I am writing to formally request a review of my vehicle for inclusion in NHTSA Recall 21V-635, issued May 11, 2022, concerning fuel leaks at the fuel tank flange assembly. My vehicle is a 2012 Land Rover LR4, VIN [XXX] . The recall letter for 21V-635 states that all Land Rover LR4 vehicles model years 2010–2016 are included. My vehicle falls squarely within that model year range. Given this, and the fact that my vehicle is exhibiting the precise defect identified in the recall, I am concerned that my VIN may have been inadvertently excluded from the recall population. This issue presents a serious safety concern due to the risk of fire associated with fuel leakage. The symptoms and failure location match the recall description. I respectfully request that NHTSA review my case to determine whether: My VIN was mistakenly omitted from the recall population, or The recall population should be expanded or corrected to include my vehicle. I am prepared to provide photographs, repair estimates, diagnostic reports, or inspection documentation confirming the fuel leak at the flange assembly upon request. Because this defect involves fuel leakage and a significant potential fire hazard, I believe it warrants careful review to ensure all similarly affected vehicles are properly covered. On February 16 I started my vehicle to drive home from work and the vehicle threw multiple error codes and shutdown while driving and would not start again. It was towed to the dealership. The dealership advised the problem was remedied. 11 days later the same problem occurred, vehicle threw multiple error codes and shutdown while driving. Vehicle needed to be towed again to the dealership.

On the week of 10/13/2025, I had an oil sensor alert appear after a recent oil change. I took the vehicle in on 10/30/2025 for a diagnostic check and was told the oil separator sensor was defective and needed to be replaced. After the service was completed, the total was $906.42 of which I paid $580.99 out of pocket post my extended warranty claim payment. I received a recall notice in November

My 2025 Honda CRV Hybrid seems to randomly accelerate on its own. This has happened at least 4 or 5 times over the past year. At first I thought I was imagining it but then I noticed it happened again, multiple times. Each time I was NOT pressing the gas pedal nor at any time was I using cruise control. I called my local Honda Dealer Service Department and was told that unless I can recreate the random and spontaneous acceleration at the dealership then they won't be able to diagnosis or write for the issue. The Honda Service Rep said sense I have no idea when it is going to happen again he cannot set a appointment for me to come in without having a current issue. Because the acceleration is random and because I cannot recreate it at will, the problem has NOT been confirmed by a dealer or service center and the vehicle has not been inspected. The acceleration issue is random and spontaneous with no warning lamps, no messages and no other symptoms prior to. Once it stops it may no happen again for weeks or months. On 5 different occasions over a period of 6 months the motorcycle has cut off while slowing down or at a stop. Each time it has taken several attempts to get the motorcycle to start again. I picked up my vehicle and a week later 2/20/26 the same thing happened after I got gas. The check engine light came on and an emission warning on my Chevy app. I stopped back into the same dealership. And the same thing happened. Scanner, codes, and then they disappeared. The third time I got gas the same thing happened again with the check engine light and emission system warning. This was on 2/28/2026. I went to a different dealership and the exact same thing happened..... scanner, codes received, schedule an appointment to get it in the following week, started the vehicle and pulled out. The check engine light was still on at that time. And the check engine light disappeared once again as I was driving down the road. Each time this has happened I have been able to use my remote start via my Chevy app along with trying to use my handheld key fob remote start. The dealerships have told me this is normal for this type of check engine light emission warning. When researching this issue there are a lot of people experiencing the same thing. There were no prior warning signs before this incident; the warnings and loss of performance occurred suddenly while the vehicle was being driven under normal conditions. The sudden reduction in engine power created a safety concern due to the inability to maintain normal traffic speed. The estimated cost of repair provided by the dealership was approximately 7550. The vehicle had no prior warning lights or symptoms before the sudden loss of power. A sudden engine failure resulting in loss of propulsion at highway speed presents a serious safety risk to both the driver and surrounding traffic. This was an extremely dangerous failure.Read more

