Fire spreading to dozens of adjacent vehicles. The explosion occurred without any prior warning, posing a lethal threat to anyone nearby. 3. The failure was officially confirmed and documented through a State Forensic Examination. The Ministry of Internal Affairs’ Forensic-Criminalistic Department conducted a comprehensive technical audit. They ruled out any external factors (arson/accelerants) and confirmed an internal electrical short circuit as the sole cause. 4. Yes. The vehicle was inspected and seized as evidence by the Criminal Police and the State Forensic-Criminalistic Department of Georgia. 5. There were no warning lights or messages displayed on the dashboard prior to parking the vehicle. I am writing to formally advise Toyota Customer Relations of significant, unresolved performance and safety issues regarding my 2026 Toyota bZ, purchased new on February 14, 2026. Despite having only 976 miles on the odometer, this vehicle is currently demonstrating a battery state of health (SOH) of 82.7% and a maximum range of only 140 miles at 100% charge—even in moderate 40-degree temperatur
... es. Description of Technical Failures: Battery & Range: The vehicle provides a maximum range of 140 miles at full charge. On an 80-mile roundtrip commute, I am forced to drive without cabin heat to ensure I can return home without a mid-trip recharge. Internal diagnostics currently show an 82.7% battery health, which is unacceptable for a vehicle less than a month old. Regenerative Braking: The regeneration system is not functioning properly, failing to recoup energy as designed. Infotainment & Safety: I have experienced consistent Bluetooth disconnections, audio drops during calls, map freezing, and—most concerning—the screen blacking out and resetting while the vehicle is in motion. Social Media Documentation & Peer Reports: Due to the persistent nature of these issues, I have documented my experience on the official "Toyota bZ (2026)" Facebook owner groups. My post has received numerous responses from other owners experiencing similar defects, suggesting a systemic issue rather than an isolated incident.Read more

Component/System: Low-pressure fuel pump/fuel delivery system related to Recall 25S75. Vehicle is a 2022 Ford Explorer Hybrid. Description: My vehicle is subject to Recall 25S75 involving the low-pressure fuel pump that can cause loss of fuel pressure and engine stalling. The vehicle has stalled six times between about 15,000 and 35,000 miles. The issue is intermittent but ongoing. The vehicle is
... available for inspection. What failed or malfunctioned: The fuel delivery system appears to lose pressure consistent with Recall 25S75, leading to engine stalling and temporary inability to restart. Safety risk: One stall occurred while driving on a highway on 9/30/23 at about 15,000 miles. The engine lost power without warning and I had to coast to a gas station. Sudden loss of propulsion in traffic creates a serious crash risk. Other stalls happened in driveways or parking lots after the vehicle had been running. In several cases the vehicle would not restart for 20–60 minutes, which could be dangerous if it occurred in traffic or an unsafe location. Reproduction or dealer confirmation: Two incidents required towing to Ford dealerships. The vehicle has been taken to Ford dealers multiple times. Earlier incidents were not linked to a recall at the time. Now that Recall 25S75 has been issued, the symptoms match. Ford has not repaired it because the remedy is still under development and parts are unavailable. Inspections: The vehicle has been inspected by Ford dealerships and towed twice by AAA for diagnosis. No permanent fix has been made because the recall remedy is not yet available. Warnings or symptoms: Stalls often occurred after the vehicle had been running. The vehicle then would not restart for 20–60 minutes. In one event there were dashboard errors. There was not always a clear warning before a stall. Reason for complaint: The recall has been open since July 2025 with no remedy available. The vehicle continues to stall. I do not feel it is safe to operate.Read more

