While traveling at cruising speed (70 MPH) on Interstate 95 South, the vehicle suffered a catastrophic electrical and mechanical failure, resulting in an immediate engine stall. The vehicle lost all propulsion. Due to the sudden loss of power, the vehicle could not safely clear the active lanes of traffic, coasting to a stop near an exit ramp and dangerously blocking highway traffic.Upon attemptin
... g to restart the vehicle to move it out of harm's way, the vehicle emitted a rapid clicking noise and again stalled out. Because this vehicle utilizes an electronic shift-by-wire system, the total power failure permanently locked the transmission into "Park." The vehicle could not be shifted into "Neutral" or "Drive" to be pushed safely off the road. This total vehicle failure created an immediate, life-threatening hazard for the occupants and surrounding motorists on a high-speed interstate, requiring emergency police intervention to manage traffic until a flatbed tow truck arrived.Read more

My 2018 Honda HR-V had the federally-mandated Safety Recall 24-015 (Fuel Pump Motor replacement) performed on August 16, 2025 at Honda Westborough, Westborough, MA. The vehicle had zero EVAP-related issues or warning lights before that recall work. Approximately 9 months later, in May 2026, a parking alert and check engine light appeared on the dash. I brought the vehicle back to the same dealers
... hip. The technician retrieved diagnostic codes P0455 (large EVAP leak) and P0456 (small EVAP leak), performed a smoke test, and confirmed the leak was coming specifically from the FUEL PUMP MOTOR AREA - the exact same location where the recall work had been performed. The technician's own notes state: "confirmed leak coming from the fuel pump motor area." After replacing the O-ring and lock ring, the leak persisted at the same spot. A new fuel pump assembly (part# 17045-T7X-A02) was ultimately installed to resolve it. Honda Westborough charged me $1,676.61 for this repair, including $870.50 in labor and $724.83 in parts. When I contacted American Honda Customer Relations, they declined to reimburse me, stating the repair was unrelated to the recall. I strongly dispute this. The recall involved opening the fuel pump housing to replace the motor. The subsequent EVAP leak was confirmed by smoke test to originate at that exact location. No such issues existed before the recall. This is a direct causal connection. Other Honda owners on public forums (HR-V Forum, CivicX Forum) have reported identical P0455/P0456 EVAP leaks appearing within days or months of the same recall being performed, suggesting a systemic issue with the recall repair procedure, specifically, improper resealing of the fuel pump housing after motor replacement. I am requesting: 1. An investigation into whether the Recall 24-015 repair procedure is causing downstream EVAP system failures. 2. Honda be required to reimburse owners who incurred repair costs caused by improper recall workmanship.Read more

Pulled off my street to go to work, and the car engine started hesitating, misfiring and losing power. I turned around and was able to get it to a safe location, but had to have it towed to the dealership. Dealership ran codes said it was a bad injector issue and quoted me $2,600 to replace all 4. My Accord just turned 9 and has had all recommended services done. This is my 4th Honda and have n

Since November 2025, this vehicle has experienced repeated loss of power, severe shaking during acceleration, jerking between gear shifts, abnormal humming/grinding noises, and multiple check engine light events. The condition is most noticeable between approximately 40-70 mph and has repeatedly occurred while driving on highways. On multiple occasions, the vehicle struggled to accelerate properly
... and would not respond normally to throttle input. During one occurrence, the vehicle had difficulty accelerating beyond approximately 55 mph while on the expressway. The shaking became severe enough that I feared mechanical failure could occur while driving. I was forced to pull off to the shoulder of the highway because I no longer felt it was safe. The inability to maintain speed and accelerate normally created a safety risk for both myself and surrounding traffic. The vehicle has been returned to the dealership six times for the same recurring issue. Repairs have included replacement of the ECU, catalytic converter, ignition components, fuel injectors, and other engine related components. Despite these repairs, the problem continues to return. The condition has been repoduced and documented by the dealership, and a manufacturer field technician was involved in the most recent diagnosis process. Most recently, after the vehicle was inspected and believed to be repaired, the check engine light illuminated again while the service manager was moving my vehicle to be washed inside of their garage, as I waited (today, June 11th 2026). The recurring loss of power, repeated check engine light events, and inability to permanently correct the defect raise significant safety concerns while operating the vehicle. I have all documentation in my notes, as well as the dealership repair documents.Read more

