The dual clutch PowerShift DCT transmission has failed. When placed in reverse, the engine RPM increases when accelerating but the vehicle does not move at all. While driving forward, the vehicle struggles to reach 10mph and the transmission slips severely. The vehicle is essentially undriveable. The vehicle has gotten stuck on neighborhood roads on 3 separate occasions due to complete loss of re
... verse function. I was stuck for 30 minutes to an hour which put me in way too much preassure and possibly, I could have ended up in danger. The vehicle had to be physically pushed by hand to move it. This created a dangerous situation for myself and others on the road. An independent mechanic diagnosed and replaced the clutch actuator assembly and fuel pump. After repairs the problem persists unchanged. The mechanic has noted the vehicle requires a full transmission replacement. This is a known defect with the Ford PowerShift DCT transmission affecting multiple model years. Repair costs have exceeded $1,500 with no resolution. The failed transmission puts safety at risk as the vehicle cannot accelerate properly in traffic and has no reverse function. Problem has been confirmed by an independent service center. Vehicle is available for inspection.Read more

NHTSA Recall Number22V312000 I am writing to formally file a complaint regarding my 2013 Hyundai Sonata (VIN: [XXX] ), which is subject to a manufacturer recall #189 due to a fuel line defect posing a fire risk and potential severe injury. Today, I visited Wallace Hyundai in Fort Pierce, Florida, and the service manager Mr. Louis LaRussa and associate employed what I can only describe as a warrant
... y circumvention tactic. They repeatedly delayed my recall repair by claiming I must first complete other optional recalls, unrelated to the fuel issue. They stated that they would not be paid by the manufacturer unless all recalls were done together. This is not only unprofessional but also a disregard for the serious safety hazard posed by this fuel line defect, they prioritize dealership financial interests over customer safety and contradicts the purpose of a safety recall. Furthermore, I was told I would need to leave my car for days or even weeks because parts supposedly must be ordered. As this is a nationwide recall, I expect a swift, ready-to-implement solution. I demand immediate action from Hyundai to honor the recall and ensure my safety is prioritized. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

After I purchased my 2021 Toyota Tacoma TRD Sport with 4 miles on it and at 1,332 miles on 11/26/21, the Check Engine light with Traction Control Off light came on while I was driving. I immediately pulled over and on the truck media screen it read this: "A malfunction in the electronic control of the engine, throttle or automatic transmission has been detected. Contact your Toyota dealer and have
... your vehicle inspected." The truck was rough idling and had no acceleration. I had the truck towed to dealership where it was purchased and upon diagnostic, it was determined that #3-cylinder injector was reading "Open Circuit" with code P020313. Being under warranty the dealership repaired the bad injector. No further issues until 4/22/26. On 4/22/26 with mileage at 48,361, I started my truck in my driveway and immediately recognized that the truck was rough idling and the Check Engine Light along with the Traction Control Light Off being on. The exact same message displayed on the screen as above, "A malfunction in the electronic control of the engine, throttle or automatic transmission has been detected. Contact your Toyota dealer and have your vehicle inspected.". The truck was towed back to the same dealership and upon diagnostic the same issue of #3-cylinder injector "open circuit" with code P020313 was revealed. It was recommended to replace the bad #3-cylinder injector and also recommended to go ahead and replace all of the spark plugs. The total repair bill we be ~$2600 when I go to pick it up in 3 days. This is not just a coincidence that the exact same #3-cylinder injector has gone bad at 1,332 miles and then again at 48,361 miles. Seems to be a connection between the injector going bad and the issues that they have had with the High Pressure Fuel Pumps. Toyota needs to acknowledge this problem.Read more

I was mailed a letter about a recall for engine bearings. When i called the dealership, they told me they did not have the parts and that Nissan would reach out. Then critical engine warning lights came on, so Nissan t
... owed it to the dealership. The dealership wants $5,000 for a bad turbo and says it is not covered under the recall, only engine bearings and a turbo hose are covered. I believe the ongoing engine failures, bad manufacturing, negligence on behalf of Nissan is causing these issues and it should be covered. This can also cause the throttle body to go bad, which would be another couple thousand dollars. This car is 3 years old, and i still owe 4 years of payments for an unsafe vehicle that i can not drive. I am attempting to escalate this with Nissan, but so far, it has not been covered.Read more

While driving my 2025 Ford Bronco Sport (approximately 14,600 miles), I made contact with an object on the roadway that did not feel like a severe impact. Immediately afterward, the vehicle began leaking fuel from the underside. The vehicle was not driveable due to the active fuel leak and had to be towed to a dealership for inspection. This situation posed a significant safety risk due to the p

The issue began immediately following the contact with the object. The vehicle has not yet been fully inspected at the time of this report, but it is currently at a Ford dealership awaiting evaluation. Based on the nature of the incident, there is concern regarding the durability and protection of the fuel tank when exposed to common road hazards. A fuel system failure under these conditions may present a safety concern.Read more

