There is a known issue about exhaust manifolds for my year model truck. A ram technician said in 2019 mopar created a new exhaust manifold for my truck that would be less likely to create soot buildup and warping in the exhaust manifold. In 2021 my EGR was replaced because of a recall but now my exhaust manifold has failed and needs to be replaced out of pocket even though it’s been a known issu

The vehicle would shut off when idling at lights or in reverse and have delayed acceleration after stops. It also would shut off when idling in park. I took it to the dealership summer-fall 2025 and they were too busy to investigate the issue because no alarms were present. The vehicle started doing the same thing and the check engine light came on March 2026. Now it has been in the shop for many
... weeks because of an fuel injector issue. The dealership service department says all 6 injectors need to be replace, but warranty won't cover them all. This appears to be a common issue with 2021 Hyundai Palisade vehicles and should be investigated for a safety recall.Read more

A vehicle was taken to a collision repair facility following minor front-end damage. At intake, the repair facility documented visible exterior damage but did not perform or document an underbody inspection, which is necessary to assess high-voltage battery integrity in electric vehicles. After the vehicle entered teardown, the repair facility later claimed that the high-voltage battery was damag
... ed and recommended that the vehicle be declared a total loss. This determination was made without documented evidence of required OEM diagnostic procedures and without initial intake documentation supporting battery damage. The sequence and timing of documentation raises concern that the battery damage determination was made after teardown rather than through proper inspection protocols. A subsequent manufacturer-level evaluation did not support the claim of battery damage. This situation presents a safety concern regarding whether proper high-voltage battery inspection and handling procedures are being followed by collision repair facilities. Misidentifying or improperly handling high-voltage battery conditions in electric vehicles can pose safety risks, including potential thermal events, improper classification of vehicle safety, or unsafe return to service. This complaint is submitted to request review of whether proper safety and OEM procedures were followed in evaluating the high-voltage battery, and whether this reflects a broader safety issue in electric vehicle collision repair practices.Read more

I purchased the truck in 2021 from a chevy dealer with 32k miles on it, truck looked great and performed great, at 42k miles it started to be sluggish and it would vibrate between 30-40 and 60-70 as i feathered the gas pedal to maintain speed, but if i gave it more gas or take my foot off the pedal it would stop vibrating, I've taken it to 2 chevy dealers, the first one said it drives within GM sp

I looked online and many people have similar problems. ALSO: The paint around my front wheels is peeling off down to the primer, I've had many vehicles and never had this happen to me, It's just flaking off, When i had the truck at the chevy dealer for maintenance and told him about it, He said GM will not fix it and they know about the issue. Now these trucks are not cheap, It seems like once they sell the vehicle GM washes their hands of it, I guess that's why they only give you 36,000 mile warranty because they don't even trust their product to last.Read more