We purchased this vehicle toward the end of 2024. Within the first 10 months, there were two major recall issues with this vehicle. These recalls were less than 10 months since date of purchase. There is now anot

We called the dealership and was told to bring it in. We brought it in the following week and was told they were not notified yet of a recall. We were told to call back in like 2-3 weeks. We then called back three weeks later and were scheduled to bring the car in for repair. When we arrived at the dealership, we were told "Oh we don't have the parts to fix this. Call back in a few weeks." We waited another three weeks and called back and were scheduled to bring the car in for repair. Now today (June 11, 2026) I went to the dealership and was told once again..."Oh we don't have the parts. Call back in like a month." Now, this recall states that the car should not be driven as the fuel leak is a high chance for a fire. We are desperately trying to have this taken back to the dealership as a lemon car because after we bought this car there have now been three emergent recalls on this car. We paid a lot of money to be able to use this vehicles for our non profit work and over an 18 month period, the car has been out of service for well over 6 months. We lease a 2026 Palisade with only 4k miles. Several weeks ago I was going through a toll plaza, car would not accelerate after slowing down, as if it was in neutral. While driving a 2021 Honda CR-V Hybrid, the vehicle has intermittently and without warning lost the ability to accelerate. When this occurs, depressing the accelerator pedal — even fully to the floor — produces no response, and the vehicle simply coasts and loses speed. This has happened on three occasions — January 13, 2025; December 16, 2025; and June 10, 2026 — including at highway spee
... d. Each occurrence is documented by a Honda dealer service invoice. In each instance, the check engine light began flashing immediately before the loss of power. The only way to restore normal operation was to bring the vehicle to a stop, turn the ignition off, wait, and restart. Restarting restores power but also clears the check engine light and any stored diagnostic trouble codes. Safety risk: Losing motive power without warning, particularly at highway speed or while merging in traffic, creates a serious risk of a rear-end or loss-of-control collision, because the vehicle cannot accelerate out of danger. The affected systems appear to be the power train and hybrid propulsion system, possibly originating from the engine given the flashing check engine light, which typically indicates an active engine misfire. The exact failed component has not been confirmed (UNKNOWN). The problem has NOT been reproduced or confirmed by the dealer during inspection. The vehicle was taken to an authorized Honda dealer for each occurrence, but because restarting the vehicle clears the fault codes, the dealer found no stored codes and could not duplicate the failure during a test drive. The vehicle has not been inspected by the manufacturer, police, or insurance. The vehicle is available for inspection upon request. The vehicle has experienced repeated failures related to the diesel emissions system despite multiple repair attempts. The primary symptom is an illuminated check engine light accompanied by diagnostic trouble code P2459 (Diesel Particulate Filter Regeneration Frequency). The vehicle frequently enters regeneration cycles and continues to exhibit the same symptoms after major emissions system repai
... rs. The emissions system components, including the Diesel Particulate Filter (DPF) and related DEF system components, were previously replaced by an authorized dealer. Following those repairs, the same warning light and fault code returned. The vehicle was subsequently inspected by another authorized dealer, which was unable to identify a definitive cause of the recurring condition. The check engine light was cleared, but it returned less than a week later with the same symptoms. The failure creates a safety concern because the vehicle may enter reduced-power or limp-home operating modes without warning, potentially limiting acceleration and the ability to maintain traffic speed. This is especially concerning when merging onto highways, crossing intersections, towing loads, or operating in heavy traffic. The recurring nature of the failure has reduced confidence in the reliability and safe operation of the vehicle. The condition has been reproduced multiple times and has been documented by authorized service facilities. Warning lamps and fault codes have repeatedly reappeared following repairs. The vehicle and emissions system components remain available for inspection upon request.Read more

