The fuel door actuator failed. The gas door would not open due to the actuator being seized shut, preventing access to the fuel tank. This vehicle does not have a manual release mechanism, so there was no alternative way to open the fuel door. The dealership confirmed the actuator malfunction. My family’s safety was put at risk because we were unable to refuel the vehicle. The incident occurre
... d during extremely cold weather, with a windchill of approximately -8°F. Due to the inability to add fuel, we were at risk of running out of gas and losing heat in the vehicle, which could have resulted in a dangerous exposure situation. There were no warning lights or messages indicating a problem prior to the failure. The issue was first noticed when attempting to refuel and the fuel door would not open.Read more

Upon accelerating from a stop, or at various speeds, the vehicle experiences a significant loss of motive power(pulling backwards) followed by an abrupt lunge forward. This hesitation occurs unpredictably and makes entering traffic or intersections extremely hazardous. This feels like a fuel delivery or transmission engagement failure. The engine failed while driving on the highway on two separate occasions due a fuel pump issue. The first time it happened, it was just reduced power (could only drive maybe 40 miles an hour). I brought it to the dealership and they said that General Motors told them it needs a software update which they completed. However the second time it happened (4 months later) it was full engine shutdown
... in the left lane on a 65mph highway with my family (including my toddler) in the car. The first time it happened we were able to make it home by driving with the hazard lights on very slowly. But the second time we were doing the 65 mph speed limit in the left lane on a 3 lane highway. To avoid causing a pile up of cars or a deadly accident, we had to use the momentum of the speed we were going to try to safely drift across three lanes of congested traffic just to make it safely to the shoulder. We were then stuck on the side of the highway in the sun on an 80 degree day with a car with no power. I had to remove my two year old from the car so he didn't get heat exhaustion and stand on the side of the highway holding him in the little bit of shade we could find. The car then had to be towed back to my dealership over an hour away because it would not even turn on. The dealership was not able to replicate the issue the first time it happened, but did the software update that the manufacturer recommended. To my knowledge its only been inspected by the dealership. The only warning signs/messages/lights were the ones happening as the engine was failing/shutting down. There was no advanced warning. This care is less than two years old and its a leased vehicle from the dealership down repairs, I work from home there are less than 20k miles on i, this is not neglect on my part or over usage in anyway of the vehicle.Read more

While driving at approximately 70 mph on a highway under normal conditions, the vehicle experienced a sudden loss of engine power. A check engine light illuminated at the time of the failure, and the vehicle was unable to maintain normal acceleration or consistent speed. The loss of power created a hazardous situation in active traffic, as the vehicle could not safely maintain speed with surround

Incident date: Apr 16, 2026
When operating the vehicle above the minimum temperature recommended by the manufacturer, the battery state of charge has dropped multiple tens of percentage points in moments while driving on the highway while driving the speed limit. The starting point for the state of charge in this situation was roughly 64% when beginning the trip. This has occurred at roughly 10 degrees Fahrenheit. The vehicl
... e gives a warning that the driver should pull over immediately, which is very dangerous when driving on an interstate highway in traffic. The state of charge dropped into the teens in moments. This cold weather issue was not disclosed when purchasing the vehicle. The minimum operating temperature for the vehicle is -1 to -13 degrees F depending on the operating conditions (charging, driving, etc.). This should not occur at 10 degrees F. A software fix to the battery to slow the state of charge will not remedy the issue with a faulty battery in this case. Why is this accepted when the Chevrolet Bolt vehicles were required to swap their batteries to remedy the issue? I have not had time to bring this into the dealer due to my schedule availability. I will try to reproduce this upcoming winter. Others online have reported this same problem though.Read more

My 2021 Chevrolet Suburban experienced a sudden no start condition which required the vehicle to be towed for repair. A diagnostic inspection determined that both the fuel pump and the fuel pump control module had failed. I paid out of pocket for the full repair, including replacement of both components. Several months later, GM issued a safety recall for the fuel pump control module on this vehic
... le. I submitted a reimbursement request to GM. GM agreed to reimburse only the control module , but refused to reimburse the fuel pump, even though the pump failure was directly caused by the defective module that is now under recall. Federal recall reimbursement guidelines require manufacturers to reimburse the full cost of the remedy for the defect. In this case, the defective module caused the fuel pump to fail, making the pump replacement part of the necessary remedy. GM’s refusal to reimburse the full repair cost leaves me paying for damage caused by the recalled component. I am requesting NHTSA’s assistance in reviewing GM’s refusal to reimburse the full repair cost associated with this recall related failure.Read more

