Subject: URGENT: Investigation Request – Systemic Loss of Propulsion in Jaguar Land Rover (JLR) Vehicles (2017–2026). 1. Description of Defect I am formally reporting a critical, life-threatening safety defect involving the Powertrain Control Module (PCM) and the Fuel Pump Driver Module (FPDM) in Jaguar Land Rover (JLR) vehicles. These modules suffer from spontaneous software "resets" or
... "reboot" loops during operation. When the PCM resets, it severs communication with the fuel system, causing an immediate and total loss of propulsion (motive power). 2. Safety Hazard and Real-World Impact This defect triggers without warning at highway speeds, resulting in vehicles stopping abruptly in high-speed lanes. This creates a "sitting duck" scenario, rendering the vehicle unable to move to a shoulder and significantly increasing the risk of catastrophic rear-end collisions and multi-vehicle pileups. Reported incidents include near-misses and accidents directly attributed to this sudden stalling. 3. Timeline of Manufacturer Awareness (2017 – Present) JLR has been aware of this specific PCM/FPDM communication failure since at least 2017. Internal documentation, such as NHTSA Technical Bulletin SB-10095121-1020, identified short circuits in the PCM/FPDM line for 2017 F-PACE and XE models. Subsequent bulletins, including MC-10218879-0001, admit that software "flashing" is ineffective and that FPDM hardware replacement is often required. Despite nearly a decade of internal knowledge, JLR has failed to initiate a comprehensive global safety recall for all affected VINs, opting instead for localized "Service Actions" that do not permanently resolve the hardware instability. 4. International Context and Manufacturer Inaction The Chinese State Administration for Market Regulation (SAMR) has already summoned JLR officials regarding these stalling failures. However, JLR has not provided a verified, permanent solution. 5. Demand for Action We demand an immediate, multi-agencyRead more

My vehicle recently experienced a sudden flashing check engine light, severe misfiring, and rough engine operation, creating an unsafe driving condition. An inspection by an authorized GMC dealership, Serra Buick GMC Champaign, confirmed a failed deactivating valve lifter on Cylinder 5, documenting that the lifter ceased movement after a single engine rotation. The dealership advised that the fail
... ure necessitates replacement of all lifters and extensive internal engine components, at an estimated cost of $5,650.75. This defect is not isolated, unforeseeable, or the result of consumer misuse. It is directly associated with General Motors’ Active Fuel Management (AFM) system, which has been widely reported to cause: •Premature lifter collapse •Camshaft damage •Severe engine misfires •Loss of power while driving •Catastrophic engine failureRead more

Subject: URGENT: Global Safety Defect Report – Jaguar Land Rover (JLR) Powertrain Control Module (PCM) and Fuel Pump Driver Module (FPDM) Failure. Description of Defect: I am writing to formally report a critical safety defect affecting Jaguar Land Rover (JLR) vehicles involving the Powertrain Control Module (PCM) and the Fuel Pump Driver Module (FPDM). These modules are experiencing spont
... aneous "reset" or "reboot" events during vehicle operation, leading to a sudden and total loss of propulsion. Safety Hazard: Affected vehicles are experiencing power loss at highway speeds, often resulting in the vehicle stopping abruptly in live lanes of traffic. This defect has directly led to near-misses and reported accidents. The loss of power frequently occurs without warning, rendering the vehicle a "sitting duck" on high-speed roads, creating an extreme risk of rear-end collisions and multi-vehicle pileups. Manufacturer Inaction: Despite internal technical bulletins (e.g., NHTSA TSB MC-10218879-0001) and mounting consumer complaints, JLR has failed to issue a global safety recall or provide a permanent hardware/software resolution. While the Chinese State Administration for Market Regulation (SAMR) has reportedly summoned JLR officials regarding these stalling issues, the manufacturer has yet to implement a verified fix or acknowledge the systemic nature of the failure. Demand for Action: We request that your agency launch an immediate investigation into the frequency and severity of these stalling events to compel Jaguar Land Rover to initiate a safety recall and protect the motoring public.Read more

I took my car to the dealer on [XXX] with concerns about the strong gas smell that I have been smelling. The Hyundai dealership kept my car for 3 days and stated they could not smell the gas after driving the vehicle themselves. They did repair a cracked cylinder head however they did tell me that it wouldn't have been the cause of the fuel smell. They stated next time I am out driving, and I noti

