To Whom It May Concern: I am submitting this complaint to report what I believe to be a potential safety-related defect in my Honda vehicle, as well as an unresolved dispute with American Honda regarding responsibility for resulting damage. Recently, I was informed by my case manager at American Honda that my claim had been closed and that Honda would not cover the necessary repairs. When I reques
... ted an opportunity to escalate the matter, I was told that no escalation was available. I am seeking NHTSA’s review because the circumstances involve component failures that may affect vehicle emissions, performance, and potentially safety. The fuel injectors in my vehicle failed, this failure caused the catalytic converter to become damaged, andHonda accepted responsibility for the faulty fuel injectors. Honda has refused to cover the damage to the catalytic converter. Because the catalytic converter failure was reportedly caused by faulty fuel injectors—components for which Honda accepted responsibility—I believe the manufacturer should cover all resulting damage, regardless of warranty status. I request that the National Highway Traffic Safety Administration: Investigate whether similar fuel injector failures are occurring in Honda vehicles. Review whether these failures are causing catalytic converter or other engine damage. Determine whether the manufacturer is appropriately addressing these failures.Read more

NHTSA Complaint Text (Copy/Paste) Vehicle: 2020 Ford F-250 Super Duty 6.7L Power Stroke Recall: 24S78 – Powertrain Control Module (PCM) Software Update Complaint: Ford has issued Recall 24S78 for the high-pressure fuel pump defect on my 2020 F-250, but the company has not provided an actual remedy. The recall shows as open on my VIN, and Ford’s documentation says a PCM software update is the f
... ix, but Ford released the software and then pulled it back within a week. My dealership confirmed Ford removed the remedy from their system, so they cannot perform the recall at all. This leaves my truck exposed to a known safety defect with no available repair. The high-pressure fuel pump failure this recall addresses can cause sudden loss of power, engine stall, or complete shutdown while driving. Until a working remedy is released, I’m driving a truck that Ford has admitted has a serious safety defect, but they have not provided any fix or timeline. I am requesting NHTSA investigate why the remedy was withdrawn, why no replacement remedy has been provided, and why Ford is leaving owners with a known safety risk for an extended period. This is an expensive vehicle, and the defect can cause catastrophic engine damage and create a hazardous situation on the road. Owners deserve an effective recall remedy that is actually available. Not exactly sure when the recall was first brought to light but I do know it has been months now.Read more

Several years ago, when I was driving the vehicle, I got a sudden "reduced propulsion alert" and the onboard coolant temperature gauge showed a significant spike in temperature. To avoid overheating, I had to pull over on the side of the highway. In the onboard fuse box, there was a blown F3 fuse. I replaced the fuse and the same issue happened again last year. After some online research, it is c
... lear that the EGR valve failed. Unplugging the valve and replacing the fuse has returned the vehicle to normal operations, but it's now out of compliance with local emissions regulations. GM has failed to properly produce replacement parts. As of Feb 11, 2026, a local GM dealer in Bloomington, IN quoted a backorder and unclear wait time on a replacement part and quoted me at $2,000+ just for the part. If not for my quick thinking, this part failure could have overheated my vehicle causing extensive damages. GM has failed to produce enough replacement parts and given the safety issues associated with a blown fuse and overheating, NHTSA should order a recall on this part and force GM to replace the parts for owners. This is a widespread problem documented among Volt owners, and the only stopgap issue is to drive the vehicle out of compliance with local emissions rules.Read more

