I am reporting a safety issue involving my 2016 Audi Q5. The vehicle previously underwent recall 20Z8/20W9, which addresses fuel pump flange cracking that can cause fuel leaks and fuel vapor odors. Despite this recall repair being marked as completed, I am now experiencing a strong gasoline smell inside the cabin. This appears to be the same condition the recall was intended to fix. When I call
... ed the authorized Audi dealership (Audi San Jose), I was told I would be responsible for diagnostic and repair costs because the recall had already been completed and is no longer covered. I believe this indicates that: The recall remedy may have been ineffective, or The defect has reoccurred This presents a potential fire hazard and safety risk. I am requesting that this issue be reviewed, as it appears the recall did not adequately resolve the safety defect. On March 25th the dealership said our car was on a recall for R25E2 the dealership performed the ECM software update fix at this time. On April 30th 2026 the engine suffered catastophic failure. The heat stopped working the car was not showing hot the dial was at 1/4 it is usually at 1/2. The oil light flashed my husband lost power steering and it died at a 4 way stop. There was so much white smo

There were no warning lights or anything the only warning was the heat not working. He only made it 8 miles past out house before this happened. The software failed to protect the hardware from the safety defect. The dealership said the head gasket blew from overheating but they are saying it is not the recall they dont know the reason even though my husband and I have stated our car was affected by this recall or they would not have completed the ECM software update which clearly is a failed remedy. I was driving my 2018 Chevy Volt at about 30 mph on EV mode. The battery was at 50%, the car was performing normally. Then the battery jumped to 100%, the check engine light came on, and the vehicle lost power. I was in rush hour traffic and unable to get the vehicle to go faster than 5-10 mph. I switched to "normal" mode to engage the gasoline engine, hoping this would help but it did not
... . The gas engine RPM fluctuated wildly and did not restore the ability to accelerate the vehicle. I was still unable to go faster than 5-10 mph. I was a few blocks from home and managed to get home safely. The engine code was P1E00. I put the vehicle on the charger and it seemed fine the next day. This exact situation I ran into - reduced engine power - is addressed in Special Coverage N232432680, which includes this vehicles VIN. I took the car to the GM dealer today and they are telling me that the car is fine, they can find no problems. I mentioned that this issue is covered under Special Coverage N232432680, and was told that unless they can reproduce the issue in the shop, that they cannot apply Special Coverage N232432680.Read more

2023 Cadillac Escalade ESV - passed Safety Recall N252494001 L87 in August of 2025. March 2026 experienced loss of propulsion (while driving), major noise and violent shaking. Every warning came on. There were no other symptoms Very fortunate no one was behind us as vehicle completely stopped on road. Reported problem to dealer and took into them. Said it was serpentine belt and we were charged to

In April 2026 the same exact issue occurred with complete propulsion failure (vehicle dies abruptly with significant noise and shaking) on the road near home (nearly hit by following traffic). Called dealer, told to not move the vehicle and we had to have it towed to same dealer. In shop now for 2 weeks, we have heard 3 different reasons with no fix of the issues. They will not call us. We have to phone them and it is a different issue each time we call. We were told GM is involved; but we have had no verification.Read more

