I was leaving from a friends' house and got a "Check electric vehicle system" error and the car went into "limp" mode while on the road. I drove to a hotel parking lot and dropped off my family then proceeded the next day to drive the car to a local dealership for service and the car maxed out at 12 mph before crawling to 1 mph while going up a hill before I could find a place on the side of the
... road to stop. While awaiting a tow truck the car's electrical system fully "died" leaving me stranded on the side of the road without even hazard lights to warn surrounding traffic. The problem has not yet been evaluated by a dealership, manufacturer or any other agencies. Several warning lamps and messages appeared, including the "Check electric vehicle system" error message.Read more

This has happened since purchasing my personal 2006 Mustang GT new in 2006. I also see early S-197 Mustangs (2005—2007) in my shop that cannot be reliably refilled because fuel shoots out of the filler neck, splashes the operator, the side of the Mustang, and puddles on the ground. The defective On-board Refueling and Vapor Recovery (ORVR) system causes high internal fuel tank pressures that w
... ill also cause the filling station pump to cut off and prevent putting fuel in the tank. This creates a fire hazard as the spillage contacts the left hot tail pipe and muffler as well as a puddle of fuel that can be ignited by a spark, cigarette, etc. This problem is in direct violation of the Environmental Protection Agency emission standards that clearly state the operator is “expected to fill the fuel tank without spit back or spillage during the refueling event.” EPA emission regulations also require that service vents, also known as safety and vacuum relief valves, be installed in tanks to prevent excessive pressure or vacuum that can build up “due to tank filling or emptying.” The parts of the Ford ORVR system that contribute to the fuel tank pressure spikes and fuel spills include: 1.Poor fuel filler design. The filler pipe is narrow (5/8” to 3/4”) with a severe an S-bend before it reaches the tank. There is a spring-loaded plug at the top of tank built into the fill neck that closes the fill pipe until fuel pushes the plug down to open the fuel tank. 2.Poor fuel tank design. The saddle style tank has two sections that “saddle bag” over the drive shaft making the opening between the two tanks very narrow and at the top of the tank. 3.Insufficient tank venting system. The capacity of this vent system is not sufficient to vent the fuel tank during refueling. Please see attached file that documents the problem and possible solutions. I also recommend sharing your many complaints on this problem with the EPA as the problem is also in their area.Read more

I have a 2018 Honda Accord Sport 1.5 The issue started after getting my fuel pump replaced as part of the recall. While driving, all of the dashboard lights came on. Warning messages kept flashing on the dash. This car was driving perfectly fine before the fuel pump was replaced. When the codes are cleared, the car drives perfectly fine. The dashboard lights will periodically turn off but ultimat

When the car is scanned, the codes are reading a misfire with the coils but, the coils that are allegedly misfiring were JUST replaced. The car jerks while attempting to accelerate, and after a few miles, it goes into limp mode. I have tried every type of repair that has been suggested to me by the mechanic but the issues persist.Read more

On December 1, 2025, the 2025 Honda Prologue was pulling into a parking spot between two parked vehicles at low speed. While in forward gear, the Prologue experienced an unexpected acceleration event and collided with three parked vehicles, two of whom were hit with sufficient force to impact adjacent parked vehicles. The driver is unaware of having pressed down on the accelerator and is additiona
... lly concerned about why none of the vehicle safety features such as the equipped Collision Mitigation Braking System (CMBS) activated to prevent these collisions. The vehicle was brought to a local Honda dealership, yet they were unable to draw any conclusions regarding why the unexpected acceleration event occurred. The vehicle owner requested a DTC scan report and EDR (event data recording) data from the vehicle, and received the DTC report but not EDR data. The Honda dealership advised calling the American Honda customer service line with the case number to obtain EDR data. The customer service line was not able to assist because they were not able to identify the case manager, which points to a documentation error within Honda servicing processes.Read more

In 2023, the car had a rough start during cold weather. Honda confirmed there was a service bulletin and told me my fuel injectors were likely failing. It’s a known issue. For 2 winters, I used Sea Foam to manage the problem. This winter, the dash completely lit up. I returned to Honda for an update to new version of the same service bulletin. They did the fix but it did not solve the problem. T

