The internal core of the Exhaust Heat Recovery System (EHRS), part of the center muffler/front pipe assembly (#28600-L5210), suffered a catastrophic failure. This allowed approximately 9 gallons of engine coolant to leak directly into the hot exhaust stream. Safety Risk: This failure creates a severe visibility hazard. During acceleration from stop lights and stop signs, the vehicle emits massive,
... thick clouds of white glycol smoke. The smoke is so dense that it completely obscures the driver's view of trailing vehicles and prevents following drivers from seeing the roadway or my vehicle's brake lights, creating a high risk of rear-end collisions. Additionally, the rapid loss of coolant caused the engine to overheat and lose power while in motion, nearly causing a stall in active traffic. Confirmation: The failure was confirmed by the service department at Washington Hyundai (Invoice #174281). This is the second failure of this specific component (previously failed in Jan 2025). The manufacturer issued TSB 23-EM-009H for this defect, acknowledging it affects 2020-2023 Sonata Hybrids, but no safety recall has been issued. Symptoms: The issue was preceded by a "sweet" burning smell and a sudden "Engine Overheating" warning on the dashboard. The failed component has been replaced by the dealer, but the manufacturer refused to replace the coolant-poisoned Oxygen (O2) sensors, leaving the vehicle in a technically compromised state. The failed unit is likely held by the dealer for warranty parts return.Read more

The engine stop/start system on my 2016 Honda Accord, VIN [XXX], has stalled and shut off without warning in active Los Angeles traffic dozens of times since September 2025. The vehicle loses all power mid-drive with no warning — no dashboard lights, no warning sounds — leaving me unable to accelerate or respond to surrounding traffic. The condition has progressively worsened and now includes
... repeated no-start failures requiring three documented tows. The component failing is the engine stop/start system. The vehicle is available for inspection upon request. My safety and the safety of others hav been put at risk repeatedly. I am a [XXX]. My wife, age [XXX], has [XXX] and has been a passenger during stalling events. Sudden unexpected stalling in active Los Angeles traffic creates immediate foreseeable risk of rear-end collision and loss of vehicle control. This risk has been experienced dozens of times over six and a half months. The problem has been presented to Honda of Hollywood, a certified Honda dealer at 6511 Santa Monica Blvd, Los Angeles CA 90038, across five separate visits. Each time the dealer concluded no problem could be found. On one visit I was charged nearly $2,000 for spark plugs and a gasket — work with zero diagnostic connection to the stalling complaint. The vehicle was returned to me in an unsafe condition after every visit. On [XXX] the tow truck driver who responded to my third documented tow independently experienced the failure, could not start the vehicle, witnessed the complete failure sequence, and declared the vehicle unsafe to drive. He is an independent third-party witness with no stake in this case. I have video documentation of the vehicle stalling in active traffic on multiple occasions. I have voicemails from Honda of Hollywood's own service advisor documenting their diagnostic failures. There were no warnings of any kind. The events suggest a cracked fuel injector. Even though 15 foot flames engulfed the car it was parked and unoccupied, there were no injuries. The results of the investigation have not been reported to us. CCTV footage was obtained from the police.Read more

I am reporting a recurring safety issue involving sudden loss of power while driving in my 2023 Ford Escape. The vehicle has experienced multiple incidents where, while accelerating or pulling into traffic, it suddenly loses power, hesitates, or fails to respond to the accelerator. In some cases, the RPM drops unexpectedly, and the vehicle will not move or respond for several seconds. After this
... delay, the vehicle may jerk or surge forward suddenly. This creates a dangerous situation, especially when entering busy roads or intersections, as it puts me at risk of being struck by oncoming traffic. This issue has occurred multiple times and has been brought to the dealership on several occasions. The vehicle has undergone multiple repair attempts, including major engine-related repairs such as cylinder head and cooling system work, as well as a fuel system component replacement. Despite these efforts, the problem persists. On multiple visits, the dealership has stated they were unable to duplicate the issue, even though the condition continues to occur during normal driving. The vehicle has been out of service for an extended number of days due to these ongoing issues. This defect is intermittent but poses a serious safety hazard due to the unpredictable loss of power and delayed acceleration. I am concerned about the risk of an accident or injury if this issue is not properly identified and corrected. I am requesting that this issue be investigated, as it appears to be a recurring defect that may affect other vehicles of the same make and modelRead more