I am submitting this complaint on behalf of the vehicle owner. On March 31, 2025 (136,962 miles), the vehicle was brought to Fred Anderson Nissan of [XXX] for Voluntary Emission Recall PD128 (NTB24-061), which required a new Particulate Matter Sensor and ECM reprogramming. One day later, the Check Engine Light illuminated. On April 2, 2025 (137,083 miles), the dealer diagnosed code P2002-00 (DPF
... Efficiency Below Specs). Repair Order #XXX states: "The new sensor that is now monitoring DPF is able to see this and is setting the code as designed." The dealer's own documentation confirms the fault code was triggered by the recall — not a pre-existing failure. The vehicle had no warning lights before the recall was performed. Nissan Consumer Affairs (Case #XXX) declined goodwill assistance citing mileage, without addressing the documented causal link between the recall and the fault code. The owner now faces a DPF replacement costing several thousand dollars as a direct result of complying with a manufacturer recall. Safety concern: A DPF in confirmed fault condition can trigger unexpected limp mode, including at highway speeds. If other PD128 recipients are experiencing the same recall-induced fault and cannot absorb the repair cost, those vehicles may be operating in a compromised condition. We request NHTSA investigate whether Recall PD128 is creating safety and repair liability for consumers who complied in good faith. Purchased a used 2022 Audi E-Tron GT with an open NHTSA safety recall for high-voltage battery fire risk (Recall 24V726 / Audi 931A). The recall remedy had not been performed at time of purchase. The selling dealer completed a pre-delivery inspection two weeks prior to sale with full knowledge of the open recall. Within 7 weeks of purchase, the vehicle entered limited performance mode (turtle mode
... ) while driving and had to be towed. A high-voltage coolant pump was diagnosed faulty and replaced. Days later, the vehicle powered down completely while driving. Fault codes indicated an implausible signal from the HV battery controller. The battery management controller (J840) was replaced. After replacement, additional faults were found. The HV battery switch box (SX6) was also diagnosed faulty and replaced. The vehicle was out of service approximately 18 days for this event alone. Within 11 months of purchase, the vehicle developed water intrusion in the trunk area traced to a faulty spoiler motor sealing surface. This caused corrosion to the center tail lamp connector, electrical shorts to all three tail lamps, and required repair of six wiring harness connections. A body electrical wiring repair was also documented in service history prior to purchase, suggesting a pre-existing sealing defect. Total: 9 dealer visits and 50+ days out of service in under 14 months. All repairs performed under manufacturer warranty. The open safety recall was not completed until 8 months after purchase.Read more

Re: Vehicle Safety Defect Complaint – 2023 Chevrolet Blazer To Whom It May Concern, I am submitting a formal safety defect complaint regarding my 2023 Chevrolet Blazer. This vehicle has exhibited multiple system failures that present a serious risk to driver safety and public roadway safety. The vehicle was purchased new on July 20, 2023. Beginning on April 21, 2025, at approximately 27,610 mile
... s, the vehicle developed a progressive defect initially presenting as oil-related issues. Despite multiple repair attempts, the condition worsened and required a full engine replacement in January 2026. Following the engine replacement, the vehicle continued to experience critical safety failures, including engine stalling, reduced engine power warnings, and electronic stability control (ESC) alerts. On April 26, 2026, the vehicle experienced a driver assistance system malfunction in which it attempted to steer off the roadway. I was forced to manually disable the system to maintain control of the vehicle. On April 27, 2026, the vehicle exhibited a fuel system failure, including fuel leakage and fuel odor, along with multiple simultaneous warning indicators: • Reduced Engine Power • Service ESC • Steering Assist Reduced / Drive with Care • Power assist driver system disabled This combination of failures presents both a fire hazard and a loss-of-control risk. Additionally, I was informed by the dealership that a high-pressure fuel line, a one-time-use component, was not replaced during the engine installation. This raises concerns regarding improper repair and continued unsafe operation of the vehicle. The vehicle has undergone multiple repair attempts and a major powertrain replacement, yet continues to present serious safety concerns. Due to these ongoing issues, I do not feel safe operating this vehicle. I respectfully request that NHTSA review this matter for potential safety defects that may impact other vehicles of the same make and model.Read more

Went to park car in a park to drop daughter off and it sputtered out and stalled out of nowhere with no warning. I had received a lamp warning very sporadically prior to this and on occasion that stated 'reduced steering assist drive with care' this resulted in heavy steering for that drive until car was shut off and then it would go away and be fine again, fixed the car through Knudtsen Chevy due

