In June 2024, my 2019 Ford Expedition experienced serious transmission and powertrain problems. Diagnostic testing identified catalyst efficiency codes (P0430), and I was given a repair estimate that included catalytic converter replacement. The vehicle also displayed repeated “Service Engine Soon” and “Powertrain Malfunction/Reduced Power” warnings. Because the vehicle became undrivable a
... nd the repair costs were unaffordable, I was forced to release it to auction in June 2024, resulting in a significant financial loss. In April 2025, Ford issued Recall 24E13 acknowledging that PCM programming errors could falsely trigger catalyst efficiency codes (such as P0430), potentially leading to unnecessary catalytic converter replacements, exactly the condition my vehicle experienced. This recall defect directly contributed to the loss I suffered, as I still owed about $11,000 on the vehicle when I could no longer use it. I have already opened a case with Ford (Case #CXH05645573C2W6R6), but I have not received meaningful assistance. When I contacted Ford Customer Service, I was not offered escalation or resolution. I was only told to call another phone number, which left my case unresolved. I respectfully request that NHTSA review Ford’s handling of this defect, including the financial impact on owners like myself who incurred substantial losses before the recall was announced.Read more

On the afternoon of Wednesday August 6th I was no less that 3-4 miles from my home when the car stopped in the middle lane of a very busy 3 lane highway and never started again. It was 90+ degrees outside and even hotter inside the vehicle with leather seats. I became extremely nervous and my small 10 pound puppy began to have anxiety and overheat. When I pulled away from my home the car was buc
... king and jerking. The car had just completed an OTA software download and installation a few hours earlier so I assumed it was the after affect of the software update. As I pulled onto the main road the car was still driving very roughly and when I cam to a red light the car started to shake back and forth very roughly and once the light turned green I was unable to accelerate. Error messages where coming up on the screen. I tried to start the car to move out of the middle lane but it would not start. A state trooper had pulled behind me and got out to assist. She noted she saw gray smoke coming from behind the vehicle as I was driving up to the light which prompted her to get behind me initially. She and I both worked to try to get the car into park but it would not shift gears. She noted fluid was running out of the car but we both assumed it was coolant or antifreeze. She radioed the local police to assist since we were in the middle lane of a dangerous highway and I contacted VW roadside assistance. The local police officer arrived on the scene and was able to assist with getting the manual parking brake on. When the tow truck arrived he asked if the car would go into Neutral we advised NO he then stated we were standing in gasoline and if we tried to start the vehicle the car could have exploded or the engine could have caught fire. If someone had rear ended or hit us there would have been a fire because the entire gas tank has leaked onto the ground that was the fluid the state trooper saw running from the back of the vehicle.Read more

I am reporting a recurring issue with my 2023 Hyundai Elantra where the low oil pressure warning light illuminates consistently during strong acceleration. On Sunday, September 21st, 2025, my 2013 Kia Optima EX suffered sudden catastrophic engine failure while driving on the highway. The check engine light was flashing, the car misfired badly, lost complete power, and became unsafe to operate. The vehicle was then towed to Wagner Kia of Shrewsbury, MA. On Monday, September 22nd, Kia diagnosed the engine with holes in the intake valves (Cylinder 3)
... and multiple misfire codes (P2191, P0272, P0301, P0304, P0172, P0303). The dealership documented that major engine repairs would be required, including two intake valves, two exhaust valves, valve cover gaskets, hraf bolts, two ignition coils, and a valve job. This sudden engine failure created an immediate safety risk of stalling or losing power while in motion, which could have led to an accident. Despite Kia’s history of Theta II engine recalls and settlements, my VIN is not included, leaving this major engine defect unaddressed. I am requesting that NHTSA investigate and consider expanding Kia’s existing engine recalls to cover this engine failure, as it represents a serious safety issue consistent with Kia’s documented engine problems.Read more

