Fuel pump leak
Incident date: May 15, 2022
Hello, I would like to file a safety complaint regarding my Ford Escape. Vehicle Information: VIN: [XXX] Make/Model: Ford Escape Year: 2018 Issue Description: While driving, my vehicle suddenly overheated. The temperature gauge moved from normal to hot within seconds, and a warning appeared to stop the vehicle immediately. This created a serious safety concern, as the failure was sudden and wi
... thout warning. After inspection by a certified technician, I was informed that the engine is consuming coolant internally, and the coolant is being burned through the exhaust system. There is no visible external leak. The engine is now running rough and requires full engine replacement. I have learned that this is a known issue with Ford Escape engines, often referred to as “coolant intrusion.” Despite this being a widely reported defect, Ford has declined to provide any financial assistance. I have contacted Ford and opened a case (Case Number: XXX), but they confirmed this is not a recall and will not cover repairs. This issue creates a significant safety risk due to sudden engine overheating and potential engine failure while driving. I am requesting that this issue be investigated, as it appears to be a widespread defect affecting many Ford Escape vehicles. Thank you for your attention to this matter. Upper fuel pump module, on top of fuel tank, is leaking fuel from a crack in fuel pump port where fuel hose connects. Fuel can be seen on top of upper fuel pump module and can be seen leaking from fuel pump port, and can be seen on exterior of gas tank. Also fuel odors were present. This could be a fire hazard. I am writing about my concerns on a 2016 Hyundai Sonata SE that I purchased for my daughter in October 2023. After purchasing I took it to my local Hyundai dealer to have it checked out. It was one of the cars that came with a lifetime warranty for the rods DO TO A DEFECT.On January 2024 my daughter was driving on the freeway when all of a sudden it just stopped accelerating and when checked it wa
... s a Knock sensor detection system. The dealer took it in and inspected and said it was a defective sensor and replaced it.This made me uncomfortable as I felt the sensor did its job and it was just a matter of time before the rod blew through.During the next few months it became very apparent that the car was going through unbelievable amounts of oil.It had a ticking so we replaced all the spark plugs and noticed oil saturating the plugs and realized a leak in the valve cover gasket which was also replaced. We thought this might be the cause but no oil was on the ground and nowhere on the engine.I just thought it was burning oil but no indicator lights were going off. The car could have no oil and it would not let us know. She's lucky to drive 300 miles a month. She has complained of a strong gas oder and we replaced 2 gas caps and went back to the Hyundai dealer. They told us there's a problem with the fuel filter and pump.If we pay $273 of inspections it might cover it if it's a failing part, and when I did the oil change the car was very low on oil despite the fact that we check it monthly and put 3 quarts of oil in the car.Her car is consuming 3-4 quarts of oil per every 250 miles. This could cause her engine to fail and possibly throw a rod which given the fact she has already taken out one Knock sensor it is just waiting to happen. So as oil disappears every few hundred miles without a warning the engine locks up and if a rod punches through onto the exhaust this can cause it to stall and potentially catch on fire especially when it wreaks of gas from the fuel filter.Read more

3 separate safety-related components have failed on my 2021 Ford Bronco, all of which correspond to open federal safety recalls that remain unremedied: 1. No remedy has been provided. 2. Digital Instrument Cluster / Dashboard: my digital dashboard intermittently fails to illuminate at vehicle startup. This occurs randomly and without warning. When it fails, the driver cannot see critical safety information including current speed, fuel level, engine temperature, or any warning indicators. Video documentation of this failure exists. This component is available for inspection upon request. 3. Low Pressure Fuel Pump (Recall Campaign 25V455): my vehicle has stalled on multiple occasions while in motion. During stall events, all dashboard warning lights illuminate simultaneously. This is consistent with the fuel delivery failure described in Recall 25V455, issued July 8, 2025, which states the fuel pump can fail causing loss of fuel pressure and an engine stall while driving. No remedy has been provided since the recall was issued in July 2025 (~9 months ago). All components are available for inspection upon request. While driving this car on April 4, 2026 at around 6PM, I experienced the distinct smell of gasoline. I had driven for about 5 minutes on our way to a local restaurant that is approximately an 8 minute drive from our residence. My wife said that she had just filled the gas tank that afternoon. After arriving at the restaurant, I checked the gas cap to ensure it was tightly sealed. I also checke

