My vehicle constantly receives an error message while driving stating, "SERVICE HIGH VOLTAGE SYSTEM" and "REDUCED ACCELERATION DRIVE WITH CARE." Error symbols appear on the dashboard that show an orange turtle, a red battery with a big exclamation point through it and and a red car outline with a big exclamation point going through the middle of it. When this happens, the vehicle acceleration is
... significantly reduced and the regenerative braking or "one-pedal" feature automatically becomes disabled. If you're relying on that system to brake, it will not do so when you need it to and you have to manually press the brake to slow and stop the vehicle. This error clears after a few days of driving but never fails to return. The dealer said they found nothing wrong when I brought it in (when the error wasn't being displayed, of course) but it went back today with the error displayed so they could witness it for themselves. Separately, the vehicle makes an incredibly loud knocking noise when accelerating with the wheel turned. It appears this is coming from the CV joints.Read more

My 2020 Lincoln Aviator Grand Touring Plug‑In Hybrid has experienced multiple high‑voltage powertrain failures that create an immediate safety hazard. The vehicle has twice lost propulsion while driving, displaying “Powertrain Malfunction / Reduced Power” and “Service Engine Soon” before shutting down completely. During the first failure, the vehicle stopped in active traffic and would
... not restart; a police officer stopped to assist and called a tow truck because the disabled vehicle posed a roadway hazard. In addition, the digital instrument cluster has repeatedly gone completely black while the vehicle is in motion, eliminating all visibility of speed, gear position, warning indicators, and hybrid system status. During the most recent blackout, an [XXX] child was in the vehicle with me, significantly increasing the danger. These failures are accompanied by hybrid‑system fault code P0A1E (Starter/Generator Control Module), a pink charging‑port indicator signaling a charging fault, and irregular hybrid‑battery behavior (range not updating, battery not depleting). The vehicle has been inspected four times by authorized Lincoln/Ford dealers and has been out of service more than 34 cumulative days. Although the P0A1E code and symptoms were documented, the dealers repeatedly stated they were “unable to verify” the concern and returned the vehicle unrepaired. Required high‑voltage diagnostics were not completed. The problem is ongoing, intermittent, and has not been corrected. Sudden propulsion loss, dashboard blackout, and high‑voltage control failures put occupants and other motorists at significant risk. I am requesting NHTSA investigate this as a potential safety defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My Mercedes Benz GLE450e Hybrid SUV has had a persistent but sporadic safety issue that suddenly causes the vehicle to go into "turtle mode" with a "performance extremely limited" warning when trying to accelerate quickly or sometimes when breaking quickly. In turtle mode, the car quickly loses acceleration, drops in speed and is then unable to accelerate properly. The sudden drop in speed and in
... ability to accelerate causes a major safety issue while driving - particularly on roads with other vehicles also travelling at high speeds. Last Friday, I was driving with my family on Interstate 95 when a tractor trailer truck did not see me beside it and started to switch into my lane. I tried to quickly accelerate to get out of the way before it hit us but my car went into "turtle mode" and the speed suddenly dropped by 15 or 20mph and would no longer drive correctly. Luckily, I was in the right lane and able to swerve onto the shoulder of the highway and narrowly avoid the large truck. Otherwise, if I had been in a middle lane, I would have either been hit by the truck or likely hit another car next to me. This issue has been sporadically occurring with my car for about 5 months now. I have had it into the Mercedes dealer three times and, each time, they say they think they fixed the issue. But, each time the same issue has returned. They said that Mercedes USA knows it is a big issue with their new hybrid SUVs but Mercedes doesn't yet know how to fix the problem and has been unwilling to publicly acknowledge the problem or offer it's customers any assistance. Doing a quick Google search online for "Mercedes hybrid performance extremely limited" also quickly brings up different online forums where other owners of this same car have had the exact same issue that Mercedes has been unable to fix or resolve. Driving a car that can suddenly lose the ability to accelerate is a major safety hazard and I would sincerely appreciate if the NHTSA would help to address itRead more

