I am reporting a serious and ongoing safety issue involving my 2015 Ford Fusion. The vehicle experiences intermittent power steering failure while driving. The steering assist will suddenly disengage and re-engage without warning, making the vehicle extremely difficult to control. This condition becomes even more dangerous in wet weather, where the vehicle is sometimes nearly undriveable. Additi
... onally, I am now experiencing a popping noise in the steering wheel. This raises concern of a possible mechanical failure consistent with known recalls involving steering wheel bolt defects that could result in the steering wheel detaching. These issues are consistent with known recalls, including: - Power Steering Failure Recall (15V340000) - Steering Wheel Bolt Recall (18V167000 / 18S08) Despite these known safety defects, Ford has refused to repair the issue, stating that the recall has expired. This presents a serious safety hazard, including risk of loss of steering control, crash, injury, or death. I am requesting immediate investigation and enforcement of manufacturer responsibility for this safety-related defect.Read more

The high-voltage battery pack in my 2018 Porsche Cayenne S E-Hybrid (VIN: [XXX] , approximately 81,000 miles) has experienced a noticeable and progressive reduction in battery range and performance. On February 20, 2026, Porsche White Plains (an authorized Porsche dealer) performed an inspection but was unable to duplicate an active fault code and declined to generate a battery State of Health (
... SoH%) report. No warranty repair was authorized. Porsche Cars North America (PCNA), Case #XXX, has since moved to close my statutory warranty claim, citing the absence of a duplicated fault. However, under New York State's adopted CARB emission warranty standards (6 NYCRR § 218-9.1), high-voltage propulsion batteries in TZEV/PZEV vehicles are warranted for 10 years or 150,000 miles from the date of original sale. My vehicle was placed in service in 2018 and remains within that statutory coverage window through 2028. PCNA has not addressed this statutory obligation or provided written confirmation of coverage. I also have not received the inspection documentation I formally requested on March 17, 2026, despite no case closure having occurred at the time of that request. As of the date of this filing, PCNA has not responded to my follow-up correspondence. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Hyundai's recent emissions software update (9C9) has disabled engine braking, creating a risk of brake fade on grades. The vehicle is no longer able to hold speed while descending grades, regardless of downshifting, forcing the brakes to be implemented to avoid excessive speed. This was never an issue prior to the software update and happened immediately after the software was installed. Instea
... d of holding the vehicle with the engine (foot off the gas, and downshifting) the engine will continue to increase speed up to the point of approaching maximum RPMs. Further evidence of this behavior is that trying to use the engine braking no longer shows maximum MPG on the dash, indicating the software update is still feeding fuel to the engine with the foot off the gas.Read more