I am reaching out regarding an ongoing issue with my 2021 Chevrolet Blazer. The vehicle is experiencing a noticeable stutter or hesitation when shifting between second and third gear. In addition, there are instances where the vehicle accelerates unexpectedly or more aggressively than intended. This behavior is particularly concerning, as it presents a potential safety issue during normal driving
... conditions. These concerns have become increasingly noticeable, and I am concerned they may indicate a transmission or related mechanical problem that requires prompt inspection and resolution. Please advise if there are any known service bulletins, recalls, or warranty coverage related to these issues. For reference, the vehicle has been properly maintained and this behavior is not consistent with normal operation. Given that this is now the third major issue I’ve experienced with this vehicle, I am hoping this matter can be addressed without additional out-of-pocket expense. After additional research, I have learned that other owners have experienced the same issue. Others have stated the following: 1. Hesitation, stuttering, or jerking between gears Owners of 2020–2021 Blazers report hesitation and jerking during acceleration, sometimes described as gears “shifting in and out rapidly.” Forum users mention sluggish shifting or slipping feelings, especially at certain speeds. Others report hard or delayed shifts at low speeds (like 15–25 mph). 2. Shuddering / vibration (often tied to torque converter) There are repeated mentions of “shudder,” “surge,” or vibration during acceleration. Some cases were linked to: Contaminated transmission fluid Torque converter clutch (TCC) issues One owner said the problem felt like a bad torque converter and required service attention. 3. The vehicle is a 2012 Jeep Grand Cherokee with open safety recalls T59 and V62. The brake warning light is currently illuminated, and based on the symptoms I believe the brake booster may be failing. The braking system is available for inspection upon request. The safety concern is that the vehicle may have reduced braking assist or impaired braking performance while being driven. This creates a r
... isk of increased stopping distance, loss of braking confidence, or inability to stop safely in traffic. I contacted the FCA/Stellantis-recommended Jeep dealership in Lapeer, Michigan multiple times this week to schedule recall-related service. I was repeatedly transferred to service voicemail and did not receive return calls. When I called back and asked to speak with someone else, I reached the service manager. I explained that the vehicle has an illuminated brake warning light and that I suspect the brake booster may be failing. The service manager focused on where I lived, why I was not using a closer dealer, and mentioned that the vehicle is 14 years old, rather than clearly arranging recall service or addressing the brake safety concern. The problem has not yet been inspected or confirmed by a dealer because I have been unable to obtain timely service despite repeated attempts. The vehicle has not been inspected by the manufacturer, police, insurance representatives, or others. The warning lamp currently present is the brake warning light. The exact first date the warning light appeared was within the last 2 weeks.Read more

I own a 2025 Jeep Gladiator with approximately 48,000 miles. The vehicle was fully operational as of approximately 5:00 PM on Monday, April 27, but by 7:30 AM the following morning it had become inoperable due to a rodent chewing through the fuel line overnight. The vehicle required towing to a dealership, Beaman Chrysler Dodge Jeep Ram. At the time of the incident, the vehicle was parked in a gra
... vel driveway at an RV park, which is a typical residential parking environment. Damage to a fuel line presents a potential safety hazard, including risk of fuel leakage, fire, or sudden vehicle disablement. In this case, the failure occurred without warning and rendered the vehicle unusable. A significant amount of fuel was observed leaking beneath the vehicle and onto the ground. The dealership confirmed the damage and advised that the repair would replace the component with the same or similar material. This repair does not provide any mitigation to prevent recurrence of the issue. It is my understanding that some automotive components may utilize plant-based materials, which have been widely reported to attract rodents. If applicable, this may increase the likelihood of repeated damage to critical vehicle systems. This situation raises concern regarding the durability and safety of fuel system components, as such damage can render a vehicle inoperable without warning and may pose additional safety risks depending on when and where the failure occurs.Read more

