The electrical wiring harness connecting to the hybrid system electric fuel pump(s) was found to have significant corrosion and a missing factory-installed protective covering. This is a manufacturing defect. The protective covering was never installed at the factory, leaving the wiring exposed to moisture and corrosion from the time the vehicle was built. COMPONENT THAT FAILED: Electrical wiring
... harness to hybrid electric fuel pump(s). Missing factory protective covering caused corrosion. The failed component is available for inspection upon request. SAFETY RISK: Corroded wiring in a hybrid vehicle electrical system poses serious risks including electrical shorts, potential fire hazard, and sudden loss of hybrid propulsion while driving. The vehicle is currently inoperable and unsafe to operate. CONFIRMED BY DEALER: Yes. The vehicle was taken to an authorized Kia dealership where it remained for approximately 8 months. The dealership and manufacturer inspected the vehicle and confirmed the wiring issue but denied warranty coverage. THIRD-PARTY CONFIRMATION: A third-party warranty representative independently reviewed photographic evidence and confirmed the missing protective covering is a manufacturing defect, not wear-and-tear or owner-caused damage. MANUFACTURER RESPONSE: The manufacturer denied warranty repair, falsely claiming the hybrid component warranty does not transfer to subsequent owners. Federal warranty law requires transfer unless explicitly excluded in the warranty terms. WARNING SIGNS: No warning lamps, messages, or symptoms appeared prior to the failure. The defect was hidden beneath the vehicle and not visible during normal operation or routine maintenance.Read more

The failure appears to involve the vehicle’s auto start/stop system and related electrical control systems, including the battery management system and/or body control module. The vehicle shuts off unexpectedly while driving and fails to automatically restart, displaying “Auto Start/Stop – Manual Restart Required” and “Shift to Park.” The vehicle is available for inspection upon reques
... t. The vehicle has shut off unexpectedly in active traffic, causing a sudden loss of power. This creates a dangerous situation where I am unable to accelerate or move with traffic flow, increasing the risk of being rear-ended or involved in a collision. This has occurred on public roads with other vehicles present, putting both myself and others at risk. The issue is intermittent and has not consistently been reproduced on demand. However, it has occurred multiple times during normal driving conditions. I am in the process of having the vehicle inspected and documented by a dealership. The issue has been reported to the manufacturer, and a case has been opened with Ford Motor Company. At this time, the vehicle has not yet been inspected by police or insurance representatives. There were no consistent warning lamps prior to the failure. At the time of the incident, the vehicle displayed messages including “Auto Start/Stop – Manual Restart Required” and “Shift to Park.” These messages appeared simultaneously with the loss of power. The issue has occurred intermittently without advance warning.Read more

Pulling out in front of High Speed Traffic, Engine went into Safe Mode with Check Engine Light. Fortunately a car at high speed avoided rear ending my car. Upon evaluation, DTC Code P0303 (Cylinder 3 - Misfire) was present. Coils and Spark Plugs were changed but car was still misfiring. Honda has a Warranty for 150k/ 10 year on Fuel Injectors but this VIN was not on list based on Honda Service De
... partment. The Honda Service Department evaluated and Car has Head Gasket Leak in Cylinder 3. This is a widely known problem reported on many Honda Owner websites and Honda Service acknowledged it was an issue but is not a “Recall” despite the potential Safety Risk on the Road when car goes in to Protect / Shutdown Mode. The nature of this issue occurring on a well maintained car with 147k miles that is 8 years old and not being a Recall and a Safety related item is being reported by me as me and my family could have been impacted by a rear end collision as car shut down / protect mode. This could and likely has occurred to other owners and possibly attributed to accidents and Safety of passengers. Honda wants over $11k to repair the car which includes Fuel Injectors / Plugs / Coils / New Head as they reflect old head can’t be used even if machined / Head Gasket. This issue is a Safety related issue and should be a Honda Recall as the Head Bolts are a known design issue causing premature failure of the Head Gasket. Submitting this to document my incident, protect the public driving this model and year and hopefully providing information to warrant a recall so that all repairs will be performed under Recall Warranty. Also to protect the public from a Honda Accord Safety issue for 2018 Honda Accord LX.Read more