This complaint concerns failure of the remedy for Safety Recall Z46 / NHTSA Campaign 22V406 involving the high pressure fuel pump (HPFP) on a 3.0L diesel pickup. The HPFP had previously been replaced under the recall by an authorized dealer. About 19 months later, while the vehicle was at a dealership for service, the replacement HPFP failed again during dealer testing. The dealer documented meta
... l contamination throughout the fuel system caused by the HPFP failure. The dealer then replaced the contaminated fuel system components. After those repairs and during post-repair test driving, the vehicle developed new bottom-end engine noise. The dealer later documented connecting rod bearing damage / rod knock and recommended complete engine replacement. The vehicle had been brought in with no bottom-end engine noise, and the dealer’s intake inspection documented oil condition as OK. An independent oil analysis later showed the engine oil was normal and did not indicate long-term lubrication failure. This suggests the engine damage occurred acutely after the HPFP failure and resulting contamination event, not from routine wear. This condition creates a serious safety risk because HPFP failure can cause loss of fuel pressure, engine stall, or loss of motive power, and the subsequent engine damage raises the risk of sudden engine failure or seizure while driving. That could increase the risk of a crash, especially in traffic or at highway speeds. The failure was confirmed by the dealer and is documented in the repair order. The vehicle remains inoperable and is available for inspection upon request. I am requesting investigation into whether the recall remedy was adequate, since the recall replacement HPFP failed again and the resulting contamination appears to have led to catastrophic secondary engine damage.Read more

When I had my truck in for a recall on 4/19/26, I asked a Toyota service manager if this is a common problem. He told me it is very common in the 2024 Toyota Tundra, with a V6 twin turbo. The steel fuel tank shield and mounting cradle assembly on my Land Rover Range Rover (L405) is experiencing catastrophic structural failure due to excessive corrosion. This appears to be a result of galvanic corrosion between the steel cradle and the aluminum chassis components. The corrosion has caused the metal straps and support brackets to rot through completely, compromising the physical sec

The vehicle experienced an overheating condition involving the fuel system, which aligns with an active recall stating that a damaged check valve can allow air into the fuel tank, causing it to expand and come into contact with hot exhaust components, potentially melting the fuel tank. While operating the vehicle, this created a serious safety concern due to the risk of fuel tank damage, fuel leak
... age, and potential fire. This was not a minor issue, as it directly involved a component that could lead to a fire hazard. The vehicle was later evaluated and deemed unrepairable due to the severity of the issue. Because of this, the vehicle is not currently being used due to safety concerns. Despite the known safety risk and recall-related defect, the situation has not been properly resolved. The handling of this issue raises concerns given the potential for fuel system failure and fire risk associated with this defect. I am filing this updated complaint to supplement my prior complaint (NHTSA ID No. XXX, filed January 27, 2026) regarding a recurring, safety-critical engine defect in my 2020 Lexus IS300 RWD, 8AR-FTS engine. ... l events involved flashing MIL, reduced engine power, shaking, and abrupt deceleration. DIAGNOSIS: Authorized Lexus dealership—following 34-day retention with Lexus Technical Assistance (TAC Case XXX) and personal on-site inspection by a Lexus Field Technical Engineer—confirmed worn intake valve guide on Cylinder 3. Repair estimate: $10,355.61 subtotal per Service Bulletin L-SB-0007-21 ($10,671.72 total incl. shop supplies and tax). Estimate cites L-SB-0007-21 without mileage qualification, confirming technical applicability. TSB PATTERN — NO RECALL: Lexus has issued multiple TSBs on the 8AR-FTS engine: L-SB-0153-17 (September 2017) and L-SB-0007-21 (March 2021, revised April 17 and July 2, 2024), covering 2015-2024 model year vehicles—progressively widening applicability over three years. Both 2024 revisions occurred during the active warranty period. No recall has been issued and this consumer received no notification of any kind. A TSB is an internal dealer communication — not consumer notification. A CPO purchaser relying on Lexus's representations of comprehensive inspection and rigorous certification has a reasonable right to assume known defects were addressed, without searching technical databases. MANUFACTURER RESPONSE: Lexus declined to authorize repair under warranty or as goodwill and declined to confirm authorization in writing. REQUEST: Please investigate whether the pattern of multiple TSBs covering progressively wider applicability—with no recall despite safety-critical symptoms—warrants a formal investigation and escalation to a safety recall. My Jeep Wrangler 4xe now has controlled/limited electric propulsion while driving in electric only mode after 68c recall was performed. With no warning while driving in electric only mode, my vehicle will not accelerate without accelerator press almost to the floor to activate the internal gas engine. This behavior on a cold run starts out fine, but as the vehicle(electrical system) warms, the l
... imitations come on strong can catch the driver completely off guard if needing to accelerate quickly to merge into traffic or enter freeway on ramps. This erratic behavior is both dangerous to the driver along with other drivers on the road. My dealer has driven the vehicle on a short(in town) test drive and concluded the vehicle drives as intended by the manufacture. No warning are lamps visible, and as outside temps rise, this issue seems to be more apparent. This electrical behavior started the day I received my vehicle from the dealer after 68c recall was performed. The software that is now installed on these 4xe vehicles seem to be limiting/cutting the electrical output of the onboard high voltage battery to limit or eliminate heat stress on the internal battery known to catch fire and burn under heavy load. This creates a safety hazard to owners who drive these vehicles that basically stall out(no acceleration/dead pedal) without warning given to the operator prior to the software limiting the battery output to the vehicle drive train at will. When this occurs the electrical boost gauge on the 4xe dash will not rise above 25% no matter how far down you push the accelerator pedal, where normally(when not limited by software) the gauge can reach 100% during the electrical acceleration process.Read more