Summary: Sudden loss of engine power while driving due to lifter failure. Details: While driving my 2022 Chevrolet Suburban equipped with the 5.3L V8 (RPO L84), the engine suddenly lost power without warning. The vehicle began misfiring and lost propulsion in traffic, creating a hazardous situation. I was unable to maintain normal speed and had to maneuver to safety while the truck rapidly lost p
... ower. The dealer later diagnosed the problem as a collapsed lifter/valvetrain failure — an issue that GM has already documented in service bulletins (PIP5776G, 23-NA-043) and a service update (N212353840). These bulletins show GM has long been aware of defective lifters in 5.3L engines. This is not just a drivability concern. A sudden engine failure and loss of propulsion while driving poses a clear safety risk, especially when merging, crossing intersections, or towing. I maintained the vehicle properly and have all service records. I request NHTSA investigate these widespread lifter failures in GM’s 5.3L V8 engines as a safety defect, since they can and do cause sudden loss of power in motion.Read more

My VIN was not included in the recall but suffered the same failure as the recalled units. No warning lights or symptoms prior to the failure. This continues to be a safety issue because without warning the vehicles just stop running. The Equinox had 45,939 miles on the odometer at the time of the failure. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The low-pressure fuel pump (fuel delivery module) appears to be failing, consistent with Recall 25V-586 / Alfa Romeo Campaign 93C. This causes fuel starvation and loss of motive power. 2. How was your safety or the safety of others put at risk? The vehicle has experienced loss of motive power (L
... OMP) while driving. At times it jerks or hesitates when accelerating from a stop, and while climbing a hill it suddenly lost power, displayed a Service Electronic Throttle warning, and went into limp mode. This created a risk of being unable to accelerate safely in traffic, increasing the chance of being rear-ended or unable to merge. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. I described the symptoms to Alfa Romeo Service dept, and they told me it is related to the recall and that other customers have reported the same symptoms and warning lights. However, they advised that there is currently no remedy available, and I must wait for the fix before scheduling an appointment. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not yet. Alfa Romeo advised that no remedy is available at this time, so the vehicle has not been formally inspected by the dealer or manufacturer. Advised not to drive long distances until fix determined. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. Check Engine Light and Service Electronic Throttle came on as car lost power and entered limp mode with code P0299. Alfa Romeo Service Dept tech said these warnings match the recall. Rep said this shows how loss of motive power from fuel starvation manifests electronically & ECU interprets fuel starvation as underboost, which then triggers limp mode.Read more

[XXX] [XXX] Phone: [XXX] Email: [XXX] [Date] National Highway Traffic Safety Administration Office of Defects Investigation (ODI) 1200 New Jersey Avenue, SE Washington, D.C. 20590 Subject: Safety Complaint Regarding 2020 GMC Yukon XL Denali – Repeated Engine Failures (VIN: [XXX] ) To Whom It May Concern: I am writing to formally file a safety complaint regarding my 2020 GMC Yukon XL Dena
... li (VIN: [XXX] ) which I purchased in 2021. This vehicle has experienced repeated and dangerous mechanical failures that raise significant safety concerns. •March 2024: The oil pump failed, resulting in immediate engine issues. Repairs were completed under warranty, but the check engine light remained on afterward. •April 2024: The vehicle broke down again. At that time, the cams and lifters were replaced in addition to other service, but the underlying issues were not resolved. •August 2025: The vehicle failed once again. After taking it in, the service department could not determine the cause. When they believed they had identified the issue and took it for a test drive, the vehicle broke down again and had to be towed back to the shop. At that point, I was informed that the vehicle now requires a complete motor replacement and a new fuel pump. Each of these failures has left me stranded and placed me and my family in unsafe situations on the road. Despite contacting the dealership (Joseph Cadillac of Dublin), General Motors, and the warranty company, none have taken responsibility to properly resolve the issues. I believe this represents a potential defect in the engine or related systems that could impact many other consumers. I respectfully request that the NHTSA investigate this matter to determine whether a broader safety recall or manufacturer action is warranted. Thank you for your attention to this matter and for helping ensure consumer safety. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

