My 2012 Honda Pilot is experiencing the same engine defect that Honda acknowledged in later model years (2013–2015) and issued recalls/extended warranty coverage for. The defect involves the Variable Cylinder Management (VCM) system causing excessive oil consumption, spark plug fouling, cylinder misfire, rough idle, fuel odor, and drivability issues that create an unsafe driving condition. I bro
... ught the vehicle to a Honda dealership in 2023 due to rough idle and a strong fuel smell — symptoms consistent with Technical Service Bulletin 13-082 for misfire and piston ring failure in 2009–2013 Honda Pilots. The dealer did not repair the issue or disclose that this matched a known defect. The vehicle now exhibits severe engine performance issues that pose a safety risk, including hesitation, engine shuddering, loss of power during acceleration, and strong fuel odor that may indicate improper combustion. Honda has issued recalls and warranty extensions for the identical defect on 2013 and newer Honda Pilot models with the same 3.5L V6 VCM engine but refuses to repair or acknowledge the defect for my 2012 Pilot, even though it uses the same engine architecture and has the same symptoms. This discrepancy indicates a potential gap in recall coverage that leaves owners of 2012 Pilots with a known safety-related defect unprotected. I am requesting that NHTSA investigate why the 2012 Pilot was excluded, despite identical engine design and failure patterns.Read more

Good morning ! I am an owner of a 2013 ML 350 . I live in San Diego ,CA ,92103 . I was driving my car on [XXX] and it was raining a lot . After around 30 miles I got to my destination parking area and my car died 20 feet away from the parking zone .I tried to reignite it but it wouldn’t start . I checked it with a OBD2 scanner and it was indicating a code U0109 - Lost communication with fuel
... pump control module . After looking into solutions and reasons for the issue online I came across a post regarding an existing recall with NHTSA with ID 22v955 regarding water intrusion in the spare wheel well portion of the trunk( The Wheel well is not water proofed and doesn’t have a drain port for water to escape ) . I proceeded with inspecting the area and of course it was full of water . The fuel pump control module is located in that area and it was damaged which shorted it and cause the malfunction of the vechile and the error code U0109 stated above . Tried calling Mercedes Benz but they confirmed about that specific recall but said my VIN is not part of it and I should file a claim or complain with you . I will attach proof and pictures of the water and the damaged component. My car now need to be towed and fixed on my expense because of Mercedes Benz negligence… Hopefully you can add my car to this recall and when this happens I would get some kind of reimbursement… Thank you for your time and understanding. The vehicle uses hydrogen fuel which is stored in three high pressure cylinders. Any fires or damage to the vehicle that affects the fuel system can result in an explosion. During a recent recall, customers were recommended not to park near structures because the car can explode and cause property damage. This is applicable to all hydrogen fuel cell vehicles available in the US.

Initial Fault Code: P0303 Replaced all 4 spark plugs and all 4 ignition coils. The check engine light extinguished momentarily but returned, logging P0303 and a new P0301 Replaced all 4 fuel injectors. CEL extinguished momentarily and then returned, logging P0303. Replaced the entire engine electrical harness. Concurrently, all related electrical components and connectors were meticulously cle
... aned with professional contact cleaner. CEL extinguished for significantly longer period of time, and then returned, re-logging P0303 and P0301. Thid suggested the ECU was "handshaking" with new components but re-logging the fault upon detecting the persistent core issue. A full compression test was performed. All cylinders reported results within specification. Using a borescope I inspected cylinder walls in piston tops. There was significant carbon residue buildup but nothing out of the ordinary for similar GDI engines. I still performed a PEA GDI specific chemical cleaning. It was a a little smoother afterwards, but the misfire persisted and CEL remained on. I also removed, cleaned, and tested common sensor faults, with no result. Also p0128 code and no start / electrical drain, and engine light started flashing numerous occasions had to stop the vehicle. I tried swapping the coils, but all that proved was that the coil was good, I did a power and ground test, it passed the connector had 12 plus volt power and a perfect resistance to static ground. The signal wire from the coil connector had perfect continuity to the ECU but had OL to chassis ground showing there was no short. A test light was used to see if anything was coming from the signal wire to the ignition coil and that test failed, there was no signal pulse. The diagnostic process has definitively proven a component-level failure inside the engine control unit. See attached file for more detailed information.Read more