Major fuel leak within engine bay
Incident date: Nov 26, 2025
While driving this vehicle under normal conditions, the engine developed abnormal ticking/knocking noises, loss of power, and misfire symptoms that progressively worsened. The vehicle was frequently used to transport children. Due to the nature of the failure and unpredictability of progression, continued operation posed a serious safety risk. An authorized dealer later confirmed that the engine
... suffered a catastrophic internal valvetrain failure involving collapsed lifters/rocker arms and severely damaged camshafts. The dealer technician explicitly stated this failure mode is “typical” for this engine design and documented progressive internal damage affecting both cylinder banks, not an isolated component. This failure mode is not sudden; it develops gradually over tens of thousands of miles and can lead to sudden engine power loss, stalling, or seizure without warning. A loss of propulsion at highway speed, during lane changes, intersections, or while transporting children creates a clear risk of crash, injury, or worse. There was no evidence of driver abuse, lack of maintenance, lubrication failure, or misuse. The dealer confirmed oil condition and service history were appropriate. The defect appears to be internal, latent, and manufacturing-related, consistent with numerous consumer complaints, technical service bulletins, and ongoing litigation involving this engine family. Had the vehicle continued to be driven, the failure could have escalated to: •Sudden loss of engine power at speed •Inability to accelerate to avoid hazards •Engine seizure causing loss of steering assist •Increased crash risk with child passengers onboard The vehicle was removed from service due to safety concerns. This defect is repeatable, predictable, and well-documented, yet not subject to recall at this time. I am reporting this to support investigation into a systemic engine defect that presents a serious safety risk to drivers and passengers.Read more

While driving, the vehicle loses power, hesitates, and jerks, then stalls and shuts off without warning, especially in extreme heat and stop-and-go traffic. The failure occurs more frequently when the fuel level drops below approximately half a tank. After stalling, the vehicle will not restart until it cools down for 20–30 minutes, then temporarily operates normally before the issue repeats.
... Warning symptoms prior to failure include loss of acceleration, hesitation, and engine overheating. A fuel pump–related fault code has been detected. The vehicle has stalled while driving, creating a serious safety risk due to sudden loss of power in traffic. This condition matches NHTSA Recall 23V858000, which states the fuel pump may fail and cause engine stall while driving. The fuel system component believed to be involved is available for inspection upon request. The issue has occurred multiple times and is reproducible under similar conditions.Read more

Component / System Failure The exhaust system, specifically the exhaust manifolds and previously the EGR system components, appear to have failed or malfunctioned, allowing exhaust gases to enter the passenger cabin. The dealer initially replaced the EGR cooler diverter on December 23, 2025. However, the condition persisted. On February 3, 2026, cracked exhaust manifolds were identified by the d
... ealership, and a major front-end teardown was advised to complete the repair. The vehicle is currently available for inspection upon request. ⸻ How Safety Was Put at Risk The malfunction allowed exhaust gases, including carbon monoxide, to enter the passenger cabin during vehicle operation. Carbon monoxide readings inside the cabin reached up to 50 parts per million (ppm) while driving. Carbon monoxide is a toxic gas that can cause: •Headaches •Dizziness •Impaired concentration •Nausea •Vision disturbances Because carbon monoxide is odorless and colorless, this presents a significant safety risk, particularly at highway speeds where driver alertness is critical. The exposure occurred while operating the vehicle in normal driving conditions. This created a potential risk of driver impairment and increased risk of collision. ⸻ Has the Problem Been Reproduced or Confirmed? Yes. The vehicle was presented to the dealership three times for the same exhaust intrusion concern: •December 16, 2025 •December 29, 2025 •February 3, 2026 During the February 3 visit, the dealership confirmed cracked exhaust manifolds requiring major disassembly for repair. The carbon monoxide readings were observed during vehicle operation. The exhaust odor and intrusion symptoms were consistently present following the first attempted repair. ⸻ Has the Vehicle Been Inspected by Manufacturer or Others? The vehicle has been inspected by the authorized dealership on multiple occasions. Two manufacturer customer care cases were opened.Read more

• Component/System: The high-voltage battery system in the plug-in hybrid propulsion system is subject to an active safety recall related to potential fire risk. The vehicle is available for inspection upon request. • Safety Risk: The manufacturer has instructed owners not to charge the vehicle near structures and to park it away from buildings due to the risk of battery fire. This creates an
... ongoing safety concern because normal charging and garaging cannot be performed safely, altering how and where the vehicle can be operated and stored. After speaking with my homeowners insurance, if the home were to catch fire through this vehicle, the claim would be denied because there is an active recall stating not to charge the vehicle. • Reproduction/Confirmation: The condition has been confirmed by the manufacturer through the official recall notice. The dealer has not attempted a repair because no remedy is currently available. Despite visiting the dealer, the dealer still states not to charge the car in the house. • Inspections by Others: The vehicle has not required inspection by police or insurance representatives. The issue is acknowledged by the manufacturer as part of the recall, and no repair procedure has been released to dealers. • Warning Lamps/Symptoms: There were no specific warning lamps prior to receiving the recall notice. The issue was identified by the manufacturer and communicated through recall notification rather than a driver-observed failure. The recall notice is the first indication of the problem.Read more