NHTSA Complaint Statement Subject: Stop‑Drive Recall – Vehicle Held Since March 5, 2026 – No Remedy, No Transportation, Buyback Delayed I am filing a complaint regarding a stop‑drive safety recall that has left me without access to my vehicle since March 5, 2026, and the manufacturer has not provided a timely remedy, transportation assistance, or a completed buyback. My vehicle has been at
... the dealership since March 5 due to a manufacturer-issued stop‑drive recall. The dealership has confirmed they cannot legally release the vehicle because of the stop‑drive order. The vehicle is not repairable at this time, and the manufacturer initiated a buyback process. However, the manufacturer has been extremely slow to communicate or complete the buyback. Despite multiple attempts to contact them, I have not received a clear timeline, and the process has stalled. I have also not been provided with a loaner vehicle or consistent rental reimbursement, even though the stop‑drive order prevents me from using my car. Because of the delay, I am now facing financial hardship, including a past‑due car loan on a vehicle I cannot legally drive and cannot retrieve. I am effectively without transportation and without a remedy. I am requesting NHTSA’s assistance because the manufacturer has not met its obligations to provide a timely remedy or adequate support for a stop‑drive recall that has removed my vehicle from service for an extended period.Read more

My 2024 Ford Bronco Sport has had an open unrepaired safety recall (Ford 25S76 / NHTSA 25V467) for about 10 months. I purchased this car new on December 31, 2024. I have owned it for less than 16 months, and in that time, we have had an issue occur at least 5 times where we get an alert on the screen that says, "Stop Safely Vehicle Will Shut Off Soon". Less than 20 seconds later, the car completely dies. On each of the 5 instances, we were lucky to be going relatively slow and were in a safe place with a shou

We brought this to the attention of Herb Chambers Chrysler Dodge Jeep RAM FIAT of Danvers (not where we bought the car, but the closest Chrysler dealership for maintenance) and they were unable to provide any help. They said they have never seen this, and since they couldn't get any codes, they didn't know what was going on. My research informs me that this is a known issue, so I'm disappointed that they didn't know about this. While traveling at normal highway speeds (55 MPH), the vehicle's battery capacity rapidly dropped from 80-90% down to 10% in less than 30 seconds, resulting in activation of the vehicle's "turtle mode" and power throttling, which made it impossible to maintain posted highway speed of 55 MPH. A warning appeared on the dashboard, which stated, "Warning Power Reduced". The vehicle traveled at approxi

Upon pulling over to the side of the freeway, the battery charge rebounded back up to 75%, but maintained power throttle, making it unsafe to reenter the roadway, as the vehicle was incapable of accelerating to the posted speed limit of 55 MPH. It was extremely slow to accelerate at all, making it from 0 - 40 in about a minute, which again, was extremely hazardous to others on the roadway. Nissan has since acknowledged that this was due to a factory defect, and covered under their factory warranty. I experienced repeated stalling, loss of drive power while on the highway, limp mode, and all dashboard warning lights over several years. I brought the vehicle to an authorized Honda dealership multiple times and specifically asked whether these symptoms were related to the known fuel pump recall. Each time I was told the recall was not the cause. I was also told recall parts were unavailable or

Based on dealership misdiagnoses, I paid out of pocket for multiple repairs including spark plugs and ignition coils which they claimed were the reason for my car acting up, that did not resolve the issue. Among other repairs I did on my own like replace the battery twice and what not. I have since received Honda’s official recall notice confirming that a faulty fuel pump causes the exact symptoms I reported. My vehicle is now completely inoperable it does not start and cannot be driven or transported. I have filed two claims with Honda Corporate, both denied based on mileage. This indicates that the recall repair was either not performed or not completed correctly. The issue appears to involve the fuel system, as the symptoms worsened after refueling and include unstable engine performance consistent with improper fuel delivery or air entering the system. This condition creates a safety risk due to potential engine stalling while driving, as well as the known recall risk involving fuel tank deformation and possible fire hazard. Vehicle: 2022 Volkswagen Taos Mileage at Incident: 61,170 On April 21, 2026, while driving shortly after leaving my home, the EPC (Electronic Power Control) warning light suddenly illuminated and the engine warning light began flashing rapidly. The vehicle immediately began jerking, hesitating, and losing power. It started slowing down unexpectedly while in motion. This created a serious safety

After turning the vehicle off and restarting it, both warning lights disappeared. Notably, on April 3, 2026 (60,428 miles), I had an oil change performed. On April 6, the engine light came on and remained illuminated until April 18, then turned off without repair. Three days later, the EPC failure occurred. The dealership was unable to reproduce the issue but charged $190 for diagnostics and stated the problem was a cracked N80 valve line (small leak), quoting approximately $700 for repair. I am aware of reports involving similar issues affecting thousands of vehicles of this model year. Sudden loss of engine power while driving is a clear safety concern, especially when warning lights appear and disappear intermittently. The vehicle remains available for inspection.Read more

