After fueling, codes P0496 will show and car will not start or struggle to start. Stalling when on freeway and rough idle. My vehicle shut off on the freeway causing me to almost crash and had to stop immediatly. In my investigation, it seems I am not the only one suffering this issue. I am on my 2nd engine replacement from the original recall as well.

There is a recall out for “2016 Audi A6 vehicles” the recalls are “fuel system rail” as well as “coolant pump”. Both recalls are vin specific and what I find to be devastating is that both recall risk pertains to possible fire! I have the exact issues with my vehicle I even have the epc light coming on and the code is “fuel rail system” I also have an ongoing issue with my coolant
... system. I have documentation that I’ve paid out of pocket to take care of these issues, however being that there is no current open recalls under my vin number I do not believe mechanics know exactly what to do to fix the issues. The vehicle is misfiring, shutting off and the EPC code is stating “fuel system rail” along with the coolant leaking. I have a [XXX] daughter and to be such an expensive vehicle, I thought I would feel safe and valued. I’ve worked as a service BDC manager for years and at other dealerships when a situation like this would occur, we still wanted them to bring the vehicle into the dealership and if it was in fact relatable to the recall we would take care of it, regardless to it being “vin specific”. There is a such thing as “silent recall”. I really hope that you all can take this as an emergency case and help with such terrible situation. Thank you so much. My 2016 Hyundai Genesis started flashing a code stating “Check PSB” only a week after buying the car. PSB means Pre-active Seat Belt. The seat belts somehow interlink with the fuel pump. My car has a faulty fuel pump module, causing the PSB warning to flash, even when the vehicle is in motion. It also causes a loud humming coming from the rear of my car, as I accelerate. I’ve done research,
... and it says MANY owners of the same vehicle report the faulty fuel pump module as the culprit, causing the Check PSB. As well as many owners online stating their vehicle has done the same as mine. There is active recalls on the newer years for this issue, such as the 2023. When the same issue is reported multiple times on the 2016s too, there should be a recall. This is a common issue, & quite frankly a dangerous one. If the fuel pump module is miscommunicating with the seat belts, that could cause them to not react in a collision. I have my vehicle scheduled to be serviced in a few days.Read more

Summary of Incident: On March 28, 2026, while traveling at 70 MPH on the Florida Turnpike, the vehicle suffered an instantaneous loss of motive power. The engine died abruptly with no warning. Upon reaching the shoulder, the electronic transmission locked, refusing to shift into Neutral or Park. The engine cranks but will not start. 1. Component Failure & Availability: The exact component is curr
... ently unknown, but the failure involves the Powertrain, Fuel System, and Electronic Transmission Control. The vehicle is currently at an authorized dealership and is available for inspection by the manufacturer or NHTSA upon request. 2. Safety Risk: The sudden loss of power at highway speeds created an immediate risk of a rear-end collision. Because the electronic transmission locked and would not allow the vehicle to be shifted into Neutral, it became a fixed object on a high-speed shoulder. It could not be pushed to a safer location, increasing the risk of a secondary accident. 3. Problem Reproduction: The vehicle was towed directly to an authorized Chevrolet dealership. As of this filing, the dealership is in possession of the vehicle, but the failure has not yet been reproduced or confirmed by a technician as they are awaiting formal diagnostic commencement. 4. Inspections: The vehicle was inspected by a roadside towing professional for transport. It is currently at the manufacturer’s authorized service center awaiting a formal diagnostic inspection. No police or insurance inspections have occurred. 5. Warning Symptoms: There were no warning lamps, "Check Engine" lights, or messages prior to the failure. No symptoms like rough idling or unusual noises occurred. The vehicle performed normally until the moment of total power loss.Read more