I am filing this complaint regarding NHTSA Recall 26V103 (Manufacturer Recall R10362) for my 2025 Volvo EX30, VIN [XXX] . The recall was issued February 18, 2026 for a fire, overheating, and smoke safety risk due to potential overheating of the High-Voltage battery cell. The remedy requires battery module inspection and replacement, however Volvo has acknowledged that no replacement parts are ava
... ilable and has provided no estimated timeline for repair. I have not received any repair. No parts have been installed. No remedy has been performed on my vehicle. Despite this, my dealer marked recall R10362 as Closed on or around April 30, 2026 without performing any repair. When I contacted Volvo Cars customer care directly, they confirmed verbally that the recall remains open for my vehicle and that parts are still unavailable. However Volvo stated they will only provide written confirmation to the dealership, not to me as the vehicle owner. As a result of the unrepaired recall, I have been instructed to limit charging to a maximum of 70% state of charge to reduce fire risk. This has been in effect since February 2026. My vehicle has lost significant market value as a direct result. I am requesting NHTSA investigate the improper closure of this recall on my VIN without remedy being performed, and take appropriate action against the manufacturer and dealer. Vehicle Safety Complaint – 2021 Ford F-350 I am submitting this complaint regarding a safety-related failure involving my 2021 Ford F-350. While driving under normal conditions, the vehicle suddenly entered “limp mode,” resulting in a significant and immediate loss of power. Approximately 20 miles later, the vehicle completely shut down, leaving me stranded and requiring the truck to be towe
... d to Gene Messer Ford for diagnosis and repair. The failure created a potentially dangerous situation, as the loss of power limited my ability to maintain safe speeds and respond to surrounding traffic conditions. After the vehicle was delivered to the dealership, it took approximately one week for inspection and an additional two weeks to complete repairs. During the repair process, I was informed that newly installed components—including the fuel rail and two fuel injectors—were defective and required replacement again. This raises concerns about whether the root cause of the failure was properly identified and whether contamination or systemic failure within the fuel system was present. I am also aware of widely reported issues involving high-pressure fuel pump (HPFP) failures in Ford diesel engines, which can result in catastrophic damage to the entire fuel system, including injectors, fuel rails, and lines. Given the nature of my vehicle’s failure and the components involved, I am concerned that this incident may be related to a broader, known defect. This type of failure—sudden loss of power followed by complete shutdown—poses a serious safety risk, particularly if it occurs at highway speeds or in high-traffic conditions. I am requesting that this complaint be reviewed as part of a broader investigation into potential fuel system defects in Ford diesel trucks. I believe this issue may warrant further scrutiny and possible recall action to prevent similar incidents.Read more

I am reporting a potential safety recall violation by Honda Mall of Georgia (3699 Buford Dr, Buford, GA 30519). My vehicle experienced engine noise, vibration, and warning indicators, which the dealership attributed to 'fuel contamination' to deny warranty coverage (attached), and charged approximately $3,392.99 for repairs. However, I have a certified laboratory analysis (attached) from the Georg
... ia Department of Agriculture confirming the fuel met all state quality standards and had no contamination at Costco Gas. Costco also provided supporting document (attached) that there was no contamination and there were no other customers reporting such issue. The vehicle required a second repair (attached) for the engine issue immediately after the first, further proving the failure was not 'bad fuel' related. Despite my state-certified proof, the dealer refuses to provide any documentation for their claim of 'bad fuel.' I believe the dealership has intentionally misdiagnosed a known defect covered under Federal Safety Recall 24V-763 / QK8 (cracked high-pressure fuel pump) to avoid performing a free recall repair.Read more