I own a 2020 Kia Niro PHEV. In October 2024, my dealership performed Technical Service Bulletin SA512 on my vehicle addressing the inverter coolant system. I was not informed at that time that the issue was likely to recur or that periodic maintenance would be required. In March 2026, the inverter coolant warning light returned. I brought the vehicle back to the dealership, where I was told the co
... olant had crystallized. The dealer confirmed this is a known recurring issue with this vehicle and stated it would need to be flushed approximately every two years since we drive relatively low miles per year (despite driving every day). I was not warned of this at any point. Furthermore, many owners likely drive it short distances every day since it is a plug-in electric vehicle. Kia denied goodwill coverage and I paid out of pocket for the repair. When I contacted Kia Customer Care, representatives gave conflicting information. One confirmed SA512 was performed as a service campaign, another stated it was only a TSB and declined coverage since my car is out of warranty (even though the warranty should still cover the battery and this is an electrical issue). A total case specialist I was assigned to said she would talk to the dealer and call me back but never did. The inverter cooling system is safety-relevant as it prevents the battery from overheating. The problem was confirmed and repaired by the dealership. The component is no longer available for inspection as the repair has been completed. Kia's issuance of SA512 suggests this is a known design issue affecting multiple owners of this model, and they informed the dealership that they could not cover the cost because if they did so, they would have to cover everyone with the same problem (again implying that the issue is quite widespread.)Read more

While charging my 2023 Porsche Taycan 4S at an Electrify America station, white smoke began emitting from the front wheel/charging port area. The vehicle also produced a burning odor inside the cabin. I immediately stopped charging and unplugged the vehicle. The smoke ceased within approximately 1–2 minutes. The vehicle was towed to an authorized Porsche dealership. Initial explanation from th
... e dealer was steam from when radiators heats-up, which is inconsistent with the conditions and prior use history. Upon further inspection, the dealership identified battery-related issues. Ultimately, I was told that 8 out of 10 battery cells were replaced. The vehicle was out of service for approximately one month. The dealership stated that the battery cell issue was not related to the smoke event, which is concerning given the timing and severity of the incident. This vehicle had previously been subject to a battery-related recall addressed via software update only. I believe there is also ongoing litigation related to battery safety in this model. Given the smoke event, burning odor, and significant battery component replacement, I am concerned about the safety of this vehicle, particularly risk of thermal event or fire during charging. I am reporting a Safety Remedy Denial by Audi of America (Case #[XXX]) regarding a failed suction jet pump and charcoal canister in my 2015 Audi S3. Although Audi issued a 15-year/150,000-mile warranty extension ([XXX]) for the 8V chassis A3 population, they are denying coverage for m

I am filing this complaint because my Volkswagen Tiguan has a serious and ongoing engine issue involving excessive oil consumption and a faulty cylinder head. The vehicle burns through an abnormal amount of oil between changes, requiring constant top‑offs, sometimes as frequently as every few hundred miles. Despite regularly adding oil and following all manufacturer‑recommended maintenance, th
... e issue continues to worsen. The dealership has confirmed that the cylinder head is defective, which is causing the engine to consume oil at an extremely accelerated rate. However, I have been told the replacement cylinder head is on national backorder, and there is currently no estimated timeframe for when the part will be available. This means I am forced to continue driving a vehicle with a known, documented engine defect that can: Lead to engine stalling Create loss of power while driving Increase the risk of engine seizure Potentially cause a safety hazard on the road Additionally, being required to constantly add oil just to keep the vehicle operable is not only unsafe but financially unreasonable. The problem remains unresolved despite multiple service visits, documentation, and repeated oil consumption tests. This is a safety issue, not just a maintenance concern, because the engine can fail unexpectedly if the oil drops too quickly. I believe this is a significant defect affecting multiple Volkswagen vehicles and should be investigated by NHTSA. I request that NHTSA open a formal investigation into the excessive oil consumption and cylinder head defects on Volkswagen Tiguans, as owners should not be left with unsafe vehicles due to known manufacturing issues and unavailable critical engine components.Read more