Known Fuel Tank crack GM Memo "Special Coverage Adjustment 17469 Fuel Leaks at the Primary Fuel Pump Module Flange", whose previous warranty extension has expired and shouldn't be! GM assumes no responsibility for notifying vehicle owners who purchased used vehicles from non-dealers, which is what I did. I never received any notices about this leak from them since 2018, when I bought the vehicle u
... sed from a non-GM dealer. I just recently discovered the leak and had it repaired for $ 5,300.00. I had been driving a vehicle that could have exploded for over 7 years and did not know it. There must be others who are still driving these without knowing. Below is my request to GM to reconsider their 'not covered' response to my email. I suggest that a known Special Coverage Adjustment 17469 Fuel Leaks at the Primary Fuel Pump Module Flange of the gas tank is a life-threatening issue and should never expire! Especially, since many of us bought these vehicles used, 2018 in my case, and have NEVER received any notifications from dealers or GM about this issue. How many others are still outstanding and could blow someone up? The pictures below were taken at my Watson Chevrolet dealer in Tucson, AZ this month, which shows that the crack is exactly what was outlined in your publication 17469, that I never received. Total bill was $5,330.89 (copy attached). I am asking GM to reimburse me for this horrible adventure, not to mention the danger that I've been in for these many years. While I should have detected this issue sooner, how was I to know? I finally gave a friend a ride who said she smelled a really bad gas odor, which led me to the dealer! Now, GM says "you're too late" for us to cover our known defect? Again, this is a life-threatening issue, and your warranty should never expire!Read more

I was driving to work on the morning of Nov. 12th, 2025 in East Greenwich RI. The car had been fully charged on my home Level 2 charger the night before. Less than a mile from my house, and without any warning or prior issues, I heard a loud pop from underneath the vehicle. Immediately the car displayed an error on t

The car still seemed driveable so I began to drive back home, noting that the car wouldn't exceed ~30 mph, but before I went very far at all the error changed to "Stop vehicle and check power supply." I pulled the car over on a side road and called for a tow to the nearest Hyundai service center in North Kingstown, RI. The next day the Hyundai service center reported that their diagnostics revealed that the ICCU had failed. I subsequently learned that they had been unable to drive the car even from their lot into the service bay - even with a jump charger the car wouldn't drive and they had to carry the scanner to the lot for the diagnosis. The Hyundai app reports 96% battery charge even now, which says that the 12V battery has been fully discharged, consistent with ICCU failure. The service center has not reported that the ICCU has been inspected by any other authorities other than them. The replacement part is due to arrive today, 11/15, and should be installed on Monday 11/17. Then, I received a diagnostic code of P-1101, and the recommended solution is to replace the intake ma
... nifold and all necessary gadgets. After speaking with professional technicians and doing thorough online research, I discovered that General Motors issued a service bulletin No. 19-NA-101, dated July 2024, covers certain 2013–2019 model vehicles (including Buick Encore with 1.4L turbo) for oil consumption and/or leaks caused by a missing non-return valve in the intake manifold. The bulletin acknowledges that the intake manifold design on these 1.4L turbo engines has a non-return (check) valve or PCV system component inside the manifold that can fail or go missing, leading to issues like boost/vacuum leaks, oil consumption, engine performance faults. •The PCV valve is non-serviceable (you can’t replace it separately), meaning the only fix is replacing the manifold — which GM calls out in the bulletin. If GM or Buick hasn't issued a recall they should offer goodwill assistance especially since this is a defect on their end.Read more

I have a 2022 Lexus RX350 F Sport. There are many complaints for this problem with this era of Lexus that happen daily. When pulling out into traffic the vehicle stutters or hesitates; this could cause a serious accident with injuries or death. I think it is a problem with the transmission shift programming. In an attempt to be fuel efficient, Lexus created a serious safety problem in my
... opinion. My car is under warranty, I brought it to the dealer and they said that is normal and a known issue with that era of Lexus; there is no fix or repair. Despite being under warranty, I paid to have the transmission system reset to learn my style of driving. The reset had no effect positive or negative. There were 750,000 Lexus RX's made between 2016 and 2022. Nearly all are affected by this problem. Note: Lexus RX450 hybrid uses a different transmission and likely does not have this problem. Please force Lexus to find a fix for this likely injurious, potentially deadly issue. Gemini AI reports this as a very common problem with this generation of Lexus RX. https:XXX I know it may have been helpful to have had a crash, injury, or fatality to help bolster my argument, but isn't the point to avoid that? The digital instrument cluster in my 2020 Jeep Gladiator has failed. The center display (LCD) went completely black, even though the backlight still glows faintly. This screen typically displays critical information such as vehicle speed, warnings, messages, and alerts. Without this display, I cannot monitor key safety information while driving. The analog gauges still function, but the central d
... igital panel remains blank regardless of ignition mode (RUN or ON). After diagnosing the issue myself, I confirmed power was present at the fuse terminals. I inspected and replaced fuse F36, which was damaged and likely contributed to the display losing power or shorting. I also inspected related fuses F43 and F57 (BCM and IOD power supply), which were intact. This is not an isolated problem. Research shows that other Jeep Gladiator owners (especially 2020 models) are experiencing identical symptoms — black instrument displays, “Push Brake to Start” messages that stay on after shutdown, and modules not entering proper sleep mode. These issues align with the symptoms listed in TSB 08-002-21A, which relates to software and communication faults between the BCM and IPC. Jeep dealership confirmed the failure and quoted $700–$1,000 for full cluster replacement, stating there are no DTC codes and no reflash updates available, and that the cluster must be replaced outright. Jeep has not issued a recall, yet this is clearly a widespread failure affecting visibility and safety of vehicle operation. The problem poses a safety risk to myself and others. I cannot see alerts or diagnostic messages while driving. If this happened at night or during highway travel, it could increase accident risk. Jeep should issue a recall or warranty coverage extension for the instrument cluster failure in affected 2020 Gladiators.Read more