After we arrived home, I check the vehicle exterior for leaks and found no liquids under the car. The following day, April 5, 2026 at approximately 1115 AM, having once again found no liquids or gasoline under my car, I drove the car to my local repair shop; an 8 minute drive. About 5 minutes into the drive, I smelled gasoline again. I parked the car at the repair shop, but while adjusting the parking to fit better between the painted lines, I saw and heard a large puff of smoke from the hood area. I immediately put the car into Park, shut off the engine and ran from the car. After exiting, I saw that the car ignited with visible fire under the engine compartment. I called 911 to request fire department help. No damage to any structures or other cars occurred. Q1 Above: No the car is totaled and unavailable for inspection as far as I know. Incident Detail: In April 2026, my 2022 Toyota RAV4 Prime (VIN: [XXX] ) suffered a critical failure where the charging cable became physically locked in the vehicle’s high-voltage (HV) charging port. I transported the vehicle to Ken Garff Cheyenne (authorized Toyota dealer), where management allowed non-certified, non-technical staff to perform unauthorized physical interventions on the HV port.
... No manufacturer-certified technician was provided for a safe diagnostic. Safety Hazard and Immediate Consequence: Following these unauthorized interventions, my wife (an educated automotive technician) determined the vehicle was in a hazardous state, risking thermal runaway or electrical fire. This hazard was so severe that the owner of the property where I was staying in Cheyenne—a [XXX] dependent senior—was too frightened to allow the vehicle to remain on the premises. Due to this immediate safety risk and the dealership's refusal to coordinate a safe tow, I had to have the vehicle towed to a neutral facility to protect the life and property of a vulnerable widow. Legal and Technical Context: I am a permanent resident of San Diego, California, and this vehicle is covered under the California Song-Beverly Act. The vehicle is also subject to Federal Recall 23V478 (DC-DC Converter Fire Risk). The dealership’s unauthorized tampering with the HV system, combined with ambient temperatures in Cheyenne often falling below the 41°F safety threshold for this system, created an unmitigated fire risk that the manufacturer has failed to address. Request: I am seeking a federal investigation into the dealer’s failure to follow High-Voltage safety protocols. I demand that Toyota Motor North America provide a Field Technical Specialist (FTS) to perform a safe diagnostic at the neutral facility where the car is now located, as the dealership’s actions rendered the vehicle a public safety hazard. Subject: Formal Complaint – Unresolved Safety Issues Following Recall Service (2018 Honda Accord) I am submitting a formal complaint regarding ongoing and unresolved safety issues with my 2018 Honda Accord, which began during the time frame of a fuel pump recall and have not been properly resolved despite multiple attempts with Honda service departments. Approximately two years ago, my vehicle
... was subject to a fuel pump recall. During the extended wait period for the replacement part, my vehicle began displaying multiple warning messages and experiencing performance issues. These issues included reduced acceleration and erratic driving behavior. Once the recall repair was completed, I was informed that an additional repair—a battery sensor replacement—was necessary, which cost approximately $800 out of pocket. Despite completing both the recall repair and the recommended sensor replacement, my vehicle has not operated properly since that time. Currently, the vehicle intermittently displays multiple system warnings, including but not limited to: - Emission system problem - Brake system and brake hold system problems - Electric power steering problem (reduced steering assist) - Vehicle stability and traction control warnings - Adaptive cruise control system problem - Collision mitigation braking system problem - Road departure mitigation system problem In addition, the vehicle frequently enters a reduced power or “limp mode,” making it difficult to accelerate safely, particularly at lower speeds. Restarting the vehicle temporarily resolves the issue, but the warnings and performance problems return shortly after. I have brought this vehicle to Honda service departments on multiple occasions. The vehicle was held for extended periods without resolution, and I ultimately had to retrieve it without a clear diagnosis or repair plan. I also contacted American Honda for assistance and did not receive meaningful support.Read more