Failed Head Gasket in the engin affected fuel injectors and spark plugs to fail or not perform normally and started misfiring in the engin. My 24 year old son drives this car and the problem happened at night when he was driving home at night on 3/4/2026 flashing all waring signs on the screen. NHTSA Complaint SummaryVehicle: 2017 Ford Escape SEEngine: 1.5L EcoBoostVIN: [XXX] Defect: Engine Block Design Flaw / Coolant IntrusionTechnical Description of the Issue:My vehicle is experiencing a well-documented manufacturing defect specific to the 1.5L EcoBoost engine. The engine block was designed with a "v-groove" between the cylinders that is prone to structural failure. This allows engine
... coolant to leak into the combustion chambers (coolant intrusion) and allows engine oil to contaminate the cooling system. My vehicle currently exhibits "milky," tan-brown sludge in the coolant reservoir, confirming that oil and coolant are cross-contaminating due to internal engine failure.Extreme Safety Hazards and Dangers:This defect creates several critical safety risks for the driver, passengers, and other motorists:Sudden Loss of Power at High Speeds: The mixing of oil and coolant destroys the engine’s ability to lubricate internal bearings. This can lead to a catastrophic engine seizure while the vehicle is in motion, causing an immediate loss of power and power steering, which creates a high risk of collisions on highways.Engine Fire Risk: When coolant enters the combustion chamber, it can cause severe engine misfires and overheating. Overheated oil or pressurized coolant spraying onto a hot exhaust manifold during a mechanical failure presents a significant fire hazard.Hydro-locking: If coolant floods the cylinder while the vehicle is in operation, the engine can hydro-lock, causing the engine to stall instantly and potentially throwing metal debris onto the roadway, endangering other drivers.Visibility Hazards: Excessive white smoke from the exhaust (caused by burning coolant) can suddenly obscure the driver’s rear-view visibility and the visibility of trailing motorists. Conclusion:Ford Motor Company was aware of this defect, as evidenced by Customer Satisfaction Program 21N08 and TSB 19-2346. I'm reporting a safety issue with BMW vehicles equipped with the B58 engine, primarily 2019-2021 models. These vehicles have an oil pump with an internal plastic component that regulates oil pressure, and this part is fracturing in the field, causing sudden loss of oil pressure. The failure typically occurs during cold starts in low temperatures. When it fails, oil pressure becomes erratic or is l
... ost entirely, often with little to no warning before engine damage or vehicle disablement. Warning signs, when present, include inability to measure oil level electronically, abnormal oil pressure, or sudden drive train malfunction warnings. These can rapidly progress to engine shutdown or severe damage from oil starvation. This creates a dangerous situation because the vehicle can become disabled without warning while driving or after startup. This is especially hazardous when cars become immobilized in traffic or during winter conditions when failures occur most frequently, leaving occupants exposed to freezing temperatures while awaiting assistance. Pump replacement requires extensive disassembly and commonly costs $6,000-$8,000, assuming no engine damage. A critical concern: owners cannot determine if their vehicle has the original plastic pump or the later metal revision. BMW's VIN-based parts lookup now shows updated components even when original plastic parts remain installed, and BMW has removed the original plastic part number from dealer networks entirely. Failures are reported across multiple production years, and the scope of affected vehicles remains unclear. Because this condition causes sudden vehicle disablement without adequate warning, especially in cold weather, I believe this warrants investigation as a potential safety-related defect.Read more

After 5-10 seconds the pedal suddenly became responsive and the car began to accelerat
... e at a bit of a slower rate than typical for the first few seconds then accelerated as it should. 2nd incident: I was turning from [XXX] , into the driveway at my place of employment. The drive way is on an incline and halfway up the incline the gas pedal again became unresponsive. Due to the incline the vehicle did not continue to coast forward and quickly lost speed at which point I hit the brakes to avoid possibly rolling back into the highway traffic. I then tried the gas pedal again and the vehicle accelerated. This could become extremely dangerous if it were to occur during merging onto a busy highway with traffic moving at high speed, as just one example. The two incidents happened about a month apart. The dealership was not able to replicate the issue as it is a very random occurrence with no warning of when it will happen. No warning lights became visible on the dash. Through a quick google search it became apparent this is a known issue, with several possible quick fixes, which I relayed to the dealership when I took the vehicle in for them to fix it. It was not replicated on their test drive and no codes were showing so I was told they couldn’t do anything unless a code would pop up or they were able to replicate the issue, which is very unlikely to happen during a quick test drive. I am reaching out regarding an ongoing issue with my 2021 Chevrolet Blazer. The vehicle is experiencing a noticeable stutter or hesitation when shifting between second and third gear. In addition, there are instances where the vehicle accelerates unexpectedly or more aggressively than intended. Please advise if there are any known service bulletins, recalls, or warranty coverage related to these issues. For reference, the vehicle has been properly maintained and this behavior is not consistent with normal operation. Given that this is now the third major issue I’ve experienced with this vehicle, I am hoping this matter can be addressed without additional out-of-pocket expense. I own a 2025 Jeep Gladiator with approximately 48,000 miles. The vehicle was fully operational as of approximately 5:00 PM on Monday, April 27, but by 7:30 AM the following morning it had become inoperable due to a rodent chewing through the fuel line overnight. The vehicle required towing to a dealership, Beaman Chrysler Dodge Jeep Ram. At the time of the incident, the vehicle was parked in a gra
... vel driveway at an RV park, which is a typical residential parking environment. Damage to a fuel line presents a potential safety hazard, including risk of fuel leakage, fire, or sudden vehicle disablement. In this case, the failure occurred without warning and rendered the vehicle unusable. A significant amount of fuel was observed leaking beneath the vehicle and onto the ground. The dealership confirmed the damage and advised that the repair would replace the component with the same or similar material. This repair does not provide any mitigation to prevent recurrence of the issue. It is my understanding that some automotive components may utilize plant-based materials, which have been widely reported to attract rodents. If applicable, this may increase the likelihood of repeated damage to critical vehicle systems. This situation raises concern regarding the durability and safety of fuel system components, as such damage can render a vehicle inoperable without warning and may pose additional safety risks depending on when and where the failure occurs.Read more

