While driving on a highway at approximately 50 mph, the vehicle suddenly lost acceleration without warning and was unable to exceed approximately 10–15 mph. This created a dangerous situation requiring immediate maneuvering across multiple lanes of traffic to reach safety. At the time of the incident, the check engine light was illuminated. Diagnostic trouble codes P0191, P2177, and P2179 were
... later identified, indicating fuel pressure irregularities and air/fuel imbalance. The vehicle was inspected by a dealership, which diagnosed a purge valve vacuum leak and faulty high-pressure fuel system components. The repair required replacement of the purge valve and high-pressure fuel system components. The failure involved components within the fuel/propulsion system and engine systems, which are controlled in part by the electronic control module (ECM). This resulted in a sudden and unexpected loss of propulsion while driving at highway speed. Prior to the incident, there were issues with refueling, including difficulty filling the fuel tank when near empty, indicating a potential developing issue within the fuel system. This condition created a significant safety risk due to the sudden and unexpected loss of vehicle speed in active highway traffic, increasing the risk of collision.Read more

I just started the truck, put it in gear, and began driving slowly through the parking lot. The gas pedal was unresponsive—even when pressed all the way to the floor, the truck would not accelerate or move. I have been waiting since September 2024 to use my level 3 charger port again. I can't safely use it without a risk of fire severely limiting the cars use and range. On top of this my main battery was tested at the Nissan Dealer November 2025 and needs replaced under warranty. It will rapidly lose charge when on the highway potentially leaving me stranded on the side of a busy road. I only hav

This is my only car and I cant continue driving it like this. I was informed of the charging issue by recall notice and started seeing the battery issue Summer 2025. If they ever do fix this, I'm going to have a hard time trusting driving it on the highway. Nissan still can't give me an ETA for the replacement battery. The Toyota dealership stated (March 2026) that there may be a problem with the fuel pump, but they were u

Original letter to Hyundai below. The car is currently at a dealer again for repair of the same issue. Fuel pump replaced in Nov 2025 but issue has returned in March 2026. Videos of the issue recently occurring are available. Good morning, We had our 2024 Santa Fe Hybrid lose power while merging onto the freeway in November 2025. I was pressing the accelerator and it did not do anything, just k
... ept rapidly decelerating. Pulled the car to the shoulder and turned it off completely and back on, and the car started up with the engine running which was unusual- typically the car starts up in silent electric mode. It would not accelerate past a few miles per hour. This has now happened on multiple occasions, disappearing for a few days only to return. It has gotten very dangerous to lose speed and acceleration into a busy freeway and almost caused an accident yesterday. We were thankfully close to a dealer (Glassman Hyundai in Southfield, MI) and were able to limp it a mile down the shoulder at low speed to drop it off. However, they are telling us they have no loaners available to provide us which seems unacceptable with a car under warranty with 18k miles on it. Is there anything you’re able to do to assist? Are there any known fixes for the issue that can be reported to the dealer? Thanks, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Incident date: Apr 7, 2026
1. What component or system failed or malfunctioned, and is it available for inspection upon request? The fuel delivery system and engine power management. The vehicle suffers from sudden "Limp Mode" activation and total loss of acceleration. The vehicle is available for inspection. 2. How was your safety or the safety of others put at risk? My safety was severely compromised when the vehicle sud
... denly lost power while traveling at highway speeds. This occurred twice in high-speed traffic, creating an immediate risk of a rear-end collision as the vehicle could no longer maintain the flow of traffic or accelerate away from danger. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? No. The issue was reported to a dealer in 2024, but because the malfunction indicator lamp (MIL) turned off after a restart, the dealer was unable to reproduce the failure or find stored error codes at that time. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes, it was inspected by an authorized Honda dealer in 2024, but no repairs or actions were taken as they could not replicate the fault. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No. There were no prior symptoms or early warnings. In both incidents (2024 and yesterday), the engine light illuminated simultaneously with a sudden, localized loss of speed and a total inability to power up or accelerate while on the highway.Read more

A motorcycle fire occurred involving a Royal Enfield Himalayan 450 while the vehicle was parked and not in operation. The specific component that failed is UNKNOWN. However, based on the observed damage pattern, the fire appears to have originated in the rear-mid section of the motorcycle, in the area near the battery, wiring harness, or fuel-related components. The incident presents a significa
... nt safety concern because the fire occurred without active operation of the vehicle. This raises the possibility of an electrical short, battery-related issue, or fuel system malfunction that could lead to spontaneous ignition. If such a condition were to occur in a confined space (such as a garage) or near structures, it could pose a serious risk of property damage, injury, or loss of life. There were no known warning lights, error messages, or prior symptoms observed before the incident. The failure occurred suddenly. The vehicle has not been inspected by the manufacturer. No technical inspection has been conducted to determine the root cause of the fire. A local fire department investigated the incident and determined that the cause of the fire is “undetermined.” No external ignition source or confirmed arson was identified. The damaged motorcycle remains available for inspection upon request. Given the severity of the incident and the lack of a determined cause, further investigation into potential electrical or fuel system defects is warranted to ensure there is no broader safety risk affecting similar vehicles.Read more

My 2018 Hyundai Santa Fe Sport equipped with a Theta II GDI engine began exhibiting excessive oil consumption, engine knocking, and cylinder misfires consistent with the known Theta II engine defect covered under the Hyundai class action settlement. Prior to the failure, the check engine light illuminated and I experienced repeated misfires and audible engine knocking. The vehicle required frequen
... t oil additions between oil changes due to excessive consumption. The vehicle was taken to an authorized Hyundai dealership where a diagnosis confirmed a burnt valve inside the cylinder head. The dealership recommends replacement of the cylinder head or complete engine replacement to correct the issue. The dealership performed a bearing failure test and attributed the burnt valve to carbon buildup. However, excessive oil consumption in a GDI engine directly accelerates carbon buildup on intake valves, as burning oil deposits carbon on valve surfaces preventing proper seating and causing overheating and valve failure. The carbon buildup is therefore a consequential result of the Theta II oil consumption defect, not an independent cause. The safety risk is significant as engine misfires and knocking while driving create unpredictable loss of power and potential for stalling in traffic. The vehicle is currently not reliably operable. The manufacturer has been contacted regarding coverage under the Theta II settlement. The component is available for inspection upon request at the servicing dealershipRead more

