Incident date: Jan 22, 2026
This complaint involves a 2023 Jeep Grand Wagoneer L Series III, a full-size luxury SUV. The vehicle has experienced repeated electrical and powertrain failures culminating in complete loss of propulsion. On 01/22/2026, while traveling approximately 45–50 mph, the vehicle abruptly shut down without warning. The engine lost power, dashboard displays went black, and multiple warning lights illumi
... nated simultaneously. I was forced to steer to the shoulder while a trailing vehicle had to swerve off the roadway to avoid a collision. This was a near-miss event and represents a serious safety hazard involving sudden loss of motive power at highway speed. Prior to that event, the vehicle had already stalled at a traffic light and had exhibited repeated check engine warnings, low-voltage faults, and module communication errors. The servicing dealer documented multiple diagnostic trouble codes affecting the Body Control Module (BCM), Ignition System Control Module (ISCM), and Powertrain Control Module (PCM). Manufacturer technical service bulletin software updates were applied in attempts to correct the issue. Despite these interventions, the electrical instability persisted. On 02/12/2026, only days after an auxiliary battery replacement intended to address voltage concerns, the vehicle would not start and required towing to the dealership at my expense. Additional fault codes are currently under review. The vehicle is also subject to an open recall with no remedy available, and the power running board motor has failed with replacement parts unavailable. This pattern of electrical failure, sudden shutdown in motion, and inability to reliably start presents an ongoing and unacceptable safety risk to occupants and surrounding motorists. The vehicle is available for inspection and has been repeatedly evaluated by an authorized dealer.Read more

Description of Incident: This is a recurring safety defect that has occurred multiple times from August 2023 to February 2026. The following description refers to the most severe incidents: 1. NATURE OF DEFECT: The vehicle experienced a sudden "Limp Mode" event where all dashboard warning lights illuminated simultaneously, followed by an immediate loss of propulsion while in motion. Subsequently,
... the vehicle suffered from a dangerous 3-second acceleration lag/delay. This 2023 Lexus LX 600 creates a life-threatening risk on public highways, as the driver cannot reliably control the vehicle's speed or power to avoid collisions. 2. MANUFACTURER’S ADMISSION: The vehicle has been presented to authorized Lexus dealerships. While the dealership could not provide a permanent fix, the manufacturer’s own internal tracking system now explicitly confirms: "REMEDY: NOT AVAILABLE." This is a formal admission by Lexus/Toyota that the vehicle has a catastrophic system failure with no known fix. 3. REQUESTED ACTION: Lexus/Toyota has refused to repurchase the vehicle, forcing me to drive a dangerous asset they admit they cannot repair. I request that NHTSA immediately initiate a safety investigation into this loss of propulsion and acceleration lag issue for the 2023 Lexus LX 600, especially given the "No Remedy" status. A mandatory recall is warranted.Read more

My vehicle is covered under Safety Recall 023G involving an ECU update, inspection for DTC P0088, and high-pressure fuel pump replacement if inspection criteria are met. The recall is listed as completed, however the high-pressure fuel pump was not replaced, and the dealer has refused replacement. Only an ECU update was performed. I continue to experience fuel pressure–related symptoms consist
... ent with the recall condition. The dealer has declined further action and has not provided documentation showing inspection results or justification for non-replacement. I believe the recall was improperly completed, creating a safety risk due to potential loss of engine power or stalling. I am requesting NHTSA review.Read more

The vehicle experienced a sudden loss of motive power and became inoperable due to failure of the high-pressure fuel system. This VIN is subject to an active safety recall (NHTSA 24V957 / Ford 24S78) addressing high-pressure fuel pump failure that can result in loss of motive power while driving. Prior to failure, the vehicle exhibited reduced engine performance and drivability issues consistent
... with fuel system malfunction. No Water-In-Fuel warning or alert was displayed prior to the failure. The vehicle was inspected by an authorized Ford dealership. Ford and the dealership asserted “fuel contamination” as the cause and refused to repair the vehicle under the recall or warranty unless the customer paid for diagnostic teardown. The recall repair was not performed. The vehicle remains unrepaired and was removed from the dealership. The loss of motive power created a safety risk to the occupants and other motorists. We are concerned that a recalled safety defect affecting the fuel system is not being remedied and is being mischaracterized to avoid recall repair.Read more

Vehicle stopped charging and will not charge using a level 1 or level 2 charger. It will charge at a level 3 charger only. Took it to a Hyundai dealership and they did a software update and said it was fixed. Took it home and still would not charge. Took it to another Hyundai dealership and they said it was an ICCU issue they had been seeing and it would need to be replaced. I have read from many