This places my vehicle in a substantially higher risk category than others in the recall population with no prior symptoms. A vehicle that has already experienced the exact failure mode identified in a safety recall presents a materially greater ongoing safety risk. Additionally, my vehicle suffered a complete ERAD (Electric Rear Axle Drive) failure — a known defect that Volvo acknowledged by releasing a redesigned ERAD2 unit, confirming awareness of this systemic issue. These repeated failures left my vehicle un-drivable and parked at the dealership for more than 6 months, with replacement parts repeatedly placed on backorder, causing severe and prolonged loss of use. I filed a formal complaint with Volvo Customer Care requesting a vehicle buyback. After months of inadequate communication, Volvo declined to buy back the vehicle or offer reasonable trade-in accommodation. Instead, Volvo offered a nominal monetary concession contingent on signing an NDA, which I declined as insufficient given the documented safety risk, repeated failures of a recalled component, extended loss of use, nor out of pocket car rental expenses. This vehicle has a considerate amount of oil dilution problems. I have had this vehicle taken in for service several times due to gasoline mixed into the engine oil. Again on March 6 2026 for the 4th time now the vehicle has a check engine light extreme hesitation with an almost stopping jerking motion due to dilution problems. Multiple dealerships claim there is nothing wrong with the car.. there
... is a distinct gasoline smell in the engine oil. The vehicle at times is almost unsafe to drive due to the hesitation and halting it does on its own. Every time a dealership is contacted my problem is dismissed. There is a burning smell due to the oil gas dilution issue. The car reaches higher temperatures than it should. And now the Hyundai dealership is making me wait almost a month again to have the problem looked at again which then puts me as the driver in an unsafe situation to drive a vehicle with such hesitation and fire risk. ... mbly went bad and had to be replaced. Upon the mechanic installing the new fan assembly, I had numerous fuses blown and that is when my blinker and hazard light issue started. This is a major safety concern as signaling the direction that you're turning is very important to other drivers. Additionally, it is against the law not to use signals. I'm constantly in fear that someone is going to rear in me because of my inability to signal when switching lanes and or turning. Additionally, my back up camera stopped working shows a black screen with a notice to contact the dealer. Another issue I have is that my door ajar light is illuminated all the time. I often feel unsafe and fear that the door may just come open at any time, I am unable to lock my doors with the internal lock , which means my doors are left unlocked while I'm in the vehicle. Also, the interior lights stay on for awhile when car turned off, thus putting more strain on the battery, which I've just replaced. Plus, the dome light will not go off until you accelerated to about 20 mph, however, when putting the car in reverse there's no way to get the lights to turn off in this causes visibility issues at night. That in conjunction with my backup camera not working makes it a very dangerous situation when going in reverse. I also detect the odor of gasoline which lingers in my vehicle for some time after getting fuel. I'm scared that there's a leak and if so the potential of a fire is very concerning. On February 25, 2026, BMW of South Miami performed the VANOS safety recall repair on my vehicle. The repair order documented that VANOS bolts were missing and retrieved from the engine oil pan and that VANOS components were serviced. Within approximately 24–48 hours of picking up the vehicle, the check engine light illuminated and the vehicle began operating abnormally, including rough idle and
... irregular shifting. The vehicle stored diagnostic fault code P054B (camshaft position timing over-retarded), which directly relates to camshaft timing and the VANOS system involved in the recall repair. I returned the vehicle to the dealership for further diagnosis. The dealership stated an aftermarket camshaft position sensor with two O-rings was installed and removed one O-ring to correct the gap between the sensor and the camshaft reluctor wheel. The faults were cleared and the vehicle was returned to me. However, the check engine light returned again the following morning upon cold start, indicating the issue was not corrected. My service advisor has since stated that replacing the camshaft position sensor is not part of the recall repair and the dealership has declined to repair the vehicle under the recall. Because the malfunction appeared immediately after the VANOS safety recall repair and involves the camshaft timing system connected to VANOS operation, I believe the recall repair may not have been properly completed or corrected. I am requesting that NHTSA review this matter as a potential recall effectiveness issue, as the safety recall repair appears unresolved and the vehicle continues to experience faults related to the system that was repaired.Read more

The CP4 pump in my truck exploded sending metal fragments through the fuel system. I was going 55 mph, it went into low engine power mode and then turned off leaving me stranded on the side on an interstate. This is a known issue with this model. There were no warning lamps, messages or symptoms prior to this happening. The fuel pump failure is catastrophic.