Despite this, my dealer marked recall R10362 as Closed on or around April 30, 2026 without performing any repair. When I contacted Volvo Cars customer care directly, they confirmed verbally that the recall remains open for my vehicle and that parts are still unavailable. However Volvo stated they will only provide written confirmation to the dealership, not to me as the vehicle owner. Vehicle Safety Complaint – 2021 Ford F-350 I am submitting this complaint regarding a safety-related failure involving my 2021 Ford F-350. While driving under normal conditions, the vehicle suddenly entered “limp mode,” resulting in a significant and immediate loss of power. Approximately 20 miles later, the vehicle completely shut down, leaving me stranded and requiring the truck to be towe
... d to Gene Messer Ford for diagnosis and repair. The failure created a potentially dangerous situation, as the loss of power limited my ability to maintain safe speeds and respond to surrounding traffic conditions. After the vehicle was delivered to the dealership, it took approximately one week for inspection and an additional two weeks to complete repairs. During the repair process, I was informed that newly installed components—including the fuel rail and two fuel injectors—were defective and required replacement again. This raises concerns about whether the root cause of the failure was properly identified and whether contamination or systemic failure within the fuel system was present. I am also aware of widely reported issues involving high-pressure fuel pump (HPFP) failures in Ford diesel engines, which can result in catastrophic damage to the entire fuel system, including injectors, fuel rails, and lines. Given the nature of my vehicle’s failure and the components involved, I am concerned that this incident may be related to a broader, known defect. This type of failure—sudden loss of power followed by complete shutdown—poses a serious safety risk, particularly if it occurs at highway speeds or in high-traffic conditions. I am requesting that this complaint be reviewed as part of a broader investigation into potential fuel system defects in Ford diesel trucks. I believe this issue may warrant further scrutiny and possible recall action to prevent similar incidents.Read more

The vehicle required a second repair (attached) for the engine issue immediately after the first, further proving the failure was not 'bad fuel' related. Despite my state-certified proof, the dealer refuses to provide any documentation for their claim of 'bad fuel.' I believe the dealership has intentionally misdiagnosed a known defect covered under Federal Safety Recall 24V-763 / QK8 (cracked high-pressure fuel pump) to avoid performing a free recall repair.Read more

I have the 2015 Hyundai sonata with the 2.5 GDI engine that eventually will seize due to internal damaged caused by manufacture design flaws. My Hyundai threw a P0010 code which causes my car to idle high and causing low vacuum affecting my brakes. ... ne, They quoted me 3700.00 but the engine was to far gone and this repair would only accelerate the engine to seize. They said since the engine isn't seize Hyundai will not cover the cost to repair or exchange the engine, I contacted the Hyundai customer service who stated since the repair that is needed wasn't the root cause of the problems associated with the engine swap warranty they were not going to help me even though I explained to them that the cars high accelerating and the braking system been jeopardized and been told the engine is beyond repairs I feel Hyundai should reconsider their decision and help a loyal customer. I have purchased 12 Brand New Hyundaj since 2001. The customer Representative was Extremely un Professional even suggesting I scrap my car, Also I contacted My Hyundai service Technician who said Hyundai customer service never contacted them about my car, When they said they spoke with the technician. Also I received an Extended warranty on the 2.4 GDI engine from Hyundai who many Hyundai customers are or have dealt with.I also purchased an Extended warranty Which I believe should have been extended as well Thank YouRead more