From this point forward the car drives in “low engine power mode” even with repairs completed and codes cleared. 2- 2 years later the same part “brake vacuum pump” exploded again. Another $1000 repair. At this time we discovered that the fine particles that were not removed from motor has now knocked the timing off by 8%. Also it has caused a knick in a seal causing an oil leak under 100k miles $1800 repair. After these extensive repairs…… The car emits gas fumes into cabin. The smell is so strong it can’t be parked in the garage because it circulates through the house. Even when car is not running. The Car can’t be driven because risk of fire or explosion from gas. Fuel system gets cleaned. Injectors replaced. The car temporarily runs without the gas smell. 2 weeks later the gas smell returns Come to find out the fuel pump failed. Search online and find there is a recall for fuel pumps in this same make model and year. But not this vin number. The car has been a headache from day 1. Incident Date: 12/28/2025 Location: [XXX] Related Recall: 23V905 / 23S64 On December 28, 2025, while driving at highway speeds on [XXX] , my 2020 Ford EcoSport suffered a sudden engine fire. There were no prior dashboard warnings or "Check Engine" lights until the fire began, the vehicle speed started to slow down itself, and I immediately pulled over on the side of the interstate. I turned of
... f the vehicle, and lifted the hood, since I saw smoke, and there was a fire under the hood. I called [XXX] , and the [XXX] /Cobb County Fire Department responded to the scene and inspected the vehicle under the hood. I have video evidence of the fire department on-scene and the emergency inspection of the burning engine. This vehicle is subject to Safety Recall 23V905 (Ford 23S64) for oil pump belt/tensioner failure leading to engine seizure. Crucially, a recall "repair" had been previously performed on this vehicle, yet the engine still suffered a catastrophic thermal event. This indicates either an ineffective remedy or a negligent installation. The vehicle was towed to Hennessy Ford [XXX] ). Their diagnosis confirmed: "COIL PACK BURN AND BURNED THE #2 INJECTOR AND HARNESS AND VALVE COVER." Despite the federal mandate that recall-related failures be handled at no cost, the dealership attempted to charge me over $2,000 for the diagnosis and repairs. Furthermore, Ford Customer Relationship Center (escalated 03/27/2026) has stalled the case by moving it to legal counsel with a 15-day waiting period. I am reporting this as a serious safety defect and a failure of a prior recall remedy. I am requesting a full investigation into the fire and a manufacturer buyback as the vehicle's safety integrity is now compromised. The component that has malfunctioned is the hybrid inverter assembly, a loose internal bolt within the hybrid propulsion system. This vehicle is currently held at Fred Haas Toyota Country (Houston, TX) and is available for inspection upon request. My safety and the safety of the public are at risk because, as stated by the recall notice given on December 16 2025, this defect involves a bolt in the
... hybrid powertrain that "can become loose and could lead to a loss of motive power or could lead to a fire when the vehicle is on." The safety risk was confirmed by Fred Haas Toyota on March 6, 2026 and while there is no formal "Stop Drive" order, the dealership agreed to ground the vehicle and lend me a rental. As of today, March 28, 2026 there is still no remedy despite Toyota saying they "estimate the remedy can be available in the 1st quarter of 2026." Since my vehicle is currently in possession of the dealership, I am not sure if they have inspected it. Before I gave my car to Fred Haas Toyota Country I did not see any warning lights or messages, but the risk was still there (NHTSA 25V869000). My vehicle has been out of service for 22 days with no estimate repair date or remedy announced and the manufacturer has failed to provide a comparable loaner and has been unresponsive to my calls and messages. I am reporting a failure to remedy a known safety defect in a reasonable timeframe and am requesting an investigation into this delay and assistance with an appropriate rental or vehicle repurchase.Read more

Incident Date: February 10, 2026 Mileage: 1,260 On the morning of February 10, 2026, my 2026 Chevrolet Equinox EV (1,260 miles) would not start. The instrument cluster simultaneously displayed three warning messages: 1)“Service Battery Fault — Vehicle Won’t Restart,” 2)“Charging is unavailable,” and 3)“Charge Vehicle Soon.” The vehicle could not be placed in neutral and required do
... lly tow from my garage, as the front wheels were locked and could not roll freely. AAA and I measured the 12V battery at 11.8 volts. AAA technician made attempt to jump 12 volt battery with no success. He made the observation that it was very rare for an EV not to start once the 12 volt battery was jumped. The vehicle was towed to Folsom Chevrolet (Repair Order #558923, opened February 10, 2026). The technician performed a global reset, BECM reprogramming, standalone radio programming, and a manual reset procedure. The repair order was closed with “confirmed no further concerns,” however the root cause — identified in the repair notes as a possible HV module communication fault — was never confirmed diagnosed or repaired. Seven days later (February 17, 2026, RO #559167), the vehicle returned to the dealer with overnight 12V voltage loss, starting at 12.12V and continuing to drop. A parasitic draw test found an 11mA draw. After a second overnight test the dealer closed the repair order as “no concerns found” without identifying or repairing a root cause. Safety concern: A vehicle that cannot be started, cannot be charged, and cannot be placed in neutral poses a serious safety risk — particularly if such a failure occurs on a roadway rather than in a driveway. The inability to shift to neutral prevents the driver from clearing the vehicle from traffic. The root cause of this dual electrical failure has never been confirmed repaired despite two repair attempts.Read more