My 2019 Cadillac Escalade ESV experienced a major engine failure that required a complete engine replacement. The failure was severe enough that it posed a safety risk had it occurred at highway speed, and it required over $20,000 to replace the engine plus thousands more in related repairs. Multiple other major components failed around the same time (suspension components, thermostat, and A/C compressor). I am filing this report because the failure pattern appears identical to what GM has acknowledged on recalled Escalade engines, and I am concerned that the defect may affect more vehicles than currently included in the recall. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? Answer: The vehicle’s 6.2L L87 V8 engine suffered a sudden internal mechanical failure involving the connecting rod, rod bearings, and crankshaft assembly, resulting in catastrophic engine damage. The failure mode is consistent with the defect described in GM Recall N252494001. The engine and a
... ll damaged components are fully available for inspection upon request. 2. How was your safety or the safety of others put at risk? Answer: The engine failed without warning while the vehicle was in motion, causing an immediate loss of propulsion. This created a hazardous situation where the vehicle could no longer accelerate or maintain speed in traffic, increasing the risk of a rear-end collision or loss of vehicle control. GM’s own recall documentation identifies this exact failure mode as a significant crash risk, and that same hazard occurred in my case. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Answer: Yes. A service technician confirmed catastrophic internal engine failure consistent with connecting-rod or crankshaft damage. The dealer also verified that the engine is inoperable and requires full replacement. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Answer: No, the engine failed while in motion and was taken to the closest mechanic. Inspections by an insurance representative or manufacturer representative can be arranged. The vehicle and damaged engine remain available for further examination. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Answer: No. There were no warning lamps, diagnostic messages, or drivability symptoms prior to the catastrophic engine failure. The failure occurred suddenly and without any advance indication. Recall 2024100002 for my 2019 Mercedes-AMG GT 53 is listed as “Completed” in the manufacturer’s system, but the dealer’s service record from November 2024 clearly states that the recall part was on backorder and the repair was not performed. There is no later record of the recall ever being completed. After the recall was marked “Completed,” multiple paid repairs were performed in the
... same system, including extensive electrical and engine-related work, totaling several thousand dollars. Warning messages and check-engine lights continued to appear, and the underlying issue was never properly resolved. The dealer repeatedly cleared fault codes and performed symptom-based repairs without identifying or correcting the cause. In 2025, the dealer also presented a very high repair estimate related to the same components affected by the recall. This raises concerns that safety-related repairs may have been converted into paid work while the recall remained incomplete. The vehicle was also marked “Disabled” by the dealer, even though it remained drivable. Release of the vehicle was delayed without a clear explanation. The dealer has not provided any confirmation that the recall was actually performed. The manufacturer acknowledged receiving my concern and forwarded it to the dealer, but no one has responded for several weeks. The unresolved status of this recall and the repeated issues affecting the electrical, engine, and propulsion systems raise concerns of a safety defect and possible recall non-compliance. I am requesting that NHTSA review the recall completion status and investigate whether the required safety remedy was performed correctly or reported inaccurately.Read more

My fuel tank was practically emptied by "gas thieves". The fuel filler compartment remains unlocked regardless of whether the vehicle is locked or unlocked. And the filler tube where the gas nozzle is inserted is capless, with just a rubber seal that expands when the nozzle is inserted. In previous models Dodge used to have a button inside the cabin right below the headlights switch to release the
... fuel filler door, and a safety puller string in the back of the vehicle, should the button fail to release the locked door. Not anymore, and this appears to be a cost saving change/decision in detriment of the consumer. This lack of safety feature is also true for RAM 1500 pick up trucks. Dodge does not offer a locking cap option for the capless fuel filler tube, nor could I find an aftermarket one. A major concern of mine is what would happen should the vehicle roll down or lay on a side? Would the fuel in the tank come gushing out, and possibly cause a fire or even an explosion? Will appreciate your further investigation and response to this issue.Read more

Car started stalling and misfiring. When I took the vehicle for service they said it was a known issue and they were replacing all 4 fuel injectors. This was my wife’s car. About a year and a half ago the same thing happened to my 2022 Hyundai Tucson N-Line. I have googled around and found many many issues of the same thing happening on 2022 Tucsons.

