On March 6th, 2026, I spent the day running errands in normal driving conditions and it created a situation that did not charge the battery or switch to gasoline. The dealership service department charged the battery and gave me advice to run the car at least 30 minut

A significant engine failure occurred while driving, presenting as loud knocking from the engine. The issue developed without prior warning lights or messages. The vehicle was inspected by a dealership and the problem was confirmed as internal engine failure (piston slap/engine knock). The dealership recommended full engine replacement. The engine is available for inspection upon request. There
... was no evidence of sludge or lack of maintenance noted during inspection. The most recent oil change had been performed less than 1,000 miles prior to the failure, indicating the issue was not caused by lack of maintenance. This condition creates a safety concern due to the potential for sudden engine failure, loss of power, or engine seizure while driving, which could increase the risk of an accident, especially at highway speeds. The issue has been confirmed by a dealership and has not been repaired due to cost.Read more

Our 2018 Honda Odyssey repeatedly and without warning completely loses acceleration power, creating extremely dangerous driving conditions. This occurs in two distinct situations:(1)while driving at highway speeds when attempting to change lanes or accelerate, and(2)when stopped at a stop sign or intersection and pressing the gas to merge into or turn into traffic. In both situations, pressing the
... accelerator results in little to no response — the vehicle will roll but fails to accelerate as needed. These failures have placed our family in multiple life-threatening situations. On Interstate 285 in Atlanta, Georgia — one of the busiest and highest-speed highways in the country — our vehicle has failed to accelerate during lane changes on multiple occasions, nearly causing serious accidents. Most recently, within the past week, my husband was driving on I-285 when he pressed the accelerator and the vehicle had absolutely zero acceleration response. He narrowly avoided a collision. We have also experienced complete loss of acceleration when turning from a stop sign into traffic and when merging — situations where immediate acceleration is critical to safety. The vehicle lags for several seconds before any power is delivered to the wheels, leaving us dangerously exposed in oncoming traffic. All manufacturer recalls have been completed, including the fuel pump recall. The problem persists and has not been resolved by any dealer service or recall repair performed to date. We are aware that this is a known, widespread defect affecting 2018 Honda Odysseys equipped with the ZF 9-speed automatic transmission. Hundreds of other owners have reported identical symptoms to NHTSA and to Honda directly. Honda has acknowledged the transmission's behavior to some owners but has stated it is "how the car is supposed to drive" — a response that is wholly unacceptable given the documented near-accidents this defect is causing. We are requesting that NHTSA open or expand its investigationRead more

This motorcycle has had a history of stalling while riding, this issue has persisted through two "repairs" that the dealership deemed safe. It stalled on the way back from retrieving the motorcycle from the supposed repair and put me in a life threatening scenario when the motorcycle stalled in traffic in front of a diesel in a construction zone without any available shoulders. The error that disp

Doing so all while rolling to a stop in the middle of highway traffic. This has caused me to injure my leg due to the extremely fast nature of attempting to start a stalling motorcycle on a busy highway. This issue first appeared 2 years ago and it has persisted through all "repairs" from the dealership of the motorcycle brand specially.Read more

