After a manual reset of the Check Engine Light (CEL), the vehicle stalled again during a turn. Diagnostic codes P228C (Fuel Pressure Regulator 1 Exceeded Control Limits) and P0089 (Fuel Pressure Regulator 1 Performance) were present. First Attempted Repair: The dealership performed a Technical Service Bulletin (TSB)/recall procedure to calibrate the fuel pump and pressure sensors. Recurrence: Immediately upon retrieval, the vehicle stalled again in a "no parking" zone at a major airport after 45 minutes of operation. The vehicle would not restart or maintain idle, necessitating a second tow. Final Repair: The dealership replaced the fuel pump assembly. Additional Safety Concerns: Approximately 4,000 miles prior to the fuel pump failure, the vehicle experienced multiple "Reduced Engine Power" events and "Service ESC" flashes while traveling at highway speeds (approx. 70 mph) in rainy conditions. The dealership replaced all fuel injectors and spark plugs at that time. Manufacturer Response: Despite the "Engine" designation of the diagnostic codes, GM initially claimed the fuel pump was not covered under the powertrain warranty. We have had a number of issues and several recalls associated with our 2017 Honda Pilot which has less than 78,000 miles on it. The issue I am writing about today involves the recent failure of the high-pressure fuel pump which I believe is related to the prior failure of the low-pressure fuel pump. ... our low-pressure fuel pump was delayed for 14 months because Honda did not have enough parts to cover all recalled vehicles. Therefore, we had to wait for over a year to get the low-pressure fuel pump replaced. Last week, our high-pressure fuel pump failed due to "internal failure" per the dealer. It is common knowledge that damage or failure of a low-pressure fuel pump can directly cause damage or failure of a high-pressure fuel pump because the high-pressure fuel pump relies on consistent, pressurized supply of fuel from the low-pressure fuel pump for lubrication and cooling. I believe the failure of the low-pressure fuel pumped coupled with the delay replacement of the low-pressure fuel caused the failure of the high-pressure fuel pump. Based on shared experiences online from other Honda Pilot owners, it seems like a lot of people are suffering similar issues. The high-pressure fuel pump is not currently under recall and, therefore, vehicle owners have to pay for the replacement of this part. When driving home one night the dash board in the car all of a sudden hade a lot of warning lights come on. I brought it to my mechanic to have a look at it and he suggested to change the spark plugs and the coils which was done and he cleared the codes. About two days later I went to go to work and once again all the warning lights on the the dash board come on and it wouldn't drive over 5 MPH.

For approximately eighteen (18) months prior to this discovery, I unknowingly operated this vehicle under these conditions. During that time, my minor child was a regular passenger in the rear seating area of the vehicle. My child has experienced ongoing neurological symptoms, including persistent headaches over a period of several months, which have required medical evaluation. While no definitive cause has yet been determined, the possibility of prolonged exposure to fuel vapors within the vehicle is a serious concern. Nissan’s guidance assumes owners can rely on Level 2 charging, which for my vehicle requires approximately 7–8 hours for a full charge under ideal conditions. Because I do not have residential Level 2 access, the loss of Level 3 charging renders the vehicle functionally unusable for my living situation. This defect severely compromises both my safety and mobility by forcing me into an impossible choice: either risk using the charging method associated with battery overheating and potential fire, or lose access to essential transportation. The defect also forces prolonged public Level 2 charging sessions simply to maintain vehicle operation, creating significant daily hardship. Without safe fast charging capability, the vehicle cannot reasonably support long-distance travel or interstate relocation, leaving me effectively unable to move out of state without major financial and logistical burden. I have pursued resolution directly with Nissan and through BBB Auto Line. Nissan acknowledged the recall but declined buyback or meaningful relief, instead advising continued waiting for a future remedy. The mechanic indicated that this type of failure should not occur under normal operating conditions. Additionally, the Ford parts dealer advised that the failed fuel pump could be returned for credit, suggesting the failure may not be consistent with normal wear and tear. This failure occurred suddenly, without warning, and resulted in a complete loss of power while driving, which I believe represents a significant safety concern. Due to concern for vehicle safety and possible engine damage, the vehicle was towed to Vaden Chrysler Dodge Jeep RAM for diagnosis. The dealership later advised me that the spark plug in cylinder #2 had cracked porcelain and that the vehicle requires spark plug replacement. The dealership also stated they were awaiting authorization/approval from Stellantis before proceeding with repairs. This failure occurred extremely early in the vehicle’s life and caused my family to become stranded during travel. A flashing check engine light indicates an active misfire severe enough to potentially damage emissions components and raises concerns about engine reliability and safety. I am concerned there may be a manufacturing defect, ignition system defect, spark plug defect, improper assembly issue, or another underlying engine-related problem. I am submitting additional documentary evidence from Jaguar Land Rover Portland showing that during the repair process: * I was advised the vehicle would only run while connected to external jumper power and would shut off once that external power was removed. * Two days later, I was advised the installed battery would only charge to 6 volts and that another battery was needed to continue testing. These communications raise concern that this failure event involved broader electrical consequences and further support that my vehicle experienced the same defect condition described in Recall 26V248. The dealership ultimately completed repairs and charged me for the failure, while JLR maintains my VIN is excluded. I am requesting NHTSA review whether my VIN was improperly excluded from the recall population and whether the affected vehicle population should be expanded. While driving, the engine suddenly shut off completely at a traffic intersection. After turning the vehicle off for 5 minutes, it was restarted and driven to another authorized Mazda dealership (Crystal Lake, IL) for specialized diagnostic scanning. The second dealership successfully retrieved stored diagnostic trouble code (DTC) P061B, which is directly linked to safety recall 24V-815 (Loss of Drive Power from Software Errors). Crucially, the detailed diagnostic scan revealed that this code was historically stored with a timestamp dating back to December 2024. This proves that the safety defect was active and detectable during the first dealer visit 16 months prior, but was completely missed by the first dealership, leaving my family with an ongoing crash hazard. To address the failure, the dealership replaced the low-pressure fuel pump in the fuel tank and a mass airflow (MAF) sensor. However, Mazda Corporate customer experience has formally stated to me in writing that they are "not in a position to guarantee the concern will not reoccur." Because this safety-critical propulsion failure causes the vehicle to suddenly die in traffic and has a documented history of ineffective recall remediation, it poses an unresolved, severe crash risk. When I turn the vehicle off and restart it, it temporarily returns to normal, but the problem keeps coming back. This makes the vehicle unsafe and unreliable for normal use. The vehicle is my work vehicle, and I depend on it for transportation. I am now unable to work normally or travel safely because I do not know when the vehicle will suddenly lose power again. This condition places my life and the lives of my passengers at risk. I contacted both the dealership and American Honda Corporate. However, my safety concern has been minimized, and I was told that I would have to pay for diagnosis, even though this problem began after Honda performed recall work on the fuel system.
