house that day, the engine would just rev and it would not get into the reverse gear. Then warning light came on for the power train so I called the dealership and had it towed to their service department. They said they had to completely rebuild my transmission and it took them almost a month to c
... omplete the repairs. (May 6th, while the car was at the dealership , I received several engine warnings from my Ford app. I got my car back from the dealership about 2 weeks later) But immediately after getting my car back from the dealership there were still issues; it’s slips gears , especially from drive to first gear and first gear to second gear and the engine will rev for a while and then the entire vehicle jerks when it finally catches the next gear, also it doesn’t have enough power for steep inclines , it will just roll backwards while the engine is raving trying to climb the hill. This happened intermittently after the transmission rebuild but has since progressed to be an everyday issue. And I’ve sent it back to the dealership several times over the last year for them to fix it but it never gets fixed. They keep telling me they did a software update and that should fix the issue, but it doesn’t. It’s at the dealership again right now because it has gotten so bad that I can’t even drive the car. You start the car, put it in drive and it just revs and won’t even catch first gear. It now has around 60,000 miles. I’m the only owner of the vehicle and I take excellent care of my vehicles. I just want my vehicle to be drivable and a vehicle that is worth the price that I paid for it. I paid for a brand new vehicle, but what I got is a piece of crap. And not drivable.Read more

I am reporting a series of safety concerns regarding my 2017 Chevy Volt, which have persisted despite multiple attempts to address them with the dealership. These issues pose significant risks to my safety and that of others on the road. On January 18th, 2024, I brought my vehicle to the dealer after experiencing reduced propulsion power while driving on the highway, coupled with the illumination
... of the check engine light. The mechanic performed a quick inspection and used a GM scan tool to diagnose the issue, revealing the code P1E00 for the Hybrid Powertrain Control Module 2, which was cleared without a thorough investigation. I was advised to return if the issue reoccurred. Despite the reset, the problem persisted, and on February 9th, 2024, I scheduled another appointment due to my Chevy Volt's High Voltage battery charging only to 65% capacity despite indicating a full charge. Subsequently, on February 11th, 2024, the check engine light illuminated again, and on February 12th, 2024 and again on November 25th, 2025, while driving to work, both times I experienced a loss of propulsion power on the highway, placing my safety at risk and lost all power in a traveling lane of traffic. These ongoing issues with propulsion power and the high-voltage battery charging system are not only inconvenient but also potentially dangerous. Despite multiple visits to the dealership and attempts to diagnose and resolve the problem, it persists, indicating a systemic issue that requires immediate attention. The reduced propulsion power while driving on the highway, coupled with the illuminationI urge the National Highway Traffic Safety Administration (NHTSA) to investigate this matter thoroughly to ensure the safety of Chevy Volt drivers and passengers. It is imperative that General Motors addresses these issues promptly and implements appropriate measures to prevent further incidents.Read more

My 2020 Jeep will regularly turnover but not start after pushing the start button. As of now, the vehicle will start normal after pushing the start button a second time. Research has shown many people have had and are still having this exact issue. It would seem that most of the ones with this problem have fixed the issue by having both fuel pumps replaced which is very expensive and in my opinion
... highly unreasonable being it is a very common issue for Jeep. Upon further research it would seem Jeep Grand Cherokees and Dodge Durangos which share the mechanical characteristics have been having this exact issue for at least 10 years now. While driving our 2022 Hyundai Tucson Limited with the 2.5L engine and about 39,000 miles, the check engine light suddenly came on. Right away the car started behaving strangely. The throttle response became jerky and uneven, and when idling the engine ran at a higher RPM than normal and sounded off. The vehicle also struggled to accelerate, which made it feel unsafe to drive. Thankfully this happ
... ened while we were not on the highway, but if it had occurred while merging or keeping up with traffic, it could have been dangerous. Even in city driving, the jerky response and slower acceleration created risk. What would normally be a safe turn across or into traffic could easily have turned into a slower, delayed turn and a potential accident. When we got home, I checked the code and it said it was a cylinder misfire on cylinder 3. The car had been regularly serviced at the dealer and had never shown signs of this issue before. Based on the symptoms, I suspect it may be related to the fuel injectors, but we haven’t been able to get the vehicle in for inspection because local dealers are backed up. One dealer even told us it was fine to keep driving as long as the check engine light wasn’t blinking, which feels concerning given the way the car is performing.Read more