I was driving my vehicle on a freeway with 3 passengers when i heard a muffled pop or thud from behind me. A rear passenger confirmed that it came from beneath the rear seat towards the left side of the car. The vehicle speed immediately dropped to less than 40 mph and a warning message appeared on the dash "Check electric vehicle system". I later noticed a red battery icon appear on the left
... of the dash. I pulled over after leaving the freeway and attempted to resolve the issue by turning off the car, then restarting it. The notice and red battery icon still appeared. I feel that this sudden reduction in speed is a safety issue because I could have been driving at a higher speed (65-70 vs 45-50). Following cars may not have been able to stop in time, or I cuold have been forced to attempt to get off the road by entering lanes with vehicles travelling at a much faster speed. I was about 5 miles away from home on the start of a 400 mile trip so decided to turn around. Back at home I discovered that a new DTC code had been thrown during or after this event - P1A9096. I transferred to another person's cart to complete the trip, and on return arranged for the car to be transported to a manufacturer recommended dealership to check the issue. The Dealership service department conducted diagnostic tests and determined that the issue was caused by an ICCU failure. The have placed an order for a replacement ICCU unit and fuse. My 2017 Land Rover, Range Rover has rusted fuel tank shields. I have owned this vehicle for seven of its nine years and has been babied. When I brought my Range Rover in for a NTSB MANDATED FACTORY RECALL THEY neglected even noting this issue. Which is a very common issue, I brought my rover into Hennessy Land Rover north atlanta on February 10th 2026. My service advisor was [XXX] . N
... ot only did Hennessy north atlanta REFUSE TO REPAIR THE FEDERALLY MANDATED “SEVERE SAFETY” recall regarding my front suspension (may crack leading to a severe accident) as instructed by the NTSB— BECAUSE “THEY DIDNT HAVE THE PARTS FOR THE RECALL” — BUT IF YOU PAY THE $7000 they charge TO FIX THIS ISSUE , HAVING IT DONE IS NO PROBLEM. THE PARTS MAGICALLY APPEAR 😂 for $7000. As stated, Hennessy did not even address the fuel tank cover issue. THEY DID NOT BRING IT UP The reason this problem occurs is because of a process called galvanic corrosion. The fuel tank shield is bolted onto an aluminum heat shield, and the pairing of these two different metals accelerates the corrosion. Land Rover has acknowledged this problem and it's given priority due to the fact that this is a safety issue being that these shields are also responsible for holding the tanks in place. ANOTHER SEVERE SAFETY ISSUE THAT LAND ROVER NORTH AMERICA IS CURRENTLY, SIMPLY IGNORING. TOTALLY UNACCEPTABLE. IF HENNESSY CAN’T DO WHAT IS RIGHT THEY SHOULD BE SHUTTERED AND PETE ALVAREZ SHOULD BE FIRED. Please pass this along to Land Rover North America. My 2017 Chrysler Pacifica experienced repeated engine timing and oil system failures. The vehicle first developed a check engine light and engine timing code (P000D – camshaft position slow response). The vehicle was inspected by a dealership service department. Shortly after, a major engine oil leak was discovered from the oil cooler / oil filter adapter assembly. The vehicle required repair o

The dealership later confirmed failure of the cam phaser components associated with the engine timing system and performed a repair. Within days of the repair, the check engine light and engine timing code returned again. The failure involves the engine valve timing system and oil pressure dependent components based on Autozone code check. Improper camshaft timing can affect engine operation, cause loss of power, rough running, or possible engine stall while driving, which creates a safety risk in traffic. The issue has been inspected and confirmed by a dealership service department. The vehicle is available for inspection upon request.Read more

Now car won’t drive past 25-30mph dealership indicated this is a faulty gas pedal which cost over $600 to repair. Did my research and this is apparently a “known” problem common in 2016-2022. Vehicles but is no

I just submitted a complaint but wanted to add that after my car lost all power on a 45mph road and went into limp mode it then was towed to the closest shop. I do have a warranty but when I called Honda they said it wasn't covered The shop I took it to charged me 4100 dollars for a blown head gasket which from researching it looks like this is happening to all 2018 Honda accords This is low m

The failure appears to involve the vehicle’s auto start/stop system and related electrical control systems, including the battery management system and/or body control module. The vehicle shuts off unexpectedly while driving and fails to automatically restart, displaying “Auto Start/Stop – Manual Restart Required” and “Shift to Park.” The vehicle is available for inspection upon reques
... t. The vehicle has shut off unexpectedly in active traffic, causing a sudden loss of power. This creates a dangerous situation where I am unable to accelerate or move with traffic flow, increasing the risk of being rear-ended or involved in a collision. This has occurred on public roads with other vehicles present, putting both myself and others at risk. The issue is intermittent and has not consistently been reproduced on demand. However, it has occurred multiple times during normal driving conditions. I am in the process of having the vehicle inspected and documented by a dealership. The issue has been reported to the manufacturer, and a case has been opened with Ford Motor Company. At this time, the vehicle has not yet been inspected by police or insurance representatives. There were no consistent warning lamps prior to the failure. At the time of the incident, the vehicle displayed messages including “Auto Start/Stop – Manual Restart Required” and “Shift to Park.” These messages appeared simultaneously with the loss of power. The issue has occurred intermittently without advance warning.Read more