After safety recall 73B was performed on my 2018 Chrysler Pacifica Hybrid on 08/25/2025, the vehicle's check engine light illuminated on 09/09/2025. I was inundated with multiple failure lights including; check engine, service electronic throttle control, service hybrid electric vehicle system, blind spot alert, and I felt like I lost power steering. I was unsure if the van was safe to drive
... , but was able to get it to the dealership. They were able to pull multiple codes. Including the one for the check engine light. They decided that the problem was related to the ABS control module. They replaced it and the van has been fine so far. It seems these error codes are widespread after performing recall 73B. Many other customers are complaining of the same thing. I believe these errors are from the testing to the High Voltage battery, and their 'software fix'. It's not safe to lose your power steering while driving. It's not safe to have multiple error codes continuously going off while operating the car. It's not safe to have a family car that we have been told twice to not park in the garage, charge, or park near a structure. This is their second attempt at 73B, and overall the second fire recall. I own a 2018 Honda Accord Sport. About 2–3 months ago, the car began having repeated engine misfires (P0301), hesitation, and stalling while driving. At the same time, multiple dashboard warnings appear, including Steering Assist Reduced, Brake System Problem, Collision Mitigation Braking System Problem, Adaptive Cruise Control System Problem, Emission System Problem, and Hill Start Assist Probl
... em, these failures occur suddenly while the vehicle is in motion, creating a serious risk of loss of engine power, reduced braking performance, loss of steering assist, and disabled crash-avoidance systems. I have already performed extensive maintenance: new ignition coils, a full tune-up, compression test, spark test, and battery/alternator check, yet the issues remain. Diagnostic scans also showed U0416-68 (invalid data from ABS/VSA module) in addition to the misfire. This suggests not only a fuel/propulsion defect but also a possible computer/control module failure that links engine, brakes, steering, and ADAS systems. These conditions match known recalls for the Accord, including 23V858000 (fuel pump failure), 20V314000 (low-pressure fuel pump), and 20V771000 (BCM software issue), yet my VIN does not currently show as included. The fact that multiple critical safety systems fail simultaneously makes this a major safety hazard that could easily lead to an accident if the car stalls, loses braking, or loses steering assist in traffic. I opened a case with Honda Customer Service (Case #15868389), but they refused to cover the repair since my VIN is not listed, despite the failures being identical to those described in the recalls. • What component or system failed or malfunctioned, and is it available for inspection upon request? The plastic fuel tank failed and developed a leak. Yes, the tank is available for inspection upon request. • How was your safety or the safety of others put at risk? A leaking fuel tank poses a serious fire and explosion hazard, especially when the vehicle is operating or parked near heat sour
... ces. Gasoline fumes were noticeable, creating an increased risk of ignition and exposure to toxic vapors. • Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The problem was confirmed by an independent mechanic, who verified the leak in the fuel tank. • Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has not yet been inspected by BMW or other officials. Only an independent mechanic has inspected it so far. • Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. A check engine light appeared shortly before the failure. In addition, a strong smell of gasoline was noticed, followed by visible leaking fuel. There is an open recall on the BMW F30 BMW 320i, production date 2013 to 2018, production dates July 02, 2012 to March 23, 2018 for the “plastic fuel tank.” I own a 2018 BMW 320i, manufactured April 2018 which is not included in the recall for the “plastic fuel tank.” However, I noticed a gasoline smell and the fuel tank is leaking. Upon further research, inspection and after notifying a mechanic, the fuel tank for my vehicle is the exact same one as the plastic fuel tank indicated in the BMW recall. My production date is a week later than the recall production date, but the fuel tank is the same exact fuel tank that has been recalled. I am reaching out regarding ongoing and escalating issues with my 2022 VW Tiguan R-Line Black. Over the past year, I have had to stop at Jiffy Lube multiple times because the oil level kept running low, and they repeatedly topped it off. I found this unusual, given how recently oil changes and services had been performed. This summer, the vehicle began experiencing episodes of stuttering and stall
... ing, accompanied by smoke from the tailpipe. Over Labor Day weekend, after another stall, I brought the car into the VW dealership. They investigated but were unable to identify the cause. Unfortunately, the situation has now worsened. Today alone, the car stalled twice — both times in the middle of traffic. The “check oil level” warning light came on, followed shortly by the “check engine” light. There was also a significant amount of smoke coming from the tailpipe. This is not only a mechanical issue but also a serious safety concern. I have two children, and I am deeply concerned for their lives and safety when driving a vehicle that stalls unpredictably in traffic. At this point, the car feels unsafe and unreliable, and I am unsure how to proceed. Please advise me on next steps regarding potential legal recourse, warranty coverage, or claims I may pursue. This vehicle does not have a traditional alternator. There was a TSB and an extended warranty offered to some models, but not all included in the VIN range for this vehicle. The vehicle was scanned and pulls the same co

