While traveling from upstate New York, the vehicle displayed an engine overheating warning shortly after startup while the engine was still cold. Because I was away from home, I stayed overnight and the next morning took the vehicle to a Kia dealership for inspection. The dealership inspected the vehicle and diagnosed a problem with the exhaust heat recovery system pipe. According to the service
... department, coolant is leaking internally through this component and is being burned in the exhaust system. The cooling system was pressure tested and held pressure externally, indicating the coolant loss is occurring internally through the exhaust heat recovery system rather than from an external leak. This condition can cause coolant loss and overheating while driving. If the engine overheats while the vehicle is in operation, it could create a safety risk due to sudden loss of power or engine damage. The dealership confirmed the diagnosis after inspection and testing and provided a repair estimate to replace the affected exhaust heat recovery system component. The overheating warning message was the first symptom observed prior to diagnosis. The problem was confirmed by the dealership after inspection and testing. This issue may indicate a defect in the exhaust heat recovery or thermal management system used in this hybrid vehicle, as coolant is entering the exhaust stream internally rather than leaking externally. If this type of failure occurs in other vehicles, it could lead to overheating while driving and potential engine damage.Read more

My 2010 Volkswagen CC became completely inoperable because of the active fuel pump control module recall (NHTSA 17V509). The vehicle stalled and nearly caused an accident. The check engine light came on just before I noticed a burning smell coming from the backseat of my car immediately after smelling that the car suddenly stalled out in the middle of a busy intersection and the engine would not s
... tart back up. I had to push it to a gas station parking lot across 3 lanes of traffic. I lifted up the backseat and found that the fuel control module was overheated and melting on top of the fuel pump so I carefully and properly disconnected it from the vehicle. The next day I had the vehicle towed to Lokey Volkswagen in Clearwater, FL. Today 3/14/2026 They did the recall repair and told me my vehicle is safe to drive. Today 3/14/2026 I drove the car 1 mile from the dealership when the exact same thing happened again the car stalled out, and I had to carefully maneuver to the shoulder of hwy us 19 s narrowly missing vehicles in the other lanes. The vehicle was then towed back to lokey Volkswagen. It is pretty clear that the issue was not resolved, and that this fuel control module failing may have affected other parts of the fuel system/wiring and or the fuel control module failed again. I have pictures of the fuel control module, and I gave the initial melted/failed fuel control module to the service department and they took it. The dealership service department is stating that this may be to do something other than the recall repair and that I may be liable for the diagnostic and towing charges. This is a very serious safety issue that almost cost me to get in accidents on the highway twice. I feel that they (Volkswagen) needs to be held responsible for rectification and to find out if there is a connection between this recall and the continuing failure of the vehicle to operate safely.Read more

On February 21, 2026, my 2022 Cadillac Escalade ESV experienced a sudden and complete engine seizure at highway speed at 57,825 miles just 8,613 miles after passing GM's PicoScope inspection under Safety Recall N252494000 (NHTSA Campaign 25V274). After fueling, I entered a highway on-ramp and accelerated to approximately 70 mph. Within 30 seconds, the engine lost power completely with no prior war
... ning lights, no diagnostic trouble codes, and no audible warning. The only dashboard notification was a prompt to press the Start button. The vehicle behaved as if shifted into neutral, decelerating rapidly from 70 mph to 40 mph while I was navigating an exit merge ramp. I reached the right shoulder safely. Under normal weekday traffic, this failure on an active merge ramp could have caused a serious collision. The engine would not restart. The vehicle would not shift out of Park. OnStar could not remotely start the vehicle. A tow truck was dispatched; the operator could not shift the vehicle to neutral and required a dolly. The dealership confirmed the engine had seized and the engine would not crank. A complete engine replacement was performed under the recall warranty (RO 606490). The recall remedy was performed August 20, 2025 at 49,212 miles. GM records confirm the PicoScope result: "Engine Passed." The engine seized 8,613 miles later with no warning. This is consistent with 30-plus NHTSA complaints reporting loss of propulsion after the prior remedy, and demonstrates the inspection-based remedy is insufficient to identify engines at imminent risk of seizure.Read more

