I am the owner of a 2024 Jeep Wrangler Rubicon X 4xe (JL PHEV). The vehicle is subject to safety recall 25V‑741 (manufacturer recall 68C) for high‑voltage battery fire risk, and also recall 78C affecting the 2.0L engine. For recall 68C, Jeep and NHTSA instruct owners not to charge the vehicle and to park it outside and away from structures and other vehicles because the high‑voltage battery
... could fail internally and start a fire, even when parked. This means I cannot safely park in my garage or charge at home. I have a [XXX] infant in the home, so any increased fire risk near the residence is especially concerning. Jeep has not yet provided a final remedy or a clear timeline for a fix for 68C, so this situation is indefinite. In practice, the vehicle now operates as a less‑efficient gas‑only vehicle, with reduced usable range, higher fuel costs, and loss of the plug‑in electric function I paid for. I am also instructed to leave a potentially defective high‑voltage battery pack on my property and outdoors, which feels unsafe for my family. In addition, recall 78C raises concerns about potential engine defects and possible fire risk or major engine damage, further impacting the safety and reliability of this vehicle. My dealer and the manufacturer have confirmed the recalls but so far have not provided a loaner, buy‑back, or other adequate remedy, despite the “park outside / do not charge” safety instructions and the presence of an infant in the home. I am requesting that NHTSA closely review these recalls, the lack of an available remedy, and the real‑world safety and hardship impact on owners who cannot safely park or charge their vehicles as originally intended. On top of this, I put about $10,000 down on the lease expecting to purchase the vehicle as a long‑term PHEV. That down payment now feels like a sunk cost into a product that cannot be safely used as advertised, and the situation is starting to resemble a potential lemon‑law case. My truck went into "limp" mode and had very little power. It was towed to the dealership where they diagnosed the issue as a bad solenoid causing the engine misfires. The solenoid was replaced, but the issue was not fixed. Now the dealership is stating the engine has bent lifters and the repairs will not be covered under the ongoing massive engine recall. The vehicle had the pico test on 9/17 and

Safety Risk: The sudden loss of power in an intersection put me and other drivers at risk of a high-speed side-impact or rear-end collision. Warning Symptoms: The "Check Transmission" message appeared moments before the failure. Inspection Status: We have had the recall taken care of on January 25 , 2023 with 8810 miles at Peoria KIA initially. My car has always had slight issue getting consistent fuel pressure causing the engine to rev abnormally when giving gas during acceleration. On 11/25, my car failed to start due to the fuel pressure being too low. I brought my car into two mechanics including a Honda certified dealership to inspect an issue with my fuel pump. I was informed that other Honda Pilots of the same year as mine have a

I experience a lack of power issue when driving the vehicle and attempting to accelerate past 40-60 mph. I will floor the accelerator and the car still stalls and will not accelerate. This has been terrifying when merging onto the highway and realizing this issue is occurring, as it is sporadic and not noticeable on residential/low speed drives. In the videos I've taken, you can see the engine RPM
... increase to approximately 4k RPM; the gear indicator continues to show "D," indicating the RPM increase was due to a throttle-induced downshift as opposed to shifting with the paddles; and the vehicle speed fails to increase as would be expected at the necessary throttle application for the RPM to be at that level. I have taken my vehicle to to the genesis service dealership at least 5 times for this issue and it is still a recurring issue. Many genesis owners complain about this issue on forums with no resolution from Genesis.Read more

While driving my brand new 4-day old 2025 Audi Q3 at highway speeds, the vehicle suddenly lost engine power and shut off without warning. At the time of the incident the fuel gauge indicated approximately 1/4 tank remaining and there were no prior warnings. I was forced to pull over in traffic, creating a dangerous situation. The vehicle had to be towed to the dealership. I was initially told th

This same failure occurred a second time under similar conditions, again while driving at highway speeds with fuel indicated on the gauge. The vehicle stalled and shut off unexpectedly, requiring another tow. During the second repair visit, Audi technicians confirmed that the fuel tank's internal level sensor was defective and that the vehicle was inaccurately reporting fuel levels. Audi's Technical Assistance Center instructed the dealer to replace the entire fuel tank assembly on this brand-new vehicle. This defect caused repeated loss of engine power while driving, creating a serious safety risk to myself and other motorists. While driving under normal conditions, my 2023 Toyota Tundra TRD Pro Hybrid experienced a sudden and severe loss of propulsion. The vehicle abruptly entered fail-safe (limp) mode and would not accelerate beyond approximately 5–10 mph. This occurred without warning and created an immediate safety hazard in traffic. Multiple warning messages illuminated simultaneously, including: Hybrid System Ma
... lfunction, Check Engine, Smart Stop System Malfunction, and Advanced Ultrasonic Detection Malfunction. The hybrid system effectively shut down and the vehicle could not maintain normal road speed. I brought the vehicle to a Toyota dealership. The dealership performed only ECM and hybrid ECU software updates and cleared codes. No mechanical inspection, oil pressure analysis, particulate inspection, turbo system evaluation, hybrid motor synchronization testing, or engineering-level diagnostics were performed. Toyota Corporate subsequently stated that the software update was the “final” resolution and declined further diagnostics. I was advised to return to the dealership if the issue reoccurs. This failure mode matches the documented loss-of-propulsion behavior described in Toyota’s V35A engine machining-defect recall (sudden power loss, limp mode, cascading system failures). Hybrid models are currently excluded despite using the same engine platform and exhibiting identical symptoms. Sudden loss of propulsion at road speed presents a serious crash risk. This appears to be a safety-related defect that is not being adequately investigated or remedied for hybrid models.Read more