2021 Hybrid CR-V - within the last 10 months the vehicle has twice lost engine power while driving on expressways with just enough time to pull off the road. In the same period of time, at least 6 times, when initially starting the engine, the engine runs roughly and will not accellerate. So far, this has happened in cold or cool weather but not extremely cold weather. In an effort to restart th
... e system, we then shut the system off, wait a few minutes and try restarting. Occaisionally, the engine will start normally after this "reset". More often, if the "reset" doesn't work, we shut off the heat/cooling system (to keep the system from calling for the engine power) and restart the system in EV Mode. The car will begin moving in EV Mode. The engine then starts and runs normally when accellerating beyond the EV system's capacity to accelerate. We understand this problem does not register in the car's computer as a "fault" once the car's engine operation returns to normal. Therefore, the dealers are unable to diagnose the problem. We have read of many other people having the same experience without any satisfactory correction from the dealerships or mechanics. Because this is an intermitant and unpredictable failure, and because the engine and system operates normally for long periods between incidents, the error must be computer/electrical and not related to the fuel pump. We have also replaced the aged engine battery and cleaned the cables. This did not correct the problem. Any assitance in resolving this issue is appreciated. We are concerned that the system will fail completely at the most inopportune time, leaving someone stranded on the highway or being a road hazard on a highway.Read more

Year: 2022 Make: Chevrolet Model: Silverado 1500 Engine: 2.7L TurboMax (L3B) VIN: [XXX] Mileage at Failure: 119,139 Description of Problem: 2022 Chevrolet Silverado 1500 2.7L L3B TurboMax engine is completely inoperable due to Cylinder 4 exhaust roller follower failure causing exhaust camshaft failure and bent exhaust valves. Vehicle shakes severely, will not accelerate, check engine light on,
... and has gone into limp mode. Diagnostic codes P0304 (Cylinder 4 misfire) and P2101 (electronic throttle actuator fault triggered by engine shaking and limp mode) were recorded. All external components including spark plugs, coils, and injector harnesses were inspected and ruled out as causes. Failure is confirmed mechanical and internal. This failure was diagnosed and documented in writing by an authorized Chevrolet dealership on March 12, 2026 (Repair Order XXX, Service Advisor [XXX] ). The dealer's written estimate states verbatim: "Failed Cylinder 4 exhaust roller follower failure causing exhaust camshaft failure and bent exhaust valves. Cylinder head replacement required to correct this concern. Direct cause of engine noise and misfire condition." Dealer repair quote: $10,371.34. A second independent shop confirmed the same failure and cited Active Fuel Management (AFM) and Direct Fuel Injection (DFI) as known contributing design factors in this engine's oil consumption and internal wear pattern. That shop quoted between $11,876.88 and $14,897.35. This is a known pattern in the L3B engine platform. Consumer Reports has documented a 4% catastrophic failure rate in this engine. Multiple NHTSA complaints exist for this same failure across 2019-2022 model years. GM has issued technical service bulletins but has not issued a recall. General Motors was formally notified of this failure via certified mail and executive email on March 24, 2026. My vehicle was inspected by a mechanic after experiencing sudden loss of power and stalling while merging onto the expressway in active traffic. At the time of the incident, the vehicle became unable to accelerate properly, creating a serious safety hazard due to surrounding vehicles traveling at highway speeds. The mechanic determined that the fuel pump had failed. During inspection prior to rep
... lacement, the mechanic identified metal shavings present in the fuel system, which appear to have originated from the fuel pump. These metal fragments contaminated the system and pose a risk of further engine damage. This failure appears to be related to a known issue involving fuel pump defects in Ford vehicles. The design and/or function of the fuel pump may allow internal deterioration that leads to system-wide contamination and potential engine failure. This issue presents a serious safety concern due to the risk of sudden stalling and loss of power while driving, particularly during high-speed merging conditions.Read more

The vehicle is experiencing excessive engine oil consumption. The engine was found by the dealership to be low by 3 quarts of oil after about 1000 miles during an oil consumption test, indicating abnormal oil loss. The suspected failure involves internal engine components, possibly piston rings, allowing oil to enter the combustion chamber and burn. This condition creates a safety risk because the
... engine can become critically low on oil, without warning, potentially leading to engine damage, loss of power, or engine failure while driving. The problem has been reproduced and documented by the dealership during oil consumption testing. However, there are inconsistencies in the dealership’s reporting, as later documentation stated that no oil consumption was found despite earlier records showing excessive oil loss. This vehicle has been inspected multiple times by an authorized dealership and is currently part of an open case with the manufacture. The vehicle was also held at the dealership for approximately three weeks without clear documentation of work performed. There were minimal warnings indicators prior to the issue. The vehicle showed rough engine performance, which was consistent with oil fouling the spark plugs. No clear warning light reliably indicated the severity of the oil loss before the condition was discovered during testing.Read more