Both turbos, spark plugs, catalytic converters, O2 sensors. If turbos blow up while drive it will shoot vanes in the motor. Yes JD automotive replaced all the above mentioned parts. will include copy of the invoice, not the only ford owner that has had issues out of the ford twin turbos failing. not sure if dealer inspected turbos, but they replaced the spark plugs 3x in less then 3 months (l
... emon) and the throttle bottle. Didnt share this information with me when vehicle was purchased. Issue started with a P0430 codes which falsely say its a cat code. I used cataclean due to the low miles and would still get code every once in while. less the 2 years from purchasing it used had both turbos fail. not sure if it was a mis diagnosis from mclarty daniel in Bentonville Arkansas or if ford knowing made motors with bad turbos.Read more

My car is a 2022 Volvo hybrid T8, 6-passenger XC90. I purchased the SUV in August of 2023 as a "certified pre-owned" vehicle with only 17,923 miles on it at the time of purchase. This SUV has been fine until within the last two to three months (July-Sept. 2025). Now, in Sept. of 2025, my SUV only has 30,795 miles and we have had it serviced regularly, as required, and taken care of it as respons
... ible vehicle owners. Beginning around June or July of 2025, I began seeing the "check engine" light, so I called the dealership where I purchased the vehicle and took it in for servicing. After a day, they said it was fixed and that it was a blown fuse or two, and it was good to go. Not even 3 weeks to a month later, the "check engine" light came on again. I called and took it back to the dealership. Same story, after less than a day they called me back after fixing something else (don't remember off the top of my head what they said it was, but it was allegedly minor). Now, about 3 days ago, Sept. of 2025, I began seeing the "check engine" light again, so for the 3rd time in less than 3 months I arranged for it to be serviced by the dealership again on 9/30. A day later, another "Propulsion System/maintenance required!!!" light came on as well, so I called to get servicing sooner because obviously the message is scary. I was told to bring it in, they would "work me in" but had no loaners I could use to get to and from work. So, at that point, I got my husband involved and now no one from the dealership is returning either of our calls, AND I'm being told there is no recall on my car, even though a simple google search reveals there is a recall on my same make/model/year SUV. I'm getting the same messages people got about the recall, so no clue why my vehicle isn't considered as part of the recall when I am having the SAME issues as the recalled vehicles are having. I am beyond frustrated and ready to dump this SUV if they are unable to properly repair it!!!!!Read more