This vehicle is subject to Safety Recall 24V-700 (Manufacturer Recall R24B2) due to a defect in the lithium-ion battery that can cause a fire during Level 3 DC Fast Charging. Since the initial notice, I have received four separate recall communications from the manufacturer, yet as of March 2026, no permanent remedy has been provided. The manufacturer’s only "interim" solution is an instruction
... to refrain from using the Level 3 DC Fast Charging (CHAdeMO) feature. This has effectively disabled a primary advertised function of the vehicle and creates a substantial safety risk should the port be used. This defect has rendered the vehicle unsellable on the secondary market and significantly impairs its daily utility, as I am unable to use charging infrastructure at my place of employment. The manufacturer continues to delay the final remedy with "coming soon" notifications, leaving me with an unsafe and substantially devalued asset for over a year.Read more

It is important to note that fuel pump was recalled in this model, car performance has significantly declined over the last 25k miles. This car is well maintained as in; regular oil, changes, transmission flushes, rear differential fluid change, tire rotation every 5-7k miles, brakes and rotors changed nearly every 30k front and back, fixing all issues as they arise. I say all of this so that there is a clear picture of the car maintenance and the issues it is having despite the proper care. This car has had several issues from about 150k miles. In my opinion this is premature for a car this well maintained. I am reporting a fuel‑system defect on my 2018 RAM 1500. The vehicle has only 43,325 miles, which is exceptionally low for its age, and it has been properly maintained. Despite this, the fuel‑tank venting system has failed. The dealership diagnosed the issue as a faulty vent valve located within the fuel tank. This defect prevents the vehicle from being refueled at a normal rate. During r
... efueling, the gas pump repeatedly shuts off, and a standard refueling process takes significantly longer than normal. This occurs at all stations and with all pumps. The dealer’s proposed remedy is complete replacement of the fuel tank at a cost of $2,104.64. Because the vent valve is integrated into the tank, they stated it cannot be serviced separately. A critical fueling component should not fail at this mileage under normal use, and the failure of the venting system raises concerns regarding safe refueling, potential fuel backsplash, and proper EVAP emissions operation. This is not a wear‑and‑tear issue. It is a premature failure of a core fuel‑system component. Stellantis has declined goodwill assistance, stating my vehicle is "too old", nor has it issued a recall for this defect, despite the unusually low mileage and the nature of the failure. A quick search on Google or YouTube yields multiple, and similar, customer experiences. I am submitting this report so NHTSA can evaluate whether this issue represents a broader defect affecting additional vehicles of this model and generation. February 17 2026 — seven days before my warranty expiration — the same authorized Subaru dealer physically identified and documented a CVT oil pump cover leak. The same jerking complaint was again documented in writing. No repairs were made and the warranty expired one week later. The CVT defect has been physically confirmed by an authorized Subaru dealer within the warranty period and Subaru of America has denied my warranty claim in writing. This issue presents a safety concern. The unpredictable jerking and hesitation when accelerating from a stop creates dangerous situations in traffic, particularly at intersections and when merging. While operating my 2021 Honda Civic Type R (VIN: [XXX] ) in high-ambient temperatures (approximately 95°F), the vehicle experienced a sudden and total loss of motive power during acceleration while exiting a busy intersection. This resulted in an immediate engine stall in the path of oncoming traffic, creating a severe risk of a collision. Following the stall, the vehicle was extremely difficult