My 2024 Jeep Wrangler 4xe is subject to Recall 78C (NHTSA #25V-766) regarding sand contamination in the engine. Hello, I am reporting a safety issue involving my Ford F250 vehicle related to Safety Recall 25S75 (low-pressure fuel pump failure). Approximately one month ago, while driving on the freeway at approximately 65 mph, my vehicle experienced a sudden loss of power. This created a dangerous situation as I was traveling at highway speed and the vehicle became unable to maintain normal operation. The
... vehicle is now inoperable. The failure is consistent with the condition described in Recall 25S75. This is not a preventative concern — the defect has already occurred and has rendered the vehicle undrivable. I contacted Ford Customer Relations and explained that the failure matches the recall condition. However, my case was closed without resolution, without any determination of responsibility, and without conducting a repair or transportation support. Ford has indicated that the official recall remedy is not yet available. Despite this, I have been told I may be responsible for out-of-pocket repair costs, even though the failure is consistent with an active safety recall. This situation presents both a safety concern and an unreasonable financial burden, as the vehicle cannot be safely operated and no interim remedy or support has been provided. I am requesting that NHTSA review this matter, as the manufacturer is not addressing a recall-related failure that has already occurred and resulted in a loss of power at highway speed. If you need any additional details, please do not hesitate to reach out. Thanks, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The electric A/C compressor on my 2022 BMW X5 xDrive45e (VIN [XXX] ) failed internally at approximately 59,000 miles, causing metal debris to circulate throughout the refrigerant system. The failed compressor and contaminated components are at BMW of Sterling (Sterling, VA) and available for inspection. This is a safety concern because the A/C refrigerant circuit provides thermal management for th
... e high-voltage hybrid battery. BMW of Sterling's service advisor confirmed in writing that the A/C system is "a critical component of your vehicle's HV system," that there is no independent backup cooling system, and that the failure "can cause potential high voltage battery failures." Without active cooling, the HV battery is at risk of overheating, which could result in a thermal event. The failure was diagnosed and confirmed by BMW of Sterling. They categorized the repair as "EHV: FULLY ELECTRIC OR HYBRID VEHICLE DRIVETRAIN DIAGNOSTICS" with the complaint "HIGH VOLTAGE SYSTEM ERROR." The repair estimate is $11,356.59 and requires replacement of the compressor, condenser, all expansion valves, and a double system flush. BMW Service Information Bulletin SI B64 09 21 documents this exact failure pattern for the G05 X5 xDrive45e, and BMW created a dedicated repair kit (part 64 50 5 A5B 590) for it. This is my second major HV component failure. A battery cell replacement was performed under warranty in 2025. Both failures were confirmed by the dealership to be component defects unrelated to owner use. Prior to complete failure, a fan noise was present in the front driver's area for a period of time before it stopped, consistent with the compressor degrading before failing entirely. A high voltage system error was displayed. BMW NA (case #XXX) has declined warranty coverage despite the vehicle being within the 8-year/80,000-mile HV battery warranty. Component: Engine/Powertrain/Electrical System Incident Description: I am reporting a recurring, life-threatening "Loss of Motive Power" (Dead Pedal) in a 2024 Hyundai Tucson (Alabama-built). On multiple occasions, the vehicle has failed to respond to throttle input during high-risk maneuvers, specifically while merging into 45+ MPH traffic. The vehicle "chugs" and stalls, refusing to exceed 15 MP
... H despite full accelerator depression. This defect leaves the vehicle and its occupants stranded and defenseless in the path of oncoming high-speed traffic. Technical Context: This vehicle is currently subject to NHTSA Recall 23V-526 (Hyundai Campaign 246) for electronic controller damage that can cause "CAN communication disruptions." I believe the "Dead Pedal" failure is a direct result of these communication disruptions between the engine and transmission control modules. Furthermore, Hyundai has issued TSB #24-AT-001H-1 for "Hesitation when accelerating," yet the authorized dealer (Ed Voyles Hyundai) has been unable to remedy the defect after 84 cumulative days out of service. Safety Risk: This is not a convenience issue; it is a critical safety failure. The unpredictable nature of the power loss makes it impossible to safely judge gaps in traffic. The vehicle effectively becomes a stationary object on active roadways without warning. Manufacturer Response: The manufacturer has been formally notified via certified legal counsel. Despite 79 consecutive days in the current repair attempt (since Jan 16, 2026), the manufacturer has failed to provide a fix or acknowledge the severity of the safety risk. An informal third-party evaluator attempted to dismiss the claim despite the vehicle's documented inability to maintain motive power. We are requesting that NHTSA investigate the adequacy of Recall 23V-526 and TSB #24-AT-001H-1, as these measures have failed to resolve this life-threatening "Dead Pedal" condition in my vehicle.Read more