This appears to be a fuel pump or something with the fuel issue on a 2021 Toyota Highlander L series. While driving it would stop and switch off at stop lights and stop sign. I am reporting a serious safety concern with my 2016 Infiniti QX60 involving a defective cooling fan system. The vehicle currently experiences a ticking/tapping noise from the cooling fan, and the fan does not operate properly, which has caused the engine to overheat. This condition creates a significant safety hazard, as engine overheating can lead to sudden engine failure while driving, increasi
... ng the risk of an accident. The issue also makes it unsafe to use essential features such as the air conditioner in hot weather and the defroster in cold conditions, which impacts visibility and driver safety. This defect appears to be part of a known issue affecting this vehicle platform, where the cooling fan system fails prematurely. I understand there have been allegations, including in Anderson v. Nissan North America, that the manufacturer may have been aware of this defect but did not adequately disclose it. In addition, I am aware that this vehicle model has been associated with other safety-related recalls, including issues involving the ABS actuator (which may pose a fire risk) and the hybrid powertrain control system (if applicable). These existing concerns further highlight the urgency of addressing this defect. This is not normal wear and tear, but a potential safety-related defect that places drivers, passengers, and others on the road at risk. I am concerned about continuing to operate this vehicle and potentially causing an accident or injury due to overheating or loss of vehicle function. I am requesting that this issue be formally investigated to determine whether a recall or corrective action is necessary to prevent harm.Read more

The component that failed in both incidents was the vehicle’s wiring system, specifically wiring insulation that appears to be vulnerable to rodent damage. In the second incident, squirrels again accessed the engine compartment and chewed through wi
... ring affecting the braking system. This resulted in a sudden and unexpected loss of braking function. Yes, the damaged components should be available for inspection through the repair shop or service records, although I would need to confirm the exact location of the retained parts. Safety was significantly put at risk. During the second incident, the vehicle was occupied by multiple passengers, including minors. While approaching a red light, the driver attempted to brake and found that the brakes were not functioning properly. The driver had to shift the vehicle into park and use the emergency brake to stop, narrowly avoiding a serious collision. This created a high-risk, potentially life-threatening situation. The first incident was addressed by a mechanic after the check engine light appeared, confirming damage to the fuel injector-related wiring. The second incident was formally confirmed by a mechanic that the brake line was leaking from damage from, what they highly suspected and have seen in the past, a rodent . In the first incident, a check engine light appeared, prompting immediate attention. In the second incident, there were no warning lights, messages, or prior symptoms before the brake failure occurred.Read more

An “Emissions System Problem” warning message appeared intermittently on the dashboard while driving. The issue was confirmed by the dealership and is reproducible through stored fault codes, even when the warning light is not actively displayed. This condition may impact engine performance, fuel mixture, and emissions system operation. In certain situations, this could lead to reduced engine performance or hesitation, which may affect the ability to safely accelerate or merge into traffic. The vehicle has been regularly maintained according to manufacturer recommendations. There were no significant warning signs prior to the initial dashboard alert other than the intermittent warning message. I leased a 2026 Electric GV70 from Genesis in June 2025. In January of 2026, when driving the car I got a warning sign to stop the vehicle and check the battery supply. At that point, the car went into sever power reduction and would only go 20 MPH or so. After 20-30 minutes, the car completely shut down and became inoperable. Luckily, I was near home and was able to get my car home. I had to have
... the vehicle towed on wheels as it was completely frozen to the dealership. They diagnosed it as a failure of the Integrated Charging Control Unit (ICCU). They replaced the ICCU but identified other issues that to this day are unresolved and my car is still in the shop. These cars were subject to a recall for ICCU failure but only to model year 2025. These ICCUs are continuing to fail on the 2026 models, and at an alarming rate. The head of service at the dealership told me they were dealing with "hundreds" of ICCU failures on Hyundai/Genesis vehicles. If I had been on a highway traveling at higher speeds and the car suddenly lost almost all power and would only drive for another few miles before completely shutting down, it would have created a dangerous situation. These cars need to be recalled - again. Hyundai/Genesis clearly dont know how to make electric cars/battery packs that work and should be held accountable. I am seeking a case review regarding a persistent and unresolved drivetrain issue with my 2025 Toyota Tundra (non-hybrid) that began immediately following a dealership service visit and has continued for over one month without resolution despite multiple repair attempts. Since that time, the vehicle has exhibited consistent transmission-related defects including failure to shift properly into high
... er gears (notably not reaching 10th gear at highway speeds), RPM hanging under light throttle, delayed acceleration followed by sudden surges in power, and harsh or erratic downshifting that causes the vehicle to lurch forward when slowing to a stop. The vehicle also demonstrates inconsistent torque delivery, where boost builds but power is not transferred until a delayed and abrupt engagement occurs. These issues are most pronounced during normal highway driving conditions and significantly impair drivability and safety, particularly when merging or maintaining speed. I have brought the vehicle to multiple Toyota dealerships for inspection, where diagnostic scans have not produced any fault codes and I have repeatedly been told the vehicle is operating “within normal parameters,” despite the ongoing and reproducible performance issues. The dealerships have been unable to replicate or resolve the problem, and I have received inconsistent explanations ranging from normal operation to speculative driving pattern causes. I have an active case open with Toyota corporate, but after a month of delays, lack of diagnosis, and no corrective action, the issue remains unresolved. We believe the evaporative emissions control system (EVAP) / fuel vapor management system on our 2024 Mazda CX-90 MHEV (inline-6 mild hybrid) is malfunctioning, releasing strong raw gasoline vapors. The strong odor occurs everywhere around the vehicle while the engine is running and immediately after shutdown and exit. The problem is intermittent. No visible liquid fuel leaks have been observed. T
... he vehicle and related components remain available for inspection upon request. This creates a direct safety risk: the hazardous flammable gasoline vapors are strong enough to migrate from our garage into our living space (house), posing a potential fire/explosion hazard if vapors accumulate near an ignition source, as well as a health concern from prolonged inhalation for our family. Raw gasoline vapors are highly flammable and should not be present in or around a residence. The problem has been reproduced and confirmed by the dealer on at least one occasion. On the third service visit (dropped off March 5, 2026), the strong raw gasoline odor was so intense upon arrival that multiple dealership personnel, including General Manager Weldon Munsey, witnessed it immediately, and one technician looked under the vehicle to check for liquid gasoline leaks. On the two prior visits, technicians were unable to replicate the issue. The local dealer has informed us that they are aware of two other CX-90s with the exact same problem occurring at the same time within their dealership network. Despite having no root cause, no fix identified, and no timeline for resolution, the dealer wants us to return the loaner and take our car home unrepaired. The vehicle was purchased new on [XXX], and currently has 39,213 miles. It has been inspected multiple times by the dealership (Roger Beasley Mazda Georgetown, repair order [XXX]). Mazda Corporate has an open investigation (their case [XXX]) and has assigned a case manager (Chris) who is currently reviewing the issue with the service INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIORead more