I shut the power off an powered up twice thinking that might reset the sensors; but they continued to fail. As I continued to drive there was limited power to the throttle, and I eventually coasted through a red light and into my work parking lot where the vehicle shut down entirely, and eventually fully locked up (couldn't start or event put in neutral to push). I had the vehicle towed to back to the dealer, and they diagnosed the problem as a failed transmission cooler. Between that incident and 4/29/26 I went thru 9 gallons of coolant, replaced the sparkplugs, put the front end up and "burp' the system as there was constantly air bubbles being introduced from a source I could not find. I had the hoses and clamps inspected, replaced the coolant reservoir and the reservoir cap, the vehicle started to shutter upon starting even after it was warmed up and every day needed to be burped and topped off before and after driving in town but often had no coolant loss during freeway driving, finally 4/28/26 I noticed smoke from the exhaust when I started the car in the morning and that smoke was accompanied by a foul, burnt, electric smell. While burping the air and topping the coolant that morning, the engine sputtered and died 2 separate times. I decided that it was time for a cooling system diagnostics as I still could still see no evidence of an external leak. The Ford technician recorded this tech story. "The vehicle ran rough and smoked for a couple seconds after starting.the vehicle ran its self test and had p0316, b0303, p1299, and p0217. These dtcs relate to cylinder misfire on startup and cylinder misfire and cylinder 3 and over temp/protection being active. ... The tracking number assigned by NHTSA for this issue is 11733456 I am writing to update and escalate my previously filed safety complaint regarding my vehicle. In addition, multiple safety and driver‑assistance functions are not working as intended. Despite these issues, the dealership insists that everything is “within specifications” and refused to take further corrective action. Because of these unresolved defects, I am afraid to drive this vehicle. I have already experienced a traumatic incident related to this car, and returning it in this condition has caused significant fear and emotional distress. I cannot go through another dangerous situation. I believe this vehicle poses a genuine safety risk to me and to others on the road. I am requesting that NHTSA review my complaint again, including this new information, as the problem persists after inspection and attempted repair. With the recent completion of recalls 68&78c , and a new HV battery, APPEARS TO ME, thatJeep has changed the software for the throttle response when in electric mode. The result creates a safety hazard as you lose pedal response and power when the battery reaches below 35% or about 9 miles remaining. On the highway the jeep loses all power and will not accelerate over 62 mph. And the same around t
... own when you need that extra umph, the pedal becomes dead. More Scary when you’re on the highway!! The result? Since the electric power output is unavailable, you have to mash the pedal to the flow to kick it out of electric mode. But this is also unsafe as now you start the engine (often cold as I was running electric mode) and you smash the rpm’s over 6k to launch the gas engine (That can’t be good for the engine long term) . Before battery replacement and recall software updates, this process was very smooth and occurred with 5% or less battery life. Two days later, I was informed that the vehicle was no longer able to shift into gear and was now completely inoperable. I was told I would be responsible for towing the vehicle off their premises. According to the service department, the vehicle now requires replacement of the EME module, at an estimated cost of $12,000. This assessment was particularly troubling because the vehicle was fully drivable when it arrived at the dealership. At no point prior to service did the car have difficulty shifting into gear. It is deeply concerning that the vehicle became a “brick” while under the care of an authorized BMW service center, with no accountability taken for what occurred during their diagnostic process.Read more

All components are available for inspection upon request. The vehicle was taken to an authorized Chevrolet dealership for diagnosis. The dealer
... acknowledged the emissions codes but stated that unrelated components would need to be repaired at owner expense before the emissions system would be addressed, despite these codes falling within the federally mandated 8-year/80,000-mile emissions warranty. The fault codes were confirmed by the dealership. No safety incident has occurred to date, however degraded NOx and EGR function can result in increased harmful emissions output and potential engine performance issues. Warning lamps appeared without prior incident or symptom. The manufacturer has not offered to cover the emissions components under the federal emissions warranty without the owner first paying for additional repairs that may or may not be causally related to the emissions system failure. When the dealer was directly asked, "Are you saying the emissions system itself is functioning correctly, and something else caused these codes to appear?" — the dealer did not provide an answer. This question was asked in order to clarify whether the emissions components themselves are degraded or failed, which would confirm warranty coverage eligibility. The dealer's failure to answer this question directly is noted as part of this complaint.Read more

There is a known issue about exhaust manifolds for my year model truck. A ram technician said in 2019 mopar created a new exhaust manifold for my truck that would be less likely to create soot buildup and warping in the exhaust manifold. In 2021 my EGR was replaced because of a recall but now my exhaust manifold has failed and needs to be replaced out of pocket even though it’s been a known issu

The vehicle would shut off when idling at lights or in reverse and have delayed acceleration after stops. It also would shut off when idling in park. I took it to the dealership summer-fall 2025 and they were too busy to investigate the issue because no alarms were present. The vehicle started doing the same thing and the check engine light came on March 2026. Now it has been in the shop for many
... weeks because of an fuel injector issue. The dealership service department says all 6 injectors need to be replace, but warranty won't cover them all. This appears to be a common issue with 2021 Hyundai Palisade vehicles and should be investigated for a safety recall.Read more