I am reporting a safety concern involving a 2018 Dodge Journey SE (VIN: [XXX] ) related to a failure in the vehicle’s emissions control system, specifically the catalytic converter and oxygen sensors. On January 13, 2026, the vehicle began producing a strong sulfur or “rotten egg” odor while driving, and shortly afterward the check engine light illuminated. Diagnostic testing revealed troubl
... e code P0420 (Catalyst System Efficiency Below Threshold – Bank 1). This code indicates that the catalytic converter is not properly converting exhaust gases, which can result in increased emissions and potential exposure to exhaust fumes. The vehicle was inspected by an authorized Chrysler, Dodge, Jeep, and Ram dealership in Maurice, Louisiana, which confirmed that the catalytic converter and oxygen sensors are failing and must be replaced. The vehicle remains available for inspection upon request. This vehicle was purchased for our growing family and is regularly used to transport our children, including a [XXX] infant and a [XXX] child. Since the emissions system issue began, occupants have noticed persistent exhaust-like fumes and strong sulfur odors while the vehicle is operating. Our infant has experienced recurring respiratory issues, and our [XXX] began experiencing respiratory symptoms after we purchased this vehicle. While we cannot definitively determine the cause, we are concerned that exposure to exhaust fumes from the malfunctioning emissions system may be contributing to these health issues. In addition, adult occupants have reported feeling unusually tired or fatigued after longer car rides in the vehicle, which raises additional concerns about potential exposure to exhaust gases inside the cabin. Because catalytic converter failures can allow harmful exhaust gases such as carbon monoxide and other pollutants to pass through the exhaust system improperly, we believe this issue presents a potential safety risk to vehicle occupants, p INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 URead more

My 2015 Ford Transit 350 equipped with the 3.2L diesel engine experienced a failure of the high-pressure fuel injection pump at approximately 72,000 miles. The pump generated metallic debris which contaminated the entire fuel system. The vehicle began experiencing drivability problems including loss of power and intermittent stalling. The vehicle was diagnosed by a Ford dealership, where metal co
... ntamination was found in the fuel system. The repair required replacement of the high-pressure fuel pump, injectors, high-pressure fuel lines, and fuel rail, along with flushing and cleaning of the fuel tank and fuel system. The total repair cost was approximately $11,500. This failure appears to match the defect described in Ford safety recall 16S32 for certain 2015–2016 Ford Transit vehicles equipped with the same 3.2L diesel engine, where scuffing in the fuel injection pump can create metallic debris that contaminates the fuel system. My vehicle was built July 16, 2015, essentially within the same production period described in the recall, but my VIN was not included in the recall population. Because this failure required replacement of most of the fuel system components and created a sudden loss of power risk while driving, I am reporting this issue so it can be investigated and considered for inclusion with similar failures.Read more

I purchased a 2025 Subaru Forester Hybrid VIN [XXX] on July 26, 2025 from Schumacher Subaru, 3031 Okeechobee Blvd. West Palm Beach, Florida 33409. On March 11, 2026 I filled the gas tank to 100% capacity. The ambient temperature was 72 degrees Fahrenheit. I parked the vehicle in our garage with the door closed. Two hours later my wife notified me that there was a strong smell of gasoline throug

Gas was spilled into and over the recess area below the gas cap, down the right side of the right back quarter panel and in a large puddle on the garage floor. I backed the Subaru out of the garage and onto the front driveway. Using multiple sheets of paper towels we mopped up the spilled gasoline from the vehicle and garage floor. I placed all the soiled paper towels in a sealed double garage bag and put it in our garden away from the house. I called Schumacher Subaru and talked with Dylan Wagner who sold us the car. I was shocked to hear that he knew of the gas cap defect and also knew that we should not fill the tank more than 50%. I asked him why he didn't notify us and he said that Subaru was going to notify us at the end on March 2026. On February 3, 2026, my vehicle was taken to Rick Hendrick Chevrolet of Buford for diagnosis of a fuel system failure. Based on this determination, my warranty claim with General Motors was denied and my case (#89096714) was closed. After this determination, the veh
... icle was transported to an independent diesel repair shop for further inspection. During the initial inspection, the technician removed the intake cover and discovered a large crack in the fuel injector pump. The pump had catastrophically failed and internally disintegrated, sending metal fragments throughout the fuel system. This clearly indicates the contamination originated from an internal mechanical failure of the fuel injector pump, not from outside fuel contamination. The independent technician confirmed that the injector pump failure caused metal debris to spread throughout the fuel system. Additionally, there was a visible leak at the front of the engine, which should have prompted further inspection in that area during the dealership’s diagnosis. It appears the dealership’s conclusion was based primarily on fuel sampling, without performing a full mechanical inspection of the injection pump and related components.Read more