Vehicle experienced white exhaust smoke due to oil entering the exhaust system, caused by turbocharger failure. This led to contamination and failure of all O2 sensors, requiring full replacement, along with replacement of both turbochargers. The radiator also failed and had to be replaced. This appears to be a cascading mechanical failure affecting multiple major systems, including the engine, f
... uel/propulsion system, and electrical components, rather than normal wear and tear. The failure rendered the vehicle inoperable, leaving me without transportation during a period of financial hardship. The severity and extent of repairs raise concern about a potential broader defect. This also created a safety concern due to excessive exhaust smoke and reduced vehicle reliability while in operation.Read more

The issue first began in February 2026, when the vehicle started showing signs of engine malfunction and poor performance, which ultimately led to a full engine replacement by an authorized Nissan dealership. After the engine replacement, the vehicle continued to experience serious and recurring drivability issues. On multiple occasions, including March 16–17, 2026, and most recently on April 1
... 1, 2026, the vehicle failed to accelerate properly even when the gas pedal was fully pressed. The vehicle would move extremely slowly and felt as if it was stuck in a low gear, with loud engine noise and lack of power. This created a significant safety hazard, as I was unable to accelerate normally in traffic, increasing the risk of being rear-ended or unable to safely merge or cross intersections. Additional symptoms included: •Engine shaking while idling •Intermittent loss of power and poor throttle response •Transmission not shifting properly •A/C blowing warm air at idle but cold while driving •A diagnostic issue indicating loss of communication with a control module (VCR/module) The problem is intermittent and unpredictable, making it difficult to demonstrate on demand, but it has occurred multiple times in real driving conditions. The vehicle has been inspected multiple times by an authorized Nissan dealership, including technicians and a shop foreman. The dealership has stated they were sometimes unable to reproduce the issue and have attempted repairs such as performing an idle relearn, but the problem persists. Despite major repairs, including a full engine replacement, the vehicle continues to experience dangerous loss of acceleration and drivability issues, posing an ongoing safety risk. The vehicle is available for inspection upon request.Read more

