I am reporting a serious and recurring safety defect with my 2023 Hyundai Santa Fe Hybrid, purchased new in January 2024. The vehicle has suffered from over 30–40 sudden hybrid system failures, including loss of acceleration, “Stop Vehicle” warnings, and check engine lights. These incidents happen without warning, including while driving at legal speeds (40–70 mph), on residential streets
... and highways. Most recently, the car failed twice in two days, while my teenage son was driving within all road laws, including once with a friend in the car. The vehicle simply stopped accelerating in the middle of traffic, endangering them and everyone around them. These failures have happened at legal speeds ranging from 40–70 mph, on both residential roads and highways. There is no advance warning, just abrupt loss of power and sudden warning messages. We’ve taken the vehicle to the South Charlotte Hyundai dealership multiple times. Each time, they either reset the check engine light or replaced indicators: but never addressed the root hybrid system failure. We filed a lemon law case, and Hyundai responded with only a $2,000 offer including legal fees, requiring us to keep the dangerous vehicle. We rejected the offer. These failures have occurred with children and passengers in the car. We have documented as many incidents as we could, including photos, service records, and communication with Hyundai. The vehicle is available for inspection upon request. I no longer feel safe driving or allowing my family to drive this car. This is a critical hybrid system failure that continues to place lives at risk. I believe this may be part of a wider pattern affecting Hyundai hybrid vehicles, and I am pleading with the NHTSA to open an investigation before someone is seriously injured or worse.Read more

Over a distance of 1,718 miles, I measured my vehicle’s actual fuel consumption by recording odometer readings and fuel volume at each refill. I then compared these values to the average fuel economy (MPG) displayed on the vehicle's trip computer. The dashboard consistently underreported fuel consumption by approximately 8% ± 2%, always showing more optimistic (i.e., better) fuel economy than a
... ctually achieved. This discrepancy is repeatable and was observed over multiple refueling intervals. The vehicle and its onboard display system are available for inspection upon request. This inaccurate fuel economy reading does not present a direct safety hazard, but it can mislead drivers into overestimating range and efficiency, and may influence consumer decisions based on performance data that does not match real-world usage. Additionally, such behavior—if widespread—could be considered a form of consumer deception. I submitted a detailed written request to an authorized Honda dealership (Victory Honda of San Bruno) asking them to compare actual fuel consumption to the displayed value under controlled conditions. However, the dealership did not perform the required measurements or reset the trip computer, and no technical analysis or documentation was provided. The manufacturer’s customer service reviewed the complaint but closed the case without addressing the technical discrepancy or providing a specification that defines acceptable error for the fuel economy display. There were no warning lights, error messages, or symptoms indicating a malfunction. The issue was discovered independently through manual tracking of fuel usage versus mileage. I have attached documentation including my full correspondence with Honda Customer Service, fuel receipts for each fill-up, photographs of the dashboard display showing average fuel consumption, and the official Service Report from Victory Honda.Read more

I purchased a certified pre-owned 2025 Nissan Sentra SV from a local dealership here in Georgia for my wife. It had 1500miles on it at time of purchase. We have had it less than a month and observed engine shutdown, and totaled power loss about five times in traffic. This is a safety concern because it is random without prior warning. The engine shuts down, dash screen goes black, and you are
... unable to warn others with hazard lights due to total loss of power. Recently, the vehicle did give dash codes alluding to the problems described, however the dealership was unable to identify the cause or duplicate the problem. When the incident occurred again, I towed it to the dealership, and they still have not gotten answers as to why the car stalls without warning. Unable to move the gear shift to neutral so that the vehicle can be pushed out of traffic is another safety concern. This car should be fuel efficient, yet we were refueling every other day. I drive a 2019 Ford F-150 with the 3.5L engine and 10 speed transmission. I have received a warning for low fuel pressure, trouble starting, and the truck has cut off once after turning over. It never fails to crank, however, it may take 5 or so seconds of cranking to get it to start. I have had the check engine light come on one time over the last couple of months of the issues starting; the code
... it showed was a P00C6. I have not brought the vehicle in for service as it just started a couple months ago when it got hot outside and there was a recent recall that did not include this VIN but matches the issue exactly. The warning states to not drive the vehicle and to get it towed if fuel is not low. The recall states there is not a current fix for the issue and I need my vehicle. This is a daily problem, mostly after the vehicle sits for a couple of hours. I am also having an issue with the cam phaser rattle upon startup and rattling while driving. I have noticed some power reduction and it has progressively gotten worse over time. I am at 135,000 miles currently and have experienced this issue for the last 50,000 miles or so.Read more