COMPLETE shutdown of the engine while driving. No warnings - no flickering- no stumbling or misfire, just complete engine shutoff - dash and everything just turns off. Reading up on past recalls for this era of vans (mine a 2016) the 2013-2020 recall on the fuel pump sounds identical to this same thing I am experiencing. I was driving along in the city with my family in the vehicle and 2 times a

I was driving down the highway, and the dash started to flash a message about the vehicle being unsafe to drive, stop driving immediately, call dealer. I was able to stop and then turn left across traffic with limited power; and continued to make my way back to work thinking I could probably get to my parking lot. The dash continued to display warning lights, starting with brake failure, then par
... king brake failure, and then the whole series of possible failures. I continued to drive about one mile. I shut the power off an powered up twice thinking that might reset the sensors; but they continued to fail. As I continued to drive there was limited power to the throttle, and I eventually coasted through a red light and into my work parking lot where the vehicle shut down entirely, and eventually fully locked up (couldn't start or event put in neutral to push). I had the vehicle towed to back to the dealer, and they diagnosed the problem as a failed transmission cooler. Of note, I had purchased the vehicle brand new less than 3 weeks before this incident and the vehicle had less than 600 total miles on it. There were no prior signs of trouble or warning lights.Read more

I am the 2nd owner of my 2016 Ford Escape 1.6L ecoboost, my aunt and uncle were the original owners. Between us we have every maintenance record of every on time and professionally performed requirement the Escape has needed and it ran beautifully. Then, in February 2026 the idle started to get a bit rough and I was preparing to put in the OEM sparkplugs when 3/28/26 the engine overheated, I was s
... tranded on the side of a 45 mph street waiting for it to cool so I could move to safety. Between that incident and 4/29/26 I went thru 9 gallons of coolant, replaced the sparkplugs, put the front end up and "burp' the system as there was constantly air bubbles being introduced from a source I could not find. I had the hoses and clamps inspected, replaced the coolant reservoir and the reservoir cap, the vehicle started to shutter upon starting even after it was warmed up and every day needed to be burped and topped off before and after driving in town but often had no coolant loss during freeway driving, finally 4/28/26 I noticed smoke from the exhaust when I started the car in the morning and that smoke was accompanied by a foul, burnt, electric smell. While burping the air and topping the coolant that morning, the engine sputtered and died 2 separate times. I decided that it was time for a cooling system diagnostics as I still could still see no evidence of an external leak. The Ford technician recorded this tech story. "The vehicle ran rough and smoked for a couple seconds after starting.the vehicle ran its self test and had p0316, b0303, p1299, and p0217. These dtcs relate to cylinder misfire on startup and cylinder misfire and cylinder 3 and over temp/protection being active. ... I suspected a possible coolant intrusion concern into cylinder 3, performed a pressure test on the cooling system and observed a drop in the pressure. I remove the spark plug and found coolant on the cylinder walls and the top of the piston in cylinder 3." Advise to replace long block assembly.Read more

The tracking number assigned by NHTSA for this issue is 11733456 I am writing to update and escalate my previously filed safety complaint regarding my vehicle. I recently received my car back from the dealership after they informed me that all issues had been repaired and that the vehicle was safe to drive. However, after getting the vehicle back, it is not the same, and it is not safe to opera
... te. The vehicle pulls strongly to the left while driving, which creates a serious risk of losing control, especially at normal road speeds. In addition, multiple safety and driver‑assistance functions are not working as intended. Despite these issues, the dealership insists that everything is “within specifications” and refused to take further corrective action. Because of these unresolved defects, I am afraid to drive this vehicle. I have already experienced a traumatic incident related to this car, and returning it in this condition has caused significant fear and emotional distress. I cannot go through another dangerous situation. I believe this vehicle poses a genuine safety risk to me and to others on the road. I am requesting that NHTSA review my complaint again, including this new information, as the problem persists after inspection and attempted repair. A vehicle that pulls to one side and has non‑functioning safety systems should not be considered safe for normal use. Thank you for your time and attention to this matter. I am available to provide any additional details, documentation, or records you may require.Read more