Pulling out in front of High Speed Traffic, Engine went into Safe Mode with Check Engine Light. Fortunately a car at high speed avoided rear ending my car. Upon evaluation, DTC Code P0303 (Cylinder 3 - Misfire) was present. Coils and Spark Plugs were changed but car was still misfiring. Honda has a Warranty for 150k/ 10 year on Fuel Injectors but this VIN was not on list based on Honda Service De
... partment. The Honda Service Department evaluated and Car has Head Gasket Leak in Cylinder 3. This is a widely known problem reported on many Honda Owner websites and Honda Service acknowledged it was an issue but is not a “Recall” despite the potential Safety Risk on the Road when car goes in to Protect / Shutdown Mode. The nature of this issue occurring on a well maintained car with 147k miles that is 8 years old and not being a Recall and a Safety related item is being reported by me as me and my family could have been impacted by a rear end collision as car shut down / protect mode. This could and likely has occurred to other owners and possibly attributed to accidents and Safety of passengers. Honda wants over $11k to repair the car which includes Fuel Injectors / Plugs / Coils / New Head as they reflect old head can’t be used even if machined / Head Gasket. This issue is a Safety related issue and should be a Honda Recall as the Head Bolts are a known design issue causing premature failure of the Head Gasket. Submitting this to document my incident, protect the public driving this model and year and hopefully providing information to warrant a recall so that all repairs will be performed under Recall Warranty. Also to protect the public from a Honda Accord Safety issue for 2018 Honda Accord LX.Read more

While driving my 2024 Land Rover Defender 130 on Route 114 in Massachusetts at approximately 8:30–9:00 PM in dark and rainy conditions, the vehicle suddenly and completely shut off while in motion. The vehicle lost all power: • Engine shut off • No gears functioned • Vehicle would not restart • Lights and hazard lights were not
... functioning The vehicle was stranded in the middle of an active roadway with no warning and no visibility. Because of this, I had to exit the vehicle into traffic, stop oncoming vehicles, and remove my child from the car. We crossed a live lane of traffic to get to safety while leaving the vehicle in the roadway. I called 911. Police responded and controlled traffic at the scene. In the presence of the responding officer, I attempted to restart the vehicle multiple times. It remained completely unresponsive for an extended period before eventually starting again without explanation. This vehicle experienced a sudden loss of power while in motion on an active roadway. This is a serious safety concern. I do not feel it is safe to transport my child or family in this vehicle. I am seeking a clear and definitive diagnosis and resolution. If this issue cannot be conclusively identified and repaired, the vehicle should not remain in service. • RSelect systems: • Engine • Electrical • PowertrainRead more

This appears to be a fuel pump or something with the fuel issue on a 2021 Toyota Highlander L series. While driving it would stop and switch off at stop lights and stop sign. The codes that come up have to do with fuel being lean. However this also affects the braking system and causes it to be very hard to steer with this issue. The problem I have is that it cuts off without even you knowing what
... 's happening and I was almost rear-ended twice. My car is now sitting outside unable to start. It doesn't even have 85,000 miles on it with all these issues. I believe it also said something about my transmission but unsure because the codes were flashing off and on.Read more

I reported a fuel smell in the cabin of my vehicle in June 2024. It was dismissed because they could not recreate the smell. I reported it again and they said that is just how the vehicle purges fuel. I reported it again and drove it right to the dealer when it smelled and multiple mechanics at KIA confirmed they smelled it. Reported it again in September 2024, then tried to deal with it, and repo
... rted it again in June and July 2025. In July they confirmed I had raw fuel in my evaporator canister. I also had misfire notifications on my dashboard which were dismissed as they could not determine where the misfire was coming from. The misfire stopped after they replaced my fuel canister but the smell never fully went away. I received an email follow up in July 2025 and I reported the smell was not as strong, but still occurring intermittently. They closed the case. I reported and had 2 open cases with KIA, and noted that I was feeling lightheaded and nauseous, as well as 2 of my 3 kids at the time.Read more