After purchasing a used 2023 Dodge Durango GT, issues arose almost immediately following delivery. I took the vehicle to a Chrysler/Dodge/Jeep/Ram (CJDR) dealership for inspection and service. Diagnostic scans revealed multiple stored and active fault codes, including but not limited to: U0415 / U0401 – Implausible or invalid data received from ABS / ECM / PCM B195F-12 – Power liftgate chim
... e control circuit short to battery B15A1-00 – Active electrical fault Multiple body control, module communication, and liftgate-related fault codes The vehicle experienced intermittent starting failures, creating safety concerns including: Potential inability to move the vehicle when needed, including in traffic Risk of being stranded in unsafe locations Uncertainty regarding whether the vehicle would reliably restart after being shut off The vehicle was removed from service and sent to an authorized Dodge dealership for further diagnosis and repair. The manufacturer/dealer elected to replace a fuel line as part of the repair process to address the starting hesitation. The vehicle has been inspected by an authorized Dodge service department. No police or insurance inspections have occurred. Symptoms occurred intermittently and without consistent warning lights, including: Delayed or hesitant engine starting Requirement to press the brake pedal firmly in order to start the vehicle Intermittent electrical system fault codes retrieved during diagnostics Symptoms first appeared almost immediately after vehicle delivery. The vehicle has a documented history of multiple electrical communication faults, including body control and power liftgate chime control circuits. The selling dealer (Westside Hyundai) was unable to retrieve these fault codes, resulting in the vehicle being out of service and pairs being coordinated at their expense. Hello, I have a 2021 Honda Accord Sport 2.0T Engine. The engine has a 2.0 liter turbo. I have had to replace two fuel pumps, which were covered my extended warranty. Another issue that I had to repair, and one that has not fully been resolved is the head gasket. I have been losing coolant at an alarming rate. Every two weeks I have to fill up coolant. I had the head gasket repaired at a Honda Dea
... lership, and this is currently being diagnosed by another Honda Dealership, as I do not want to deal with the Honda Dealership in Cedar Rapids, Iowa. Currently, a Honda Dealership in Iowa City, Iowa is handling the issue, but the head gasket along with the cylinder head were originally repaired in October 2025. Since December 2025, I have been losing coolant on a regular basis. I still have 5,000 miles left on my extended warranty, but there have been many owners who have had their head gasket replaced, or are needing replacement, and Honda refuses to acknowledge that there is a manufacturing defect with the engine. The 2.0 liter turbo engine has a major design flaw with it's cooling passages, and Honda has yet to come up with a resolution. Having an engine overheat is a major safety concern! The current dealership is driving my vehicle on a regular basis, in fact the service advisor is driving it to work and back home (35 miles each way) to replicate the problem. At this point, a recall should be issued by Honda, as there is currently a class action lawsuit that has occurred in the state of California.Read more

This 2019 Honda Accord was part of the fuel pump recall that occurred in 23/24. The vehicle was taken in and had the fuel pump fixed at the dealership under the recall. All accessory items, Lane Departure warning, adaptive cruise control, etc, all stay on
... as if malfunctioning. The vehicle was taken into dealership and diagnostic was run, results was two failing fuel injectors. Calls were placed to Honda and request was made to have them assist in a goodwill repair since so many of these vehicles are experiencing the same issue after the recall repair was made. Honda did a review of the diagnostic report and reported problem and stated that the vehicle fell outside of the warranty on the fuel injectors, and that they would not honor any assistance in repairs. They stated case was closed and no further conversations could be had. I mentioned that I knew that they had issued an extended warranty on 2016-2019 Honda CRV's and Civics for the fuel injectors up to 150,000 miles. They stated that my vehicle VIN did not fall within those, which I acknowledge. Vehicle is still malfunctioning and we will be responsible for the repairs, even though this is a well known issue after the fuel pump recall repair. It was stated that we will just need to fix the repairs on our own, which is over $2000 and that, if in the future, there is a recall or extended warranty offered then we could try and speak with them about possibly getting our money back.Read more

I am filing this complaint regarding my 2023 Jeep Wagoneer, which I have owned for approximately 1.5 years. Since purchasing the vehicle, I have experienced repeated and ongoing mechanical failures, primarily involving the fuel injection system and chronic battery drainage. The vehicle has been taken to the dealership for fuel‑related issues more than five times. During these visits, the fuel pu
... mp was replaced, but the problems continued. The vehicle repeatedly triggered check‑engine lights related to the fuel system. At one point, the dealership bypassed or cleared the check‑engine indicator without resolving the underlying problem. After this, the vehicle began performing noticeably differently, including a significant drop in fuel efficiency—going from approximately 18 MPG to around 14 MPG. In addition to the fuel system defects, the vehicle has had persistent battery issues. The battery drains unexpectedly and frequently dies without warning. Even after replacing the battery, the new battery lasted only about three months before the same drainage problems began again. The vehicle will randomly shut down or stop functioning correctly until another battery replacement is performed. These issues raise serious concerns about the safety, reliability, and overall drivability of the vehicle. The repeated failures of both the fuel system and electrical/battery system pose potential safety hazards while driving and have made the vehicle unreliable for normal use. I am requesting that NHTSA review these recurring problems, as they appear to indicate systemic defects affecting vehicle performance and safety.Read more