On July 9, while accelerating on a busy freeway during rush hour, the car stalled in the middle lane and was unable to be driven off to the side. I called 911 and a tow truck came to tow the vehicle. I am the owner of a 2020 Ford Escape experiencing two serious issues: 1.My vehicle is exhibiting the exact symptoms outlined in Ford CSP 23B56—specifically, the failure of the driver-side door check-arm welds. The door produces loud popping noises, feels unstable when opening/closing, and has visible signs of structural stress. Despite this, my VIN has been deemed ineligible for the program. Giv

According to NHTSA, there is currently no remedy available. This adds another layer of safety concern, particularly considering the compounding risks and Ford’s delay in delivering a fix. Between the structural door failure and a pending recall involving potential engine fires, I am deeply concerned about the safety and reliability of my vehicle. I am asking that my concerns be formally investigated and that Ford take appropriate responsibility for repairs, reimbursement (if applicable), or an expanded VIN inclusion under the CSP.Read more

On [XXX], my 2019 Chevrolet Traverse (VIN [XXX] ) experienced a complete transmission failure. The vehicle began to hesitate, jerk during acceleration, and then completely lost the ability to shift or drive. It had to be towed, and the dealership confirmed that the transmission must be replaced. This is extremely concerning because GM previously issued Recall 20V668000 involving missing bolts in
... the start-stop accumulator of the transmission, which can cause fluid leaks and loss of propulsion—a potential safety hazard. Although my VIN does not show this recall as active, the symptoms and failure seem directly related to this known defect. I am requesting NHTSA investigate whether this recall should be expanded or reopened to include more affected vehicles. In addition to the transmission failure, I have had to replace the purge solenoid valve three times due to persistent check engine lights and fuel system issues, which suggests deeper reliability issues with emissions and vapor management. Lastly, there is now a hole in the exhaust pipe, which is affecting performance and may pose a safety or environmental concern due to potential carbon monoxide exposure. These multiple powertrain and emissions-related failures within the same vehicle raise serious concerns about the long-term safety, reliability, and manufacturing quality of the 2019 Chevrolet Traverse. I respectfully request that NHTSA investigate this model year and VIN range further. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I own a 2024 Jeep Wrangler 4xe (VIN: [XXX] ) that has experienced multiple hybrid system failures since the week of purchase. Despite several major repairs, the issues persist — raising significant safety and reliability concerns. Timeline of Major Issues: •[XXX]: Just two days after purchase, the vehicle was brought into the shop for hybrid system failure, requiring replacement of the high-v
... oltage wiring harness and electric cooling heater. •Shortly after, the check engine light returned (per service staff, likely another hybrid fault). I was unable to bring it in at the time, and the light later turned off on its own. •[XXX}: After overnight charging, the battery was dead and the vehicle wouldn’t start. My wife (a medical provider) had to leave work to assist, causing major disruptions to patient care, financial loss, and missed school for my son. •[XXX]: After another overnight charge, the vehicle again failed to start, with more difficulty getting it to run than before. •[XXX}: The vehicle was returned to the shop and required replacement of the hybrid powertrain chip and additional high-voltage wiring. These failures mirror the symptoms and component issues currently under investigation in Recall 95B, yet my VIN does not show any open recalls, and I was never notified of any hybrid system defect. My vehicle has experienced the same hazardous failures and received the same repairs as affected vehicles. I am submitting this complaint because I believe my vehicle was improperly excluded from safety-related notifications and may be affected by a known, systemic hybrid defect. In March 2025, I returned to Hollywood Kia because of popping sounds and a check engine light. They diagnosed and “repaired” a purge control valve but failed to address the underlying defect. Despite these repairs, the catastrophic failure still occurred. The vehicle is now completely inoperable and irreparable. Kia later offered to buy back the vehicle but failed to respond to over 200 calls and dozens of emails from me, allowing their offer to expire without explanation. NHTSA report 7.19.2015 July 19, 2025 NHSTA Report This is what happened to me [XXX]: I was at an off ramp stop waiting for cars to pass. I eased forward to see if everyone had passed. The car accelerated on its own and I tried to stop. When I braked it accelerated faster. I had both feet on the brakes and tried to stop, but each time the car accelerated. It accelerated three times until I crash