The condition appears to involve the hybrid control/electric drive system. The vehicle is available for inspection upon request. What Happened / Safet
... y Risk: The loss of electric propulsion occurs unpredictably during normal operation, including both highway and city driving. The reduction in available power and sudden loss of electric functionality creates a potential safety concern, particularly when driving in traffic or at higher speeds, where reduced or inconsistent propulsion may impact the ability to maintain speed or respond to driving conditions. Additionally, the vehicle is subject to a high-voltage battery recall involving fire risk, which raises concern regarding safe operation and storage of the vehicle. Reproduction / Dealer Confirmation: The issue has been reproduced and confirmed by the dealership (Johnson Auto Plaza, Brighton, CO). The dealership also confirmed that the same issue was reproduced on another Jeep Wrangler 4xe vehicle on their lot. Inspection by Manufacturer / Others: The vehicle has been inspected by the dealership and escalated to Jeep Engineering. The manufacturer has been unable to diagnose the issue due to the absence of diagnostic codes and has not identified a repair. Warning Lamps / Messages / Symptoms: No consistent warning lamps or diagnostic codes are present when the failure occurs. The loss of electric functionality happens without a predictable pattern and without clear diagnostic indicators. Component/System: Fuel System – Fuel Filler / Left-Side Radiator and Cooling Fan Assembly (subject to NHTSA Recall 25V536). What happened: On June 15, 2024, I was refueling my 2023 Chevrolet Corvette Z06 at a BP gas station in Phillipsburg, New Jersey. As I removed the fuel pump nozzle from the vehicle, a violent explosion occurred. I was immediately engulfed in a fireball. My clothing caught fi
... re and was burning me. I jumped clear of the flames and extinguished the fire on my clothing. I saw that the vehicle remained fully engulfed in flames with the pump nozzle still lodged in the filler. I ran to the adjacent pump island where a station attendant was attempting but was unable to open a locked fire extinguisher cabinet. I broke the glass with both of my bare hands to remove the fire extinguisher. I sustained a severe laceration to my forehead that was approximately 3 inches long and required plastic surgery of over 50 stitches to close. I then ran to the car that was burning with approximately 10 foot flames and I used the fire extinguisher to put out the fire on the vehicle and pump. I then pulled the pump handle from the fuel filler opening and called the police. The police and fire department arrived and confirmed the fire was extinguished. The flames singed the hair on my head and legs. Component availability: Documentation including photographs, police report, and medical records are available. Safety risk: A fuel-fed explosion at an active gasoline pump created an extreme and immediate risk of death or serious injury to me the station attendants and others. Dealer/manufacturer confirmation: GM subsequently issued NHTSA Recall 25V536, acknowledging the defect — fuel spillage during refueling can contact the left-side radiator cooling fan, which remains active even with the ignition off, causing ignition. This recall directly confirms the defect that caused my incident. Insurance inspection: The police and Geico inspected the vehicle. No warnings at all.Read more

I am reporting a failure of the fuel pump system in my Volvo vehicle, which currently has approximately 60,000 miles. The failed component is available for inspection upon request. The issue was confirmed by an authorized Volvo dealership, which diagnosed the fuel pump as defective and requiring replacement. This appears to be a premature failure given the mileage and proper maintenance of the ve
... hicle. This failure presents a serious safety risk. A faulty fuel pump can cause the vehicle to stall unexpectedly while driving, increasing the risk of a crash, especially at highway speeds or in traffic. Loss of engine power without warning places not only the driver and passengers at risk, but also other motorists on the road. Prior to the failure, the vehicle experienced symptoms consistent with fuel system issues, including difficulty starting and intermittent performance irregularities. These symptoms escalated until the vehicle became unreliable to operate. The problem has been confirmed by the manufacturer’s authorized service center, and the vehicle has been inspected accordingly. No external factors or misuse contributed to this failure. It is my understanding that Volvo has issued recalls and/or acknowledged similar fuel pump defects in other vehicles of comparable models and years. Based on the nature of this failure and its alignment with known issues, I believe this vehicle should be evaluated for inclusion under an existing recall or be subject to further investigation. Given the safety implications of fuel system failures, I respectfully request that this issue be formally reviewed to determine whether a broader defect trend exists that could impact other vehicles. The failure occurred at relatively low mileage and under normal operating conditions, raising concern of a systemic defect rather than normal wear and tear.Read more