They stated the failure was due to “contaminated fuel,” but they did not provide any evidence or test results showing that contaminated fuel was present in the vehicle. Because catalytic converters are emissions components that are typically expected to last much longer and are often covered by extended emissions warranties, I am concerned this may represent a broader issue affecting consumers. ... y trip a chirping/ squeak type noise was heard under light throttle and light load at approximately 1500 rpm. The noise stopped when accelerator was released. Rough running sensation followed the noise under the load. Noise returned when the accelerator was pressed and then the engine stopped running while traveling 45-50 mph. Placed the shifter to neutral and attempted to restart, engine rolled slowly for a brief moment and then locked up entirely. Starter only clicks when attempted after that. Vehicle left me stranded with no heat on the shoulder of the highway in the dark in Northern Michigan on March 5, ambient temp was 28 degrees. ... agnostic health check at an authorized dealer confirmed stored DTC fault codes P281671 and P08BA71, indicating that Pressure Control Solenoids Actuators "H" and "L" were in a "stuck" state. The vehicle's Freeze Frame Data recorded a catastrophic internal transmission slip, showing an engine speed of 1767 RPM while the vehicle was moving at only 4 MPH in 2nd gear. There are no previous warning signs or symptoms of this problem before the transmission failure. It was sudden and unexpected. The manufacturer’s technical service protocols (such as T-SB-0094-24/T-SB-0076-24) for these specific solenoid codes on this specific 8-speed hybrid transmission (L580F) – which is equipped on my vehicle – identify this as an internal mechanical actuator failure requiring full transmission, torque converter, and hybrid module replacement. However, the manufacturer has denied the repair because the vehicle’s specific VIN is not yet listed on a service bulletin and that they could not reproduce it in a short test drive. The manufacturer’s official recommendation is to continue operating the vehicle with a documented and non-repaired powertrain defect until a second failure occurs in traffic. I am reporting this as a significant safety risk, as a recurring loss of motive power during critical driving maneuvers puts myself and other motorists in danger. fuel supply line from fuel tank at the engine is leaking on top of the valve cover causing a fire hazzard. The part is not available from the dealership as they said it is on national backorder this line is not damaged in any way and should not be leaking. I own this truck and have had issues with the exhaust regeneration process for about 6 months. I have taken it to the dealer 3 times and they have done diagnostics and replaced the diesel particulate filter, but the issue continues to happen. ... multiple customers with the same issue with the diesel exhaust system not operating properly, hampering the engine performance, damaging the engine and exhaust system, polluting from the exhaust discharge. This is a widely known issue with the 2022 Ram 3500 from other owners, yet RAM refuses to properly address the issue. They advertise their trucks with certain horsepower and output specs, which aren't accurate due to their faulty exhaust systems. Consumers are now completely removing the factoring exhaust systems because they damage the engines and cost thousands of dollars to attempt to repair, which many times never work. This needs to be addressed with RAM and recalls put in place to address the failures with their exhaust systems. My truck has 28,000 miles on it, 3 years old, and is not operable. Powertrain light came on and threw code p1450 and car went into limp mode. Car will stall at stops and lights and won't go over 40. I have had multiple occurrences where as I'm accelerating into a turn at normal /slow speeds the traction control activates and my vehicle almost comes to a stop almost causing an accident with the vehicles behind me. This was on dry pavement and not heavy acceleration so the traction system should not have activated.

When I try to open the fuel cover it wont' open. I have to lock and unlock my car, sometimes up to 10 times, to get the fuel fill door to open. This eventually is going to leave me stranded somewhere stranded. Some of my same year and model are covered under recall for this issue by when I call Hyundai my VIN is not covered. I would like it to be covered and repaired.