With the recent completion of recalls 68&78c , and a new HV battery, APPEARS TO ME, thatJeep has changed the software for the throttle response when in electric mode. The result creates a safety hazard as you lose pedal response and power when the battery reaches below 35% or about 9 miles remaining. On the highway the jeep loses all power and will not accelerate over 62 mph. And the same around t
... own when you need that extra umph, the pedal becomes dead. More Scary when you’re on the highway!! The result? Since the electric power output is unavailable, you have to mash the pedal to the flow to kick it out of electric mode. But this is also unsafe as now you start the engine (often cold as I was running electric mode) and you smash the rpm’s over 6k to launch the gas engine (That can’t be good for the engine long term) . Before battery replacement and recall software updates, this process was very smooth and occurred with 5% or less battery life. This had occurred a few times and could probably reproduce it when this scenario is presentRead more

Replaced the actuator, drove about 50 miles, light comes back on. Finally get my appointment with dealer and they say my turbo is bad, they want $8k to fix. I ordered the turbo and replaced it myself along with another actuator, test drove. Light still comes on but have not lost power yet?? Dealer is not helping. This is a major issue that is all over the internet! The truck just goes to limp and your in the middle of traffic. If this has not caused an accident it’s only a matter of time before it does. On March 9, 2026, I brought the vehicle to BMW of Fairfax for a scheduled appointment. In the week leading up to the visit, the vehicle intermittently failed to start and required jump-starting using a portable battery starter. Despite this issue, the car drove normally, showed no warning lights or error messages, and had approximately 69,000 miles on the odometer at the time. After jump-sta
... rting the vehicle, I personally drove it to the dealership. I paid for a diagnostic evaluation and also authorized completion of a minor recall. Two days later, I was informed that the vehicle was no longer able to shift into gear and was now completely inoperable. I was told I would be responsible for towing the vehicle off their premises. According to the service department, the vehicle now requires replacement of the EME module, at an estimated cost of $12,000. This assessment was particularly troubling because the vehicle was fully drivable when it arrived at the dealership. At no point prior to service did the car have difficulty shifting into gear. It is deeply concerning that the vehicle became a “brick” while under the care of an authorized BMW service center, with no accountability taken for what occurred during their diagnostic process.Read more

The fuel injection system failed, specifically causing a cylinder 1 misfire (DTC P0301). The component is currently installed in the vehicle and available for inspection. The vehicle experienced a sudden loss of power and violent shaking while driving in traffic, creating a high risk of a rear-end collision or being stranded in a dangerous intersection. Multiple safety systems (VSA, Braking Assist
... , Power Steering) were disabled simultaneously. The problem was officially confirmed and diagnosed by Saccucci Honda [XXX] ) on Invoice #XXX. The vehicle has been inspected by the dealer (Saccucci Honda), who confirmed the failure is internal to the fuel injectors. There were NO warning symptoms prior to the failure. All warning lamps and messages appeared suddenly on 04/20/2026 exactly 3 days after the official Honda Fuel Pump Safety Recall was performed at a different dealership of Honda . Check engine light illuminated on my 2021 Chevrolet Tahoe. Upon diagnostic scan, the following emissions-related fault codes were present: P11D5 (NOx sensor), P14D9 (NOx sensor), P2383 (EGR pressure sensor), and P2C7A (NOx catalytic converter efficiency). All components are available for inspection upon request. The vehicle was taken to an authorized Chevrolet dealership for diagnosis. The dealer
... acknowledged the emissions codes but stated that unrelated components would need to be repaired at owner expense before the emissions system would be addressed, despite these codes falling within the federally mandated 8-year/80,000-mile emissions warranty. The fault codes were confirmed by the dealership. No safety incident has occurred to date, however degraded NOx and EGR function can result in increased harmful emissions output and potential engine performance issues. Warning lamps appeared without prior incident or symptom. The manufacturer has not offered to cover the emissions components under the federal emissions warranty without the owner first paying for additional repairs that may or may not be causally related to the emissions system failure. When the dealer was directly asked, "Are you saying the emissions system itself is functioning correctly, and something else caused these codes to appear?" — the dealer did not provide an answer. This question was asked in order to clarify whether the emissions components themselves are degraded or failed, which would confirm warranty coverage eligibility. The dealer's failure to answer this question directly is noted as part of this complaint.Read more