The 62kWh High-Voltage Lithium-Ion Battery Pack. The vehicle is currently in my possession in Santa Fe, NM, and is available for inspection. It exhibits physical degradation (40.07% Hx) directly linked to the cell defects described in Recall 25V655. The vehicle is a documented fire risk. Per Recall 25V655, internal lithium deposits create resistance that leads to thermal incidents. Because my bat
... tery already shows terminal degradation (40.07% Hx and 3.18V sag), the risk of a fire during charging or operation is imminent. Furthermore, Nissan has banned Level 3 Fast Charging, which leaves me with no way to safely recharge the vehicle during necessary travel, creating a risk of being stranded in unsafe conditions. A terminal battery failure was officially diagnosed by an authorized Nissan dealer in Albuquerque in August 2025 at 108,000 miles. However, my local dealer (Fiesta Nissan in Santa Fe) has since refused to intake the vehicle for the recall because they are not EV-certified. Yes, by an authorized Nissan Dealer. As noted, the failure was confirmed in August 2025. Additionally, the vehicle is subject to a formal manufacturer safety recall (25V655 / R25C8) specifically for this battery defect. Symptoms appeared in August 2025 including severe range loss and power turtle mode. LeafSpy Pro diagnostics confirm a terminal 40.07% Hx rating and a dangerous 3.18V cell sag under load. Nissan's March 2026 recall notice confirms there is no current remedy to address these physical symptoms, and the proposed software 'fix' only monitors for failure rather than repairing the defective cells. Car immediately shut down on me on the Highway(around 55mph) and I was able to coast out of traffic and onto the shoulder. Car indicated having battery percentage around 58%(30KWH) still remaining. car also would not shift into neutral because battery was too low so towing was difficult. no neutral overide on car if battery is "dead".Read more

The internal core of the Exhaust Heat Recovery System (EHRS), part of the center muffler/front pipe assembly (#28600-L5210), suffered a catastrophic failure. This allowed approximately 9 gallons of engine coolant to leak directly into the hot exhaust stream. Safety Risk: This failure creates a severe visibility hazard. During acceleration from stop lights and stop signs, the vehicle emits massive,
... thick clouds of white glycol smoke. The smoke is so dense that it completely obscures the driver's view of trailing vehicles and prevents following drivers from seeing the roadway or my vehicle's brake lights, creating a high risk of rear-end collisions. Additionally, the rapid loss of coolant caused the engine to overheat and lose power while in motion, nearly causing a stall in active traffic. Confirmation: The failure was confirmed by the service department at Washington Hyundai (Invoice #174281). This is the second failure of this specific component (previously failed in Jan 2025). The manufacturer issued TSB 23-EM-009H for this defect, acknowledging it affects 2020-2023 Sonata Hybrids, but no safety recall has been issued. Symptoms: The issue was preceded by a "sweet" burning smell and a sudden "Engine Overheating" warning on the dashboard. The failed component has been replaced by the dealer, but the manufacturer refused to replace the coolant-poisoned Oxygen (O2) sensors, leaving the vehicle in a technically compromised state. The failed unit is likely held by the dealer for warranty parts return.Read more

The engine stop/start system on my 2016 Honda Accord, VIN [XXX], has stalled and shut off without warning in active Los Angeles traffic dozens of times since September 2025. The vehicle loses all power mid-drive with no warning — no dashboard lights, no warning sounds — leaving me unable to accelerate or respond to surrounding traffic. The condition has progressively worsened and now includes