My vehicle is affected by Recall 25V-741 (FCA recall 68C), which involves a risk of high-voltage battery fire due to potential internal separator damage. FCA’s NHTSA filing instructs owners to refrain from recharging the battery and to park the vehicle away from structures or other vehicles until a remedy is available. The recall remedy is not available. FCA Customer Care advised me by phone th
... at the vehicle is “safe to drive in e-save mode unless symptoms appear,” which contradicts the recall notice because the defect can cause a fire without warning or symptoms. This vehicle automatically recharges the high-voltage battery through regenerative braking and engine charging, making it impossible to comply with the instruction to “refrain from recharging.” During another discussion, a technician also told me that the “stop sale has nothing to do with the safety of the vehicle” and advised me to “drive normally.” When I asked about the prior cases where vehicles covered by this recall had caught fire, the technician replied, “Well, they’re still around, right?” This guidance minimizes the documented fire risk and contradicts the official recall filings. The vehicle cannot be safely used or stored under normal conditions, and no interim solution or transportation has been provided. The dealer stated they were instructed by the manufacturer’s warranty team not to assist and reiterated that no remedy exists. I am reporting the unsafe and contradictory guidance from the manufacturer and the inability to safely use the vehicle while the recall remains unresolved.Read more

The SUV was driving smoothly at 55mph speed, suddenly I hit the break and tried to reaccelerate and felt a hard transmission shift, felt like metal hitting metal, felt like it was forced to shift to drive mode from parking mode. After that I started using 93 octane rated gasoline, do not observe the problem too much. I will really scary and unsafe, the whole SUV shook hard for 2sec.

My 2015 Chevy Volt has been experiencing ongoing mechanical and electrical issues, including random power loss, reduced propulsion warnings, and sudden vehicle shutdowns with no prior indication. These problems pose a serious safety concern, as the car can lose power unexpectedly while driving. Despite these issues, GM has refused to service the vehicle or unlock necessary components because they
... no longer manufacture or support the required parts. As a result, I am left with an unsafe and unreliable vehicle with no available remedy from the manufacturer. This is forcing me to trade in the vehicle at a lossRead more

I am an active U.S. Air Force member. My 2021 GMC Sierra 1500 failed GM Recall N252494003 on Sept 29, 2025 (L87 engine loss of propulsion). GM states this defect can cause sudden engine failure and loss of propulsion, increasing crash risk. Despite the confirmed failure, Ocean Cadillac (Miami, FL) told me to keep driving the unsafe vehicle and refused all Courtesy Transportation. The Service Manag
... er admitted in writing that the engine failed, the defect can cause loss of propulsion, and the replacement engine is delayed under GM National Control for up to 3 months — yet they still denied me a loaner or rental. His statements contradict the GM documents he attached: • GM Bulletin DCS7222 says Courtesy Transportation CAN be provided during L87 recall downtime and dealers MUST consider loaners/rentals. • GM FAQ N252494002 says Courtesy Transportation IS available for this recall. • GM FAQ lists recall symptoms (check engine light, hesitation, high RPM, reduced power) — NOT battery drain. • GM states: “If the engine fails, the vehicle WILL lose propulsion.” Ocean Cadillac ignored these rules and put me back on the road in a known unsafe vehicle. As an American manufacturer, GM has a responsibility to protect American drivers. Their failure to follow recall procedures placed me, my family, and other motorists at risk. I request NHTSA investigate whether GM/Ocean Cadillac are complying with recall safety requirements and whether customers are being told to drive unsafe vehicles after confirmed failures.Read more

As I was parked with the car running, my 2024 Equinox suddenly shut down and the check engine light came on. I tried to start it, but it didn’t. Someone tried to jump the battery, thinking it was that, but it wasn’t. I had it towed to the dealership and after 2 days they said the Fuel Pump Power Control Module needed to be replaced. Unfortunately, GM did not have the part, but the dealership f
... ound one in another state and we had to wait several days for it to be mailed to them. After a week, once the part was replaced, they tried to start the car and realized that the original issue fried the battery, so they had to replace that as well. I was told that they had seen several customers with the same issue and wouldn’t be surprised if GM eventually issues a recall. They also said they couldn’t be sure the replacement part wouldn’t have the same issue. I did some research and found that GM has had this issue with 2019 - 2024 vehicles. Some customers reported that they were driving and almost got hit by other vehicles when theirs shut down. My car only had 18000 miles when this occurred. I no longer feel safe driving my car.Read more