I am [XXX], one of which is still in a car seat and unable to unbuckle themselves to get out if something terrible like a fire was to arise. This was the 2nd time taking my car in, as it was previously fixed in Feb 2025 due to another recall on the gas tank expanding. I do not feel safe transporting my children in a vehicle that is still reeking of gas and a possible hire hazard with no resolution. Went in for a typical servicing. They noted that the downstream O2 sensor was damaged by rodents. The vehicle, with around 14k has always been stored in a weather tight garage, never stored outside. There has been no rodents in the building since I owned it from 1999. This is a presumptuous statement I refuted, but they wouldn’t take off, and I felt this is a warranty or recall item. There
... are lots of speed bumps in the North Portland neighborhood, which may have caused the problem by cable scraping. I believe they documented it as rodents to escape their extended warranty contract requirements. I purchased an O2 sensor, which my independent said I don’t need to use it after taping the scrape as there’s no electronic issues being reported in the system. I would like this investigated and them pay for the sensor I apparently don’t need to install.Read more

On the afternoon of 01/20/26, I was merging onto the highway in my 2021 GMC Sierra 1500 when a loud bang was heard from the engine compartment at approximately 60 mph. Immediately after the ban was heard, several indicator lights on the dashboard were illuminated, including the ESC, check engine light, and parking brake light. A power loss was felt when attempting to accelerate and the banging noi
... se worsened anytime the accelerator was depressed. I was able to drive the vehicle to the nearest GM dealership that coincidentally was at the closest exit off of the highway I was traveling on. The dealership performed diagnostic and determined that the damage was caused due to the ongoing GM recall associated with Safety Recall N252494000. My vehicle had previously been inspected as part of the recall and was found to have passed. Vehicle had been driven approximately 4,000 miles after recall was performed.Read more

We bought a 2021.5 Volkswagen Atlas that, starting around 15,000 miles, began having computer-related issues that were reported but never truly repaired. Over time, those issues escalated into serious drivability and safety problems. Beginning in mid-2025, the car started driving poorly. It had hesitation, dragging, poor fuel economy, and then began stalling, sometimes at idle and sometimes while
... driving. At first, repairs like a battery replacement were tried, but they did not fix the problem. In August 2025, the car stalled on the interstate in Nashville with flashing EPC and check engine lights and would not restart. Our family, including our children, was stranded in an unsafe situation. The car had to be towed, and a fuel pump was replaced. Even after that repair, the stalling and warning lights continued. Later in August, when the car continued to stall, Don Jacobs Volkswagen told us the problem was a warranty-covered EVAP (carbon) canister and that repairing it should resolve the issue. It did not. In December 2025, the car stalled again with flashing warnings and was deemed unsafe to drive. It was towed back to Don Jacobs, where a second fuel pump replacement was performed. In January 2026, the check engine light came on again and the car was returned to the dealership. At that time, we were told the EVAP canister had been left unattached from prior testing, not from the most recent repair. Since then, the vehicle has continued to drive poorly, with ongoing hesitation and dragging. In total, the vehicle has been in the shop four times, including three visits to the same dealership, and no one has been able to diagnose or permanently repair it. The dealership has told us they cannot proceed unless specific fault codes appear, even though the vehicle continues to malfunction and send diagnostic alerts through the Volkswagen app. Because of the repeated stalling, unresolved repairs, and lack of a clear path to fix the problem, we do not feel the vehicleRead more

Dear Texas Department of Motor Vehicles, I am submitting this letter in support of a Texas Lemon Law complaint regarding my vehicle, which has experienced repeated mechanical failures that substantially impair its use, value, and safety. The vehicle has been subject to multiple manufacturer recalls and remains in the repair shop at this time. It is available for inspection upon request. The vehi
... cle has exhibited a persistent knocking sound from the engine or drivetrain area, intermittent loss of power, and a brake system defect in which the brakes stick unexpectedly. These conditions have continued despite multiple repair attempts by an authorized dealership, including repairs related to manufacturer recalls. These defects constitute a serious safety concern. The loss of power occurred while driving and created a hazardous situation by preventing the vehicle from accelerating as expected in traffic. The brake sticking condition impaired my ability to maintain safe control of the vehicle and increased the risk of sudden or unintended braking, endangering myself, passengers, and other motorists. The issues have been reproduced and acknowledged by the dealership, as demonstrated by repeated service visits. The vehicle has been inspected and repaired by the manufacturer’s authorized service center on multiple occasions. No inspections have been conducted by law enforcement or insurance representatives. Prior to and following recall repairs, the vehicle continued to exhibit symptoms including the knocking noise and intermittent power loss. Any warning lamps or dashboard messages are UNKNOWN. Because these defects remain unresolved after a reasonable number of repair attempts and involve safety-related systems, I believe this vehicle meets the criteria for review under the Texas Lemon Law. This complaint concerns failure to provide a remedy for multiple active NHTSA safety recalls, resulting in a vehicle that has been determined unsafe and inoperable. The vehicle is subject to NHTSA Safety Recall 24V-160 involving the house battery relay / contactor. During a January 7, 2026 call between the servicing dealer (Lazy Days) and Jayco / Entegra technical support, the battery system was
... diagnosed as having a stuck contactor with no 12V output. Jayco / Entegra technical guidance instructed the dealer to shut down the battery and specifically advised that the battery should not be turned on or charged. The dealer was further instructed that the coach should remain at the dealer until a replacement could be obtained. As a result of this instruction, the vehicle has been rendered unsafe and inoperable. No manufacturer-directed repair or replacement timeline has been provided. In addition, the vehicle is also subject to NHTSA Safety Recall 24V-312 involving the fuel tank mounting bracket. This recall likewise requires dealer-performed remediation. No repair timeline or remedy has been provided for this recall either. Despite repeated contact with Jayco / Entegra, the manufacturer has stated that the vehicle is out of warranty and has declined to provide a manufacturer-directed remedy, referring the matter to a component supplier. Warranty expiration has been cited despite the existence of active safety recalls and a documented instruction not to operate the vehicle. The vehicle has been left at the dealer with no repair path, no timeline, and no safe method of operation despite active safety recalls. This complaint is submitted due to failure to provide a timely remedy for active NHTSA safety recalls.Read more