A vehicle was taken to a collision repair facility following minor front-end damage. At intake, the repair facility documented visible exterior damage but did not perform or document an underbody inspection, which is necessary to assess high-voltage battery integrity in electric vehicles. After the vehicle entered teardown, the repair facility later claimed that the high-voltage battery was damag
... ed and recommended that the vehicle be declared a total loss. This determination was made without documented evidence of required OEM diagnostic procedures and without initial intake documentation supporting battery damage. The sequence and timing of documentation raises concern that the battery damage determination was made after teardown rather than through proper inspection protocols. A subsequent manufacturer-level evaluation did not support the claim of battery damage. This situation presents a safety concern regarding whether proper high-voltage battery inspection and handling procedures are being followed by collision repair facilities. Misidentifying or improperly handling high-voltage battery conditions in electric vehicles can pose safety risks, including potential thermal events, improper classification of vehicle safety, or unsafe return to service. This complaint is submitted to request review of whether proper safety and OEM procedures were followed in evaluating the high-voltage battery, and whether this reflects a broader safety issue in electric vehicle collision repair practices.Read more

I purchased the truck in 2021 from a chevy dealer with 32k miles on it, truck looked great and performed great, at 42k miles it started to be sluggish and it would vibrate between 30-40 and 60-70 as i feathered the gas pedal to maintain speed, but if i gave it more gas or take my foot off the pedal it would stop vibrating, I've taken it to 2 chevy dealers, the first one said it drives within GM sp
... ec's. the 2nd one looked more promising but my warranty ran out at 75k and i was at that mileage. So today at 93k on it, it still vibrates and sluggish from a start. I looked online and many people have similar problems. ALSO: The paint around my front wheels is peeling off down to the primer, I've had many vehicles and never had this happen to me, It's just flaking off, When i had the truck at the chevy dealer for maintenance and told him about it, He said GM will not fix it and they know about the issue. Now these trucks are not cheap, It seems like once they sell the vehicle GM washes their hands of it, I guess that's why they only give you 36,000 mile warranty because they don't even trust their product to last.Read more

I am submitting this complaint on behalf of the vehicle owner. On March 31, 2025 (136,962 miles), the vehicle was brought to Fred Anderson Nissan of [XXX] for Voluntary Emission Recall PD128 (NTB24-061), which required a new Particulate Matter Sensor and ECM reprogramming. One day later, the Check Engine Light illuminated. On April 2, 2025 (137,083 miles), the dealer diagnosed code P2002-00 (DPF
... Efficiency Below Specs). Repair Order #XXX states: "The new sensor that is now monitoring DPF is able to see this and is setting the code as designed." The dealer's own documentation confirms the fault code was triggered by the recall — not a pre-existing failure. The vehicle had no warning lights before the recall was performed. Nissan Consumer Affairs (Case #XXX) declined goodwill assistance citing mileage, without addressing the documented causal link between the recall and the fault code. The owner now faces a DPF replacement costing several thousand dollars as a direct result of complying with a manufacturer recall. Safety concern: A DPF in confirmed fault condition can trigger unexpected limp mode, including at highway speeds. If other PD128 recipients are experiencing the same recall-induced fault and cannot absorb the repair cost, those vehicles may be operating in a compromised condition. We request NHTSA investigate whether Recall PD128 is creating safety and repair liability for consumers who complied in good faith. Purchased a used 2022 Audi E-Tron GT with an open NHTSA safety recall for high-voltage battery fire risk (Recall 24V726 / Audi 931A). The recall remedy had not been performed at time of purchase. The selling dealer completed a pre-delivery inspection two weeks prior to sale with full knowledge of the open recall. Within 7 weeks of purchase, the vehicle entered limited performance mode (turtle mode
... ) while driving and had to be towed. A high-voltage coolant pump was diagnosed faulty and replaced. Days later, the vehicle powered down completely while driving. Fault codes indicated an implausible signal from the HV battery controller. The battery management controller (J840) was replaced. After replacement, additional faults were found. The HV battery switch box (SX6) was also diagnosed faulty and replaced. The vehicle was out of service approximately 18 days for this event alone. Within 11 months of purchase, the vehicle developed water intrusion in the trunk area traced to a faulty spoiler motor sealing surface. This caused corrosion to the center tail lamp connector, electrical shorts to all three tail lamps, and required repair of six wiring harness connections. A body electrical wiring repair was also documented in service history prior to purchase, suggesting a pre-existing sealing defect. Total: 9 dealer visits and 50+ days out of service in under 14 months. All repairs performed under manufacturer warranty. The open safety recall was not completed until 8 months after purchase.Read more

