My 2018 Hyundai Santa Fe Sport equipped with a Theta II GDI engine began exhibiting excessive oil consumption, engine knocking, and cylinder misfires consistent with the known Theta II engine defect covered under the Hyundai class action settlement. Prior to the failure, the check engine light illuminated and I experienced repeated misfires and audible engine knocking. The vehicle required frequen
... t oil additions between oil changes due to excessive consumption. The vehicle was taken to an authorized Hyundai dealership where a diagnosis confirmed a burnt valve inside the cylinder head. The dealership recommends replacement of the cylinder head or complete engine replacement to correct the issue. The dealership performed a bearing failure test and attributed the burnt valve to carbon buildup. However, excessive oil consumption in a GDI engine directly accelerates carbon buildup on intake valves, as burning oil deposits carbon on valve surfaces preventing proper seating and causing overheating and valve failure. The carbon buildup is therefore a consequential result of the Theta II oil consumption defect, not an independent cause. The safety risk is significant as engine misfires and knocking while driving create unpredictable loss of power and potential for stalling in traffic. The vehicle is currently not reliably operable. The manufacturer has been contacted regarding coverage under the Theta II settlement. The component is available for inspection upon request at the servicing dealershipRead more

Incident Date: 04/05/2026 Vehicle Speed: 65–70 MPH Location: [XXX] Component: Engine / Power Train / Fuel System Summary of Safety Defect: While traveling at highway speeds (65–70 MPH) with four passengers (including two children), the vehicle suffered a sudden, catastrophic engine failure without any prior warning or symptomatic noise. The Malfunction Indicator Light (MIL) began flashing r
... apidly, immediately followed by a total loss of motive power. Despite throttle input, the vehicle failed to maintain speed or accelerate, creating an immediate hazard in high-speed traffic. Hazardous Conditions encountered: Upon the loss of power, I observed white smoke trailing from the exhaust and a strong burning odor entering the cabin from the engine compartment. To avoid a high-speed rear-end collision, I was forced to perform an emergency maneuver across active lanes of traffic to reach the shoulder. Due to the immediate fear of a vehicle fire (indicated by the smoke and burning smell), my family was forced to evacuate the vehicle and wait outside in a remote location for six hours. Correlation to Known Defects: This incident appears to be a direct manifestation of the defect described in Nissan Recall ID R25E3/R25E4 / NHTSA ID 26V0801 regarding Engine Main Bearing Seizure. The failure resulted in a complete loss of propulsion and a potential fire risk due to fluid ignition or a breached engine block. Post-Incident Status: The vehicle was towed to Nissan of Cumming and is currently awaiting a formal inspection for internal metal debris. The failure of this critical component under highway conditions posed a life-threatening risk to my family and other motorists. On 11/7/25 , we were driving my 2023 Hyundai Ioniq 5 to a friend's home in rural [XXX] , a trip of approximately 160 miles. At around 5:00 PM, we stopped about halfway to our destination to charge the battery. The car would not charge above 47%. While we continued to attempt to charge the battery, the console displayed a Check Electrical System Warning message, and we received the DTC P1AA700 via
... BlueLink. We were concerned about our safety on the drive up to the mountains. We were afraid the car would stop running on the highway and cause an accident. We were also concerned the car would die in a remote area and we would be stranded. However, the dealership was closed for the weekend. We drove carefully to our friend's home. I contacted Stevens Creek Hyundai on 11/10/25 and arranged to bring the car in on 11/11/25. It had 13,652 miles on it at that time. Eventually, the dealer confirmed the DTC P1AA700 and said they would need to order a new EGMP battery. The car was in the shop for 128 days, waiting for Hyundai to "build a battery" in [XXX] . Eventually, Stevens Creek Hyundai replaced the EV battery and I picked up the car. I own a 2021 Nissan Leaf subject to open recall R25C8 (NHTSA Campaign 25V655) for fire risk during Level 3 DC fast charging. The recall was issued October 2025. Nissan’s stated remedy target was late March 2026. As of today, April 6, 2026, no remedy has been provided. The affected component is the lithium-ion battery pack and its charging system. The vehicle is equipped with a CHAdeMO DC fast ch
... arging port, which I cannot safely use due to the documented fire risk. I am not avoiding this feature by choice – I am avoiding it because Nissan’s own recall identified it as a safety hazard. On April 6, 2026, I encountered a DC fast charging station and chose not to attempt charging due to the active recall warning, and due to reports that charging networks have begun refusing sessions on recalled vehicles. Diagnostic software shows my battery’s internal resistance index (Hx) at 71%, disproportionately low relative to State of Health at 87.5%. The vehicle is also consuming battery charge at an accelerated rate during highway driving, with approximately 50% of charge depleted over 30 to 40 miles. I am concerned that a software-only remedy will be inadequate for a battery with this level of internal resistance degradation, as software cannot compensate for the absence of active thermal management hardware. No dealer has confirmed the condition. I am filing to document loss of vehicle functionality, Nissan’s missed remedy deadline, and concern about remedy adequacy.Read more