I am reporting a safety concern regarding my 2023 Mercedes-Benz EQB 300 (VIN: [XXX] ), currently subject to NHTSA Recall 26V073 involving the high-voltage battery. The defect may result in a vehi
... cle fire while driving or parked. The recall was issued around February 9, 2026, and remains unrepaired with no remedy available. The manufacturer has indicated a permanent repair may not be available until approximately December 2026. I have been instructed to limit charging to 80% and to park the vehicle outside due to fire risk, yet I am still expected to operate the vehicle and transport my family. Additionally, I received third-party notification advising that my vehicle should not use DC fast charging due to this safety risk, significantly limiting normal use. Mercedes-Benz USA offered a monetary payment conditioned on waiving all current and future legal rights while the vehicle remains unrepaired. I have not been provided a guaranteed loaner or replacement vehicle, and the dealership has requested return of the loaner. Due to these limitations, I am incurring and expect additional costs and loss of normal vehicle use, including reduced access to charging benefits and potential fuel or charging expenses. This situation requires continued use of a vehicle with a known fire risk, no remedy, and restricted functionality without reliable alternative transportation. I request this be documented as a safety concern involving the defect, delayed remedy, and limitations on safe and normal vehicle operation. I was notified of a recall on my from vehicle by Hyundai via email some time in March of 2026. The message said mentioned my vehicle may have a defective/worn purge control check valve that would cause the fuel tank to deform. I was offered a buyback from my Hyundai claims manager over the situation, but I figured since its a simple fix, I'd keep my car and have it fixed. Two weeks went by with no update from the dealership, so I walked in for answers. Turns out, the subframe of my car has been damaged by the expanding fuel tank and they are still waiting for updates from Hyundai on how to proceed as they themselves don't know how to fix my vehicle. Despite having the 'latest software remedy' installed less than 30 days ago, the vehicle suffered the exact safety failure the recalls were meant to address. Gasoline contamination would typically spike this number into the 50s.GMSi protocol indicates that gasoline-contaminated diesel will melt a cup within 30 minutes.results only provided a "softened bottom," which is an inconclusive result per GMSi standards.Sinclair Refinery lab confirms the fuel sample taken from my vehicle is within 100% of factory specifications for diesel, with zero gasoline or DEF detected.Violation of GM Service Protocols even assuming the shop’s diagnosis of contamination was correct, the repair performed deviates dangerously from GMSi requirementsFor a 2024 L5P Duramax that has been cranked with contaminated fuel, GMSi (e.g., Bulletin 16-NA-376) mandates the replacement of the entire high-pressure fuel system, including the pump, both fuel rails, and all eight fuel injectors. shop replaced only the pump and cleaned the lines. This "partial repair" is not a GM-approved recovery procedure for contamination. Reusing potentially contaminated injectors and rails is the primary cause of catastrophic repeat failures. This clearly falls under warranty work and I should not had been charged for this repair. The dealership refusing to acknowledge my concerns and evidence is not only dangerous to the safety of myself and passengers, but raises concern for my vehicles future maintenance status, both inside and out of warranty. This shows a deceptive business practice and complete lack of accountability, compassion and moral understanding for the customer.I am owed a full refund and a warranty claim openRead more

Monday, March 30, 2026 my GMC Yukon Denali had already had the recall pico test and was told it passed on September 2025 six months later March 30, 2026, driving to pick up my wife from work a tapping sound coming from the engine started and about a mile down the road from when I first heard the tapping the engine started juttering and the noise got louder like a clunking sound. I had to pull off

I was told by the dealer it could take up to two months for them to receive a replacement engine and once they receive the engine, it can take roughly for maybe five days for turnaround. I inquired whether the replacement engine isn’t upgraded engine or is it the same engine that has all the problems I haven’t received an answer for that yet. I am awaiting that answer from my service advisor. He said he is looking into that.Read more

Vehicle subject to NHTSA Recall 25V796000 (fuel system defect). In July 2025, while driving, a loud explosive sound came from the rear of the vehicle followed by raw gasoline fumes entering the cabin. Vehicle was towed to a Hyundai dealership and held approximately 120 days before being returned as repaired. In December 2025, the exact same defect recurred. A third-party diagnostic scan (performed
... before drop-off) confirmed DTC Code P14EE — Fuel Tank Check Valve / Evaporative Purge System failure — directly related to the recall. The vehicle was held an additional 110+ days while the dealer awaited a new remedy from the manufacturer. Total days out of service: 230+. The recall repair has now failed twice. The safety defect was never successfully corrected after a reasonable number of attempts. The vehicle is currently being withheld by the dealership pending an unrelated disputed repair charge.Read more

Code P0087 on a Kia Telluride indicates that the fuel rail pressure is too low. This means the engine is not receiving enough fuel, often resulting in poor acceleration, stalling, rough idle, and decreased power. It is frequently caused by a faulty high-pressure fuel pump... This is the code is threw when it stalled in busy traffic on a hill. The CEL came on at this point. Then continued to stall
... . Before this incident, it would periodically stall while in park, or rough idle and prolonged startup. However, this was sporadic and couldn't be recreated when taken to a mechanic. We were coming back from vacation with 5 kids in the car ranging from a baby to a 12 year old. It was towed BY Kia to the nearest dealership. They claimed the recalls were addressed and the software updates cleared the codes. I came home tonight and it almost immediately started acting up again. This is a known and recognized issue in certain other vehicles (like the Sedona) and has been given an extended warranty. I'm not sure why this isn't addressed in all their GDI vehicles with these High Pressure Fuel Pumps. I'm afraid of it dying and being stranded in the heat again with young children, as well as being hit when/if it stalls.Read more