... ched an O‑ring during the recall work and replaced it. Next, I detected a fuel odor. After multiple refueling cycles to isolate the source, I reported the issue, and the dealership replaced an unspecified seal. Approximately one week after that repair, the vehicle developed a rich‑running condition and illuminated the check engine light with DTC P0172. On 2026‑03‑27, the dealership diagnosed the issue as failed fuel injectors and recommended replacement of all injectors, the fuel joint pipe, and associated seals. Their testing showed: EVAP system functioning normally; no induction leaks; MAF swap produced no change; fuel rail pressure at idle ~500 psi; and cylinders 1, 2, and 4 running rich relative to cylinder 3. Because the fuel system was disassembled multiple times immediately before the onset of the rich condition, and because the dealership acknowledged that contamination during recall work could plausibly cause injector malfunction, I am concerned that the post‑recall defect may be related to the recall procedure or subsequent rework. The dealership's alternative rationale was that fuel injector failures are so common on this platform that the failure was coincidental. That would imply the need for a fuel injection system recall. I am submitting this complaint because the vehicle developed fuel‑system performance and emissions issues only after the recall repair and subsequent rework, and the root cause remains uncertain. I request that NHTSA document this case and evaluate whether the recall procedure or related components may contribute to downstream fuel‑system failures.Read more

Since moving here to Nevada in 2023, I’ve had several oil changes. After about 1200 I have to add mother oil to the car. I’ve gone back three times to 5 min oil change to get free refills since they offer free top offs after your initial oil change. He said no it didn’t without even checking for facts. I read that Volvo knew about this issue and we’re fixing the issue, however when I inquired they wanted me to pay for their mistake. How is that so when they knew there is and was an oil consumption matter and did not inform me of the matter. I’m furious with Volvo for hiding or concealing this issue. How can I get them to correct the matter, at this point?Read more

[Incident Description & Manufacturer's Bad Faith] On April 6, 2026, an authorized Honda dealership (McConnell Honda in Montgomery, AL) diagnosed my vehicle (2019 Honda Odyssey EX-L, 143,097 miles) with DTC P219A (Rec replacing fuel injector set and fuel feed pipe at this time), requiring a fuel injector replacement. This exact mechanical failure is documented in Honda’s official Service Bullet
... in 21-010. The bulletin acknowledges that defective fuel injectors can cause engine misfires, loss of motive power, and engine stalling while driving. These conditions present an immediate and severe safety hazard. Honda established a 150,000-mile warranty extension limit for this defect in TSB 21-010. My vehicle has 143,097 miles and is well within this limit. However, American Honda Corporation has arbitrarily refused to repair the vehicle. Furthermore, on April 21, 2026, the assigned Honda Case Manager left a voicemail officially denying the repair. He explicitly refused my repeated requests to provide this denial in writing. In the voicemail, he falsely stated that my vehicle at 143,000 miles is "well outside the range" for assistance, directly contradicting Honda's own written 150,000-mile bulletin. Please review the attached documents, including the official dealership diagnostic report (DTC P219A), the repair estimate, and a copy of TSB 21-010. I have also secured the audio recording of the case manager's voicemail to prove their bad faith. Honda is knowingly leaving a dangerous vehicle with a confirmed, documented fuel system defect on the road, while actively providing false information to the consumer to avoid repair costs. I strongly request the NHTSA to investigate Honda's deceptive and dangerous practice.Read more

System/Component Involved: Fuel System / Fuel Injectors / High-Pressure Fuel Joint Pipe Description of Incident and Safety Risk: My 2017 Honda CR-V (1.5T engine) experienced a fuel system failure evidenced by DTC P0172 and a failed AF cylinder test, confirmed by an authorized Honda dealer. This failure leads to engine hesitation and power loss, creating a safety hazard during operation. More cri
... tically, during the repair (Case #16244235), the dealer and manufacturer refused to replace the High-Pressure Fuel Joint Pipe. According to Honda’s official Service Manual and TSBs for the Earth Dreams 1.5T engine, this pipe is a mandatory, one-time-use component that must be replaced whenever fuel injectors are serviced. Reusing this component violates Honda’s own safety protocols and poses a severe fire risk due to potential fuel leakage under high pressure ($20\text{ MPa}$+). Safety Risk to Others: Reinstalling a non-reusable high-pressure fuel line creates a high probability of fuel spraying onto hot engine components, leading to an under-hood fire while driving. This endangers the occupants and others on the road. Reproduction and Inspection: The injector failure was confirmed by [Insert Dealer Name] in Houston, TX. The subsequent safety violation (failure to replace the mandatory fuel joint pipe) is documented on my repair invoice dated April 2026. The vehicle and the old part (if still on the car) are available for inspection. Symptoms and Warning Lamps: The Check Engine Light (DTC P0172) appeared shortly before the failure, accompanied by a strong gasoline odor and engine stalling symptoms.Read more

Kia has a recall (Campaign SC 368) on a fuel pipe that may be defective causing a gas leak leading to fire. I called the dealer I bought the vehicle from to request an inspection of the part. I was told Kia will not send letters out until June 2 so dealers are under no obligation to inspect the impacted area before then. Why should a family have to wait six weeks or more on such a critical issu

The length of time that they are going to be taking shows a lack of concern for safety, especially since the initial step simply requires inspection of the part.