Post fix EDR cooler failure causing coolant fluid loss " enough complaints =pressure on Audi Reference EA189 emissions fix Subject: Goodwill Assistance Request – 2014 Q5 TDI EGR Cooler – VIN [XXX] Audi Customer Experience Team, I am writing to request goodwill assistance for my 2014 Audi Q5 TDI, VIN [XXX] , with 105,000 miles. Vehicle Issue The car is losing engine coolant with no exter
... nal leaks. Symptoms are consistent with a failed EGR cooler: low coolant warnings every few weeks, white smoke on cold starts, and no puddles or visible leaks. This is a documented failure mode on the 3.0L TDI V6 engine, especially after the emissions modification. Warranty Background - Original in-service date: [XXX] - Emissions modification completed: 2018 - Current mileage: 105,000 I understand the EGR cooler is a covered component under the TDI Extended Emissions Warranty as an emissions-related part. While the 10-year time provision from the in-service date expired [XXX], I am well under the 120,000-mile limit. The 4 years/48k miles from the 2018 modification also places the failure directly within the spirit of the extended coverage. Basis for Goodwill Request The EPA-approved emissions modification increased EGR duty cycles and thermal stress on the cooler. This failure is a direct consequence of that modification. Repair quotes from Audi dealers are $1,200-$2,000 due to the labor involved to access the cooler in the V of the engine. The vehicle has been maintained with VW 507.00 specification oil and has no aftermarket modifications. I intend to keep the vehicle long-term if repaired. Request I respectfully request Audi cover the diagnosis and replacement of the EGR cooler under goodwill. I am happy to have the work performed at an authorized Audi dealer in Houston. I appreciate your consideration and can provide service records or additional photos if needed. I can be reached at [XXX] or this email. The 2025 Hyundai Hybrid Santa Fe SEL has experienced two instances (Nov 2025, 11k miles & Apr 2026, 14k miles) where the combustion engine fails to provide power to the vehicle’s wheels. The engine continues to run, but only enough to maintain a charge to the hybrid battery system. When accelerating to speeds above 30 miles per hour or when traveling up inclines, the combustion engine’s rotati
... ons per minute (rpm) spiked, however the vehicle’s power-train still rapidly lost power and the hybrid battery drained significant charge. During one instance in November 2025, while attempting to accelerate to 55 mph up an incline, the hybrid battery drained enough that the vehicle was required to be pulled off the road to allow the battery to charge. During the April 2026 event, while containing a family of four (including to young children under six years of age), the vehicle failed to accelerate to 60 mph up an incline on an interstate freeway bridge. The vehicle rapidly slowed to between 20 and 30 mph, putting the family in grave danger from high-speed collisions. Following both instances, the on-board diagnostics computer was reset, “resolving the issue” and the Hyundai technicians elected to make no further repairs or replacements. The onboard diagnostics code P0191 was reported following the April 2026 event, however no codes or errors were identified by the Hyundai technician following the November 2025 event. No additional follow-up was performed, even though the vehicle remained under the manufacturer’s warranty.Read more

The system that failed is the high-pressure fuel pump and the intake fuel pump. The fuel system pressure is too low, and the cold start fuel pressure is too high. This is defective because I purchased the car used in November 2023. The car is a safety risk as it stalled immediately yesterday while I was driving with my children and it began smelling like gas fumes from exhaust. The vehicle has bee

On [XXX] at approximately [XXX] PDT, a 2024 Mercedes-Benz EQS SUV 450+ caught fire at an intersection in Irvine, California. The vehicle was being driven normally. It was NOT charging. It was NOT in a collision. It was stopped at the intersection when the fire began. There was no warning. Four occupants were inside: the driver (age [XXX] ), two minor children (ages [XXX] and [XXX] ), and one eld
... erly passenger (age [XXX] ). All four escaped, but narrowly — the rear child safety locks barely released in time for the children to exit. The vehicle sustained catastrophic cabin burn-through and is a total loss. Smoke and flames appeared to originate from beneath the vehicle, consistent with a thermal event in the under-floor high-voltage lithium-ion battery pack, pending expert investigation. This vehicle is within the population of NHTSA Recall 24V-372 (Mercedes-Benz battery management system software defect, 14,912 2023-2025 EQE/EQS units, including 551 units of the 2024 EQS SUV 450+). Mercedes documented in its Part 573 report that it received field reports of this defect in October 2023 and waited approximately seven months before filing the recall. Mercedes acknowledged fire risk in the EQS platform in NHTSA Recall 24V-115, stating "a risk of fire cannot be ruled out." In February 2026, Recall 26V-073 ordered full battery replacement in EQB vehicles after Mercedes admitted prior software remedies were failed remedies. On [XXX], a Mercedes EQE caught fire spontaneously in a parking garage in Incheon, South Korea while parked and not charging, damaging 140 vehicles. The fire in this vehicle occurred at zero mph with two young children and an elderly grandmother trapped behind child-locked rear doors. The fire department and police responded. The vehicle is preserved pending expert inspection. I request NHTSA ODI open a formal investigation into spontaneous battery fire risk in the Mercedes EQ platform.