My Equinox completely stopped functioning in the middle of a major intersection as I was making a left turn during 5pm traffic. I was able to push the car out of the intersection with the help of other drivers (they had to exit their vehicles to assist) and the car restarted - I drove back roads to chevy dealership with the car cutting out unexpectedly twice more while the vehicle was moving and t

When I start my car, every once in a while it acts as if it is not getting enough gas and lags. In the colder months it seems to do this more often and sometimes will not start. When the weather is warmer it will sporadically do this. I have put a new battery in thinking it was weak and this could be the cause for it to not want to start, but it has done this at least 5 times within the month that
... the battery was replaced. My VIN is not showing a recall for the fuel pump like the other 2016 IS200t models, but I’m afraid that this is the recall problem. What should I do if my mechanic says it is the fuel pump? Will it get replaced under the recall?Read more

Nissan issued a recall on the ev battery in September 2024 because risk of fire if the quick charge feature is used but has failed to remedy the problem as of March 2026 and only says maybe the solution will come in this calendar year. My 2025 Honda Odyssey has failed multiple times in the middle of the road when coming to a complete stop. Auto idle engages and when I try to press the gas the car malfunctions. I have to turn off the car and turn it back on again for it to start. Also at times the interior of the car will start to smell like gas.

I’d like to report a serious, ongoing safety concern with my 2019 Chevrolet Silverado (5.3 V8 53,028 mi) since Dec 2025. Over the last 4 months, I have experienced at least 20 engine failures/loss of power, with no engine light illuminated after restarting truck. These failures result in a complete loss of power, including steering and braking, posing a significant risk of a crash. Chevy said i
... t would be hard to diagnose with no engine warning light on. Feb 24th, engine failure, warning light stayed on and towed truck to Chevy. Truck on standby, repaired Mar 10th, replacing 2 oxygen sensors per Diagnostics. Picked it up on March 11th, but engine shutdown again the same day and 4 additional failures while driving in my neighborhood, making the truck undrivable. I reached out to Chevy service supervisor, Juliana, but have received no response for the last 10 days. I urge NHTSA to investigate this defect as it poses a severe safety risk. Subject: Formal Demand for Investigation – Fuel Tank Capacity Discrepancy Vehicle: 2021 Jaguar F-PACE VIN: SADCJ2EX9MA669268 To Whom It May Concern, I am writing to formally request an immediate investigation into a significant discrepancy affecting my 2021 Jaguar F-PACE regarding its fuel tank capacity and driving range, which appear to fall substantially below the specifications published by
... Jaguar Land Rover. I purchased this pre-owned vehicle on August 30, 2024, through a salesperson named Misko Sekulic. At no time during the transaction was I informed that this vehicle was equipped with a reduced fuel tank capacity of approximately 14–15 gallons, nor that its maximum driving range would be limited to approximately 275 miles per tank. This omission is particularly concerning given that I have previously owned two Jaguar F-PACE vehicles, both of which were equipped with fuel tanks of approximately 21–22 gallons and provided an average driving range of close to 400 miles. Within days of taking ownership, I noticed a substantial reduction in driving range. I raised this concern with the salesperson within a few weeks; however, no action was taken. Over time, it became increasingly clear that the vehicle’s limited range significantly impacts its usability, particularly given my profession as a real estate agent, which requires frequent and long-distance driving. The current range makes even routine trips inconvenient and impractical. Due to this issue, I have limited my use of the vehicle, which is reflected in its unusually low mileage of approximately 13,000 miles. Had this critical information been disclosed at the time of purchase, I would not have proceeded with the transaction. his conclusion is extremely concerning and unacceptable. A vehicle that is advertised with a 21-to 22 gallon fuel tank to but consistently accepts only 14–15 gallons For this reason, I am requesting that Jaguar Land Rover North America immediately conduct a formal reviewRead more