I have a 2018 Hyundai Elantra with about 155k miles. It has been consuming oil excessively and has developed serious drivability issues. If I try to accelerate even slightly harder than normal—especially above 2,000 RPM—it makes a metallic, chain-type rattling noise, loses power, and produces heavy smoke from the exhaust. The smoke sometimes continues even after shutting the engine off and res
... tarting. Because of this, I’ve been avoiding rapid acceleration and slowly working up to highway speeds. The car also periodically shakes at idle, idles higher than normal at times, and the RPMs will occasionally rise on their own. In the past it has stalled randomly, and sometimes it feels like the transmission is “stuck” and won’t shift properly. Despite all this, there are currently no check-engine codes. This is terrible and clearly there’s been lawsuits and other recalls regarding the same issue and same engine but nothing for mine? I am out of warranty and had no clue the amount of issues the car was going to have until last year and I’m screwed now, insurance doesn’t cover issues with the engine or transmission. Neither will Hyundai?! Last year Hyundai performed a CVVT repair that cost about $6,000, which temporarily improved things, but the symptoms returned shortly afterward. When I brought it back, they said they couldn’t recreate the issue. The car is now out of warranty. Separately, the car was stolen two months ago due to the immobilization fault and TIk TOKs videos and the police said it has been hundreds of these Elantras stolen and destroyd and the ignition and wiper control switch/harness were damaged, which still hasn’t been repaired.Read more

I am submitting this as a supplemental statement to my existing NHTSA safety complaint regarding my electric vehicle, KIA EV6, 2023 model. Since my original filing, the vehicle has now experienced loss of power / loss of propulsion a total of three times. These incidents occur without warning, are unpredictable, and render the vehicle unsafe to operate. The vehicle can suddenly lose motive power,
... creating a serious risk of collision, particularly in traffic or at speed. The manufacturer and authorized dealership have been unable to identify the root cause or permanently repair the defect. Dealer documentation reflects that the issue “cannot be replicated,” and no corrective action beyond temporary battery charging or system reset has been performed. No explanation has been provided as to why the power loss occurred or how it will be prevented from recurring. Because the defect is intermittent and safety-critical, I no longer feel safe driving this vehicle and have stopped operating it entirely. A reasonable consumer would consider repeated, unexplained loss of propulsion to present an unreasonable risk to safety. I am submitting this update so NHTSA is aware that: The defect is recurrent (three incidents) The condition remains unresolved The vehicle is unsafe for continued use I respectfully request that NHTSA review this matter for potential investigation, as loss of propulsion in an electric vehicle poses a serious safety hazard to the driver and others on the road. Sirs: I am reporting TWO issues: One: My car should have been included in the recall campaign for rear coil spring fracture that other 2015 Golfs have experienced. Afterward, I made an appointment with another local VW dealer AT RANDOM, and one I had never done business with. Before I could identify myself or my VIN number this second dealer proceded to accuse me of being a deadbeat who does not pay his bills. He said if I came to his shop, I would pay in advance for any servicing. I would have to agree to surrender my car for an indeterminate time, and pay to strip it of all aftermarket parts at my expense, before even proceeding with diagnosis. It was apparent to me that the service manager had badmouthed me to ALL the local dealerships. I am in effect, blacklisted. I complained to Volkswagen corporation about this treatment in writing. I sent them proof of payment. They refused to help me. There is so much hostility coming from VW dealers, that I don't feel I can trust them to do safety recalls. I don't deserve this. I've always been polite and paid my bills immediately. The Ithaca dealer was paid for their diagnosis, even though they lied to me about the problem and never fixed the brakes. Subject: Consequential Damage and Failure of Remedy regarding Recall 24V-608 Incident Description: "I am reporting a failure of the manufacturer to provide an effective remedy for Safety Recall 24V-608 (Electric Water Pump) and subsequent consequential damage to the High-Voltage battery system. In September 2023, at approximately 78,000 miles, this vehicle exhibited 'Drivetrain Malfunctions' inc
... luding Fault Code 0x222626 (EME: internal fault - active short circuit requested) and 0x22271C (Overvoltage in intermediate circuit). The BMW dealer (Fields BMW) failed to diagnose the root cause and instructed me to 'keep driving'. Since the release of Recall 24V-608, it is clear these 2023 faults were early indicators of the coolant pump short-circuiting. This persistent electrical instability has now resulted in a permanent failure of Cell Supervision Circuit #3, causing the high-voltage contactors to open and leaving the vehicle disabled. The dealer is currently attempting to charge for diagnostics and battery removal. I am asserting that the lack of a timely remedy for the coolant pump (which was only recently shielded per the recall) caused a power surge that 'fried' the internal battery circuitry. This is a safety issue as the vehicle experienced a sudden loss of propulsion while in motion. I request that NHTSA investigate BMW’s refusal to cover battery failures caused by the electrical interference of the recalled coolant pump."Read more