My safety and the safety of others hav been put at risk repeatedly. I am a [XXX]. My wife, age [XXX], has [XXX] and has been a passenger during stalling events. Sudden unexpected stalling in active Los Angeles traffic creates immediate foreseeable risk of rear-end collision and loss of vehicle control. This risk has been experienced dozens of times over six and a half months. The problem has been presented to Honda of Hollywood, a certified Honda dealer at 6511 Santa Monica Blvd, Los Angeles CA 90038, across five separate visits. Each time the dealer concluded no problem could be found. On one visit I was charged nearly $2,000 for spark plugs and a gasket — work with zero diagnostic connection to the stalling complaint. The vehicle was returned to me in an unsafe condition after every visit. On [XXX] the tow truck driver who responded to my third documented tow independently experienced the failure, could not start the vehicle, witnessed the complete failure sequence, and declared the vehicle unsafe to drive. He is an independent third-party witness with no stake in this case. I have video documentation of the vehicle stalling in active traffic on multiple occasions. Our 2024 Bronco Sport Outer Banks, 1.5 liter engine, purchased new from a Ford dealer 11/2024, serviced at the dealership and driven less than 14k miles, was destroyed by a spontaneous fire 3/22/2026. After driving 12 miles, we parked in a parking lot. Upon exiting the car we noticed a burning smell and saw small plumes of steam or smoke coming from under the car hood. Within a minute a red/orang
... e disc of heated metal became visible on the hood surface as flames came from under the hood. Seven minutes later the fire department arrived extinguishing the fire. The car was destroyed. There were no warnings of any kind. The events suggest a cracked fuel injector. Even though 15 foot flames engulfed the car it was parked and unoccupied, there were no injuries. The results of the investigation have not been reported to us. CCTV footage was obtained from the police.Read more

The high pressure fuel pump “grenaded” while driving on the interstate. Caused an immediate shutdown and barely got off the main lane to the shoulder in extremely heavy traffic putting me, my wife and others in imminent danger. The components were inspected by the dealer mechanic and insurance adjuster and they said that is what happened and that there was no reason for it to have happened. A

I am reporting a recurring safety issue involving sudden loss of power while driving in my 2023 Ford Escape. The vehicle has experienced multiple incidents where, while accelerating or pulling into traffic, it suddenly loses power, hesitates, or fails to respond to the accelerator. In some cases, the RPM drops unexpectedly, and the vehicle will not move or respond for several seconds. After this
... delay, the vehicle may jerk or surge forward suddenly. This creates a dangerous situation, especially when entering busy roads or intersections, as it puts me at risk of being struck by oncoming traffic. This issue has occurred multiple times and has been brought to the dealership on several occasions. The vehicle has undergone multiple repair attempts, including major engine-related repairs such as cylinder head and cooling system work, as well as a fuel system component replacement. Despite these efforts, the problem persists. On multiple visits, the dealership has stated they were unable to duplicate the issue, even though the condition continues to occur during normal driving. The vehicle has been out of service for an extended number of days due to these ongoing issues. This defect is intermittent but poses a serious safety hazard due to the unpredictable loss of power and delayed acceleration. I am concerned about the risk of an accident or injury if this issue is not properly identified and corrected. I am requesting that this issue be investigated, as it appears to be a recurring defect that may affect other vehicles of the same make and modelRead more

I own a 2020 Kia Niro PHEV. In October 2024, my dealership performed Technical Service Bulletin SA512 on my vehicle addressing the inverter coolant system. I was not informed at that time that the issue was likely to recur or that periodic maintenance would be required. In March 2026, the inverter coolant warning light returned. I brought the vehicle back to the dealership, where I was told the co
... olant had crystallized. The dealer confirmed this is a known recurring issue with this vehicle and stated it would need to be flushed approximately every two years since we drive relatively low miles per year (despite driving every day). I was not warned of this at any point. Furthermore, many owners likely drive it short distances every day since it is a plug-in electric vehicle. Kia denied goodwill coverage and I paid out of pocket for the repair. When I contacted Kia Customer Care, representatives gave conflicting information. One confirmed SA512 was performed as a service campaign, another stated it was only a TSB and declined coverage since my car is out of warranty (even though the warranty should still cover the battery and this is an electrical issue). A total case specialist I was assigned to said she would talk to the dealer and call me back but never did. The inverter cooling system is safety-relevant as it prevents the battery from overheating. The problem was confirmed and repaired by the dealership. The component is no longer available for inspection as the repair has been completed. Kia's issuance of SA512 suggests this is a known design issue affecting multiple owners of this model, and they informed the dealership that they could not cover the cost because if they did so, they would have to cover everyone with the same problem (again implying that the issue is quite widespread.)Read more

