I am reporting a recurring safety hazard involving my 2019 Kia Optima following a repair for diagnostic code P064A (Fuel Pump Control Module). Despite the dealership attempting to repair the fuel system, the vehicle remains in a 'limp mode' state. While driving, the vehicle began shaking violently and lost significant power, refusing to accelerate above 50 mph. This creates an immediate safety ris
... k, as the vehicle cannot maintain highway speeds or safely merge, increasing the danger of a rear-end collision. The P064A code and subsequent 'limp mode' symptoms persist even after professional service. I am concerned that the existing Kia warranty extensions (such as WTY028 for the low-pressure fuel pump) or current repair procedures are insufficient to resolve this underlying mechanical/electronic failure, leaving the vehicle unpredictable and hazardous to operate in traffic.Read more

I am submitting this letter to formally report two ongoing issues with my 2018 Buick Enclave. First, I have experienced persistent problems with the emissions purge valve. My vehicle has repeatedly triggered diagnostic trouble code P0496, indicating a malfunction in the purge valve system. I am aware that this issue has been the subject of recall campaigns for other Buick Enclave models; however,
... my vehicle has not been included in any such recall, despite exhibiting the same symptoms. This malfunction has resulted in persistent check engine warnings and noticeable performance concerns, raising questions about emissions compliance and the overall reliability of my vehicle. Given that other vehicles of the same make and model year have been recalled for the same issue, I respectfully request that my vehicle be reviewed for inclusion in the recall. Second, I am formally reporting a malfunction involving the stabilizers. Recently, I have noticed unusual noises and instability when driving, particularly when turning or traveling over uneven surfaces, which indicates a potential stabilizer malfunction. These issues have persisted despite regular maintenance and inspections, affecting the handling and safety of the vehicle. I believe this may represent a manufacturing or design defect that could pose a risk to other owners of the same make and model. I respectfully request that the National Highway Traffic Safety Administration investigate this matter further. I kindly ask the National Highway Traffic Safety Administration to consider expanding the existing recall to encompass additional vehicles affected by this defect. Please advise if you require further information or documentation regarding my case. Here is an update to the issue I had raised with your office in March-April 2026: Honda Van Nuys performed the 35,000-mile routine maintenance on the Honda Clarity on February 24, 2026. At that time, Honda Van Nuys recommended (and I approved) that they clean the throttle. The odometer read 33,882 miles. I drove the car 337 miles when the unintended acceleration accident occurred on March 18
... , 2026 – 2.5 weeks after Honda Van Nuys cleaned the throttle. The airbags deployed. The throttle seems to be the part that failed based on the attached EDR report. The odometer read 34,219 at that time. The car is now considered totaled by GEICO. In addition, Honda Van Nuys has done all maintenance work on the Honda Clarity, including some major removal of the dashboard and back seats, since I purchased the car there in 2018, as outlined in the attachment. I have reached out to Honda Van Nuys and hope your office can encourage them to respond. This is a safety issue. The attachment elaborates further. Related Recall: 24-01-023G DTC P1A9096 CHECK FOR ICCU & FUSE REPLACEMENT & ICCU SOFTWARE UPDATE (RECALL 021G) On August 8, 2025 I was vacationing and attempted to charge the vehicle at a hotel's level 2 destination charger (32 amp) but the vehicle refused to charge. I then drove 20 minutes away to DC Fast Charge and it worked, and tried level 2 charging again, it still did not work. I ruled out t
... hat the level 2 charging station was the problem due to seeing someone else successfully charging from that station. When I returned home, I found that my home charger also stopped charging at 32 amps (though slower 16 amps still worked). On December 14, 2025 I was driving on the highway when I heard a loud pop noise and the vehicle immediately lost propulsion, resulting in a dangerous situation where the car needed to limp at low speeds to the side of the road. There, the car fully lost all mobility and needed to be towed to a Genesis dealership. They diagnosed the problem as a faulty ICCU, and replaced it as well as installed new software to prevent this situation from reoccurring. In May 29, 2026 the vehicle once again no longer charges at 32 amps at home. The charging would initiate and last a few minutes before fully disengaging and the car reports "Charger Not Connected". Since this is a repeat of the same ICCU failure (above), and I DO NOT wish to lose vehicle propulsion on the highway like before, I have stopped driving the vehicle until it can be diagnosed and fixed by Genesis dealership. I am awaiting for the local Genesis dealership's next availability in a week. I no longer can safely drive my vehicle until this is fixed for a second time. The issue is still unresolved as of today, June 8th 2026.Read more