Once restarted, it entered a 'limp home' mode with a severely restricted RPM limit and multiple dashboard warnings, including 'Emissions System Problem' and 'Rev Match System Problem.' A diagnostic scan performed immediately after the incident revealed a DTC P0087 (Fuel Rail/System Pressure - Too Low). The symptoms—thermal-related failure, stalling under load, and the P0087 code—are identical to the documented mechanical failures of the Denso fuel pump impeller swelling defect. Although this production year and model are known to be part of the expanded fuel pump recall population, my specific VIN is not currently showing as 'remedied' or 'open' in the manufacturer's database. I am filing this complaint regarding an unresolved safety recall (Recall 78C) on my 2024 Jeep Grand Cherokee 4xe, which has now been open for approximately 139 days without a remedy. This recall involves engine manufacturing contamination that may lead to catastrophic engine failure, loss of propulsion, and potential fire risk. Despite the seriousness of the recall, Stellantis has failed to provid
... e a remedy and has given conflicting and unclear guidance regarding safe operation. At one point, I was instructed to operate the vehicle using the gasoline engine only. I was later told Stellantis cannot provide technical guidance and to contact the dealership. The dealership has stated they have no clear direction from Stellantis. As a result, I have no clear understanding of whether it is safe to operate the vehicle, use electric mode, or rely solely on the gasoline engine. Restricting use to the gasoline engine also prevents use of the plug-in hybrid system, meaning the vehicle cannot be used for its intended purpose, resulting in increased fuel costs over several months. I have also incurred out-of-pocket rental car expenses during recall service. Stellantis indicated reimbursement would be available but has not provided a process or responded to follow-ups. Given the nature of the recall and lack of guidance, I cannot determine if this vehicle is safe to transport my newborn child. On March 5, 2026, while I and my 2 passengers were traveling on the 405 Freeway at approximately 40–50 mph, the vehicle suffered a catastrophic total loss of power. The engine shut off and all electronic systems failed simultaneously, including the instrument cluster and all interior lighting. Most critically, the hazard lights were inoperable, leaving the vehicle invisible to high-speed traffic
... during the failure. Since I was traveling at highway speeds I was able to safely glide the vehicle to the center shoulder. After approximately 10–15 minutes, the dashboard, infotainment, and climate control systems began flickering intermittently before power temporarily returned. Upon attempting to re-enter traffic to reach an exit, the vehicle stalled a second time while in motion. I returned to the shoulder again. After another 10 minutes the vehicle started and I was eventually able to exit the freeway and reach my residence. The following morning, the vehicle was completely unresponsive and would not start. It was towed to Land Rover Santa Monica dealership on March 6, 2026, where it currently remains for diagnosis and repair. This incident on the 405 freeway marks the third occurrence of a total loss of propulsion and electrical failure while the vehicle was in motion. Despite previous repair attempts, the defect persists, rendering the vehicle fundamentally unsafe for operation. I am gravely concerned for my personal safety, as well as the safety of other motorists on public streets and highways. A vehicle that spontaneously stalls at high speeds—while simultaneously losing the ability to activate hazard lights—presents an unacceptable risk of a high-speed rear-end collision. ... ge system shutdown while driving. The repair is extensive, requiring full module replacement, software programming, and system recalibration across interconnected systems. The vehicle has remained inoperable due to significant delays in obtaining this part. This defect appears to be widespread among this vehicle’s make and model, as evidenced by numerous owner reports and forum discussions describing similar SX6 module failures with limited long-term resolution. The vehicle and all affected components are available for inspection upon request. The safety risk has been severe. The lane keep and lane centering system has shut off abruptly at highway speeds without warning, causing the vehicle to drift or swerve out of its lane and creating a high risk of collision. More critically, the vehicle suffered a complete high-voltage system failure while driving on public roads, resulting in a sudden and total loss of power without warning. This creates an extremely dangerous situation, as the vehicle cannot maintain speed or safely respond to surrounding traffic, significantly increasing the risk of an accident. Yes. The issues have been reproduced and confirmed by an authorized dealership, which also diagnosed a failed module requiring replacement. No. The vehicle and its components have not been inspected by the manufacturer, police, insurance representatives, or other third parties. Yes. Numerous warning lights, system notifications, and other symptoms began appearing within a few weeks after purchase. The vehicle was purchased in September 2025.Read more

Without any warning, on March 15, 2026, I turned out of a parking lot onto a major road and I noticed it wouldn't accelerate above 20 mph, so I pulled off the road. I tried again and my vehicle still failed to accelerate. And on my third attempt, my vehicle decelerated and was about to completely stall. I managed to turn into a parking lot and avoid getting rear ended. However, this could have
... been a fatal event. While I was trying to accelerate, the vehicle was quiet as if the car wasn't even running. After shutting the engine off for awhile, I was able to drive the vehicle with caution. I reached out to two Genesis service departments regarding my vehicle failing to accelerate while driving and neither had heard any previous complaints, and the earliest appointment would be mid April. There was no sense of urgency, as I'm left to hesitantly drive my vehicle at the risk of it happening again and getting rear ended. And when my vehicle does go in for service, I'm weary whether the problem will be resolved properly since the service department is unaware of the issue. In addition, I have had this vehicle for only 4 months and 1 month into it, I already took it to service because of fuel odor that had not gone away since the day 1. But the service department didn't find anything wrong with the vehicle, so nothing was done to it. I also swung by another Genesis service department, in case they happened to be familiar with the cause of the odor. My vehicle still has the fuel odor and now I'm wondering if that is related to my vehicle not accelerating. I am seeking your assistance to ensure that the problems with my vehicle are confidently resolved. And I wouldn't want this to happen to anyone else. I am thankful, I was driving and not my teen.Read more