Knudtsen fixed that issue. Low battery voltage light did not come on however battery was older so had battery installed at park to ensure the issue was not due to the battery. The oil was fresh and showed no issues, changed 2 weeks before fresh oil no issues found during service. When tried to restart the car it would think it would try to start and then sputter out and not stay in idle. Les Schwab left and called a mobile mechanic to bring the computer. Computer was hooked up and it would try to start, however, it would not know it was already running and try to start again on it's own...the codes that came up were the PO101 code came up for Mass Air Flow issue...and then the codes would go in a wild sequence of fail.....mechanic checked for loose connections around air filter, took air filter out and it was clean no issues, checked pipe no visible issues. We walked away and walked around the park to give it some time to sit and then we tried it again and it tried and sputtered out and died again would not stay idling. Then all of a sudden we heard it try to start itself without us trying to start it and it clanked. We hauled it to Knudtsen for a diagnosis. They stated "the motor will not turn this way or that way and it is seized or locked up". This is a 2021 with only 89,000 miles it should have a longer life than this. This past year I experienced catastrophic failure with my Bosch, CP4 fuel pump in my 2017 Ford F250 with a 6.7 L diesel. Recalls have been published by Ford, but not beyond a 2018 vehicle. The dealer opened Toyota TAS Case #TA26079027. However, the dealership performed no mechanical repairs to address the root cause of the sudden power loss. They simply cleared the safety code, test drove it, and explicitly wrote on the final invoice: 'ADVICE CUSTOMER TO DRIVE VEHICLE UNTIL PROBLEM OCCUR'. Furthermore, they refused to document my stated concerns that the vehicle is unsafe to drive. Brought F150 in for scheduled service - oil change. Also asked for inspection of a problem with the fuel door hatch that was causing "Refuel Error" messages on the instrument panel and several grinding noises every time the fuel release button was pressed. Note that I have an active extended Ford Premiumcare warranty. Dealership diagnosis was that the fuel door solenoid was bad, verified that t
... he gears could be heard grinding with this part. However, there is no separate motor part, therefore the entire fuel door assembly must be replaced. Ford warranty system does not cover this part because it is considered "Body and Structural". I called Ford Protect to discuss this, and explained the motor defect and problem, that this has nothing at all to do with wear & tear or body damage. They explained that in this case, the dealership should call and get the part pre-authorized with the justification. Seelye Ford called and tried to do this on 28 April 2026, but ultimately was still denied by Ford. The attached service report from Seelye describes the technician findings and confirms the bad solenoid and audible gear grinding sound coming from the fuel door assembly - and that the entire assembly must be replaced. As this is related to the fuel system and has a defective motor, this is an example a safety defect that should not only be repaired on this vehicle, but for other vehicles with similar problems. Note that the dealership also pointed out that Ford appears to have created this part layout with the intent of having the motor be a separate part, but ultimately did not offer that option, only a full housing replacement, which is excluded because it is mostly made of plastic and metal, which is considered to be structural in nature, despite being a part of the fuel system that has active motor buttons and instrumentation sensors for driver interaction.Read more

On 04/28/2026, when diagnosing a reoccurring check engine light on the vehicle at a Chevy Certified Service, the mechanic "Borescoped cylinder 4 and found significant scuffs on the cylinder wall which along with the misfires indicate a cracked piston. The big picture appears to be the piston cracked which caused excessive oil consumption and misfires. The excess oil and unburned fuel overloaded th
... e converter causing it to fail. The engine and converter will need to be replaced." In 2017, a recall was issued by GM to mitigate Stochastic Pre-Ignition (SPI) Causing Cracked Piston (17019). Between the initial purchase of the vehicle (11/2016) and performed service required by this recall (03/10/2017), there were approximately 5000 miles driven. No engine diagnosis was performed or recommended. After the 17019 was addressed, the engine misfires continued along with reoccurring CEL. C.S.P. N182195660 Predictive Monitoring for Stochastic Pre-ignition (SPI) released in 2019 states the 1.5 L LFV engines in certain 2016 and 2017 model year Chevrolet Malibu vehicles may have a preignition condition that requires immediate repair. This condition may cause the engine to perform poorly, misfire, run rough, and/or excessively consume oil. It may also illuminate the vehicle’s service engine light. Over time, if not repaired, the condition can damage the vehicle’s pistons and engine. The VIN for this Malibu was not attached to this notice despite it having SPI recall history. On 06/2023 (after the N182195660 program ended), at 80k miles a diagnosis revealed a potential cracked cylinder with no mention of the pre-ignition condition that could cause the crack, and the only way to fix is to replace the entire engine (as per dealer recomendation). Since vehicle was out of warranty, the repair est. was nearly $10k. I firmly believe my 2016 Malibu has suffered the consequences of poor mismanagement on this recall, and am demanding mitigation from GM to fix this issue.Read more