2025 Honda Civic Hatchback experiencing dangerous mechanical binding, sticking, and notching in the steering column. The defect is strictly thermal, escalating significantly in extreme Arizona heat and/ or after driving for a prolonged duration. This requires excessive driver effort to break the steering wheel free from center, risking over-correction and loss of vehicle control. Additionally, ve
... hicle experiences intermittent powertrain jerking and hesitation during low-speed acceleration. Under active factory warranty, the vehicle was towed to a local Honda dealership. The Service Director initially pushed back significantly, attempting to dismiss the safety keywords and label a major steering defect as a vague 'multi-system failure' on the printed Repair Order. After consumer refusal to accept the vehicle, the Service Director capitulated, personally authorizing a Repair Order explicitly documenting in handwriting: 'Also Steering Sticky while CAR in motion. Also vehicle jerky at low Speeds.' The Service Director physically signed next to these terms. The Service Director explicitly acknowledged that the symptoms sound like the NHTSA recall 24V-744, but stated they must put up to 300 miles on the vehicle to 'replicate' the issue before replacing the steering rack due to fear of corporate financial chargebacks, actively attempting to force a consumer to keep an unsafe vehicle unless able to replicate.Read more

While driving downhill and using regenerative braking, my 2013 Acura ILX Hybrid suddenly experienced a hybrid system malfunction. Immediately after the event, the vehicle displayed "Check IMA System" and "Check Charging System" warnings and began exhibiting drivability and electrical-system problems. Following the failure, the vehicle experienced reduced acceleration from a stop, rough engine res
... tarts, loss of auto-stop functionality, charging-system issues, intermittent low-voltage behavior, and eventual no-start/jump-start conditions. The vehicle was inspected and diagnosed by an authorized Acura dealership. Diagnostic testing identified fault codes P0A78 (Motor Power Inverter Internal Circuit Malfunction), P1440 (Motor Power Inverter Module Output Circuit Malfunction), and P1437 (Motor Power Inverter Module Short Circuit). The dealership determined that the Motor Power Inverter had failed and required replacement. The failure affected vehicle propulsion, charging-system operation, and regenerative braking operation. Reduced acceleration and unexpected hybrid-system malfunction while driving created concerns regarding vehicle safety and reliability during normal operation. The Motor Power Inverter was replaced by an Acura dealership and the warning messages, drivability concerns, and related symptoms were resolved. I am submitting this complaint to document a confirmed Motor Power Inverter failure involving a 2013 Acura ILX Hybrid.Read more

On June 1, 2026, software update 68C was performed at the dealership on my 2025 Jeep Wrangler 4xe. On June 6, 2026, at approximately 9:00 a.m., after driving the vehicle for about 25 minutes, I experienced a serious malfunction. While making a turn at a speed of less than 10 mph, the vehicle suddenly and without warning lost throttle and propulsion. It appeared to hesitate as though shifting betw
... een electric and gasoline power, and then abruptly and erratically lurched forward, accelerating to what felt like maximum speed. The Jeep struck a chain-link fence, which slowed its movement, but it continued to surge forward. I applied the brakes and attempted to maintain control of the vehicle, but it kept lurching ahead. At no point did the vehicle emit a safety warning, sound an alert, or attempt to stop automatically, even though it normally responds to obstacles when operating properly. I repeatedly attempted to place the vehicle in reverse, but it did not respond. I turned the engine off and back on several times, and only after more than three attempts did the vehicle finally recognize reverse, allowing me to back away from the fence. A man in a truck assisted me and contacted the police. Both the responding police officer and the assisting individual expressed concern about the potential danger, particularly because the vehicle is a 4xe. I am now extremely hesitant and fearful to operate or move the vehicle. The vehicle is available for inspection and has not been inspected yet by dealership or insurance. I am writing to report a severe safety defect regarding my 2018 Jeep Renegade. The vehicle suffers from an extreme internal oil consumption defect, burning 1 quart of oil every 1,000 miles. According to certified automotive diagnostics, this is five times the normal operating rate. This rapid oil depletion causes the vehicle to abruptly lose power, stall, and completely shut down while driving in
... active traffic without warning. Sudden engine failure at highway speeds creates an immediate, critical risk of collisions. The vehicle’s history directly matches documented manufacturer issues. The original purchasing dealership notified me that the vehicle was impacted by Customer Satisfaction Notification W80 for abnormal oil consumption. My research also confirmed related under-hood wiring recalls for this model year that cause fuel pressure drops and engine stalling. However, when I presented the vehicle to an authorized Jeep dealership in Newnan, Georgia, due to repeated stalling, the service department refused recall coverage because my specific VIN was not flagged in their database. They replaced a valve cover gasket instead, but the severe oil depletion and dangerous stalling persisted. A subsequent inspection by an independent licensed automotive shop confirmed the excessive oil burning and determined the engine has suffered fatal internal wear, requiring a complete engine replacement. Because the manufacturer excludes my VIN from the official recall population, they refuse to provide any administrative or financial remedies. However, my vehicle is exhibiting the exact mechanical failures, dangerous stalling, and severe oil consumption targeted by NHTSA’s investigations into the engine. Excluding a vehicle experiencing identical safety failures based solely on an arbitrary VIN window leaves consumers unprotected. This unaddressed defect ultimately rendered the vehicle un-driveable, forcing me to surrender it, causing financial hardship and negative credit impact.Read more