While accelerating to highway speeds, the vehicle suddenly experienced a significant and continuous loss of power. Even with the accelerator fully pressed, the vehicle would not increase speed beyond roughly 50 mph. Based on the behavior and previous service history, the issue appears to be related to the engine, fuel delivery system, or overall powertrain. The incident created a serious safety c
... oncern for both the occupants and surrounding traffic. The vehicle could not properly accelerate, merge, or respond to normal highway conditions. Hazard lights had to be activated, and the vehicle remained in the slow lane for an extended period to avoid faster-moving traffic. The issue was consistent and repeatable throughout the drive. Multiple attempts to accelerate resulted in the same lack of response, and the loss of power persisted for the duration of the trip. The vehicle has not yet been inspected following this occurrence because when turned off and turned back on, the issue goes away and happens sporadically. It is also worth noting that the vehicle previously underwent a manufacturer-covered repair involving the high-pressure fuel pump, for which a warranty extension had been issued. This loss of power occurred after that repair was completed. No warning lights, alerts, or prior symptoms were present before or during the incident. Overall, the sudden loss of propulsion at highway speed posed an immediate safety risk, as the vehicle was unable to maintain appropriate speed or safely maneuver with traffic.Read more

On 3-21-26 I noticed the smell of gasoline after driving the vehicle. Upon inspection a noticeable fuel leak from the top of the fuel tank was evident from underneath the vehicle. Further inspection found fuel leakage at the fuel pump outlet line. After online research it was found that other vehicles of this type have hairline cracks on the fuel pump outlet causing fuel to spray out under pressur
... e. Fuel pump assembly replacement has repaired the problem. No indication other than the smell of gasoline was noted at any time. This is a safety hazard that should be addressed ASAP. Part removed has only been inspected by me. Part is available. If I had not been diligent in addressing this problem the consequences could have been catastrophic. The fuel outlet on a high pressure pump should have never been allowed to be made of plastic. People's lives are at risk!Read more

While parked on a slight downhill grade, gasoline started leaking and created a significant puddle on the ground. No warning lamps, messages or other symptoms. An independent service center ran numerous tests over two days, including a smoke test. Diagnosis: There is a hole in top section of fuel tank and the fuel tank needs replaced. Recommendation was to not drive vehicle with fuel tank over
... half full. Due to the potential of leaking fuel in the presence of a hot ignition source increasing the risk of fire, driving the vehicle long distance with defective fuel tank appears to represent an unreasonable safety risk. Unfortunately, the vehicle is currently 300 miles from home. Mazda’s Mexican-made plastic fuel tank specifically required for our vehicle’s VIN is not currently available. (Mazda makes a similar metal fuel tank in [XXX] which will not fit.) Mazda has an estimated three-month backlog in [XXX] for the fuel tank. Still awaiting repair due to lack of availability of the appropriate fuel tank. Major safety recall NHTSA 16V684000 for the 2015 Mazda3 addresses a manufacturing defect where a fuel tank weld could fail, leading to fuel leaks and a potential fire. Recall is for vehicles produced between 12/4/13 – 9/18/15. Our vehicle was manufactured 02/2015 in [XXX] . However, recall is limited to three specific series of VINs and our vehicle falls in the gap between subject VIN ranges. A recent NHTSA complaint XXX (1/27/26) has the same issue: fuel tank leak, but not within range of recall VINs. It seems that the list of affected VINs included in recall 16V684000 should be expanded to include additional Mexican manufactured vehicles potentially with the same issue. Or create a new recall. Having a hole in the top of a plastic fuel tank securely mounted to the vehicle is unexpected through normal wear and tear. Any fuel leakage increases the risk of fire. NHTSA should investigate if this is due to a defect in Mazda’s manufacturing process. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORRead more