Two safety defect incidents involving unintended acceleration. This defect has resulted in two loss of control incidents causing property damage. First incident: While attempting to move vehicle inside garage, I started engine with foot firmly on the brake, shifted the transmission into drive, and maintained full brake pressure. Immediately upon shifting, the vehicle bolted forward at an accelerat
... e rate, despite my foot remaining fully depressed on brake. Vehicle travel about 30 feet in only a few second, coming within three feet of a structural cinder-block wall supporting house beams before braking force finally overcame the surge. Avoided a serious collision required immediate corrective action and full driver control. Second incident: Again, vehicle parked in my driveway in front of storage garage. After briefly backing up to clear the driveway, I again shifted to drive with my foot firmly on brake. As with previous incident, the vehicle surged forward unexpectedly and forcefully, overcoming normal braking and collided with full-length of storage cabinet system inside garage, destroying the cabinetry and stopping the vehicle's forward motion. At the time, my brother-in-law was nearby. Had he been in the vehicle's path serious injury could have occurred. As in the first incident, avoiding a more catastrophic impact required immediate and precise driver intervention. Following these incidents, I presented the vehicle to Garnet Volkswagen of West Chester, 1620 Wilmington Pike, West Chester, PA 19382, on December 15, 2025. The dealership reported that: They were unable to replicate the condition. Diagnostic scans reportedly showed no stored fault codes. Incident date: Sep 22, 2025
The vehicle experienced a fuel/propulsion failure involving sudden loss of engine power. After internal engine repairs performed by the dealership (camshaft and valvetrain work), the vehicle was returned to me as repaired and safe to operate. Within minimal post-repair driving (less than one mile), the vehicle entered limp mode, lost propulsion, and the engine shut down while driving in active tra
... ffic. The vehicle required towing back to the dealership. The loss of propulsion occurred on a busy city roadway and created an immediate safety risk to me and surrounding motorists due to the sudden inability to maintain speed or power, with a foreseeable risk of collision. The problem has been confirmed by the dealership. After subsequent inspection and teardown, the dealer documented catastrophic internal engine damage, including cylinder wall scoring and a failed head gasket. The dealership acknowledged that deeper inspection during the prior internal engine repair would have revealed the need for engine replacement. The vehicle has been inspected by the dealership multiple times and by the dealership’s insurer. The manufacturer (Stellantis) has opened a customer care case. The vehicle is currently available for inspection and has been removed from service due to safety concerns. Prior to the failure, the vehicle displayed warning indicators and messages at the time of the incident, including limp mode activation and critical alerts immediately before and during the shutdown. No warnings were provided to me at the time the vehicle was returned from repair regarding unresolved internal engine risk or potential loss of propulsion.Read more