My 2019 Chevrolet Malibu has severe safety defects involving the engine, powertrain, and speed control. The vehicle will sometimes accelerate on its own with my foot off the gas, and other times it loses all power while pulling into traffic. This creates an immediate risk of a crash. Multiple diagnostic codes appear repeatedly, including P0299 (turbo underboost), P0101 (mass air flow performance)
... , P1101 (intake airflow performance), and P0506 (idle control RPM too low). The charge-air hose repeatedly blows off the throttle body while driving, causing sudden loss of power. The vehicle was sold to me by a dealer as safe and reliable, but these defects appeared shortly after purchase. The unintended acceleration and sudden loss of power make the car dangerous to drive. This appears to be a serious mechanical and safety-related defect that could cause an accident.Read more

I purchased this 2014 Jeep Cherokee from Taverna Collection in Feb 2025 and received it in March 2025. The selling dealer was required to inspect the vehicle before delivery and did not report any transmission or valve-body issues. On June 25, 2025, the vehicle received a multipoint inspection at Auto Angel in Santa Fe. No transmission failure, shifting issues, or valve-body concerns were found.
... The Jeep was not showing symptoms related to Recall U90 at that time. After this, I attempted to have Recall U90 (PCM/TCM reprogramming) performed. Lithia Chrysler in Santa Fe confirmed the recall by email and scheduled an appointment, but after towing the vehicle to them they refused to perform the recall and repeatedly stated “there is no recall,” forcing me to tow the vehicle home and then again two days later to a dealership in Albuquerque. At the Albuquerque dealership, I gave clear instructions not to perform any paid repairs—including a transmission valve-body replacement—until the U90 reflash was completed and confirmed. I never received confirmation. The dealership later claimed they followed protocol but refused to release the WiTECH session report or timestamps showing when the recall work occurred. They told me I could only see documentation after paying $2,600. I contacted FCA/Stellantis. The case manager said her supervisor stated the dealer “followed protocol,” but FCA admitted this was based solely on the dealership’s word. FCA could not confirm whether they reviewed the WiTECH diagnostic session or any recall repair records. Under 49 U.S.C. §30120(i) and 49 CFR Part 576, manufacturers and dealers must retain detailed recall records, including diagnostic data and timestamps, to verify proper recall completion. FCA’s inability or refusal to verify the recall using these required records raises concerns that Recall U90 may not have been performed correctly or in the proper sequence. I request NHTSA review this matter.Read more

... , a mechanic diagnosed the root cause of these misfires as critically low fuel pressure resulting from a failed low-pressure fuel pump. The mechanic advised that the fuel pump requires immediate replacement. These symptoms (low fuel pressure causing engine misfire and potential stall) align exactly with the safety defect described in NHTSA Recall Campaign #23V881 (and the related #23V444). That recall describes a defect where the fuel pump impeller deforms and meets resistance, causing a drop in fuel delivery and potential loss of motive power. I leased a brand-new 2025 Dodge Charger Daytona EV. About 2 weeks after receiving the vehicle, a severe and dangerous incident occurred. I had driven approximately 2 miles at normal residential speeds with my baby in the back seat. After parking and walking toward the entrance of the home, I turned around and saw both rear wheels glowing bright red, radiating extreme heat, as if they were about to
... ignite. The temperature was so high that when I poured water onto the wheels, it instantly evaporated into steam. It appeared the back end of the vehicle was on the verge of catching fire. This happened during normal, low-speed driving with an infant in the vehicle, creating a direct and immediate risk to my child’s safety. This was not caused by hard braking, aggressive driving, or any impact. The vehicle had very low mileage and was being operated under normal conditions. In addition to this near-fire thermal event, the vehicle has multiple serious electrical malfunctions: • The key fob frequently fails to unlock the doors or open the trunk, leaving us stuck outside the vehicle. • The vehicle sometimes remains powered “on” even after pressing the button to shut it off. • These issues are unpredictable and raise further safety concerns. I reported the incident to Dodge/Stellantis immediately. For over 4 weeks, Dodge has repeatedly delayed sending an inspector, and the vehicle still has not been examined. Because of the overheating, electrical failures, and the risk of fire — especially with a child in the vehicle — I have stopped driving the car entirely. It is unsafe to operate.