COMPONENT: Battery and Charging System SUMMARY: 2023 Tesla Model 3 Long Range lost 15% battery capacity (345→300 miles) in 2 months at 9,816 miles. Comparison vehicle shows this is a defect, not normal wear. DESCRIPTION: Vehicle: 2023 Tesla Model 3 Long Range, VIN: [insert VIN], 9,816 miles Defect: Between September 2025 and November 2025, displayed range at 100% charge dropped from 345 miles to
... 300 miles—a 15% loss in just 2 months. Evidence This Is A Defect: I own a second 2023 Model 3 (base model) with 66,680 miles that only dropped from 272→260 miles (4.4% loss). The Long Range with 7x FEWER miles has 3x MORE degradation, indicating a battery pack defect. Service Refusal: Tesla Service ran “range analysis” but refuses to test battery capacity or check for dead cells. Only provided scripted responses about EPA vs real-world driving, which doesn’t explain why my high-mileage vehicle has healthier batteries. Safety Concern: Rapid cell degradation may indicate failing cells, thermal management defect, or fire risk. Unpredictable range loss creates stranding risk. Request: Investigate 2023 Model 3 LR battery packs for systemic defects. Require Tesla to perform cell-level diagnostics and replace defective packs. Documentation available upon request.Read more

Driving down the road and the vehicle lost all fuel pressure and engine shut off in the middle of the road. Turns out to be a catastrophic Bosch CP4 failure. At approximately 10pm on Saturday night with temps in the single digits and a large snowstorm (that would cripple much of the US for several days) just about to hit, the 1.5 year old vehicle failed completely. Propulsion battery 81% charged, started normally, then gave "check electrical system" warning which upon search can be due to a low 12v battery; call AAA and spouse (live 2hrs away). Began
... to drive towards a station two blocks away, thinking plugging in may help system warm and/or recharge 12v battery. Car goes into limp mode on road, then dies in four-lane road. Use portable battery jumper, draining it into the 12v battery and able to limp-mode out of traffic and into Walmart parking lot entrance where dies again. With bystander help, push the vehicle across parking lot to chargers; vehicle in total electrical failure w/random lights flashing (tail, dash, etc.) and no command-functions (couldn't shift into park, open charging port). Charged 12v battery from an ICE car 30 minutes w/restoration of command function, but nothing more than limp mode. AAA still hours ETA. Personally swap 12v battery purchased from Walmart w/restoration of power, but abnormal function (lane assist forcing wheel, menus malfunctioning) & warnings remaining. Drive vehicle to -thankfully- nearby dealership before another failure. Abandon vehicle & wait for pickup by spouse, drive last hour in a snowstorm. Very lucky I wasn't stranded on the highway, hit and killed in a snowstorm. Very lucky I wasn't hit and killed in the road where it died. Very lucky I called for help early, wasn't stranded more than 20 minutes in dangerous-cold, didn't wreck in snowstorm getting home. Very lucky the malfunctioning systems did not steer/force me into another vehicle or pedestrian, causing injury or death. Dealership confirmed (longstanding and very well-known) failure of ICCU module that Hyundai refuses to properly address. These are unsafe vehicles, hand-grenades on a timer.Read more

On approximately January 24, 2026 my 2020 Chevrolet Silverado 2500HD equipped with the 6.6 Duramax Diesel displayed an error code (P20B9). DISCONNECT PUMP MODULE AND BURNED/OVERHEATED TERMINALS..PUMP MODULE AND HARNESS PART OF TANK ASSEMBLY... LABOR TIME ADDED TO PERFORM MALFUNCTION/TAMPER TEST TO CLEAR EMISSIONS MESSAGE...." I found the "burned/overheated terminals" comment to be alarming. I looked into this matter further and found a GM Service Bulletin (21-NA-006) from October 2022. This bulletin identified this issue which contained an image showing burned/overheated terminals. The identified cause within the bulletin states "This condition may be caused by a poor connection at the Emission Reduction Fluid Tank electrical connector." I believe this should upgraded to a safety recall due the the potential of this terminal connection catching fire which could be detrimental for the operator, passengers, property, and the public.Read more

My 4Runner is hard starting all the time. I’ve replaced the battery, tested the alternator etc. It sounds exactly like the problem of the fuel pump recall. The 4Runner was recalled from 2014-2015, my car was technically put together in Sept 2015, but is labeled a 2016. I believe my fuel pump is failing. I am extremely nervous it is going to not start one day while I am out camping.