The next morning I went to start my car and it was a very very rough start to get it started. I decided then to take it to the Honda dealer. On my way to the Honda dealer, my car did the "lose power" again. I had to sit there for about 15 minutes before I finally had enough power. I "inched" my way to the dealer. They called and said it was the fuel injectors and that they needed to be replaced. My car is a 2022 with 56,000 miles on it. THIS SHOULD NOT HAPPEN with that low of mileage. Honda has said this was not a covered repair even though I had no power to my vehicle. I did research and have found that there is a definite issue with the 2022 Honda CRV fuel injectors but they only have covered certain VIN numbers. My VIN number is not in that replacement -- which I don't understand. This should be a recall on ALL Honda 2022 CRV's. This is a very expensive repair and I feel Honda should cover the replacement of these fuel injectors.Read more

Life-Threatening Acceleration Lag: The vehicle is experiencing severe power loss during critical driving moments, for example, while entering intersections or attempting to re-accelerate into moving traffic. When I attempt to speed up, the engine fails to respond initially, leaving the vehicle stranded in the path of cross-traffic. The engine finally engages only because I am forced to floor the a
... ccelerator pedal out of necessity, which then causes the tires to squeal when power eventually returns. Intermittent Starting Issues: The vehicle frequently fails to start smoothly on the first attempt; a known symptom associated with the fuel pump impeller deformation described in Recall 23V-858. VCM Shudder and Hesitation: I am experiencing the distinct vibration and rhythmic shudder associated with the Variable Cylinder Management system. This symptom is most prominent during deceleration or coasting; specifically, when I lift my foot off the accelerator without applying the brakes, the system triggers a noticeable shudder as it incorrectly manages cylinder engagement/deactivation. Suspected Catalytic Failure (Codes P0420/P0430): Both catalytic converters are exhibiting inefficiency codes simultaneously, which are highly atypical and occur well before the expected component lifespan. This clearly points to upstream defects. Specifically, the catalytic substrate has been poisoned by carbon and phosphorus deposits from the documented oil-burning defect and has suffered thermal damage due to the improper fuel mixtures caused by the defective high-pressure fuel pump (Recall 23V-858).Read more

My vehicle has had 3 fuel pump failures in under a year, requiring 3 separate fuel pump replacements by an authorized Nissan dealership. The fuel pump failure causes the vehicle without warning to stall in active traffic, loss of power and the drivers seat to move back away from the steering wheel. The infotainment center has also failed 2 times and had to be replaced 2 separate times by an aut
... horized Nissan dealership. The infotainment center failures also cause the back up and side view cameras to become inoperable. The vehicle and repair records are available for inspection upon request. My safety and the safety of others is put at risk because the repeated and unpredictable fuel pump failures cause the engine to stall in active traffic and the loss of power. This is unsafe because the stalling has happened in hazardous situations like while exiting the interstate. The drivers seat also moves away from the steering wheel and brake while the car is in motion creating a risk of loss of control. Additionally, when the infotainment system fails so do the side view and back up cameras increasing the risk of an accident. Although the vehicle is currently operational, the pattern of failures creates an ongoing safety risk. An authorized Nissan dealership confirmed the failures and replaced the fuel pump three times and the infotainment system twice. The vehicle was inspected and repaired multiple times by an authorized Nissan dealership, and the issues were reported to Nissan Consumer Affairs. The failures occurred intermittently and without warning.Read more

The fuel tank shield is bolted onto an aluminum heat shield, and the pairing of these two different metals accelerates the corrosion. Land Rover has acknowledged this problem.