My vehicle 2008 Ford edge had a safety recall from 2012 and the gas tank that the previous owners never had taken care of it or resolved. I purchased a car in February. I filled the gas tank. I parked it in my condo and I used my secondary car to go to work that day. Well, the management called me and said that my gas tank exploded and that there was fuel all over the condo parking lot. Had the fi
... re department come and secure the car they took care of the gasoline in the parking lot and then I had it towed to Miller Ford when we found out that there was a recall on that gas tank. This car has been at this dealership three times it’s going tomorrow. This will be the fourth time for the same issue. I feel that this car is not safe. I feel that because nobody addressed this safety recall since 2012 that everything was corroded. I almost got into three accidents because the car stalled on me on three different occasions.Read more

The incidents most often occur when I try to accelerate after vehicle is stopped at an intersection. The car hesitates during acceleration regardless of how much I push the gas. Once it starts to go, it feels as though the car can't shift from 1st to 2nd gear. The RPMs get up 4500 while the car is going 25-40 mph and remain there for a few seconds up to ~20 seconds. I am fearful of getting t-bone
... d when my car hesitates while turning at an intersection, going through an intersection or pulling out from a parking lot. I have been stuck in several intersections when traffic was coming toward me, but thankfully those cars were able to avoid hitting me. I have reported the problem every time I take the car to the dealer for a service. The problem has been happening since I purchased the car in November 2024 and occurs randomly--sometimes every couple of months, sometimes days in a row. The dealer can't reproduce it, but they were able to pull a code from the transmission and found it had an acceleration error. They updated the software from a TSB, but that didn't help.Read more

Vehicles coolant emptied out causing vehicle to overheat and smoke from under engine.
Incident date: Jun 20, 2023
The vehicle exhibits a consistent and reproducible malfunction in the powertrain system, specifically related to acceleration response and drivetrain engagement. While driving at a steady speed, when the accelerator is pressed gradually (approximately 50–60%), the engine RPM increases but the vehicle does not accelerate accordingly. There is a delay in power delivery. When the accelerator is re
... leased, the engine may continue revving briefly, followed by a sudden and jerky engagement of the drivetrain. This issue occurs consistently under the same driving conditions and can be reproduced every time. The vehicle is available for inspection upon request. This malfunction creates a safety risk when merging into traffic, entering intersections, or making turns. The vehicle does not respond when acceleration is required, which can lead to dangerous situations where other vehicles may approach at higher speeds. After the delay, the vehicle may surge forward unexpectedly, increasing the risk of collision. The issue has been reported to multiple authorized Toyota dealerships on several occasions within the first 15 months of ownership. The vehicle has been inspected by dealership technicians, but the concern has not been diagnosed or repaired. Dealerships have stated that the behavior is normal, despite the repeatable nature of the issue. No warning lights, error messages, or diagnostic indicators have appeared at any time. No diagnostic trouble codes have been reported. The vehicle is equipped with an 8-speed automatic transmission, and the behavior appears consistent with a possible transmission, torque converter, or drivetrain control malfunction affecting acceleration response. The condition is not related to driver input and occurs under controlled, repeatable conditions.Read more

... ship nearby (Peak Kia) was able to diagnose the problem. Unknown if the failed part is available for inspection. Vehicle was seen by dealership a few days later and as far as I'm aware are the only entity to do an inspection on the vehicle. Out of nowhere and driving this morning, my engine reduced power came on. Then I started losing power in the car then it died on me once out of nowhere. I love this car, but it seems to have a lot of issues all of a sudden and I’m fixing to be at 60,000 miles which is a big problem considering warranty will be expired. I’ve already had issues with squeaking and squealing from the windows that they claim they can’t find nothing and I’ve had to listen to it for almost 60,000 miles. I had to get this car because my other car blew up on me and I can’t afford another new car. I’ll be taking it to the dealership first thing in the morning and I hope to God that something can be fixed cause we’re in one car Family.Read more