AAA flatbed towed my vehicle to a near mechanic's shop, but it was after hours and had to be left there until morning. The mechanic inspected the problem and confirmed the issue was a fuel module and fuel pump failure. How does this happen without any alerts to the driver? I opened the hood and there was a leaking sound followed by a light smoke and leakage near the battery. A video was taken if needed. The smell was extremelly strong and I am reporting this safety concern as the VIN associated with the vehicle currently has an unrepaired recall with no available remedy yet by FORD - 25V467. The safety risk associated with this recall number is fire under hood due to possible injector crack that could lead to fuel accumulating on top of the engine. Despite being shown evidence of this(photo and video of oil overfill and watery viscosity) , the manufacturer instructed me to continue operating the vehicle, placing myself, and other drivers at risk of a high-speed collision or vehicle fire. The symptoms (P0011 code and engine noise) were confirmed by an independent mechanic on March 25th 2026. However, during the official Bearing Clearance Test (BCT), the dealership intentionally overfilled the engine oil by 3 quarts (documented via photograph) to muffle mechanical noise and artificially pass the diagnostic and issued a pass on all inspections despite the oil hazard. Hyundai Corporate Case Manager Nikulmar Dhabi has admitted on record that the dealer was out of compliance by "being outside the standard notification time" yet they refuse to provide a safety-compliant remedy. Warning Symptoms & Appearance: Symptoms first appeared months ago when I repeatedly got a P0011 code, however On march 24th 2026 I received an engine light with a P0011 and the car going into limp mode while i drove. Warning signs included a persistent Check Engine Light (P0011) and audible metallic knocking. Upon checking the oil myself after the dealer "inspected" it, I discovered the oil was 3 quarts over the "Full" line, which is a deceptive practice used to mask rod bearing failure.Read more

There is no incident, yet, just a recall that I have for this vehicle, coming up on a year here soon, for a fuel leak that could cause a fire under the hood while driving, and Ford doesn't seem to think that's urgent enough to get the parts to fix it. What's up with that? Safety recall notice, 25S76 NHTSA Recall, 25V467

Vehicle experiences sudden, intermittent loss of motive power, severe engine hesitation, and stalling while operating at highway speeds. Diagnostic scans confirm fault codes P2015 and P0341, indicating a failure of the intake manifold position sensor and camshaft position correlation. These mechanical failures, combined with extreme carbon buildup on the valves, create a hazardous operating condit
... ion where the vehicle cannot maintain safe speeds in high-traffic environments. The manufacturer (Audi) has acknowledged the intake manifold failure—which is a known defect under an extended 120,000-mile warranty—but refuses to remediate the consequential engine damage (carbon buildup) caused by the faulty manifold and a previous oil overfill incident at a certified dealership (Chapman Audi of Tucson). Despite a 12-month documented history of seeking repairs for these safety symptoms, the dealership repeatedly failed to diagnose the manifold defect, instead clearing codes and returning the vehicle to the road. This systemic failure resulted in a hazardous operating condition that now puts the driver at immediate risk of rear-end collisions due to unpredictable engine stalling. By refusing to perform the necessary top-end cleaning required to restore the engine to a safe state, Audi is knowingly allowing an unsafe vehicle to remain in service. This represents a significant risk to public safety and a failure to address a documented powertrain defect that compromises vehicle control.Read more

I am getting increasing frequency of cylinder misfire, so far cylinder 1 and 2, and the car goes to a limb mode and loose power. It happens during acceleration to a highway ramp. Engine lights and other many warning lights come on, the car goes into limb mode, I pull over and when I turn the engine off and turns back on the warning lights clear and the car drives normally until next time, to star
... t all over again. I took it two Honda dealers at three occasions. Since the engine light goes off, and they cannot capture the issue, they are telling me there is nothing they can do. It seems to happen during a normal driving than sport mode. It is frustrating and dangerous situation. I thought I would report it, to save others from similar situationRead more