This vehicle is subject to NHTSA Recall 25V467 involving potential fuel injector cracking and risk of fuel leaking onto the engine, creating a fire hazard. The recall status shows “Remedy Not Yet Available,” leaving the defect unresolved. While driving, the vehicle suddenly displayed an engine overheating warning and immediately entered limp mode, causing abrupt loss of power. The vehicle dec
... elerated unexpectedly and was not able to maintain normal driving speed. This created a dangerous situation in active traffic and required a safety tow. The sudden power reduction could have resulted in a rear-end collision or loss of vehicle control. There were no prior overheating warnings immediately before this incident. The failure occurred abruptly. A warning message appeared at the time of the event, followed by limp mode activation. The vehicle had to be towed to a dealership for inspection. Given that the open recall involves fuel injector cracking and potential fuel accumulation near hot engine components, there is concern that the unresolved recall condition may be related to engine overheating and loss of power. The vehicle remains under an active fire-risk recall, yet no repair remedy is available. Owners are left operating vehicles with a known safety defect and no corrective action timeline. The defect has not been remedied because parts are not available. The vehicle was inspected by a dealership after the overheating event. The primary safety concern is the combination of an open fire-risk recall and a sudden overheating/limp mode event causing abrupt power loss in traffic, creating a serious risk of injury. Due to this open recall with no available remedy, the vehicle cannot be listed or rented on a peer-to-peer vehicle sharing platform, resulting in loss of legitimate income. The vehicle was previously eligible for use, but is now restricted solely because the safety recall remains unresolved without a repair solution. Owners are financially impacted.Read more

As we drove toward the highway to begin our journey home in our 2024 RWD Ioniq 5 with our entire family and luggage in the vehicle, we received a "Check Electric Vehicle System" warning and immediately pulled over and called Hyundai Roadside Assistance. They initially were going to send someone to jump the car, assuming it was an ICE, but once we said it was an EV, they said they didn't have the
... equipment to jump an EV. We then spoke to Hyundai Customer Care, and we reported that we received the "Check Electric Vehicle System" warning. The HCC rep said that she did not see any issues with the vehicle on her end, so we would be OK to drive it. We specified that we had a 1.5 hour drive home and asked her explicitly if we would be OK to drive this length of time with this warning and make it home? She again said that we would be OK to drive it. After getting off the phone with her, we proceeded onto the highway and within about 2 minutes, received the "Stop Vehicle and Check Power Supply" warning and then, within a few seconds, began to lose power. This was extremely dangerous, as we were going down a hill on a highway frequented by semi trucks in the rain. Fortunately, we were still able to turn on the hazard lights and coast into a nearby hotel parking lot. As we were losing power, we were afraid we would not be able to get out of the car because all the doors were locked and the windows were up due to the rain, so we quickly unlocked two of the doors as soon as we pulled into the parking lot. One of us then had to stay with the car at all times because we couldn't lock or unlock the car. We eventually had the car towed to the Hyundai dealer closest to us who determined that the ICCU and fuse had failed and needed to be replaced, but they could only replace them with new parts of the same design that could potentially fail again; Hyundai does not yet have a fix for the ICCU failure.Read more

The fuel injector failed which cause safety issues such as reduced speed, failing safety issues, and the car is misfiring and not running safely. To Whom It May Concern: I am submitting this complaint to report what I believe to be a potential safety-related defect in my Honda vehicle, as well as an unresolved dispute with American Honda regarding responsibility for resulting damage. Recently, I was informed by my case manager at American Honda that my claim had been closed and that Honda would not cover the necessary repairs. When I reques
... ted an opportunity to escalate the matter, I was told that no escalation was available. I am seeking NHTSA’s review because the circumstances involve component failures that may affect vehicle emissions, performance, and potentially safety. The fuel injectors in my vehicle failed, this failure caused the catalytic converter to become damaged, andHonda accepted responsibility for the faulty fuel injectors. Honda has refused to cover the damage to the catalytic converter. Because the catalytic converter failure was reportedly caused by faulty fuel injectors—components for which Honda accepted responsibility—I believe the manufacturer should cover all resulting damage, regardless of warranty status. I request that the National Highway Traffic Safety Administration: Investigate whether similar fuel injector failures are occurring in Honda vehicles. Review whether these failures are causing catalytic converter or other engine damage. Determine whether the manufacturer is appropriately addressing these failures.Read more

NHTSA Complaint Text (Copy/Paste) Vehicle: 2020 Ford F-250 Super Duty 6.7L Power Stroke Recall: 24S78 – Powertrain Control Module (PCM) Software Update Complaint: Ford has issued Recall 24S78 for the high-pressure fuel pump defect on my 2020 F-250, but the company has not provided an actual remedy. The recall shows as open on my VIN, and Ford’s documentation says a PCM software update is the f