A 2018 Honda Accord 1.5T developed an engine misfire, coolant loss, and reduced acceleration shortly after purchase from a dealership on 01/07/2026 (around 156,000 miles / $15,000). I was told that continued driving could worsen the condition, leading to overheating and further engine damage. After that diagnosis, the vehicle’s drivability worsened, and it became unsafe to rely on due to misfires and loss of acceleration, especially on the highway. The selling dealership refused to assist, claiming the defect was not present at the time of sale; Honda America acknowledged the head gasket concern but denied a goodwill claim, citing the high cost of the issue and lack of NHTSA enforcement on this issue. An independent repair shop then provided a second inspection/estimate consistent with the same head-gasket repair scope. The vehicle and repair records are available for inspection. Per Stellantis Technical Service Bulletins, this code indicates a severe failure requiring a physical hardwa
... re replacement to ensure the high-voltage system operates safely. To avoid performing this costly warranty repair, the dealership deliberately falsified the diagnosis on Final Invoice #31349. Under Line C (Customer States Check Engine Light is On), the dealership officially recorded the cause and correction as: 'DUE TO RECALL MODULE UPDATES AND BAD BATTERY.' This is an affirmatively false written diagnosis. A dead 12V battery and a recall software update do not cause, nor do they resolve, a P0E15 High-Voltage Electric Coolant Heater failure. The dealership lied on the legal invoice to scrub the hardware defect from the record. They then applied the 68C software patch as a smokescreen, falsified the 'Date Out' to 12/26/2025 to prematurely close the federal recall ticket, and attempted to return a vehicle that remains mechanically compromised and unsafe. This is intentional warranty fraud masking a high-voltage system failure. Stellantis and its authorized repair facility are refusing to properly remedy the vehicle. While driving on the highway at 65 miles per hour, my Atlas shut itself off. I had to put it into park and restart the ignition. It turned on after 2 attempts but just barely. It turned on and gave enough power to move forward very slowly but virtually no propulsion. It turned on briefly, I pressed the gas and then it shut off but had enough to cruise to get to the shoulder across the highway. T
... here was no shoulder next to the lane I was in. Luckily the cars around me saw that I put my hazzard lights on immediately and did the same. Once at the shoulder, tho the system had shut down again... the Atlas suddenly engaged and sped up on its own. The control panel of the car was "resetting" itself still from the initial shutdown. The system appeared to reboot itself after putting into park and pushing the ignition button. And was still "processing" while I attempted to get it to the side of the road while it shutdown again. Once it was "rebooted" that's when the system appeared to "catch" and the car "sped up" by itself. Once all that happened, probably in the span of 5 minutes or so, I was able to drive 15 miles away safely to a gas station, turned off the car, got gas and it re-started like nothing had happened and drive fine home, about 2 miles further. The instrument panel including map and "driving screen" has shut down before while driving a few times, but never anything with the engine. While stopped at a light a few times, the car has previously "shut itself off" and restarted fine after being put into park and restarting ignition. This is by far the worst and most dangerous experience I've had that could have resulted in a fatal collision, had the cars around me not also been paying close attention and helped me out by slowing behind me and creating awareness for others approaching at 65-75 MPH as well.Read more

Vehicle stalled out. Then would not start. Daughter was stuck for 30 minutes until it finally turned over took it into dealership. Was told it’s the fuel pump. My issue is that now. These fuel pumps are back ordered indefinitely found out because all of the fuel pumps are going bad on the jeeps. Now we have a faulty part that the dealership is aware of that you cannot get replaced due to so many
... of them needing to be replaced, which tells me they know this part is faulty was told they’re not even sure if or when they would be able to get the part now you have all these people driving jeeps that can stall out on the road and get into a terrible accident. My daughter is [XXX] and this is not acceptable. She has a faulty car part and Jeep has no parts to replace this faulty part. The oil pressure light is coming on . Even after doing oil change at dealership it’s not going away. Confirmed that the issue was with the oil pressure switch at dealership, which is a common problem for 3.3L V6 engines . Similar issue was observed in other older models with similar engine and the warranty was extended to cover this issue (WTY026).

After the automatic start/stop system engages, the engine stalls and completely shuts off shortly after the vehicle begins moving again. This occurs after the vehicle restarts and travels only a few feet. When the stall happens, the vehicle loses power and the engine stops functioning while the car is in traffic. This has occurred two separate times: once on a busy highway and once while entering
... a roundabout. Both incidents created a dangerous situation where the vehicle could not maintain power in active traffic and could have resulted in a crash. A yellow emissions warning light also appeared on the dashboard. The dealer indicated that an intermittent oil pressure sensor code was present but no parts were replaced and no corrective action was taken. The dealer reset the system and stated they could not reproduce the problem. The vehicle is now being returned to me without repair despite the safety risk. An engine stall immediately after the start/stop system restarts the vehicle creates a serious hazard because the vehicle can lose power in the middle of traffic. I believe this issue may be related to the automatic start/stop system or an oil pressure sensor fault affecting engine operation.Read more

Vehicle fails to start intermittently. After having this happen on several occasions and waiting for 1-2 hours it would start. However, the last time, while traveling it would not start after taking brief stop. Had to leave the vehicle overnight and return the next day to haul it back home 300 hundred miles. That was Feb. 23, 2026 and still will not start. Vehicle has 70,000 miles.

