On November 26, 2025, my 2025 BMW XM experienced complete drivetrain failure while my wife and two children were in a Kansas City parking lot. The vehicle displayed "drivetrain malfunction," would not engage any gear, and was completely inoperable. They were stranded in an unsafe area for 40 minutes awaiting a flatbed tow truck—the only option for this 6,100 lb hybrid. At Baron BMW (Invoice #696
... 591), diagnostics found fault codes 032B51, BA092C, BA09B8, BA0A45, BA0023—all related to the Electric Motor Electronics (EME) module. BMW's test plans indicated EME replacement was needed. However, BMW engineering (TSARA Case #4548336) instructed the dealer to simply CLEAR THE CODES and release the vehicle if faults didn't return after two drive cycles. NO PARTS REPLACED. NO SOFTWARE UPDATED. NO ROOT CAUSE IDENTIFIED. BMW erased the diagnostic evidence without fixing anything. Their own system flagged a critical component for replacement, yet the "fix" was deleting the fault codes. SAFETY CONCERNS: 1) Complete propulsion loss strands occupants in dangerous locations 2) 6,100 lb vehicle losing power in traffic creates collision risk 3) Cannot be conventionally towed, limiting recovery options (i.e. parking garages, parallel parked on a hill, etc..) 4) No root cause fix means repeat failures likely This defect poses unreasonable risk. BMW's failure to correct the underlying issue despite flagging a critical component suggests a systemic software problem in the XM hybrid drivetrain. While driving on the freeway I heard a pop and a saw a warning notification on the dashboard to check the vehicles electrical system. This was followed by a warning that power was limited which resulted it the vehicle speed decreasing to 30mph, and finally a notice to stop the vehicle immediately. ... road without an accident. After this the vehicle became inoperable including hazard flashers. I had to wait on the side of the freeway for about an hour to get a tow. The vehicle was towed to a dealer which confirmed that the ICCU and high voltage fuse were faulty. My safety during this process was put at risk because A) during a snow storm I had to pull the vehicle over to the side quickly across 4 lanes of traffic and B) then remain parked on the shoulder with limited ability to notify other drivers that I was on the shoulder. The dealer has confirmed the failure, the vehicle is available for inspection, and the manufacturer has not inspected the component.Read more

The vehicle's battery was replaced twice within one year, which seemed to temporarily alleviate the issue; however, the warning lights recurred afterward. Recently, Honda conducted a thorough diagnosis and identified the root cause, leading to the replacement of the spark plugs and fuel injectors at an approximate cost of $2,000. If unaddressed, this issue could pose safety risks such as rough idling, decreased fuel efficiency, engine misfires, difficulty starting, poor acceleration, unexpected stalling, complete loss of power, and electrical problems. This issue is recognized by Honda and is currently covered under a recall for the 2023-2025 Honda Pilot and the 2023-2025 Honda Accord Hybrid. While the Honda Accord Hybrid and Honda CR-V Hybrid share the same fuel injection system as part of Honda’s two-motor hybrid-electric powertrain, the Honda CR-V Hybrid has not yet been included in this recall. Nonetheless, many Honda CR-V Hybrid owners are experiencing similar issues and are incurring repair costs out of pocket. THE fuel pressure line from the fuel pump to the flex fuel sensor chaffed a pin hole leak near the front top of the fuel tank. This caused a significant fuel smell and trace leak at the front of the fuel tank which could lead to a fire or explosion. The heat from the fire caused the brakes to start to fail and the amber check engine light to come on within about 5 seconds before smoke was observed coming from the engine compartment. Fortunately, the fire occurred as the vehicle was pulling into the owner's driveway and a fire extinguisher was nearby and used to put out the fire. My 2013 Chevrolet Equinox (~132,000 miles) has a serious and unsafe oil consumption problem. I had a full oil change, and only two weeks later the dipstick was completely dry. The vehicle provided no warning lights, no low-oil indication, and the GM Oil Life Monitoring System gave an inaccurate reading, failing to alert me to the critically low oil level. ... my foot firmly on the brake, the vehicle began constantly jumping and jerking forward as if it was trying to move on its own. During driving, I also began hearing a loud crunching, metal-on-metal noise whenever I accelerated or drove under 50 mph. Because these symptoms started suddenly and were severe, I took the vehicle to my mechanic the same day. On November 26, 2025, the mechanic confirmed that the engine had suffered major internal damage due to running with almost no oil. The timing chain and VVT components needed to be replaced. The mechanic confirmed that oil starvation