The"MIL" reported "Emmisions System Problem" The Dealer stated "DIAG Code P219A" was the underying cause, and the reccomended solution was to "replace fuel injectors and check for PGM updates". After some research I discovered that these symptoms, codes, replacement parts etc.. are all consistant with a Honda Service Bulletin (MC-10183514-0001)

After reviewing the document I found that my VIN is not included in the scope of the "warentee extension" I opened case number 16127621 with AMERICAN HONDA MOTOR CO., INC. to inquire further and get a reasonable answer as to why my VIN was excluded from scope, they were responsive, but provided no explanation other than my VIN " is not included in this campaign for fuel injector concerns. Unfortunately, your vehicle is not eligible for reimbursement at this time." I would like to have it "on record" that I have voiced concerns suspecting this is not a coinsidence, and the scope of the recall should be expanded.Read more

The car was driven from Cheshire, CT to Old Tappan, NJ about 90 miles, just under two hours drive time. Combination of highway and back roads. The car was in drive in comfort mode proceeding slowly (approx 20 mph) down a street within a condo complex. As I approached the driveway I lightly braked, turned into the driveway then lightly pressed the accelerator to bring the car up the driveway apron
... onto the single car length driveway. At that point the car accelerated to what felt like max power. The car accelerated uncontrollably causing the car to advance in a right turn through the closed garage door striking a garaged vehicle in the right rear quarter and the right wall of the garage. Fortunately no one was hurt but this could have resulted on severe injury or death. No pre warning lights just a slight burning smell just before. This unrequested acceleration occurred twice before the crash incident leaving a traffic light from stop. Porsche has analyzed the car and find no fault. The insurance company has sent an adjuster. The same issue has happened twice. I'm driving on the highway, going maybe 60 or 65, and with no warning, the check engine light starts flashing and the car won't respond to the gas pedal. The car immediately starts to rapidly decelerate, and I can only try to get to the breakdown lane before I get rear-ended. I've been lucky (especially since my child was in the backseat both times). But when I s
... top the car, turn it off, then turn it on again, everything is fine. I can get back on the highway, and no problems occur. The codes are not stored, however, so I can't know what caused the problem. When this happened the first time (last year), I took it to the Honda dealer where I bought it, and they said they had never heard of this happening. When it happened the second time in January, I called Honda, spoke to an agent who gave me a case number (XXX), and said she has heard of something like this from other drivers, but the solutions were all different, so she couldn't say what would fix it for me. I did some internet research, and found other drivers who describe the exact same issue: [XXX] I've switched to premium gasoline ($$) and won't let it get lower than 1/4 full, and have an OBDII scanner in my glove compartment ready to go if it happens again. That said, this is a very dangerous defect, and I feel unsafe in my car. I am disturbed that a problem of with such potentially serious consequences has no solution that anyone can offer me. While turning my 2022 BMW 540i onto a busy, 50 MPH road, the vehicle suffered a catastrophic drivetrain failure, without any physical trauma such as encountering a curb or pothole. The car experienced a total and immediate loss of motive power (propulsion), leaving me coasting in live traffic. This created an extreme safety hazard as I was unable to accelerate or maintain speed to clear the roadwa
... y safely. The failure occurred immediately after a significant temperature stress: taken from a heated garage, driven with no issue for 40 minutes, then parked in 15 degree weather for two hours, then complete failure within moments of starting up after the two hour cold parking block. My reading since the incident reveals that BMW quietly switched a critical drivetrain part from a composite/plastic material to metal in later 2022 production runs, due to temperature tress failures precisely as I experienced. This 'silent fix' suggests a known design defect in early 2022 models that poses a recurring risk of sudden propulsion loss. BMW did not do a recall for this, but they should have: I and other drivers were in real danger when my car spontaneously lost all motive ability in high speed traffic.Read more

Incident #1: Car suddenly and without warning had complete loss of power, with gas engine shutting off and only using battery power. Car was traveling at 45mph in morning traffic. I was able to pull off road and turn off car and