Throttlebody hose pops off of the throttlebody due to a missing gasket ring as well as the retention clip/spring clip. The plastic pieces on the spring clip have popped off since the issue has been presented. I am submitting this complain regarding my 2021 Toyota Venza, which has developed a serious defect involving the fuel system. Although my vehicle is fairly new, I began experiencing problems with the fuel door not opening properly. Initially, I could not access the fuel cap, which left me unable refuel in a timely manner. Soon after, I noticed additional fueling issues: while attempting to ref
... uel, the gas pump would repeatedly shut off and stop the fueling process. After taking the vehicle for inspection, it was determined that the actuator responsible for controlling the fuel door and fueling system required complete replacement. I am concerned this defect represents more than just an inconvenience. Inability to open the fuel door or complete a fueling cycle can create dangerous situations - such as being stranded without fuel or blocking a fueling station. Additionally, if the actuator fails while in use, it could interfere with the proper venting of the fuel system. I have since learned that other Toyota Venza owners are experiencing the same problem with the actuator, suggesting this is not an isolated case but a potential widespread defect. I request that Toyota investigate this issue and that the National Highway Traffic Safety Administration evaluate whether a safety recall or service campaign is warranted. 2021 Toyota Venza LE [XXX] 90,0000 Sincerely, [XXX] and [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While the vehicle was approximately ten months old with approximately 11K miles, it was continuously not functioning properly and would not start/accelerate. My 2024 KIA Forte, which was purchased new from a KIA dealership, has continuously failed to function properly. It had a water pump replaced after failing to be operated for more than a week and has since continued to fail - this time for not
... starting at all, not accelerating properly, and losing all power while driving (switching off completely) on at least two separate occasions. The dealership stated that I should drive the vehicle and document it when it occurs again (video) to assist them with identifying the issue since they are unable to correct the malfunction. It has been at the dealership for more than a month with no definitive resolution concerning the safety of the vehicle. I refuse to drive the vehicle and am afraid that I may injured/killed in an accident due to the safety concerns aforementioned. As of today's date, 09/22/2025, my 2024 KIA Forte remains at the KIA dealership with no foreseeable actions that may lead to a safe resolution. Further: 1) Component or system failed/malfunctioned - Engine, fuel pump, water pump. 2) Is it available for inspection - Yes. 3) How was your safety or that of others put at risk - Vehicle switched off multiple times while operating in traffic and barely escaping accidents. Suffered a scrape on the passenger's door as vehicle would not accelerate and stalled in a separate incident. 4) Vehicle or component been inspected by manufacturer - Yes, at the KIA dealership again. 5) Were there any warning lamps, messages, or other symptoms of the problem prior to the failure - No, there were no prior warning lamps, messages, or symptoms. KIA dealership stated that the 2024 KIA Forte has been "reprogrammed", but the dealership has been unable to identify the safety issues it has. I do not feel safe to operate this vehicle nor have any family members operate it.Read more

My 2024 Volkswagen Tiguan (purchased used) has a recurring and unsafe issue related to acceleration and the automatic start/stop feature. When accelerating from a stop or while maintaining slight pressure on the gas pedal, the vehicle lurches forward as if aggressively accelerating, even though my foot pressure has not changed. Additionally, when the start/stop feature re-engages after idling, th
... e vehicle sometimes lunges forward despite my foot being firmly on the brake pedal. This behavior has occurred multiple times, creating near-miss situations that could have resulted in accidents. Service History: The issue was replicated by one Volkswagen technician during an overnight test drive but was not accurately reflected in the official service notes. A different technician (shop foreman) later claimed the concern could not be replicated and dismissed it as a “normal vehicle characteristic,” despite my report. The dealership performed a factory reset but the problem persists. Safety Concern: This is a serious safety risk that poses a danger to myself, my passengers, and others on the road. I am especially concerned because my teenage daughter is about to begin driving this vehicle, and I do not feel it is safe for her. Manufacturer Response: Volkswagen of America has declined my buyback request, stating that the situation “does not merit an offer.” I strongly disagree, as the vehicle’s behavior is not normal or acceptable, and could result in unintended acceleration accidents. Request: I am requesting that NHTSA investigate whether this issue is a defect in the 2024 Volkswagen Tiguan and whether other vehicles may be experiencing the same unsafe condition.Read more

This truck has had to have several high temp burnoffs because of failures in the EGR and Carbon build up. The recent recall for Rams does not cover the 2012, and began at 2013, however, we are experiencing the same exact issues with the same type of system. In addition, we have had repeated airbag issues due to the positioning of the cockspring and wiring getting choked out.