While being driven on a residential street, the vehicle suddenly displayed a 'Stop Safely' warning on the dashboard. Upon an attempted restart, the vehicle suffered a total electrical blackout and complete loss of propulsion power while in the middle of a live roadway. All internal and external lights, steering assistance, and power systems failed simultaneously. The vehicle was manufactured in 20
... 25 and has been out of service since March 16th. The dealership (Napleton Downtown Hyundai) is attributing this total system collapse to 'bad fuel' to deny warranty coverage, despite the failure being a catastrophic high-voltage electrical shutdown. While driving at highway speed with cruise control engaged, the vehicle experienced a sudden loss of propulsion. The engine began jerking and the vehicle rapidly decelerated from highway speed (50 mph) to 10–15 mph. The transmission appeared to disengage or shift to neutral around 10 mph, and the vehicle was unable to maintain speed, creating a hazardous situation in traffic. The vehicle then s

When attempting to restart, the engine would not turn over properly and produced a loud clicking/struggling noise, similar to a failed or seized engine. The vehicle was towed and is currently awaiting diagnosis at a dealership. This failure occurred without warning and created a serious safety risk due to sudden loss of power while driving.Read more

I am reporting a serious safety concern with my 2016 Infiniti QX60 involving a defective cooling fan system. The vehicle currently experiences a ticking/tapping noise from the cooling fan, and the fan does not operate properly, which has caused the engine to overheat. This condition creates a significant safety hazard, as engine overheating can lead to sudden engine failure while driving, increasi
... ng the risk of an accident. The issue also makes it unsafe to use essential features such as the air conditioner in hot weather and the defroster in cold conditions, which impacts visibility and driver safety. This defect appears to be part of a known issue affecting this vehicle platform, where the cooling fan system fails prematurely. I understand there have been allegations, including in Anderson v. Nissan North America, that the manufacturer may have been aware of this defect but did not adequately disclose it. In addition, I am aware that this vehicle model has been associated with other safety-related recalls, including issues involving the ABS actuator (which may pose a fire risk) and the hybrid powertrain control system (if applicable). These existing concerns further highlight the urgency of addressing this defect. This is not normal wear and tear, but a potential safety-related defect that places drivers, passengers, and others on the road at risk. I am concerned about continuing to operate this vehicle and potentially causing an accident or injury due to overheating or loss of vehicle function. I am requesting that this issue be formally investigated to determine whether a recall or corrective action is necessary to prevent harm.Read more

The component that failed in both incidents was the vehicle’s wiring system, specifically wiring insulation that appears to be vulnerable to rodent damage. In the first incident, squirrels chewed through wiring connected to the fuel injector system, which triggered a check engine light and was repaired. In the second incident, squirrels again accessed the engine compartment and chewed through wi
... ring affecting the braking system. This resulted in a sudden and unexpected loss of braking function. Yes, the damaged components should be available for inspection through the repair shop or service records, although I would need to confirm the exact location of the retained parts. Safety was significantly put at risk. During the second incident, the vehicle was occupied by multiple passengers, including minors. While approaching a red light, the driver attempted to brake and found that the brakes were not functioning properly. The driver had to shift the vehicle into park and use the emergency brake to stop, narrowly avoiding a serious collision. This created a high-risk, potentially life-threatening situation. The first incident was addressed by a mechanic after the check engine light appeared, confirming damage to the fuel injector-related wiring. The second incident was formally confirmed by a mechanic that the brake line was leaking from damage from, what they highly suspected and have seen in the past, a rodent . In the first incident, a check engine light appeared, prompting immediate attention. In the second incident, there were no warning lights, messages, or prior symptoms before the brake failure occurred.Read more

An “Emissions System Problem” warning message appeared intermittently on the dashboard while driving. The warning light would sometimes turn off after restarting the vehicle, but the issue persisted in the vehicle’s stored diagnostic system. The vehicle was taken to an authorized dealer, where a diagnostic scan confirmed a P0430 code (catalyst system efficiency below threshold). The dealer
... identified the likely cause as failing fuel injectors and recommended replacement of the injector system. The issue was confirmed by the dealership and is reproducible through stored fault codes, even when the warning light is not actively displayed. This condition may impact engine performance, fuel mixture, and emissions system operation. In certain situations, this could lead to reduced engine performance or hesitation, which may affect the ability to safely accelerate or merge into traffic. The vehicle has been regularly maintained according to manufacturer recommendations. There were no significant warning signs prior to the initial dashboard alert other than the intermittent warning message. The failed components (fuel injectors) should be available for inspection upon request through the servicing dealership.Read more