caused this failure despite a recent oil change, and the inaccurate oil life monitoring system contributed to the hazard by failing to alert me to the critically low oil. This defect presents a significant safety hazard, as the vehicle can jerk unexpectedly, stall, lose power, or suffer sudden engine failure while driving, all without warning. I have since learned that excessive oil consumption and inaccurate oil life readings are widespread in GM 2.4L Ecotec engines. All responses from American Honda customer service have been generic email acknowledgments, and no substantive assistance or resolution has been provided despite my repeated attempts to contact the assigned case manager. Because the fuel odor consistently appears after refueling, I am concerned the repair may have resulted in a leak or improper installation involving the fuel pump or related components. The entire fuel delivery system should be thoroughly re-checked to ensure there are no leaks or safety hazards. This presents a clear safety risk, including the potential for fire. The combination of engine malfunction, electrical issues, and fuel leaks posed a risk to my safety and the safety of others while driving. Confirmation by Dealer / Mechanic: Independent mechanics inspected the vehicle and confirmed the engine failure and alternator issues. Multiple mechanics advised not to drive the car due to the fire hazard. The original selling dealer (Larry H. Miller Toyota) inspected the car and acknowledged engine problems in writing on a business card. Inspection by Manufacturer / Others: Hyundai corporate has been notified, but they declined warranty coverage as the vehicle is a second-owner car and beyond the original warranty. The dealer has also been made aware of the engine failure. Warning Lamps / Symptoms: No warning lamps indicated imminent engine failure prior to the engine dying. The first symptoms were mechanical issues, gas smell, and eventual total engine failure. Alternator failure and repeated maintenance issues occurred within 8 months of purchase. ... even after HOF. My rusted-through fuel tank strap and holes in the rear cross member qualify. HOF RO# XXX. Claim #XXX My 2008 Tundra SR5 TRD was treated with HOF in Nov 2017. In November 2025 my mechanic told me the gas tank was falling out as the straps that hold the tank in place were completely rotted out. And rust perforation was on the rear cross member. He instructed me to get a safety inspection at a Toyota dealership to document the rust. Toyota of Waldorf didn’t even schedule a appt, they told me to call the Brand Engagement Center and that there was no possibility of repair because Toyota had no frames. Brand engagement refused to provide any guidance. I scheduled an inspection at Koons Toyota of Annapolis, which it failed inspection with multiple >10mm frame holes and a rusted-through tank strap (held by bungee). They deemed it unsafe to operate and unrepairable. They kept leaving out the information about the rust or the holes until they realized I was not leaving until they documented it properly. I have 3 versions. Despite ZH7 expiration, this is a post-HOF recurrence and safety hazard. I requested goodwill repurchase at 1.5x KBB (~$12k) under the settlement’s intent. The Brand Engagement Center had me call them and [XXX] ” announced she was my case manager, then almost as quickly announced it was going to “Internal Dept” for repurchase review, that she would no longer be my case manager, and that they would get back to me in 60 days. Recall 95B / NHTSA 24V-720 states that the battery pack may contain cells susceptible to separator damage which, combined with internal cell interactions, may lead to a vehicle fire. Owners are advised not to recharge and to park away from structures. Recall 68C / NHTSA 25V-741 states that under normal vehicle operation, the high-voltage battery may become damaged and lead to a vehicle fire, again instructing owners not to recharge and to park away from structures. These are virtually identical safety defects occurring back-to-back in the same high-voltage battery system. The 68C recall states explicitly that no remedy is currently available and there is no ETA, leaving me with a vehicle that cannot safely be parked at my home, charged, or operated as intended. I am now unable to use the hybrid/electric functionality at all, despite paying a premium for a plug-in hybrid vehicle. I am forced to operate the vehicle only in degraded gasoline mode with significantly reduced functionality and safety. The ongoing fire risk, inability to recharge, and requirement to park away from cars and buildings create a daily hardship and significant safety concern for my family. This is a serious safety defect affecting the same critical system twice in less than a year, and with no available remedy the vehicle remains unsafe to charge, unsafe to park near structures, and unable to function as advertised. My high voltage drive inverter experienced a failure resulting in loss of all power as well