High voltage battery management system generated fault codes BMS_u008 and BMS_f071 during a Supercharger charging session, prohibiting drive mode. Vehicle had been driven normally to the Supercharger with no drive-prohibiting alerts during transit. After connecting to the Supercharger, fault codes manifested. Despite fault condition, vehicle continued charging to 93% state of charge before requiri
... ng towing. This is the second such event — original HV battery also generated BMS fault alerts prohibiting drive mode during a charging session in January 2024 and was replaced with a remanufactured assembly. Third-party firmware alert data shows recurring hardware charging fault alerts (CHG_f018, CHG_f076, customer message: Unable to charge) occurring in January and February 2026, weeks prior to the March 2026 failure. Alert DI_u014 also fired repeatedly on March 18, 2026 indicating insufficient 12V supply contributing to undriveable condition — a separate system from the HV battery. Tesla Service Center has not investigated whether Supercharger charging events contributed to either failure, has not produced Supercharger session logs, and has not attempted software-level fault resolution before recommending full hardware replacement. Tesla's own firmware documentation describes BMS_u008 as potentially self-clearing and BMS_f071 as having a documented clear condition. Remanufactured battery subsequently drained from 88% to 0% over 11 days while unplugged at service center — 6-9% per day versus normal 1-3% per day vampire drain, consistent with active BMS fault causing abnormal parasitic draw.Read more

My 2022 Kia K5 GT-Line has had a recurring check engine light fault (P0471 — exhaust pressure sensor circuit) since approximately 9,874 miles in September 2022. The vehicle has been brought to the servicing Kia dealer a minimum of five times for this same fault across nearly 65,000 miles. It has never been permanently resolved. Most critically: dealer repair records from May 2025 (56,501 miles)
... confirm a technician identified fault code P047100, noted it as a history code, cleared it, test drove the vehicle, and returned it without performing any repair and without informing me. Ten months later, the same dealer presented this identical fault as a $1,121.07 customer-pay repair on a vehicle under active 10-year/100,000-mile powertrain warranty. The repair was only approved after I contacted Kia Consumer Affairs directly — the dealer had not contacted them despite claiming otherwise. The February 2026 repair failed within 15 minutes of pickup. I made five documented phone calls to the dealer over 33 days reporting the failure. No action was taken. The April 2026 repair — sensor and hose replacement — failed within one hour of pickup on April 1, 2026. Of additional concern: Recall SC288/SC288A (Fuel Tank Inspection and ECM Re-Update) was performed on this vehicle twice — July 2024 and August 2025. The ECM governs the exhaust pressure sensor circuit. The P0471 fault first appeared in dealer records between these two recall dates and has persisted through both completions. CARFAX confirms the April 2026 repair involved the engine/powertrain computer module. I am requesting NHTSA investigate whether SC288/SC288A is connected to recurring P0471 faults in 2022 Kia K5 vehicles. Kia Consumer Affairs Case escalated April 2, 2026.Read more

The vehicle is experiencing multiple system malfunctions involving the engine performance, fuel system, and electronic throttle control, including engine shaking while idling, activation of the check engine light, a “start/stop unavailable” message, limp mode (inability to switch between driving modes), and illumination of the electronic throttle control warning light. One dealership suggested
... the issue may be related to an open fuel pump recall after noting that previously replaced spark plugs were heavily carbon-fouled, indicating excessive fuel combustion; however, another dealership diagnosed the engine as “scored” and recommended major repair. This diagnosis is disputed, as an independent mechanic performed a borescope inspection and reported no visible internal engine damage. The issue is intermittent—some days the vehicle operates normally, while other days it becomes undrivable. This creates a safety risk, as the vehicle may unexpectedly enter limp mode, limiting acceleration and drivability in situations such as merging or navigating traffic. I have not been involved in any accidents related to this issue and avoid driving the vehicle when symptoms are present. The vehicle has been inspected by both a dealership and an independent mechanic, but not by police or insurance representatives. Warning lights and symptoms, including the check engine light, electronic throttle control light, and start/stop warning, began appearing prior to the drivability failure and around the same time I received notice of the fuel pump recall. The vehicle is available for inspection upon request.Read more

While driving my 2021 Ford F-150, the vehicle suddenly began shaking, lost power, and shut off completely while in motion. This created a dangerous situation as I was left without acceleration while driving. Prior to the failure, there were no major issues with the vehicle. After the engine shut off, a battery warning appeared, but the battery had been in good condition and less than a year old.
... The vehicle would not restart and had to be towed to a Ford dealership, where it has remained for over two weeks. Independent mechanics and the dealership have both indicated the issue is related to the fuel system. I later discovered this matches an active recall for the low-pressure fuel pump. Despite this, I was informed there is currently no remedy available for the recall. The vehicle is still at the dealership and has not been repaired. I have not been provided with a loaner or rental vehicle and have been left without transportation for work or to care for my children. I have also been asked to pay out of pocket for towing and battery replacement, even though the failure appears to be related to a known recall issue.Read more