The fuel delivery system, specifically the fuel pump, has malfunctioned. This vehicle is subject to a fuel pump safety recall, which was reportedly repaired in 2022; however, the vehicle continues to experience the same failure symptoms and is available for inspection upon request. The vehicle has stalled multiple times at highway speeds, resulting in sudden loss of power while driving and creatin
... g an immediate risk of collision with surrounding traffic. My family was in the vehicle during these incidents, significantly increasing the risk of serious injury or death. The problem has been reproduced multiple times, including after the vehicle was taken to an authorized Acura dealership. During the most recent visit, diagnostic trouble codes were cleared, but the recalled fuel pump was not repaired or replaced. After being released by the dealership, the vehicle stalled again under similar driving conditions. The vehicle was inspected by the dealership, but no corrective repair was performed beyond clearing codes. The manufacturer has not yet properly remedied the recurring safety defect. Prior to stalling, the vehicle exhibited intermittent hesitation, loss of power, and engine shutdown. Warning indicators and fault codes were present at times and were cleared by the dealership without resolving the underlying issue. These symptoms have continued intermittently since the recall repair. This appears to be a failure to properly diagnose and correct a known safety defect, resulting in repeated dangerous highway stalls and placing occupants at serious risk.Read more

I am reporting ongoing safety concerns with my 2018 RAM 2500 (6.7L Cummins) related to repeated emissions/aftertreatment failures and the lack of timely repair options. These failures directly affect drivability and can create hazardous situations if the vehicle enters derate mode while towing or driving at highway speeds. Over the past year, my truck has suffered multiple emissions-related compo
... nent failures, including DEF dosing issues, SCR/DPF system faults, repeated warning messages, and the threat of speed-limiting derates. The vehicle is used for normal personal use, is well-maintained, and has not been modified. Despite proper maintenance, the same types of failures continue to recur. A major concern is that local RAM dealers have extremely limited ability to service these issues in a timely manner. Appointments are often weeks out, and when the truck is already in a failure state it is unsafe to continue driving. I have previously reported safety-related defects to NHTSA (ABS/HCU), and those issues later resulted in recalls—so I know that reporting matters. The combination of emissions failures, derate risks, and lack of dealer support creates a situation where the vehicle may suddenly lose power or become unsafe to operate, especially when towing or traveling in rural areas with limited service availability. I am requesting that NHTSA review whether there is a broader pattern of emissions-system failures on RAM heavy-duty trucks with the 6.7 Cummins and whether these failures pose a safety risk due to the forced-derate logic and inability to obtain timely repairs.Read more