The car stalls upon acceleration while driving. At times it will stall in the middle of the road; skipping repeatedly or simply not accelerating at all. There are often no warning signs at all, though when it skips repeatedly upon acceleration, the engine check light will appear. The car can be available for inspection upon request.

Incident date: Dec 31, 1969
Following a manufacturer over-the-air (OTA) software update released in Fall 2025 and subsequent completion of Safety Recall 68C, my vehicle exhibits new and dangerous driving behavior that did not exist prior to these updates. While operating in electric mode, the vehicle can unexpectedly lose power during acceleration, including at roadway speeds and while climbing hills, creating a risk of sudd
... en deceleration in traffic. Under electric operation, the vehicle accelerates normally up to approximately 45 mph. Beyond that point, electric power output progressively decreases even as accelerator input is maintained or increased. Vehicle speed can decline while the accelerator is further depressed, and the electric power indicator visibly drops. This often results in abrupt internal combustion engine (ICE) engagement, aggressive downshifting, and sudden acceleration. This loss-and-surge behavior is unpredictable and hazardous, particularly in traffic or on inclines. Prior to the OTA update and recall action, the vehicle provided consistent electric power well beyond 45 mph, and ICE engagement was smooth and nearly imperceptible. I consistently achieved approximately 22–23 miles of electric range on the same daily route. Post-update and post-recall, usable electric range has been reduced to approximately 14–15 miles under similar conditions, regardless of weather. The issue also occurs at low speeds, including complete loss of electric momentum while climbing hills, followed by harsh ICE engagement that can cause wheel spin. This behavior raises safety concerns related to power loss and unexpected acceleration. Based on the timing and nature of these changes, I am concerned that software updates may be intentionally limiting battery power output and usable capacity to reduce stress on potentially defective high-voltage batteries rather than correcting the underlying hardware defect. This materially alters vehicle performance and safety and requires investigation.Read more

I received a recall notice for my 2013 Kia Optima, VIN #: [XXX] related to engine issues and an increased risk of fire. After receiving the recall notice, my vehicle began smoking and briefly caught fire while in operation. I immediately shut the vehicle off, and the smoke and fire stopped. For safety reasons, the vehicle was towed by my insurance company to my aunt’s residence, which was the
... closest safe location at the time. The vehicle was unsafe to drive. No injuries occurred, and the fire department was not called because the fire stopped once the vehicle was turned off. I believe this incident is related to a known safety defect and recall involving Kia Optima engine fires. I have contacted Kia multiple times regarding this issue, and there have been delays in addressing or repairing the vehicle. This vehicle is equipped with a feature known as "e-pedal" also known as one-pedal driving which is quite common on EVs. When this mode is activated, the accelerator pedal acts as a speed command (vs. power) and when the pedal is not depressed, the vehicle applies brakes until it comes to a standstill. Recently, the system has demonstrated a fault condition, where the Vehicle Control Module (VCM)
... issues a fault warning and disengages the e-pedal mode. This appears to occur randomly after the vehicle has been at a standstill for over one minute (e.g. waiting at a traffic signal) where the driver is using the one-pedal system vs. continuously applying pressure to the brake pedal. The safety issue is that when the system detects a fault and disengages e-pedal, the vehicle starts to move forward, requiring the driver to take immediate action to stop unwanted motion. This can lead to a collision with a vehicle or pedestrian in front of the vehicle. The desired behavior is that when the VCM detects a fault with the e-pedal system, it should engage the Park mode and prevent the vehicle from moving forward. The driver must then depress the brake pedal and affirmatively command the Drive mode before moving. Just disengaging the e-pedal (brake) without prior warning of a fault is a safety hazard.Read more

Check engine light came on after attempting a remote start and failed. Upon using an OBD-II code reader, it gave the code: P2421 - EVAP Vent Valve stuck open. This part is not covered under the 8/80k federal emissions warranty but is still an issue and is affecting emissions. There were no other alerts or warnings issued before attempting the remote start that caused the issue.

The ICCU failed at 11,000 miles and 11 months into the lease. When the ICCU fails, the car becomes inoperable. This was a major safety hazard since it broke down on a major US highway and I was barely able to get off to the side of the road. This seems to be affecting a large number of Ioniq 5 and Ioniq 6 models.

We heard a pop noise and then received low 12 volt low battery warning. The Integrated Charging Control Unit (ICCU) might be the root cause of the Hyundai Ioniq 5 electric vehicle. Car went into slow turtle mode and we could barely move the car. we pulled over and found a safe spot to park car. we had car towed to Hyundai dealership since car was not operational.