I am submitting this complaint regarding a serious safety‑related failure involving the Totally Integrated Power Module (TIPM) in my 2017 Dodge Durango. Although my vehicle is not included in the 2011–2013 recall window for Safety Notice R0 / NHTSA 14V‑115, it experienced the same dangerous malfunction described in that recall. The TIPM failed without warning and caused a catastrophic failu
... re of the fuel pump. As a result, the vehicle would crank but would not start, leaving my wife stranded and requiring a tow to an automotive repair facility. The repair shop confirmed that the TIPM malfunction caused the fuel pump to fail internally and come apart inside the fuel tank. The TIPM installed in the vehicle is part number P68251629AC A, vendor code 16238DA, with an additional number of 16317G. The TIPM was not inspected anyone other than the repair facility and there were no warning lights or codes prior the failure. The quoted cost for parts and labor to correct this safety‑related defect is $2,100. FCA has refused to cover the repair because my vehicle falls outside the recall years, despite the failure matching the symptoms and safety risks identified in NHTSA Campaign 14V‑115. Given the severity of the failure, the safety risk posed to my family, and the clear similarity to the known TIPM defect, I am requesting NHTSA’s assistance in reviewing this incident, determining whether the recall should be expanded to include additional model years, and supporting my request for FCA to cover the full cost of repairs or reimburse me for the necessary corrective work. This failure created a hazardous situation and resulted in significant financial burden for a defect consistent with an already‑recognized safety issue. I respectfully request that NHTSA investigate this matter and take appropriate action.Read more

On or about Feb10 2026 near [XXX] heading North I experienced an commanded accelerator input. ... to the infotainment system and the infotainment system resets. However this time the throttle advanced to almost full throttle without any input on the accelerator as my foot was on the brake in order to slow. I had to use full application of the brake in order to keep from hitting the car in front of me. Just as I started to shift the transmission to neutral the accelerator went back to idle. The event lasted about 5 seconds. My hypothesis is that some sort of radio interference is causing the system to reset and in this case it caused communication issues with the can bus or engine control module. Our 2018 Honda Odyssey repeatedly and without warning completely loses acceleration power, creating extremely dangerous driving conditions. This occurs in two distinct situations:(1)while driving at highway speeds when attempting to change lanes or accelerate, and(2)when stopped at a stop sign or intersection and pressing the gas to merge into or turn into traffic. In both situations, pressing the
... accelerator results in little to no response — the vehicle will roll but fails to accelerate as needed. These failures have placed our family in multiple life-threatening situations. On Interstate 285 in Atlanta, Georgia — one of the busiest and highest-speed highways in the country — our vehicle has failed to accelerate during lane changes on multiple occasions, nearly causing serious accidents. Most recently, within the past week, my husband was driving on I-285 when he pressed the accelerator and the vehicle had absolutely zero acceleration response. He narrowly avoided a collision. We have also experienced complete loss of acceleration when turning from a stop sign into traffic and when merging — situations where immediate acceleration is critical to safety. The vehicle lags for several seconds before any power is delivered to the wheels, leaving us dangerously exposed in oncoming traffic. All manufacturer recalls have been completed, including the fuel pump recall. The problem persists and has not been resolved by any dealer service or recall repair performed to date. We are aware that this is a known, widespread defect affecting 2018 Honda Odysseys equipped with the ZF 9-speed automatic transmission. Hundreds of other owners have reported identical symptoms to NHTSA and to Honda directly. Honda has acknowledged the transmission's behavior to some owners but has stated it is "how the car is supposed to drive" — a response that is wholly unacceptable given the documented near-accidents this defect is causing. We are requesting that NHTSA open or expand its investigationRead more