... ed. I crashed into a utility trailer and the wheel well of the trailer caught on my front left bumper on the driver's side. I was drug along the road around 60 miles per hour until the bumper bent and I could pull over onto the side of the road. The car was a total loss. The attached notice from Mazda matches the uncontrollable acceleration of my car. I contacted Mazda Corporation on the same day ([XXX]) and was told there is no VIN record of the car since I owned it. Additionally, they told me it was repaired in 2005 as a throttle recall. But there is a subsequent notice attached dated July, 2012 and no record of that recall repair. I called WA state Dept of Licensing to see if there is a new owner. They do not have a record of such. I contacted Copart in Arlington WA today to find out if the car was there for parts so nobody would buy the accelerator cable. Copart told me today they sold the car in 2024 but cannot release the name of whoever bought it. They said my insurance company would know. Whoever owns the car now needs to know there is a faulty accelerator cable causing a accident and they need to have it examined and replaced correctly. Please help find who owns this car and remedy a potential crash. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Subject: Known Carbon Buildup Issue in 2016 Volkswagen Tiguan TSI – Request for Recall and Accountability To Whom It May Concern, I am writing to formally express my concern and request action regarding a well-documented and dangerous issue affecting Volkswagen TSI engines—specifically, severe carbon buildup. I am the current owner of a 2016 Volkswagen Tiguan TSI, which I purchased in April 2
... 025 with approximately 84,000 miles on it. In June 2025, I had to have the intake manifold replaced due to carbon buildup—a repair that was still covered under warranty. However, shortly after, I was informed that the vehicle now requires a full engine decarb and replacement of all four fuel injectors, at a total cost of $3,296. This raises several serious concerns: •Carbon buildup this severe at such low mileage is not typical wear and tear. It indicates a design flaw in the TSI direct-injection engine. •My extended warranty provider refuses to cover the repair, citing that it is a maintenance issue—conveniently shifting the responsibility away from the manufacturer. •This type of engine failure can result in dangerous performance issues, including: •Sudden engine misfires •Loss of power while accelerating •Poor fuel economy •Stalling or hesitation while merging or turning •Catalytic converter damage •Potential long-term engine failure I am stunned that this is not considered a safety issue worthy of recall, especially given the number of reports online and in service centers regarding this exact problem. The symptoms of carbon buildup can easily result in on-road danger to the driver and others. What’s most disheartening is that Volkswagen has long been aware of this flaw, yet no recall or extended warranty has been offered for affected consumers. I previously owned a 2015 Passat and was a loyal VW owner—but now, as a single mother, I am expected to pay nearly $3,300 for a repair tied to poor engineering and design, not misuse or neglect.Read more

My 2018 Kia Optima LX began using excessive oil (>3 quarts/1000 miles) just over 100K miles. I researched and found that the Theta II engine in my car is known for this. When I took my car to dealership (Stokes Hodges Kia, North Augusta, SC), with the check engine light on, they told me me plugs were fouled (they were just replaced 1 yr prior), cylinder 3 misfiring, and that I needed to pay a few
... thousand dollars to have repaired. When I asked them to guarantee the soaking process they recommended, they responded that they couldn't, that it "only worked 80% of the time." I have the KSDS download since 2019 and asked them to follow the guidelines of their TSB ENG 222 for the class action lawsuit and start a 1000 mile oil consumption test. they refused, basically telling me the only way to get a new engine from Kia, since my car is over 100K miles, is to drive it until the engine seizes. I expressed my fear for personal safety, but they wouldn't help me. I am original owner and have all services, etc documented. I called Kia customer affairs, then was told only local dealership could help. they stuck me in a loop between them without helping. Please help me.Read more