This leaves my truck exposed to a known safety defect with no available repair. The high-pressure fuel pump failure this recall addresses can cause sudden loss of power, engine stall, or complete shutdown while driving. Until a working remedy is released, I’m driving a truck that Ford has admitted has a serious safety defect, but they have not provided any fix or timeline. I am requesting NHTSA investigate why the remedy was withdrawn, why no replacement remedy has been provided, and why Ford is leaving owners with a known safety risk for an extended period. This is an expensive vehicle, and the defect can cause catastrophic engine damage and create a hazardous situation on the road. Owners deserve an effective recall remedy that is actually available. Several years ago, when I was driving the vehicle, I got a sudden "reduced propulsion alert" and the onboard coolant temperature gauge showed a significant spike in temperature. To avoid overheating, I had to pull over on the side of the highway. In the onboard fuse box, there was a blown F3 fuse. I replaced the fuse and the same issue happened again last year. After some online research, it is c
... lear that the EGR valve failed. Unplugging the valve and replacing the fuse has returned the vehicle to normal operations, but it's now out of compliance with local emissions regulations. GM has failed to properly produce replacement parts. As of Feb 11, 2026, a local GM dealer in Bloomington, IN quoted a backorder and unclear wait time on a replacement part and quoted me at $2,000+ just for the part. If not for my quick thinking, this part failure could have overheated my vehicle causing extensive damages. GM has failed to produce enough replacement parts and given the safety issues associated with a blown fuse and overheating, NHTSA should order a recall on this part and force GM to replace the parts for owners. This is a widespread problem documented among Volt owners, and the only stopgap issue is to drive the vehicle out of compliance with local emissions rules.Read more

Hello, I have a 2021 Honda Accord Sport 2.0T Engine. The engine has a 2.0 liter turbo. I have had to replace two fuel pumps, which were covered my extended warranty. Another issue that I had to repair, and one that has not fully been resolved is the head gasket. I have been losing coolant at an alarming rate. Every two weeks I have to fill up coolant. I had the head gasket repaired at a Honda Dea
... lership, and this is currently being diagnosed by another Honda Dealership, as I do not want to deal with the Honda Dealership in Cedar Rapids, Iowa. Currently, a Honda Dealership in Iowa City, Iowa is handling the issue, but the head gasket along with the cylinder head were originally repaired in October 2025. Since December 2025, I have been losing coolant on a regular basis. I still have 5,000 miles left on my extended warranty, but there have been many owners who have had their head gasket replaced, or are needing replacement, and Honda refuses to acknowledge that there is a manufacturing defect with the engine. The 2.0 liter turbo engine has a major design flaw with it's cooling passages, and Honda has yet to come up with a resolution. Having an engine overheat is a major safety concern! The current dealership is driving my vehicle on a regular basis, in fact the service advisor is driving it to work and back home (35 miles each way) to replicate the problem. At this point, a recall should be issued by Honda, as there is currently a class action lawsuit that has occurred in the state of California.Read more

Warranty has now expired, and an independent scan shows ongoing network/data bus/12V monitoring faults consistent with continuing electrical/network instability. I am requesting NHTSA review for a potential safety defect investigation/recall.Read more

Driving under normal conditions, heard a “pop” near the rear of the car and within seconds had an immediate loss of power. The dashboard had lit up with “Check electric vehicle system” and was limited to 20 MPH on a 45 MPH road with no safe place to stop. Was able to limp car home, dealer confirmed ICCU was faulty and had to be replaced. My family was very very lucky to not be on a highway
... when this occurred. Hyundai has continuously failed to not only fix the ICCU issues with this vehicle, but also hasn’t extended the warranty on the faulty component here in the US like they’ve done in Asian markets. As a consumer, I’m begging the NHTSA to hold Hyundai accountable for these ongoing EV issues.Read more

My vehicle failed while in motion. I lost power to the vehicle and power steering. Just before the car failed, I heard a pop and a warning light came on that said "Check Electric Vehicle System". The car's ICCU failed and caused failures of the battery and 12v battery. My safety and everyone on the road around me's safety was put at risk when I lost the ability to accelerate or use power steering.
... The car was towed to a dealer and the dealer confirmed the issue. The ICCU failure and failure of the vehicle while in motion is directly related to a previous safety recall (025G) for which repair work was done on my vehicle. The recall repair was ineffective as the very issue it was intended to resolve still happened to me in a 1 year old car with only 8,000 miles.Read more

There is a safety recall concerning fire hazard during normal operation. Kia is refusing to buyback the inoperable vehicle
Incident date: Dec 29, 2025
While driving at approximately 70 miles per hour, the vehicle experienced a sudden loss of power and felt as though the engine was going to shut off. This created an immediate safety hazard and forced me to pull over to the side of the road. Despite feeling unsafe, I continued driving directly to the dealership to have the vehicle inspected due to an engine warning light related to the fuel system
... . The vehicle has only been serviced and inspected by the dealership. I was informed that there is currently no available remedy for Recall 78C. With the recall unresolved, the vehicle continues to pose a significant safety risk. The potential for engine failure while driving at highway speeds places me, my passengers, other motorists, and nearby property in danger. Due to these conditions, I do not feel the vehicle is safe to operate or park until the recall issue is properly resolved.Read more