The next winter it shook and felt like it was going to shut down. I took it in for diagnosis while it was under warranty. They denied anything was wrong. This winter it did it again. It shook and the check engine light came on. Even though I told Kia service in Queensbury NY that a computer said there was a misfire in cylinder 4 they insisted that if the check engine light was not flashing I could drive it. They wanted me to wait almost two weeks to get it diagnosed. Had I driven it as they stated I could have ruined the engine, it could have caused a fire or for me to have an accident. I called back told them it was shaking and dangerous to drive. They allowed me to take it in earlier. I had it towed in. Now that it is out of warranty and 64000 miles they admitted the fuel injectors had to be replaced and the spark plugs. Because of the issues I had experienced from the beginning, I believe the fuel injectors had been faulty for a long time and could have been a serious risk for myself and other drivers. While driving, the vehicle experienced a sudden brake system malfunction without prior warning or gradual degradation. The vehicle was being operated under normal driving, road, and weather conditions. No recent brake service, repairs, or aftermarket modifications had been performed prior to the failure. Multiple warning lights illuminated on the dashboard, including the ABS warning light, brake
... warning light, and traction control warning light. Shortly after the warning lights appeared, the vehicle began emitting continuous warning beeps. Audible buzzing or pumping noises were also present from the brake system during the malfunction. During this event, brake performance was reduced and braking felt inconsistent. The brake pedal became hard at times and spongy at other times, requiring increased effort to slow the vehicle and resulting in unpredictable braking response. This created a safety concern while the vehicle was in motion. The driver was forced to alter driving behavior and exercise extreme caution due to reduced braking confidence and unpredictable pedal feel. The malfunction has occurred intermittently during normal driving. The condition temporarily resolves on its own and later reoccurs without corrective action. The suspected failed component is the brake actuator / ABS actuator assembly. A diagnostic scan retrieved fault codes C1252 (Brake Booster Pump Motor ON Time Abnormally Long), C1253 (Pump Motor Relay Malfunction), and C1256 (Accumulator Low Pressure), which are consistent with brake actuator malfunction. The failure occurred at approximately 154,000 miles. This vehicle model and generation have a documented history of premature brake actuator failures with similar safety-related symptoms reported by other owners. The condition places the driver and others at risk due to unpredictable braking behavior, reduced braking effectiveness, and repeated warning alarms while driving. Warning lamps and audible alerts continue intermittently.Read more

Reproducible and sudden loss of indicated driving range at low state of charge (SOC) for 6 months. First documented occurrence on 9 September 2025 during my final service visit while the vehicle was still covered under Tesla’s 8-year unlimited-mile high-voltage battery warranty, at which time no corrective action was taken. After repeated documented low-SOC range collapses, back to service on 13
... November 2025. Tesla acknowledged the fault, verified the condition with diagnostics and customer provided screenshots, and stated that CAN log data indicated the Battery Management System (BMS) was not calculating SOC correctly. Tesla replaced the BMS assembly under warranty as a continuation of the original issue and advised that the vehicle would need to be driven and charged for recalibration. Following this repair, I completed multiple full charge cycles to 100% and allowed time for recalibration. Despite this, the issue persisted with 100% reproducibility, with rapid loss of available range after driving and then parking when the vehicle indicated approximately 14–17 miles remaining. This condition resulted in a near-stranding event involving my family, including three young children, due to the unexpected loss of usable range without warning. I returned the vehicle again on 10 December 2025 to the Tesla Service Center in El Paso for the same unresolved issue. Tesla declined to perform low-SOC diagnostic or drive-down testing, instead stating the behavior was normal and required further recalibration despite prior calibration attempts and a failed warranty repair. Tesla has since stated that the vehicle is now out of warranty and has attempted to reclassify this unresolved condition as goodwill rather than treating it as a continuation of the original in-warranty defect. This condition presents a safety risk due to sudden and unexpected loss of usable driving range at low SOC, creating a risk of vehicle stranding.Read more

The vehicle is a 2016 Kia Sorento equipped with a 2.0L Turbo Theta II engine subject to the KSDS (Knock Sensor Detection System) safety recall. The original engine was replaced at approximately 70,000 miles under Kia’s engine service program. The replacement engine currently has approximately 32,000 miles. The vehicle developed internal engine damage diagnosed by a Kia dealership as a burnt exha

These are conditions the KSDS recall was intended to detect and mitigate. Kia Consumer Affairs denied warranty coverage, stating that the extended warranty applies only to rod bearing failure and that no further escalation would be pursued. Kia Consumer Affairs declined to escalate the matter for District Parts & Service Manager review and directed the owner back to the dealership. During the prior engine replacement, the dipstick was observed to be glued in place, raising concerns regarding oil level accuracy and lubrication integrity on the replacement engine. This complaint is submitted due to concern that a KSDS safety recall-related engine failure involving abnormal combustion was not properly reviewed or escalated, and that recall procedures were not administered in accordance with their intent.Read more