Incident #2: 2 days later, while driving my kids to school the car again, suddenly and without warning had complete loss of power, with gas engine shutting off and only using battery power. Again, warning dash lights: "Check Hybrid System. Stop Safely!". I did so and car ran normally back to my house (distance approx: 2.5 miles). I immediately called KIA of Beaverton and they said to have it towed right away. Once at dealer, they were unable to diagnose any issues and said that no codes showed up, but also said that the car "deleted codes". Car was in shop for 2 days and determined "car operates as designed." I picked up the car. Incident #3: 7 days later, with kids in the car, again car had complete power loss with no warning. Warning lights again told of "Check Hybrid System. Stop Safely!" Pulled over (in heavy Friday rush hour traffic) and turned off car. Turned on again, but this time car drove for less than a quarter mile before complete power loss happened again with all warning lights. This time we took video of the car not having power, all the warning lights etc. as evidence. We turned off the car and upon turning it on, it instantly had no power. We pulled car over and had to park it that night at a local business for towing. Car was towed again to Beaverton KIA. Again, dealer said they were not able to replicate problem after driving 250 miles on it. Insisted we pick up car, despite numerous comm's saying how unsafe we felt taking it. I am writing to report an additional safety‑related incident involving my 2024 Jeep Wrangler Rubicon 4xe. This vehicle previously experienced propulsion issues that I reported under NHTSA Case Number 11692887. On the morning of February 4, 2026, while driving to work on a busy interstate (approximately 12 miles from home), the vehicle again began exhibiting propulsion instability. The vehicle e
... xperienced repeated surging and thrusting movements that became progressively more severe during the drive. During these events, the check‑engine light intermittently illuminated and then turned off. This incident occurred exactly seven days after my vehicle was evaluated by Sam Leman Dodge on January 27, 2026, for three open safety recalls. At that visit, I was provided paperwork indicating that the issues had been addressed. However, as of today, the official Jeep/Mopar recall website still shows multiple serious and active safety recalls for my VIN, including recalls involving fire risk and sudden loss of motive power. After the February 4 incident, I contacted the dealership, and their service department advised me not to drive the vehicle and recommended that it be towed to the dealership for evaluation. I followed this guidance due to the severity of the propulsion behavior and the safety concerns involved. Given the recurrence of propulsion instability, the worsening severity of the surging behavior, the dealer’s recommendation not to drive the vehicle, and the presence of multiple unresolved safety recalls, I am requesting NHTSA’s assistance in ensuring that the manufacturer addresses these issues and restores the vehicle to a safe operating condition. Thank you for your attention to this matter. Recognize that this is the second time I have emailed the NHTSA regarding propulsion issues with this vehicle (Earlier Case Number 11692887). 2018 Kia Sorento 2.4 GDI. In late 2024 a Kia dealer performed an oil-consumption test and confirmed the engine was burning excessive oil. In Feb 2025 the engine long block was replaced. Within about 4,000 miles the vehicle began setting P1326 warnings, misfiring, losing power, and struggling to climb hills. On a highway trip from South Carolina back to Indiana, the vehicle could barely maintain 3
... 0–40 mph on grades with the accelerator floored while traffic was moving much faster. The check-engine light returned repeatedly. The vehicle eventually died on an exit ramp and had to be pushed out of traffic and towed. I felt it was unsafe to drive but was repeatedly told by dealers that it was “OK to drive” or that the issue was “bad gas.” After visits to multiple dealers, the current Kia service center finally diagnosed a severely clogged exhaust. They report the manifold catalytic converter and underbody converter have broken down internally and are heavily coated in carbon/oil deposits, and state this likely came from the engine burning oil before and around the time of the rebuild. Exhaust temperatures have been recorded at very high levels, and the engine will only run briefly with an O2 sensor removed to vent exhaust; with the exhaust sealed it will not stay running. The dealer has warned there may now be internal engine damage from prolonged backpressure. The vehicle has been undriveable for many months and the failed converters are being retained and are available for inspection. My concern is that excessive oil consumption on these GDI engines is causing secondary catalytic-converter failure, extreme power loss, stalling risk, and possible fire risk from overheated exhaust components, even after engine replacement.Read more

I have a 2026 F-250 diesel with 6K miles on it that caught fire about 3-5 minutes after using it to jumpstart a UTV. Jumping the UTV went just fine as expected, and I disconnected the cables and shut the truck hood with no sparks, smoke, or flames at
... the time. Within 3-5 minutes, I noticed there were flames coming from inside my truck at the center dashboard area. We attempted to extinguish the fire ourselves, but the fire department was ultimately called. If my son had not gotten out of the truck to get into the UTV with me, he would have been in the back seat when the fire started. The truck was pulled close to my barn when the fire started, which caused damage to the barn. A fire investigator hired by my insurance, and the fire marshal came out to my home to inspect the truck, and both determined it was not my fault the truck caught fire. There were no warning labels indicating the drivers side battery should not be used to jump start vehicles. The fire investigator stopped at a local ford house, and none of the employees were aware the drivers side battery should not be used to jumpstart vehicles either. I found information online that mentions fires occurring in 2025 & 2026 due to cracked fuel return lines, and engine heater block problems. The fire investigator stated if I was able to find that information online, that the manufacturer likely knows there are problems, and chose to sell the vehicles anyways. My truck is currently waiting to be inspected by a Ford Engineer to try and determine the cause. This fire put myself, my son, barn, and animals at risk. If Ford knows about any potential fire hazards, they should have never released the truck for sale. Due to the fire, I had smoke inhalation, and visited my PCP a few days after. I'm still waiting for the fire departments report.Read more