OnXXX, as we approached what I call the 6 o’clock position of a round-about in [XXX], we had braked almost to a full stop (0-5 mph) to wait on the car ahead of us to enter the round-about when we felt increasing movement. Despite braking, to wait on that car, our car with increasing speed started shoving that car into the circle. Fortunately, he was able to drive away from us, but that car had
... been damaged on the back and right side. With that obstacle out of the way, our car rapidly and exponentially increased speed - all while we were fully braking. Two witnesses said they saw our brake lights as we sped along. We hit the curb as we approached the 12 o’clock exit, and that turned us so that we crossed both lanes of the road and headed towards the far sidewalk and storefronts. When we hit that curb it pushed us diagonally to face the street again. Hard to estimate but we must have been doing 50 mph when we hit a very large brick planter and we finally were wedged to a stop when we slammed in between two vehicles parked parallel to the sidewalk. The motor ran for a few seconds after we stopped, and finally quit on its own. We KNOW the brake was applied the whole time; there were no visual or audible alarms from the car as we entered “UNINTENDED ACCELERATION”. Despite the incredibly strong impact (our car has been deemed totaled) the front air bags did not deploy. Only the driver’s side curtain deployed. The bumper of the parked transport van on our front left was damaged, and the small SUV on our front right was deemed totaled. No people were in those two parked cars; the first car was hit at the initial slower speed so those people were not injured. By some grace, no pedestrians were in our path or they would surely have been killed. While driving my 2024 Jeep Wrangler 4xe on the Garden State Parkway and approaching traffic on the Driscoll Bridge, a turtle warning icon suddenly appeared on the dashboard, indicating limited vehicle performance. Moments later, the dashboard displayed a message instructing me to "Shift to Park." Immediately after this warning, the vehicle suffered a complete hybrid/electrical system failure and
... lost all motive power. The vehicle became completely unresponsive, and I was unable to accelerate, move the vehicle, or restart it. The vehicle came to a stop in an active travel lane of the Garden State Parkway on the Driscoll Bridge, leaving me stranded in live highway traffic. This created an immediate and serious safety hazard, as vehicles were approaching from behind and I was exposed to the risk of a high-speed rear-end collision. The vehicle could not be moved under its own power and had to be removed from the travel lane and towed to a nearby parking lot. Roadside assistance later performed a battery reset, which temporarily restored vehicle operation and allowed me to drive the vehicle home. No permanent repair was performed, and the cause of the failure remains unresolved. The vehicle currently has multiple open safety recalls, including Recall 68C involving the high-voltage battery system. Stellantis has also recently announced an additional recall involving electric hydraulic power steering pump wiring and fire risk. The primary defect is the sudden loss of propulsion and complete electrical/hybrid system shutdown while driving, resulting in the vehicle becoming disabled in active highway traffic without warning. This defect created an immediate risk of a serious crash and could have resulted in severe injury or death.Read more

I have experienced a severe hesitation or delay in acceleration when accelerating from a stop or near-stop condition. While the issue has only occurred a few times over the past several months, on multiple occasions it happened while merging into traffic and nearly resulted in an accident. Toyota has issued Technical Service Bulletin (TSB-0032-26) addressing this condition. However, when I brough
... t my vehicle to the dealership, Toyota documented that they were unable to reproduce or confirm the hesitation, and no repair was performed. This defect presents a significant safety risk. Unexpected loss of acceleration during traffic merges, intersections, or other situations requiring immediate power can place drivers and passengers in danger and could lead to serious injury or death. Given the safety implications and the fact that the remedy appears to be a software update requiring less than an hour to perform, I believe this issue warrants a formal safety recall rather than a voluntary Technical Service Bulletin. I urge NHTSA to investigate this matter and require Toyota to provide a recall remedy for all affected vehicles.Read more

Hyundai Motors America has failed to comply with its statutory obligations in conducting the recall campaign 25V796, and is instead engaging in behavior substantially similar to that which has been previously censured by NHTSA. My Sonata suffered irreparable damage related to this campaign as confirmed by HMA. 49 U.S.C. § 30120(a)(1)(A) allows HMA to choose one of three remedies: repair, replace
... ment, or refund. However, they 1) say it cannot be safely repaired, 2) will not replace it, and 3) are not offering a repurchase as required by statute, i.e. "by refunding the purchase price, less a reasonable allowance for depreciation." Instead, Hyundai's case manager Aditya informed me on 5/29/2026 at 12:57 PM that they will make a repurchase offer "based on the Kelly Blue Book value." I protested and cited the statute, but he maintained that Hyundai has their process and subsequently gave an unconscionably low offer that was not based on any reasonable measure of depreciation. I insisted on a reasonable, defensible calculation. I proposed 1) using the state formula for calculating reasonable depreciation as applied in both Texas and California (HMA and my home states), or 2) using actual market values (of which I have tangible data) to determine the actual market loss in value. Hyundai has rejected both of these reasonable proposals in favor of an arbitrary formula that would leave me with thousands of dollars in lost value. On June 9, 2026, when I pressed Executive Case Manager Jenifer for an explanation of their proposed depreciation formula, she only stated that "it’s a standard calculation across the board" and that it applies "regardless of state" and they "will not change it." The NHTSA discussed this behavior in its Final Order and Determination against Fiat in Federal Register Vol. 45 No. 7, and endorsed a "reasonable" model similar to my proposal #2 above. It found Fiat non-compliant for its behavior which had a nearly similar impact.Read more