I am reporting the failure of Ford Motor Company to perform a mandatory safety recall (Recall 22S43 / NHTSA ID 22V413) on my 2019 Ford Escape. This failure is compounded by evidence of falsified internal service records (OASIS reports) that misrepresent the vehicle's recall status as 'completed.' Furthermore, the failure to perform this specific recall directly prevented a necessary diagnostic
... inspection of the transmission linkage and adjacent engine compartment. Had this recall been performed, the technician would have identified the progressive corrosion and mechanical stress that subsequently destroyed the high-pressure fuel pump and the Transmission Control Module (TCM). The manufacturer meticulously recorded routine maintenance while ignoring a critical federal safety mandate. Their negligence directly caused a cascading mechanical failure, rendering my vehicle an unsafe, non-operational hazard. I am seeking a formal investigation into these falsified service records and the manufacturer's failure to remedy this safety hazard. I attempted to resolve this manner at the lowest levels, before taking this route. Unfortunately, to their detriment; Ford basically told me I was on my own. I have already alerted the Attorney General for the State of Tennessee, as well. The subsequent loss of the vehicle as a result of the abhorrent behavior of Ford Motor company has resulted in time lost with my kids and my mental health appointments with the Veterans Administration. I can not make it to my therapist or get my medication. I have the Carfax and Oasis reports as further evidence to back these claims.Read more

The affected component is the fuel injector system covered under Ford’s safety recall. I use this vehicle as my primary means of transportation and have concerns about my safety and the safety of passengers due to the possibility of a fire occurring before a repair is available. The issue has been confirmed by Ford through its issuance of the safety recall. I have contacted Ford and dealership service departments regarding the recall. However, I have been informed that repair parts are not yet available, leaving the vehicle unrepaired despite the acknowledged safety defect. The vehicle has been inspected by authorized Ford dealerships in connection with related concerns. I have also communicated directly with Ford regarding the recall and the lack of an available remedy. No specific warning lights or messages related to a cracked fuel injector appeared prior to my awareness of the defect. On November 20, 2025, my 2022 GMC Sierra 1500 AT4 equipped with the 3.0L Duramax diesel engine began experiencing intermittent engine overheating conditions. Concerned about a potential safety issue, I contacted a GMC dealership and was advised that because no diagnostic trouble codes were present, there was nothing to diagnose or repair and that I should continue driving the vehicle. Over the fol
... lowing months, the overheating condition continued intermittently. On May 24, 2026, while driving on the highway, the vehicle suddenly overheated, displayed fault code P1098, and entered reduced power ("limp mode"). The vehicle lost normal performance and acceleration while traveling in traffic. This created a significant safety hazard because the vehicle was unable to maintain normal highway speeds, increasing the risk of a rear-end collision or other traffic accident. The loss of power occurred while driving and without sufficient warning, leaving the vehicle unable to safely perform as expected in highway traffic. After researching this issue, I found numerous reports from other GMC and Chevrolet owners describing similar overheating, coolant control valve, thermal management system, and P1098-related failures. Many owners report symptoms involving engine overheating, reduced engine power, and limp mode operation while driving. I am concerned that this may indicate a broader defect involving the vehicle's cooling system and thermal management components. The vehicle exhibited overheating symptoms for approximately six months before the final failure occurred. Additionally, I understand that certain GM vehicles have been subject to service actions involving coolant control valves and thermal management system components. Given the similarity of symptoms, I am concerned that the overheating condition reported in November 2025 may be related to the same type of failure mechanism, yet my vehicle has not been included in any recall or special coverage program.Read more