The high-pressure fuel pump (HPFP) in the vehicle’s diesel fuel system failed. This component is subject to NHTSA Safety Recall 22V-767 (FCA Recall Z96). The HPFP has failed more than once, including after recall replacement, and the vehicle is currently in dealer custody, making the component available for inspection upon request. The HPFP failure caused the vehicle to stall and become inopera
... ble, resulting in an unexpected loss of motive power. This created a safety risk by leaving the vehicle disabled and unable to be safely driven. The recall itself identifies HPFP failure as a condition that can lead to engine stall without warning, increasing the risk of a crash. The repeat failure after recall repair further increased safety risk. The failure was confirmed by an authorized Ram dealership (Gene’s CDJR of Fairbanks). The dealer documented HPFP failure and fuel system contamination, performed the recall repair, and later confirmed repeat fuel system issues. Most recently, the replacement HPFP failed again during an extended test drive performed by the dealer, confirming recurrence of the defect. The vehicle has been repeatedly inspected by an authorized Ram dealership, including: Recall repair inspection and replacement of the HPFP and related fuel system components, Multiple follow-up inspections after continued symptoms, Diagnostic inspections following check-engine lights, noise, fumes, and loss of drivability. No police or insurance inspections were performed, as this was a mechanical failure handled directly by the dealer under a safety recall. There was no warning prior to the HPFP going out the first time.Read more

I initially brought my vehicle in for service due to a strong gasoline odor and the vehicle failing to start. At that time, I was informed that no gasoline smell was detected and that the issue was instead a faulty starter, which I authorized for repair. Approximately two weeks later, my vehicle again would not start. I was then informed that the issue was a lower fuel injector, which required add
... itional repairs. Just three days after that service, the vehicle once again failed to start, and I was told this time that the upper fuel injector was the cause. As a result of these events, I incurred a total of $3,946.96 in repair costs across two service visits and was forced to have my vehicle towed three separate times. Before the third visit, I was informed that I would need to pay an additional $1,500, on top of the nearly $4,000 I had already paid. I challenged the accuracy and consistency of the diagnoses at that point, and ultimately, the vehicle was repaired without charging me the additional $1,500. While I appreciate that the final repair was completed without further cost, I am deeply concerned about the pattern of repeated breakdowns, inconsistent diagnoses, and the financial burden placed on me as a customer. The original concern of a gasoline smell appears to have been valid from the outset, yet it took multiple failures, repairs, and significant expense before the underlying issue was resolved.Read more

I purchased this 2018 Honda Accord Sport brand new from the dealership, relying on Honda’s longstanding reputation for reliability and longevity. At approximately 120,000 miles, the vehicle suffered a blown head gasket during normal driving conditions. The vehicle has been properly maintained according to Honda’s recommended schedule. I have already opened a claim with Honda corporate regardi
... ng this issue and am currently awaiting their response. Despite proper maintenance, this catastrophic engine failure occurred, suggesting a design or manufacturing defect rather than misuse. This issue presents both a safety concern and a substantial financial burden, as I am a single mother of four who depends on this vehicle for daily transportation. Based on reports from other owners, this appears to be a widespread defect affecting 10th-generation Honda Accords with the 1.5L turbo engine, which I respectfully request NHTSA investigate.Read more

About 4 weeks ago my vehicle began shaking uncontrollably while driving. The check engine light illuminated and multiple warnings appeared including emissions system, variable valve timing system, engine and transmission failures shown on the GMC app. I contacted the selling dealership and was given an appointment 4 days later. Later that day I drove less than 5 miles to pick up my children from s
... chool. During that drive the vehicle filled with a strong gasoline odor and exhaust fumes were overwhelming. I feared the vehicle could catch fire. I contacted the dealership again, stated the vehicle was unsafe, dropped it off at approximately 8:00 PM and was required to obtain my own rental vehicle. After one week I was told the engine required a camshaft and lifter replacement. Inspected & approved by extended warranty, parts were ordered and installed. I retrieved the vehicle on 23 days after the initial failure. The following morning (24th day) while driving approximately 75 mph on a major interstate with my children the vehicle suddenly shifted into neutral and would not re-engage. I was unable to shift into any gear and coasted to a narrow shoulder inches from traffic and guardrail. The dashboard stated the vehicle could not shift into gear and instructed me to push the start button. The engine would not restart. We were stranded approximately two hours in 10-degree weather on a major interstate with heavy semi-truck traffic. Roadside assistance was required. 3 days later the dealership stated the failure was a fuse block. After replacement the vehicle was test driven and stated to be operating normally. A second dealership employee test drove the vehicle, it died again and had to be towed back to the dealership. The battery was replaced after draining while disabled. Repeated mechanical and electrical failures resulted in complete loss of power at highway speeds and placed my children and me in immediate danger. I no longer believe this vehicle is safe to operate.Read more