Incident Date: 04/05/2026 Vehicle Speed: 65–70 MPH Location: [XXX] Component: Engine / Power Train / Fuel System Summary of Safety Defect: While traveling at highway speeds (65–70 MPH) with four passengers (including two children), the vehicle suffered a sudden, catastrophic engine failure without any prior warning or symptomatic noise. The Malfunction Indicator Light (MIL) began flashing r
... apidly, immediately followed by a total loss of motive power. Despite throttle input, the vehicle failed to maintain speed or accelerate, creating an immediate hazard in high-speed traffic. Hazardous Conditions encountered: Upon the loss of power, I observed white smoke trailing from the exhaust and a strong burning odor entering the cabin from the engine compartment. To avoid a high-speed rear-end collision, I was forced to perform an emergency maneuver across active lanes of traffic to reach the shoulder. Due to the immediate fear of a vehicle fire (indicated by the smoke and burning smell), my family was forced to evacuate the vehicle and wait outside in a remote location for six hours. Correlation to Known Defects: This incident appears to be a direct manifestation of the defect described in Nissan Recall ID R25E3/R25E4 / NHTSA ID 26V0801 regarding Engine Main Bearing Seizure. The failure resulted in a complete loss of propulsion and a potential fire risk due to fluid ignition or a breached engine block. Post-Incident Status: The vehicle was towed to Nissan of Cumming and is currently awaiting a formal inspection for internal metal debris. The failure of this critical component under highway conditions posed a life-threatening risk to my family and other motorists. I own a 2021 Nissan Leaf subject to open recall R25C8 (NHTSA Campaign 25V655) for fire risk during Level 3 DC fast charging. The recall was issued October 2025. Nissan’s stated remedy target was late March 2026. As of today, April 6, 2026, no remedy has been provided. The affected component is the lithium-ion battery pack and its charging system. The vehicle is equipped with a CHAdeMO DC fast ch
... arging port, which I cannot safely use due to the documented fire risk. I am not avoiding this feature by choice – I am avoiding it because Nissan’s own recall identified it as a safety hazard. On April 6, 2026, I encountered a DC fast charging station and chose not to attempt charging due to the active recall warning, and due to reports that charging networks have begun refusing sessions on recalled vehicles. Diagnostic software shows my battery’s internal resistance index (Hx) at 71%, disproportionately low relative to State of Health at 87.5%. The vehicle is also consuming battery charge at an accelerated rate during highway driving, with approximately 50% of charge depleted over 30 to 40 miles. I am concerned that a software-only remedy will be inadequate for a battery with this level of internal resistance degradation, as software cannot compensate for the absence of active thermal management hardware. No dealer has confirmed the condition. I am filing to document loss of vehicle functionality, Nissan’s missed remedy deadline, and concern about remedy adequacy.Read more

I am submitting this complaint regarding a serious safety‑related failure involving the Totally Integrated Power Module (TIPM) in my 2017 Dodge Durango. Although my vehicle is not included in the 2011–2013 recall window for Safety Notice R0 / NHTSA 14V‑115, it experienced the same dangerous malfunction described in that recall. The TIPM failed without warning and caused a catastrophic failu
... re of the fuel pump. As a result, the vehicle would crank but would not start, leaving my wife stranded and requiring a tow to an automotive repair facility. The repair shop confirmed that the TIPM malfunction caused the fuel pump to fail internally and come apart inside the fuel tank. The TIPM installed in the vehicle is part number P68251629AC A, vendor code 16238DA, with an additional number of 16317G. The TIPM was not inspected anyone other than the repair facility and there were no warning lights or codes prior the failure. The quoted cost for parts and labor to correct this safety‑related defect is $2,100. FCA has refused to cover the repair because my vehicle falls outside the recall years, despite the failure matching the symptoms and safety risks identified in NHTSA Campaign 14V‑115. Given the severity of the failure, the safety risk posed to my family, and the clear similarity to the known TIPM defect, I am requesting NHTSA’s assistance in reviewing this incident, determining whether the recall should be expanded to include additional model years, and supporting my request for FCA to cover the full cost of repairs or reimburse me for the necessary corrective work. This failure created a hazardous situation and resulted in significant financial burden for a defect consistent with an already‑recognized safety issue. I respectfully request that NHTSA investigate this matter and take appropriate action.Read more

On or about Feb10 2026 near [XXX] heading North I experienced an commanded accelerator input. I was stopping for a light on a section of the road where I always lose connection with the apple carplay in the vehicle. I suspect that there is an illegal radio jammer being used. On this particular occasion the traffic stopped at this location where I typically lose the connection from the phone
... to the infotainment system and the infotainment system resets. However this time the throttle advanced to almost full throttle without any input on the accelerator as my foot was on the brake in order to slow. I had to use full application of the brake in order to keep from hitting the car in front of me. Just as I started to shift the transmission to neutral the accelerator went back to idle. The event lasted about 5 seconds. My hypothesis is that some sort of radio interference is causing the system to reset and in this case it caused communication issues with the can bus or engine control module. I have not gone back to try and repeat the issue but the radio interference is still there in the same area. RF interference should not effect the function of the engine or vehicle controls.