I was looking at common problems associated with the make and model of my car. I came across an issue that could possibly cause engine failure. I am unsure if any one has reported it yet. here are the details - Problem overview An VVT-i oil line on the Lexus RX350 is known to burst. This is a very large leak which covers the ground, engine, and undercarriage. If the oil line ruptures while driv

Re: Vehicle Safety Defect Complaint – 2023 Chevrolet Blazer To Whom It May Concern, I am submitting a formal safety defect complaint regarding my 2023 Chevrolet Blazer. This vehicle has exhibited multiple system failures that present a serious risk to driver safety and public roadway safety. The vehicle was purchased new on July 20, 2023. Beginning on April 21, 2025, at approximately 27,610 mile
... s, the vehicle developed a progressive defect initially presenting as oil-related issues. Despite multiple repair attempts, the condition worsened and required a full engine replacement in January 2026. Following the engine replacement, the vehicle continued to experience critical safety failures, including engine stalling, reduced engine power warnings, and electronic stability control (ESC) alerts. On April 26, 2026, the vehicle experienced a driver assistance system malfunction in which it attempted to steer off the roadway. I was forced to manually disable the system to maintain control of the vehicle. On April 27, 2026, the vehicle exhibited a fuel system failure, including fuel leakage and fuel odor, along with multiple simultaneous warning indicators: • Reduced Engine Power • Service ESC • Steering Assist Reduced / Drive with Care • Power assist driver system disabled This combination of failures presents both a fire hazard and a loss-of-control risk. Additionally, I was informed by the dealership that a high-pressure fuel line, a one-time-use component, was not replaced during the engine installation. This raises concerns regarding improper repair and continued unsafe operation of the vehicle. The vehicle has undergone multiple repair attempts and a major powertrain replacement, yet continues to present serious safety concerns. Due to these ongoing issues, I do not feel safe operating this vehicle. I respectfully request that NHTSA review this matter for potential safety defects that may impact other vehicles of the same make and model.Read more

I am reporting a safety defect involving my 2023 Kia Carnival SX Prestige. On June 6, 2025, I brought the vehicle to Yonkers Kia Service Center because of a strong gasoline odor. The dealership told me the fuel tank was swollen and that the vehicle was unsafe to drive. The vehicle remained at the dealership from June 6, 2025 through October 17, 2025. Kia later indicated that the fuel tank, caniste
... r, and vent valve had been replaced. When the vehicle was returned to me on October 17, 2025, it still smelled like gas. Within 24–48 hours, the same or related safety issues returned, including strong fuel odor, loss of engine power, check engine light, abnormal fuel consumption, and later a visible fuel leak, which I documented on video. Because the vehicle did not feel safe, I stopped driving it on October 19, 2025. In March 2026, the vehicle was towed back to Kia for renewed diagnosis. Kia later reported that one fuel injector was leaking and performed additional repairs. My VIN now shows open unrepaired Recall SC368 / NHTSA Recall 26V232. That recall states that the high-pressure cross-over fuel pipe nuts may loosen over time, resulting in a fuel leak and fire risk. The recall warning signs include fuel smell and/or check engine light, which directly overlap with the exact symptoms I repeatedly experienced and reported. I am reporting that this vehicle had repeat fuel-related safety failures, a prior extended repair attempt that did not resolve the issue, and was previously returned to me in what I believe was an unsafe condition.Read more