Hello, I am reporting a safety issue involving my Ford F250 vehicle related to Safety Recall 25S75 (low-pressure fuel pump failure). Approximately one month ago, while driving on the freeway at approximately 65 mph, my vehicle experienced a sudden loss of power. This created a dangerous situation as I was traveling at highway speed and the vehicle became unable to maintain normal operation. The
... vehicle is now inoperable. The failure is consistent with the condition described in Recall 25S75. This is not a preventative concern — the defect has already occurred and has rendered the vehicle undrivable. I contacted Ford Customer Relations and explained that the failure matches the recall condition. However, my case was closed without resolution, without any determination of responsibility, and without conducting a repair or transportation support. Ford has indicated that the official recall remedy is not yet available. Despite this, I have been told I may be responsible for out-of-pocket repair costs, even though the failure is consistent with an active safety recall. This situation presents both a safety concern and an unreasonable financial burden, as the vehicle cannot be safely operated and no interim remedy or support has been provided. I am requesting that NHTSA review this matter, as the manufacturer is not addressing a recall-related failure that has already occurred and resulted in a loss of power at highway speed. If you need any additional details, please do not hesitate to reach out. Thanks, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The electric A/C compressor on my 2022 BMW X5 xDrive45e (VIN [XXX] ) failed internally at approximately 59,000 miles, causing metal debris to circulate throughout the refrigerant system. The failed compressor and contaminated components are at BMW of Sterling (Sterling, VA) and available for inspection. This is a safety concern because the A/C refrigerant circuit provides thermal management for th
... e high-voltage hybrid battery. BMW of Sterling's service advisor confirmed in writing that the A/C system is "a critical component of your vehicle's HV system," that there is no independent backup cooling system, and that the failure "can cause potential high voltage battery failures." Without active cooling, the HV battery is at risk of overheating, which could result in a thermal event. The failure was diagnosed and confirmed by BMW of Sterling. They categorized the repair as "EHV: FULLY ELECTRIC OR HYBRID VEHICLE DRIVETRAIN DIAGNOSTICS" with the complaint "HIGH VOLTAGE SYSTEM ERROR." The repair estimate is $11,356.59 and requires replacement of the compressor, condenser, all expansion valves, and a double system flush. BMW Service Information Bulletin SI B64 09 21 documents this exact failure pattern for the G05 X5 xDrive45e, and BMW created a dedicated repair kit (part 64 50 5 A5B 590) for it. This is my second major HV component failure. A battery cell replacement was performed under warranty in 2025. Both failures were confirmed by the dealership to be component defects unrelated to owner use. Prior to complete failure, a fan noise was present in the front driver's area for a period of time before it stopped, consistent with the compressor degrading before failing entirely. A high voltage system error was displayed. BMW NA (case #XXX) has declined warranty coverage despite the vehicle being within the 8-year/80,000-mile HV battery warranty. On June 13, 2025, the fuel pump was replaced due to a recall. After the fuel pump was replaced and, in particular, after reading the Honda forums for this car, all the warning lights in the instrument cluster came on. After my mechanic checked my car and, with his knowledge of this type of vehicle, he recommended replacing the fuel injectors at 86,000 miles. Honda recommends replacing the fuel inj

Component: Engine/Powertrain/Electrical System Incident Description: I am reporting a recurring, life-threatening "Loss of Motive Power" (Dead Pedal) in a 2024 Hyundai Tucson (Alabama-built). On multiple occasions, the vehicle has failed to respond to throttle input during high-risk maneuvers, specifically while merging into 45+ MPH traffic. The vehicle "chugs" and stalls, refusing to exceed 15 MP
... H despite full accelerator depression. This defect leaves the vehicle and its occupants stranded and defenseless in the path of oncoming high-speed traffic. Technical Context: This vehicle is currently subject to NHTSA Recall 23V-526 (Hyundai Campaign 246) for electronic controller damage that can cause "CAN communication disruptions." I believe the "Dead Pedal" failure is a direct result of these communication disruptions between the engine and transmission control modules. Furthermore, Hyundai has issued TSB #24-AT-001H-1 for "Hesitation when accelerating," yet the authorized dealer (Ed Voyles Hyundai) has been unable to remedy the defect after 84 cumulative days out of service. Safety Risk: This is not a convenience issue; it is a critical safety failure. The unpredictable nature of the power loss makes it impossible to safely judge gaps in traffic. The vehicle effectively becomes a stationary object on active roadways without warning. Manufacturer Response: The manufacturer has been formally notified via certified legal counsel. Despite 79 consecutive days in the current repair attempt (since Jan 16, 2026), the manufacturer has failed to provide a fix or acknowledge the severity of the safety risk. An informal third-party evaluator attempted to dismiss the claim despite the vehicle's documented inability to maintain motive power. We are requesting that NHTSA investigate the adequacy of Recall 23V-526 and TSB #24-AT-001H-1, as these measures have failed to resolve this life-threatening "Dead Pedal" condition in my vehicle.Read more