National Highway Traffic Safety Administration Office of Defects Investigation 1200 New Jersey Avenue SE Washington, DC 20590 Subject: Safety Defect Complaint – Integrated Charging Control Unit (ICCU) Failure Vehicle: 2026 Genesis GV70 Electrified (EV) VIN: [XXX] To Whom It May Concern, I am submitting this complaint to report a safety-related failure involving my 2026 Genesis
... Electrified GV70, which experienced a sudden “Check Electric Vehicle System” warning consistent with a failure of the Integrated Charging Control Unit (ICCU). Incident Summary On February 5, 2026, at approximately 4,480 miles, the vehicle displayed a “Check Electric Vehicle System” alert and was no longer safe to operate. Due to the risk of propulsion loss and electrical system malfunction, the vehicle required flatbed towing to an authorized Genesis dealership for evaluation. A dealership service advisor subsequently informed me that the vehicle has been diagnosed with an ICCU failure, a component responsible for charging the 12-volt battery and maintaining critical electrical systems. Failure of this component can result in loss of drivability, propulsion limitations, or vehicle shutdown, creating a significant roadway safety hazard. Safety Concern Failures of the ICCU have been widely reported across Hyundai, Kia, and Genesis electric vehicles and have been the subject of prior recalls due to the potential for unexpected loss of motive power while driving. Despite prior recall actions, failures continue to occur, including in my vehicle. Current Status The vehicle remains out of service pending repair, and no written repair completion timeline has been provided. The failure rendered the vehicle unusable and required emergency towing. I respectfully request that the Office of Defects Investigation review continuing ICCU failures affecting Genesis, Hyundai, and Kia electric vehicles to determine whether additional corrective action or expanded remedies INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIORead more

My car is equipped with a forward-facing camera, mounted on the forward area of the interior rearview mirror. Additional cameras are mounted on the front bumper, trunk door, and on the rearview mirrors on each side of the car. Various sensors mounted throughout both bumpers face the front, rear, and each side of the car. At the time of this incident all sensors and cameras were clean and free of
... obstructions, and software was up to date. I was driving uphill, negotiating a tight left curve at approximately 30 MPH in a rural, forested area in the Santa Cruz Mountains (California.) There were no other conveyances, objects, or obstruction on the roadway in front or behind me. The weather was clear and the daytime visibility was excellent. The road was dry and I estimate its gradient at 30%. As I was about to enter the curve, the vertical dirt hillside appeared ahead and directly in front of the forward-facing cameras and sensors, which I supose, detected this topography. With minimal pressure, I had my right foot on the throttle to keep the aforementioned speed while turning. The cruise control was not in use. As I began to turn, and while still facing the hillside, the throttle stopped responding and the car decelerated. Simultaneously, a warning appeared on my dashboard screen, indicating the forward collision warning system was unavailable. I fully depressed the throtle but instead of gaining speed, the car slowed down to 12 MPH as I completed the turn. In quick succession, I lifted my right foot of the throtle and depressed it all the way to the floorboard but the car did not accelerate. The car eventually regained forward acceleration when I had the throttle pedal all the way against the floorboard but despite this severe throttle application, the car only accelerated very slowly. Thereafter, the throtle resumed normal operation and I continued my trip over the hills and back. I believe I could have been rear ended during this scary incident.Read more

Despite the accelerator pedal being fully depressed, the vehicle would not accelerate beyond approximately 62 mph. Based on the symptoms and prior service history, the malfunction appears related to the fuel/propulsion system, engine, or powertrain. The vehicle is currently parked and available for inspection
... upon request. This failure placed the safety of the occupants and other motorists at risk. The vehicle was unable to safely accelerate, merge, pass, or respond to traffic conditions. Hazard lights were activated, and the vehicle was forced to remain in the slow lane of the highway for an extended period to avoid being struck by faster-moving traffic. The problem was reproducible and occurred repeatedly during the same trip. Acceleration was tested multiple times, and each attempt resulted in the same loss of power with no response to throttle input. The condition persisted for the entire drive. The vehicle has not yet been inspected following this incident. However, the vehicle previously received a manufacturer warranty repair involving replacement of the high-pressure fuel pump, and the manufacturer has issued a warranty extension for this component. The loss of motive power occurred after that repair. No warning lamps, alerts, or messages were observed prior to or during the failure. There were no warning symptoms immediately preceding the loss of motive power. This loss of propulsion at highway speed created a serious and immediate safety hazard due to the inability to maintain normal traffic speed or maneuver safely.Read more

... e recalled problem they dropped the cots to $2384. I believe that is an agreement that the manufacturer should cover this. Of course I will have to pay to get my car but I would like the manufacturer to reimburse me for it seeing it was a manufacturer default. I spoke to the manufacturer they flat out refused to cover it. I obviously disagree. Would like you to pressure them to change their decision.