Case Number: 1170739 – Additional Evidence Submission I would like to provide an additional document for Case #1170739. The manufacturer (Porsche Cars North America) stated in writing that the vehicle is “safe to drive even with warning indicators illuminated,” but in the same communication offered a rental vehicle if I found this “inconvenient.” These two statements cannot logically c
... oexist, and they raise concerns that the manufacturer may be minimizing or mischaracterizing a potential safety-related defect. Furthermore: The warning indicators involve the coolant and fuel systems of an E-Hybrid model, which may be connected to thermal management and combustion-related safety. The vehicle remained in the dealer’s possession for an extended period during repeated repair attempts. During a visit, I found the hood partially open while the vehicle was being stored long-term, raising questions regarding proper safety and storage protocols. Additional system warnings appeared after prolonged non-operation, suggesting possible deterioration related to improper storage or incomplete diagnostics. I respectfully request NHTSA’s assessment of whether it is appropriate for a dealer or the manufacturer (Porsche Cars North America) to advise a customer to operate a vehicle on public roads while warning indicators are illuminated. From a consumer’s standpoint, this does not appear reasonable or safe, and I wish to operate my vehicle only in a condition that is objectively safe for road use. The fact that such guidance was provided to me is deeply concerning, and I would appreciate the agency’s evaluation of whether this type of advice aligns with I am submitting the manufacturer’s email for your review, as it appears inconsistent with standard safety guidance and may reflect an attempt to address the matter through customer-service accommodations rather than technical evaluation. Vehicle: 2025 Honda Pilot VIN: 5FNYG1H44SB075283 Mileage: ~6,328 miles Summary of Safety Defect: Serious acceleration failure combined with existing safety recalls (brake pedal pivot pin defect and improperly routed third-row seat belt). Vehicle hesitates, fails to accelerate when needed, and creates dangerous crossing and merging conditions. Detailed Description: I own a 2025 Honda Pilot that is
... currently subject to multiple safety recalls, including the brake pedal pivot pin recall and the third-row seat belt routing defect. In addition to these known defects, the vehicle has a severe and unpredictable acceleration failure that creates a significant safety hazard. On December 10, 2025, I attempted to turn left onto a main road from a complete stop. When I pressed the accelerator, the vehicle failed to accelerate quickly enough to clear oncoming traffic. The throttle response was delayed, sluggish, and unresponsive for several seconds. An oncoming driver had to brake hard to avoid hitting my vehicle. This was a near-collision and placed me and others in immediate danger. This hesitation and lack of power has occurred multiple times when: Turning left across traffic Merging onto highways Changing lanes at speed Attempting to accelerate from a stop The vehicle does not respond appropriately or consistently when pressing the accelerator. This is especially dangerous in situations requiring timely throttle response to avoid collisions. Combined with the existing factory safety recalls, this creates a serious and ongoing safety risk. A new vehicle should not fail to accelerate when entering or crossing traffic. I have already notified Honda in writing. I am requesting NHTSA to investigate this issue, as I believe this is a defect affecting the safe operation of the vehicle, particularly in situations requiring quick acceleration. This defect is significant enough that I no longer feel safe driving the vehicle.Read more

The vehicle is affected by manufacturer recall R25C8 (NHTSA 25V-655), which identifies a risk of battery overheating and potential fire during fast charging. The warning light first appeared within the first week of ownership and has remained intermittent. The vehicle shows signs of unstable engine operation and ongoing system malfunction. The issue has been observed by the dealer, but they declined to perform proper diagnostics or repair. The defects are persistent and pose ongoing safety concerns.Read more

Since I purchased this vehicle, it has experienced severe safety-related failures in the electrical system, battery, vacuum pump, powertrain, and other components. All warning lights would activate on the dashboard, messages appeared such as “Service Immediately” and “Power Reduced,” the vehicle became limited to about 10 mph, and on several occasions it shut off completely while in use. S
... ix days before taking it to Midway Ford, Metro Ford replaced the battery, but within days the vehicle became inoperable again. Midway Ford kept the vehicle for weeks and eventually claimed the battery was defective, but attempted to charge me over $2,000 for sensors and a fuse box despite my Ford 5-year/8-year warranties and an extended service contract. I opened formal complaints with Ford, Metro Ford, and Midway Ford. Ford acknowledged factory defects, and the vehicle has more than six prior repairs since 2022, consistent with known issues in similar models (vacuum pump failures). During active state and federal investigations—and without my authorization—Midway Ford moved the vehicle off their property. I have evidence from the Ford app showing it was relocated to an apartment complex, then a private house, and finally to an unauthorized tow yard. When I returned to the country, police accompanied me to the location where I found the vehicle hidden, vandalized, with deflated tires, scratches, and surrounded by stripped and crashed cars. Midway Ford initially denied having the vehicle and later changed their statements. Afterward, all alerts were cleared from the Ford app without explanation. Midway Ford acted without permission, moved the vehicle during an open investigation, manipulated its condition, and returned it damaged. I request a full safety investigation due to repeated failures, loss of power, shutdowns while driving, and the dealer’s negligent handling of the vehicle.Read more

While driving my 2024 GMC Sierra Denali Ultimate on the highway, the engine suddenly failed without warning. The truck shut off completely while I was traveling at highway speed. All fuel injectors need to be replaced. I know 2 other people with CRV's that have had the same exact issue and had to have them replaced. It is also all over forums on the internet. Service people at Honda also told me it is a known issue with the fuel injectors in the Honda 4 cylinders until 2023. I have a quote for almost $1700. I currently own 2 Honda CRV's and one is a 2022 so I am expect
... ing that one to eventually need the fuel injectors also. I am bringing the car in for service on 12/15 to have this done. I feel like this should be investigated to bring a recall. When people that work at repair facilities say the only way to bring this to light is to report it to this agency and try to get a recall since Honda isn't going to issue the recall on their own.Read more