I am reporting a critical safety hazard involving my 2021 Nissan Leaf S Plus (approx. 117,000 miles). Since January 2025, the vehicle has exhibited dangerous battery degradation and voltage sag during operation. On my 30-mile highway commute, while driving at normal highway speeds—especially on inclines—the vehicle would suddenly display a low-power warning, and the remaining battery percentag
... e would instantly crash. This caused a sudden, dangerous loss of propulsion in high-speed traffic, creating severe crash risks. I primarily charged at home and rarely used DC fast charging. On May 22, 2026, while actively driving, the dashboard flashed: "Service EV system unable to restart after power off." I pulled over, and upon turning the car off, it completely bricked. The auxiliary electronics power up, but the vehicle permanently refuses to enter "READY" (Drive) mode due to a high-voltage safety lock. The vehicle was towed to an authorized Nissan dealer. The certified technician's diagnostic report confirmed that the high-voltage traction battery has completely failed and is internally damaged. Crucially, this vehicle is currently under an active, unremedied safety recall (NHTSA Recall 25V655 / Nissan R25C8) due to excessive lithium deposits that cause severe battery degradation, sudden power loss, and fire risks. The physical failure of my battery is a direct manifestation of this exact manufacturing defect. Nissan’s software-only "interim remedy" did not fix the hardware defect; it simply bricked my car after exposing my family to highway propulsion loss. Nissan must be held legally responsible for replacing these defective, recalled battery packs regardless of mileage, as they present a severe, systemic risk of catastrophic failure and traffic accidents.Read more

My 2018 Jeep Compass, with approximately 107,000 miles, has developed symptoms consistent with the oil pump/oil pressure defects reported in other recalled Jeep Compass vehicles. The vehicle frequently struggles to start, runs rough with noticeable vibration, and has displayed an oil pressure warning light. In addition, the vehicle frequently stalls or suddenly loses power/slows while driving. The
... stalling and loss of power have created a serious safety concern because they can occur while the vehicle is in motion, increasing the risk of a crash, especially in traffic or at highway speeds. These symptoms have become recurrent and appear to be worsening over time. The vehicle was evaluated by a Jeep dealership. The dealer scanned the vehicle but did not provide a repair for the underlying issue and instead advised us to continue adding coolant as needed. Despite this, the vehicle continues to exhibit the same dangerous symptoms, including the oil pressure warning light and stalling while driving. I am aware that certain 2018 Jeep Compass vehicles were recalled for oil pump-related defects; however, my vehicle was not included in the recall population. Because my vehicle is experiencing similar symptoms, I believe additional 2018 Jeep Compass vehicles outside the current VIN range may be affected. I am submitting this complaint so that NHTSA can investigate whether the scope of the recall should be expanded. The vehicle is available for inspection upon request.Read more

For the past year, I have been dealing with a recurring check engine light on my Ford Fusion related to Diagnostic Trouble Code P0301 – Cylinder 1 Misfire. Despite multiple repair attempts by qualified mechanics, the issue continues to return, leaving me with mounting repair bills, lost confidence in my vehicle, and serious concerns about its reliability and safety. Every time the check engine l
... ight has appeared, I have taken my vehicle in for diagnosis and repair. Each time, I was assured the problem had been fixed. However, the repair would only last anywhere from a week to a month before the exact same code would return. I have now experienced this issue on August 8 2025, January 9 2026, April 20 2026, May 19 2026, and June 1 2026. I have paid to address the same problem, along with related repairs, five separate times. Even more concerning, my vehicle has broken down and left me stranded on the side of the road three out of those five occurrences. No vehicle owner should have to repeatedly face the same unresolved issue after spending significant money on repairs and maintenance. After conducting extensive research, I discovered that I am far from alone. I have found numerous reports from other Ford Fusion owners experiencing the exact same Cylinder 1 misfire issue, many of whom have also been unable to find a permanent solution despite repeated repairs. This appears to be a widespread problem affecting many customers, not an isolated incident. As a Ford customer, I expected a dependable vehicle that I could trust to safely get me where I need to go. Instead, I have spent the last year dealing with recurring failures, unexpected expenses, and the constant fear of being stranded again. When so many owners are reporting the same defect with no lasting repair available, it raises a serious question: Why has this issue not been investigated for a recall or other manufacturer supported corrective action? I urge Ford to thoroughly investigate this recurring issue.Read more