I know this is an ongoing problem with GM’s 6.2 engine. Complete engine failure while driving at 75 mph. No engine warning sign. Screen went blank. I will no longer trust this truck or have any confidence in the truck once I get it back. I sure hope GM pays for this. This has caused me endless anxiety.Read more

To Whom It May Concern, I am writing to file a formal safety complaint regarding my 2019 Mercedes-Benz CLA250 involving a leaking fuel rail that caused the vehicle to shut down and become unsafe to operate. The failure of the fuel rail resulted in a fuel leak and engine shutdown. A failure within the fuel delivery system presents a serious safety concern, as it can cause sudden loss of power whi
... le driving and poses a potential fire hazard if fuel comes into contact with hot engine components. During the repair process, I was informed that the necessary replacement component, Fuel Rail Part Number 2700701995, is no longer being produced by Mercedes-Benz. Because this critical component is unavailable, the vehicle cannot currently be repaired and is therefore undrivable. It is deeply concerning that a safety-related fuel system component for a 2019 vehicle is no longer available. Without access to this essential part, the vehicle is effectively disabled. Additionally, based on the nature of this failure, I believe this fuel rail defect should be included in the recall list for this vehicle. A leaking fuel rail capable of shutting down the engine represents a significant safety risk that may affect other vehicles of the same model. I have attempted to contact Mercedes-Benz Customer Care at [XXX] on seven separate occasions and have left messages requesting assistance, but I have not received a return call. Because this issue involves both a serious fuel system failure and the inability to obtain a critical replacement part, I respectfully request that the National Highway Traffic Safety Administration investigate whether this defect affects other 2019 Mercedes-Benz CLA250 vehicles and determine whether a recall is warranted. JEEP Wrangler 2024 Issues PLATE NUMBER [XXX] BIN # [XXX] To whom it may concern, I would like to request a courtesy incentive or compensation due to the repeated recalls and repair situations with my Jeep Wrangler. ... my lease from trips to the dealership, appointments, and test drives. The electric engine issues have been particularly concerning, as the failure has rendered the vehicle inoperable on some occasions, posing a significant safety risk. There have been instances where the vehicle has completely stopped, created a dragging effect, and failed to start in winter, while experiencing malfunctions in all electric components. My fuel consumption has significantly increased over the past three months due to these repairs, and I am no longer able to effectively use the electric engine feature. The service department has been unable to diagnose the electric engine failure after multiple attempts, and as a result, the battery is being overcharged without engaging the electric motor. There have been numerous unresolved issues and repairs that have not been successfully completed. I acquired this hybrid vehicle after considering the advertising regarding its favorable fuel efficiency and the electric rechargeable engine feature. However, this feature has not been utilized this winter due to repeated malfunctions, recalls, or a combination of both. Since late November 2025, this electric engine has consistently ceased to function. Several scheduled repair diagnostic appointments were postponed by the dealer due to a busy service schedule and a shortage of staff at the repair facility. Additionally, I need to request unpaid time off from my job, as the dealer is unable to provide a loan vehicle while my jeep is being repaired. This has caused significant inconvenience and fina INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more

I own a 2021 Jeep Wrangler Rubicon 392 with the 6.4L engine (VIN: [XXX] ). The vehicle has a dangerous condition where the engine stalls unexpectedly when the fuel level is approximately 1/4 tank or lower and the vehicle is traveling downhill. When the vehicle is positioned on a decline or downhill slope, the engine suddenly shuts off without warning. This appears to be fuel starvation on decline
... when the fuel level is below approximately 1/4 tank, resulting in engine stall and loss of power steering and braking assist after stall. This has occurred multiple times and creates a significant risk of crash due to the sudden loss of engine power and reduced steering and braking assistance while the vehicle is in motion. The issue is particularly concerning when descending hills or driving on angled terrain. The vehicle was taken to an authorized dealership (Carvana Jeep San Diego) where technicians determined the likely cause is a defective fuel pump module associated with the Wrangler Rubicon 392 6.4L fuel pump module. However, the dealership informed me the replacement fuel pump module is currently on national backorder from the manufacturer (Stellantis/Jeep). They stated there is no confirmed availability date and that many vehicles may be waiting for the same component. My vehicle has been awaiting repair for approximately four months due to the unavailable fuel pump module. Because the vehicle can stall unexpectedly while traveling downhill, this defect presents an ongoing safety hazard and potential risk of crash. This complaint is being submitted so the issue can be evaluated as a potential safety defect involving fuel delivery failure, fuel pump module malfunction, and engine stall under low fuel conditions in the Jeep Wrangler Rubicon 392.