I had pickup my vehicle from the dealership where it had been since the 29th of May due to a previous incident exactly the same as what occurred this time. The vehicle had been returned to me as the service department could find nothing wrong from the May 29th incident. On my way home from the dealership, the vehicle started the same as on May 29th. When accelerating from a stoplight, I lost al
... l propulsion. After about 60 seconds the vehicle recovered and I turned to go back to the dealership. At the aforementioned intersection (crossing over I35) it again lost propulsion, only this time for a longer period of time. I was in heavy traffic and the car behind me was honking even though I had my hazard warning flashers operating! I made it to the dealership where the service manager agreed to do a ride along. We were about to the end of the ride along when the vehicle lost propulsion power. It wasn’t as severe and recovered quickly. The service manager said he had noticed the brake pedal move at the time I accelerated. His theory was that the brake being applied at the same time as the acceleration cause the computer to get confused and it’s reaction was to remove propulsion. No offer was given to take the vehicle back for further testing. I dropped the service manager off and proceeded to return home, being cognizant of my foot placement and insuring it was well away from the brake when accelerating. KIA Care has so far refused to get involved. While I recognize braking at the time of acceleration is counterproductive, it should not cause a catastrophic propulsion failure. Plus the system sets no flags that it had this event! I have tried recreating this failure by using my left foot on the brake while accelerating with my right. This doesn’t recreate the problem so it must be a timing issue. I do not know if this issue is unique to my vehicle or is shared with other hybrids! Otherwise, I would have had to PAY FORD $2,000 to make up the difference in “value” (I disputed the value as I had one KBB report and they had a “different” one) and my loan payoff. I stated several times I cannot put thousands of dollars down to purchase a new vehicle at this time; however, I do not want to drive this unsafe hazardous vehicle for another day. They would not work with me at all nor did they offer to fix it or even look at the vehicle. Under NO circumstances should an owner have to pay Ford for their negligence and poor manufacturing, especially with a recent DO NOT DRIVE ADVISORY issued, no matter how much the owner owes on their loan. This is 100% a FORD issue, not a user issue, not wear and tear issue. These recalls are wildly unsafe and defective issues that lie within FORD that we as owners, should not bear the burden for. How is FORD being held accountable and at what point will they start buying these unsafe and terrible cars back without penalizing the owners? It seems far more logical and safe to buy back and lose out on $2,000 rather than taking the risk on paying millions and millions of dollars in lawsuits. This is a disgusting business practice on behalf of FORD and they should be held accountable.Read more

I am the owner of a 2020 Jaguar I-PACE, VIN: [INSERT VIN], subject to NHTSA Recall 26V067. The interim software remedy under campaign H570 has been installed on my vehicle, capping the battery charge at 90%. I reside in a designated fire hazard zone in Santa Monica, California, in a densely populated residential area. I contacted NHTSA directly on June 5th 2027 at appx 10:20 AM and was advised th
... at it is still not safe to park the vehicle inside or charge it indoors, even after the H570 software update has been applied. However, the Executive Liaison at Jaguar Land Rover North America's Customer Relationship Center, Judianne Di Leo, stated in writing yesterday that, following the H570 update, all parking and charging precautions are lifted and the vehicle can be operated as normal. This directly contradicts the guidance I received from NHTSA. I submitted a formal repurchase request to [XXX] under the California Song-Beverly Consumer Warranty Act. Jaguar Land Rover North America denied that request in writing, with no explanation or legal basis provided. The permanent remedy under campaign H572 remains open and under development with no committed timeline. I am filing this complaint because I believe Jaguar Land Rover North America is providing customers with safety guidance that contradicts NHTSA's own recall documentation, and because I am unable to safely comply with the parking and charging instructions in a fire hazard zone with no remedy INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Subject: Request for Investigation / Status Update: Unresolved Safety Recall for 2023, Lincoln Aviator To Whom It May Concern, I am writing to formally request an update and urge an investigation into the lack of a remedy for an active safety recall on my vehicle. ... ade parts available. Here are the details of my vehicle and the recall: Vehicle: 2023 Lincoln Aviator VIN: [XXX] NHTSA Recall Campaign Number: 25S75/25V455 Date Recall Issued: July 2025, 10 months ago Federal law requires manufacturers to remedy safety defects within a reasonable timeframe. Waiting nearly a year for a safety-critical fix leaves me driving a vehicle with a known hazard, which is an unacceptable risk to my safety and the safety of others on the road. When I contact the manufacturer or my local dealership, I am repeatedly told that "parts are not available" or that "the remedy is still under development." Could you please provide an update on the status of this campaign? Furthermore, I ask that NHTSA review this delay to determine if Lincoln is failing to meet its legal obligation to remedy this defect in a timely manner. I was not going to put in a complaint about this failure but after learning about GM creating a special extended warranty program for this issue, I felt it should be addressed and treated as a recall. Incident date: May 31, 2026
Car has only 3307 miles and incurred a DTC P1A9096. This meant the vehicle only had power from the 12 volt battery, not traction battery. Luckily, the event happened 3 miles from home and we were able to just make it. Next day upon calling for flatbed tow service, the car generated an additional DTC P05616. 12 volt battery was exhausted and nothing functioned on the car. Had to then call for a
... different tow truck with dollies. The failure of the ICCU (Integrated Charging Control Unit) was a cause for recall on earlier year's models. This is a major safety issue as failure of the ICCU leaves you stranded with an inoperable vehicle where you cannot even lock the doors.