The vehicle experienced engine failure due to coolant intrusion into the engine cylinders. The issue presented with engine misfire, including diagnostic trouble code P0302, rough running, and coolant entering the combustion chamber. The condition was confirmed by a dealership, which determined that the engine has internal damage and requires full engine replacement. There were no external coolant
... leaks observed. This failure occurred at approximately 88,000 miles and was not consistent with normal wear and tear. The issue developed without significant prior warning other than engine misfire and rough operation. This condition may create a safety risk, as engine misfire and internal damage can lead to loss of power, reduced vehicle performance, or potential engine stall while driving. Technical Service Bulletins have been issued for similar vehicles equipped with the same engine, including TSB 19-2172, TSB 19-2208, and TSB 22-2229, which describe coolant intrusion into engine cylinders and require engine replacement. This suggests the issue is not isolated and may affect other vehicles with the same engine design.Read more

The vehicle has 64,240 miles and has had zero symptoms prior to the DTC appearing. This problem is noted in Service Bulletin 21-010 for Honda Pilots year 2017-2019, but my Vin was not included in the "Vin range". I am taking the vehicle in to
... the dealer for an inspection and diagnostic, but I believe the "Vin range" should be updated to include my vehicle.Read more

The vehicle suffered a recurring mechanical failure of the fuel injectors and a flooded EVAP charcoal canister, causing the engine to run lean (Fault P2177) and lose power. This defect creates an unreasonable safety risk as it can cause the engine to stall or lose motive power unexpectedly at highway speeds. Furthermore, the fuel system defect led to a flooded EVAP system, which poses a potential
... fire hazard and fuel vapor accumulation. Volkswagen Technical Assistance confirmed the mechanical failure of all four injectors . The defect was initially misdiagnosed as 'fuel contamination,' which delayed the critical safety repair for over 25 days and resulted in a total electrical system failure (battery death) while the vehicle was inoperable.Read more

Since purchasing the vehicle, I have had ongoing issues: a rattling noise when accelerating, a squeaking noise in reverse, and a fuel odor. The fuel injector recall (NHTSA 25V467) is still open, and Ford has not provided a remedy." Keep it simple and focused on the recurring noise and the recall.

My wife was driving our 2016 Mazda CX-5, and was getting ready to pull into our daughters' driveway. My wife slowed down to a stop before entering the driveway. She gave the vehicle some gas to move forward. She stated that there was a pause, then the vehicle accelerated forward slamming into the garage door and the corner of the garage/house. There was also a Tesla parked inside the garage that w
... as knocked back about a foot and a half. The distance from the edge of driveway and street to the garage door is about 27 feet. This happened at 1:30 PM. The weather conditions were great, sunny and dry. Thankfully, no one was hurt. If my wife was another foot to the right, she would have hit the support post for the upper balcony, and there would have been a good chance of the balcony falling on top of her. Or, if someone was standing in front of my wife when she entered the driveway, they could have seriously been hurt! There were no warning lights on prior to the accident. This happened last Sunday, April 19, 2026. We had the car towed to a local body shop on the same day as the accident. The vehicle was towed away the very next day to our insurance company's salvage lot in Bakersfield. I do have all the information, address and phone number, of the lot where our car was towed to. Hopefully, the car can still be inspected by the proper authorities. Thank you for your time!Read more

I've been living with fuel pump issues for a year or so, i must prime the pump, meaning cycle the fuel pump without starting to get the car to start, if not it just cranks and doesn't start, my vehicle year is definitely on the expanded recall list, but for whatever reason my specific VIN doesn't come up on recall list, this is very frustrating as it is very clear my fuel pump is not holding press