10 day roundtrip from [XXX] to [XXX]. 5 incidents of catastrophic engine failure on interstates. Truck started to jerk, sputter, lost power at an alarming rate. No engine light until failure ocurred. Very dangerous with heavy holiday traffic. Lives were in danger, us and other vehicles around us. No one expects travel to go from 70 mph,to 50, 30, 15 in such a short period of time. Pulled over to "
... investigate" code failures we deemed as o2 sensors. Afterwards the truck started back up, no engine lights and drove as normal. Started using premium gasoline, hoping it would help. Started driving in right lane as much as possible but had a failure ocurr right in front of on ramp as well at interstate speeds. Had another family following us so they also witnessed and pulled over all incidents. Brought into Ford 7/18/24 upon return home. Paid $125 inspection fee, concern not present, come back if happens again. Makes no sense that codes not present to pull. 12/13/2024 engine light on, catalytic converter replaced. 06/19/25 truck had catastrophic failure returning from work on hwy. XXX on [XXX], just two miles from leaving home another failure with no warning lights in heavy Friday traffic in construction zone. Couldn't clear the traffic lane completely. Very scary. Waited for clearing and limped it to a nearby off ramp. Same jerking, loss of power. Vehicle towed to shop for 2 weeks, testing, fuel pump replaced. Picked up 07/07/25. Next day received recall notice 25S75 in the mail 7/08/25. Everything made sense. 1/24/26 - Battery failed inspection @ 40% during oil change. Replaced battery ourselves from Costco. 04/23/26- Warning light, P0420 pulled, shop recommended engine tune -up. Paid $175 inspection fee + $671.20 tune up. 5/29/26 engine light on, P0420 pulled, recommend catalytic convertor replaced but not under factory warranty anymore so wants us to pay $4000 bill. Escalated up through Ford. As of 6/5/26 - Ford will pay half. I refuse. Should be covered by recall. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF IRead more

The systems affected with failure notifications were Electric Parking Brake, TPMS, Emissions, Brake, Brake Hold, Power Steering, Vehicle Stability Assist, Hill Start Assist, Adaptive Cruise (including regular cruise), Collision Mitigation/Avoidance, Road Departure Mitigation. The stored codes in the vehicle that I was able to read were P0300, P0303, P0304. This failure bears a striking resemblance
... to that of TSB 21-010, directed at Keihin manufactured fuel injectors, PN: 16010-RLV-315, which was later replaced with updated ...-305 after the failure was determined, and the vehicles were given a 10 year 150,000 mile warranty. The CRV had injector PN: 16010-5PA-305, also manufactured by Keihin, which was also later revised to 06160-..., but the CRV was not given the same warranty extension, suggesting Honda acknowledged the same defect pattern across both injector families but chose not to extend equivalent coverage to the CRV. 21-010 also details that debris from the high pressure pump may contribute, and the CRV experienced the same fuel pump recall for low density impellers that these vehicles did. Honda is also obligated under TSB 98-081 to perform emissions services due to OBD inadequacy, which this vehicle experienced. Misfires were felt for months with no CEL. The Norm Reeves Honda parts catalog classifies injectors as emissions components, implying they should be covered. However, a Honda dealer in CA, mandated by CARB and EPA, refused to do the repairs or make any goodwill attempt on my behalf. I then reached out to American Honda, who delayed assigning a case manager as long as possible, then required phone contact after initially accepting alternate communication methods, then assigned a case manager whose hours conflicted with my stated availability, then assigned a second who has not answered the phone. I am still awaiting a callback. The vehicle has been immobile and in limp mode for the past two weeks while Honda's callback process remains unresolved.Read more