Vehicle: 2022 Audi Q5 (VIN [XXX] ) Mileage at incident: ~20,000 Component: Powertrain – Engine / Turbocharger – Sudden Loss of Power NARRATIVE: On or about [XXX], my 2022 Audi Q5 experienced a sudden loss of engine power while driving approximately 60 mph. The vehicle began vibrating/shaking severely, jerking on acceleration, and displayed warnings including “Drive system: Please switch eng
... ine off” along with a check engine light. The vehicle entered reduced power/limp mode, creating a hazardous situation in traffic at highway speeds. The vehicle was taken to an authorized Audi dealer in Wayzata BLVD Minneapolis who diagnosed catastrophic internal engine failure and documented metallic debris in the engine oil. Audi replaced the engine with a new long block (warranty repair). However, despite confirmed metal contamination in the oil system, Audi directed the dealer to flush and reuse the turbocharger rather than replace it. The turbocharger is supplied by engine oil and may retain embedded metal in its bearing assembly, creating an ongoing risk of turbo bearing seizure/failure and another sudden loss of power at highway speed. I am reporting this because (1) the vehicle already experienced sudden power loss at highway speed, and (2) the repair decision to reuse a contamination-exposed turbocharger may create continued risk of sudden loss of power and unsafe operation. Dealer repair: Engine replaced; turbocharger reused after “flush.” Current safety status: I do not feel the vehicle is safe for highway driving due to risk of turbo failure and sudden power loss. Requested action: Please log and review similar complaints for potential investigation related to sudden loss of power and contamination-exposed turbo reuse in warranty repairs. On 12/06/2025 I filled the vehicle with gasoline. As I was driving home I noticd a strong odor of gasoline. When I arrived home I parked the car in my carport. A short time later I went outside and noticed that I could still smell gasoline and I looked under the car, which was approximately 3 feet from my house and saw a puddle under it. This could have caused a fire, so I immediately took th
... e car to a repair shop. Approximately one hour later I received a call from the shop informing me that the tank was broken at the top and it would cost approximately $2,600 to replace the tank. The problem was confirmed by Gemba Automotive, Surprise, AZ. I am unaware of whether they have the part available for inspection. There were no warning messages or lights displayed on the driver information system.Read more

I was driving up the road truck cranked up fine started a morning commute, on cruise control the truck instantly out of nowhere lost power completely with no signs of anything and engine won’t attempt to turn over now. I fear the worst has happened which would be catastrophic internal failure for to a faulty engine & its internals.

Fuel tank failure at 18,000 miles and less than 3 years of ownership. I noticed that the vehicle had a strong gasoline smell when I was around it. When looking under the truck, I could see that there was a fuel leak coming from the fill pipe inlet into the fuel tank itself. Took the vehicle under warranty to the ford dealer, and was told that the hose clamp was loose connecting the fill pipe into
... the fuel tank itself. Was sent on my way with the vehicle, and noticed a smell of gasoline and fuel not even two months later. Took it back to the dealer and was told that they were unable to replicate it. Once the weather got cold again, took the vehicle back after noticing a much larger leak and smelling fuel, and am now being told by ford that the fuel tank does in fact need to be replaced as the tank is leaking from the fill pipe inlet. I am no longer under warranty and was quoted $1100 for this repair. Not only do I find this to be a safety concern as leaking gasoline is extremely dangerous and flammable (plus there are vapors in the air in my garage) but felt that my dealership did not do their due diligence to resolve this issue properly, and now I am on the hook for an $1100 repair that should’ve been taken care of when the vehicle was under warranty. No warning lamps were lit alerting me to this problem, I noticed it by smelling gasoline in my garage near the vehicle.Read more

While driving, the car all of a sudden lost power - forcing me to pull over. If this were to happen while on the freeway, a crash would have been imminent. This is a known issue with kia/Hyundai EV's where the ICCU fails and causes the vehicle to lose power suddenly.