While charging my 2023 Porsche Taycan 4S at an Electrify America station, white smoke began emitting from the front wheel/charging port area. The vehicle also produced a burning odor inside the cabin. I immediately stopped charging and unplugged the vehicle. The smoke ceased within approximately 1–2 minutes. The vehicle was towed to an authorized Porsche dealership. Initial explanation from th
... e dealer was steam from when radiators heats-up, which is inconsistent with the conditions and prior use history. Upon further inspection, the dealership identified battery-related issues. Ultimately, I was told that 8 out of 10 battery cells were replaced. The vehicle was out of service for approximately one month. The dealership stated that the battery cell issue was not related to the smoke event, which is concerning given the timing and severity of the incident. This vehicle had previously been subject to a battery-related recall addressed via software update only. I believe there is also ongoing litigation related to battery safety in this model. Given the smoke event, burning odor, and significant battery component replacement, I am concerned about the safety of this vehicle, particularly risk of thermal event or fire during charging. Note, I have a video of the smoke when car was charging, not able to upload video hereRead more

I am reporting a Safety Remedy Denial by Audi of America (Case #[XXX]) regarding a failed suction jet pump and charcoal canister in my 2015 Audi S3. This failure resulted in a fuel leak and strong fuel odor near the exhaust, creating an immediate fire hazard. Although Audi issued a 15-year/150,000-mile warranty extension ([XXX]) for the 8V chassis A3 population, they are denying coverage for m
... y S3 variant. This is in direct contradiction to Audi's own TSB 2068171/5, which explicitly states the A3 model group 'includes S and RS versions.' The dealership (Audi Rancho Mirage, RO# [XXX]) confirmed the failure of the suction jet pump (Version 1.0/1.1) but refused the safety repair at no cost because the 'U49 Warranty Key' was not in their digital system. This component is part of NHTSA Recall 24V-110 and Investigation RQ23007. Audi is failing to honor federal safety protections for the S3 variant of the 8V chassis. My vehicle is currently under a recall (recall SC356), but I'm currently very far from a Kia dealership. The nearest one is about 175 miles away. I contacted Kia and they informed me that they can't reimburse me for having my car towed to the dealership. So now I am stuck with a car that could possibly catch fire at anytime, according to the recall notice. And they refuse to help me.

On 1/26/2024 approximately 9am, my husband had been driving on the highway for about 2 hours and pulled over to shut his eyes for a few minutes. He shut off the engine, closed his eyes and immediately smelled smoke. Black smoke began coming from under the hood and quickly got worse. He quickly got out of the car and walked away. He heard a loud pop sound and the engine area was engulfed in flames
... . He called 911. It took about 10 min for the fire dept to arrive and by then, the fire was being fed by the gas in the tank. They tried to put out the fire but the car burnt completely due to the gasoline. No one was hurt, thank goodness. There were no active lawsuits that I could find to get some compensation for this loss. We had to pay our high deductible and lost personal belongings in the car. This car did have an ongoing oil consumption issue. We had to replace the oil more frequently than normally required and it used much more oil than normal. We were told we had to wait for engine failure to get any kind of resolution.Read more

On January 19, 2026, the vehicle lost power while driving and required a tow to the local dealership. After inspection, the dealership determined that the ICCU was at fault and needed to be replaced. The ICCU part was backordered. The vehicle was inoperable for 6 weeks as we waited for the part to be available and installed.

I am filing this complaint because my Volkswagen Tiguan has a serious and ongoing engine issue involving excessive oil consumption and a faulty cylinder head. The vehicle burns through an abnormal amount of oil between changes, requiring constant top‑offs, sometimes as frequently as every few hundred miles. Despite regularly adding oil and following all manufacturer‑recommended maintenance, th
... e issue continues to worsen. The dealership has confirmed that the cylinder head is defective, which is causing the engine to consume oil at an extremely accelerated rate. However, I have been told the replacement cylinder head is on national backorder, and there is currently no estimated timeframe for when the part will be available. This means I am forced to continue driving a vehicle with a known, documented engine defect that can: Lead to engine stalling Create loss of power while driving Increase the risk of engine seizure Potentially cause a safety hazard on the road Additionally, being required to constantly add oil just to keep the vehicle operable is not only unsafe but financially unreasonable. The problem remains unresolved despite multiple service visits, documentation, and repeated oil consumption tests. This is a safety issue, not just a maintenance concern, because the engine can fail unexpectedly if the oil drops too quickly. I believe this is a significant defect affecting multiple Volkswagen vehicles and should be investigated by NHTSA. I request that NHTSA open a formal investigation into the excessive oil consumption and cylinder head defects on Volkswagen Tiguans, as owners should not be left with unsafe vehicles due to known manufacturing issues and unavailable critical engine components.Read more