as regenerative braking. Mechanical braking and steering remained available. ... ety recalls on this vehicle (including electrical system/propulsion defects), but no repairs or remedies are available, and the manufacturer is refusing to provide towing, inspection, or safe transportation. I reported the total power loss to the manufacturer. Despite this being an extremely dangerous event consistent with the recall symptoms, I was repeatedly told the vehicle is “safe to drive in gas mode,” even though they did NOT inspect it, did NOT ask for diagnostic information, and did NOT offer any safety assessment. They refused to authorize towing, refused to authorize inspection, and told me to pay out of pocket for a rental despite the vehicle being unsafe and unusable. The dealership also stated they have no way to repair the recalls and provided no solution or timeline and refused a loaner vehicle. The vehicle cannot be safely driven the 40+ miles to the dealership due to the prior failure. This vehicle is unsafe to operate, has already experienced the failure described in the open recalls, and the manufacturer is not providing any remedy, inspection, transport, or safe alternative transportation. We paid over $100,000 for this vehicle and currently cannot use it at all. I am reporting this because the defect has already occurred and poses a serious safety hazard to us and other drivers. We need NHTSA involvement because the manufacturer is refusing to address or acknowledge the danger despite documentation of total power loss in motion. The dealer refuses speak to us and states that the recall was done and therefore not their issue. The issue was confirmed with the new mechanic. These failures show a repeated pattern of serious defects. Lincoln Customer Relations and my dealership have stated they “cannot do anything” because there is no remedy, and told me I must continue driving the vehicle despite the documented highway shutdown. I am extremely concerned for my safety because the recall states that loss of power increases the risk of a crash, and I have already experienced this. I purchased a used 2024 Jeep Grand Cherokee 4xe (approx. 64,000 miles) from EIDE Chrysler in Waite Park, MN on 9/30/2025. On the initial drive home, a “Service 4WD” warning appeared. Before I could return for the axle repair, a Check Engine Light for the hybrid system also appeared. After purchase, I also began experiencing exhaust fumes entering the cabin, which is a health and fire concern.
... At the time of sale, the dealership provided two contradictory recall printouts for the only recall they disclosed—one stated a remedy was available and one stated no remedy was available. I was verbally told the recall had been repaired. This was inaccurate. After the sale, I discovered that my vehicle now has two open NHTSA safety recalls, both with no available remedies: • 25V741 – High-voltage battery fire-risk recall (federal stop-sale order applies). • 25V766 – Engine contamination recall that may cause engine damage or failure. These recalls did not exist at the time of purchase, but the dealership never notified me when they were issued. Stellantis only notified me of one recall; I had to discover the second by checking my VIN on NHTSA’s site. The drivetrain warning, hybrid-system fault, and exhaust fumes occurred despite very low mileage after purchase. The vehicle currently has multiple unresolved safety defects, including fire risk and potential engine failure, and cannot be repaired due to lack of available remedies. I have opened a case with Jeep/Stellantis (Case #: 94881053). I am reporting these issues because the combination of drivetrain malfunction, hybrid-system warning, cabin exhaust fumes, and two unrepaired safety recalls poses a significant risk to occupants and other drivers.Read more

12-01-2025 driving to work in the morning approx 7:00A.M. and turned on the heater and had a very strong smell like fuel so immediately turned it off. Cracked my windows to air it out and then was super cold again and had turned on the heater to try again. Immediately had to turn it off and rolled windows down to air it out because the smell was intense and had started to feel nauseas. Later
... in the day, about to head to lunch and had started the vehicle and smelt a lot like gas and turned it off and had someone come out to double check the smell. Co worker had strong smell from the hood area and looked underneath the car and can see a little drop puddle. Opened the hood and lifted the cover to engine where you can see the leakage coming from the center hose and spill puddle. Service tech came to take a look and advised its the fuel feed to the high pressure pump. This is a safety concern because fuel is extremely flammable and can put myself in dangerous situation of firing igniting in the car or hurt others around. No check engine light and prior the vehicle incident car has been consuming oil putting in about 1- quart a week or so with commute miles about 270 weekly.Read more