2024 Kia EV6 leased January 2025. Vehicle is subject to NHTSA Recall 24V-867 / Kia campaign SC327 for the Integrated Charging Control Unit (ICCU). The recall remedy was performed at an authorized Kia dealer in July 2025 (software / operational logic update). The defect was not cured. Three months after the recall remedy was performed, the vehicle suffered complete loss of electrical power and woul
... d not move. It was towed to the Kia dealer. The dealer's own diagnostic recorded code P1A9096 — the exact diagnostic code Kia's recall instructs technicians to check for — and replaced the ICCU and high-voltage fuse. This is the recall-defined failure occurring AFTER the recall remedy was applied. Approximately three months later, the vehicle again suffered total loss of power and would not start in cold weather. It was towed in. The dealer replaced only the 12-volt battery and did not address the ICCU, despite the documented prior ICCU failure on this VIN. The 12V battery failure is a known downstream symptom of ICCU failure on this platform. Approximately one month after that, the vehicle returned for charging system failure ("vehicle charges briefly then stops") and a "check electrical/hybrid system" warning that appeared while driving. Diagnosed as charging port failure. Vehicle was out of service approximately 27 consecutive days. Total cumulative days out of service across four visits: approximately 35-40. Two safety-related stranding incidents resulted from these failures. Both occurred in winter conditions in Pennsylvania. In the first, the driver was stranded in a parking lot at night during a snowstorm in sub-zero conditions; three tow trucks were dispatched before a flatbed could recover the vehicle because the EV could not be towed conventionally with no power. In the second, the vehicle died in a school parking lot during a snowstorm; the first dispatched tow truck could not recover an EV with no power, and the driver had to walk multiple miles home in freezingRead more

I lease a 2024 Hyundai Ioniq 5 AWD. Prior to a planned trip, the vehicle displayed an electrical system warning. I contacted the dealership and was advised to bring it in. While driving approximately 5 miles to the dealership, the vehicle lost all power including hazard lights, leaving me stranded on a busy road. I contacted roadside assistance and the vehicle was towed to the dealership. The deal
... ership confirmed the failure was caused by the ICCU (Integrated Charging Control Unit). The repair took approximately two weeks. The vehicle was unavailable during this time, requiring a rental car. Hyundai partially reimbursed rental costs but did not compensate for other losses incurred. The ICCU failure is a known widespread issue affecting the entire Hyundai/Kia E-GMP platform across multiple models and model years. A dedicated owner community tracking these failures can be found at [XXX] . The sudden and complete loss of vehicle power including safety systems such as hazard lights while in traffic represents a significant safety risk to the driver and other road users. First owner of a newly purchased 2017 Honda CR-V that has been well and regularly maintained. Driving at 25mph in a city and every single warning light and dash light simultaneously came on. Every single one -- brakes, headlights, emergency brake, engine, etc. Eventually codes read off the car's computer showed P0172, P219C, P219F -- all of these codes relate to the fuel injectors. I brought the c
... ar to a Honda dealer and they confirmed that all of the fuel injectors needed to be replaced and said the fuel injectors are "faulty" and "from a bad batch." Lots of Honda forums online talking about faulty fuel injectors. A whole thread on 2017 CR-V fuel injector problems can be easily found online along with service bulletins for fuel injector problems (with the code P0172) for similar year, different model Honda vehicles. The dealership confirmed the fuel injector problem but then said they couldn't source the parts despite a "global search" and putting us on a "critical list." We then went to a competent independent mechanic who was able to source Honda OEM fuel injector replacement kit from a different Honda dealership (not sure why the first Honda dealership couldn't get the parts then...) The independent mechanic said the fuel injectors looked "terrible" and "definitely faulty." He sent us home with the old parts, which we have and could supply. Faulty fuel injectors affecting the propulsion system feels unsafe and the fact that Honda told us "you are out of luck" with regard to replacements was especially aggravating. We had no chance to pursue a goodwill repair of the issue even though our independent mechanic did find the parts at Honda in spite of the first dealership's claim of a global search. The service that was performed included replacing all fuel injectors and whatever is in the associated OEM kit. In summary, fuel injector problems caused ALL warning lights to come in, dealership confirmed problem, mechanic repaired, many reports of same issue online.Read more