Ford Customer Service told me over the phone that they are “hoping” to have the parts to fix my truck in Q3 of this year (2026). I purchased this truck as it fit my everyday needs. I often tow which made this truck a good candidate. As I understand, the truck is not currently safe to drive, and definitely not capable of towing. Not having a capable truck is unacceptable. I own a 2018 Ford F-150, with the Cougar V-8 and a turbocharger. A few years ago, while on the expressway, I had to accelerate to clear a dangerous situation. A message came up on the instrument cluster saying that I was seriously low on oil. I pulled off on the shoulder and shut the engine off. I checked the oil level and it was almost empty. The truck only had about 10,000 miles on it at the tim
... e. I had checked the level prior to leaving and it was full. Luckily, I carry extra oil on long trips, a habit I got into back when I used to drive beaters. I had enough oil to fill the engine back up with oil and proceed on my way. I have not had any oil loss since then, although I rarely drive the truck, except to pick up lumber or other large loads, and it currently has about 23,000 miles on it. A few years ago, I read about an F-150 whose driver tried to pass a slow moving vehicle on a mountain road and was hit by an oncoming vehicle. Everyone in the truck was killed. When I read that article, I thought back to what happened to me. Luckily, I had room to pull over. I believe that when I did that hard acceleration (as the driver on the mountain road must have done), a large amount of engine oil must have been sucked out through the turbocharger, emptying the crankcase. If this happened to the driver in the article, he would have been left with no engine power and no way to complete the passing maneuver. This is all speculation on my part, but an emergency hard acceleration should not empty any engine of oil, leaving the vehicle's driver and their passengers defenseless.Read more

(Car was in owners possession for no more than 15 hours, and was driven 25 miles on the highway when it switched to neutral and shut down). Second attempt: The vehicle was towed back to the dealership. Additional electrical repairs were then attempted. (Fuse box) It shut down again during dealer testing. Third attempt: Subsequent internal inspection revealed cylinders 5 and 6 seized, requiring full engine replacement. This represents: • Repeated loss of motive power • Escalation from top-end failure to catastrophic lower-end seizure • Failure occurring immediately after dealer-performed repair(s) • A 2024 model-year vehicle with relatively low mileage Given NHTSA’s ongoing investigations into post-remedy failures (RQ26001) and out-of-scope bearing failures (EA25007), I request that this incident be evaluated as part of the broader defect pattern analysis.Read more
Incident date: Jan 16, 2026
Vehicle was purchased new on 5/17/2024. On January 16, 2026 the vehicle was brought into a Honda dealership because of 2 lingering issues. 1) It had low power while driving uphill on the freeway. When driving the vehicle from the Bay Area to Reno, the vehicle was unable to keep up with the flow of traffic travelling at the speed limit. Vehicles behind us were agitated. We did not know if thi
... s was normal or not and we wanted to have the car checked out. 2) The engine sounded different after the trip to Reno. Youtube videos, including some by Honda dealers had advised that the crv hybrid made noises which were considered normal. We wanted to know if the noise that we we're hearing was normal. The vehicle was in the shop for 26 days. The dealer confirmed the low power but had difficulty diagnosing the engine noise, except that it was not normal. The dealer brought in resources from Honda techline on multiple occasions to assist in diagnosis. During the course of the 26 days in the shop, the dealer ultimately found it necessary to rebuild the entire engine, including replacing the block, cylinder heads, lifters, fuel injector, etc.... We feel fortunate that we did not experience the more extreme loss of power and limp home mode that has been reported. We are very concerned about the future reliability of the vehicle. From the research that we have done, it appears that the primary cause of many of the engine failures experienced by this year/model are due to failure of the fuel injector system. What is really odd, is that the fuel injector system on our vehicle was inspected by the dealer on February 22, 2025, i.e., a year before we brought the vehicle in for the current issues.Read more
