2026 Hyundai Palisade Calligraphy, VIN [XXX] , purchased March 6, 2026 with under 2,200 miles at time of incident. SAFETY INCIDENT: While operating under adaptive cruise control at highway speed, the vehicle experienced sudden unintended acceleration followed by severely delayed braking response. At least three trailing vehicles were forced into emergency evasive action to avoid a multi-vehicle co
... llision. Family members were present. This event represents a direct and immediate threat to the safety of vehicle occupants and the public. The root cause has not been definitively identified or resolved. The authorized dealership confirmed CAN-Bus resistance irregularities — 160 ohms versus the designed 120-ohm specification — governing acceleration, braking, and all driver assist systems. Prior symptoms included flickering interior lighting. An initial module replacement failed. Repair has escalated to full wiring harness replacement with no parts availability and no confirmed timeline. The safety event remains undiagnosed and unresolved. The vehicle carries three simultaneous active safety recalls — 293, 296, and 297 — all involving electrical and wiring systems on the Calligraphy trim exclusively. None have permanent remedies. The vehicle has been in dealer custody since March 21, 2026 and is available for inspection. Hyundai Motor America active case — No. XXX. This vehicle is not safe to operate. I saw previous warning when I pulled my credit report on Experian, but I am not seeing any recalls currently. My driver side taillight is malfunctioning. I am reporting a safety concern involving a 2019 Chevrolet Malibu. The vehicle was previously involved in accidents prior to my purchase and had known mechanical issues, including a gear shifting problem that was not properly repaired. In February 2024, while the vehicle was insured under Ocean Harbor Casualty Insurance Company, it sustained a significant front-end collision impacting the driver
... side. The impact was severe enough to cause structural damage, including a dent to the vehicle frame. Despite the severity of the collision, the airbags did not deploy at all. Based on the extent of the damage, airbag deployment would reasonably be expected. Prior to this incident, the vehicle had ongoing issues following repairs, including system errors and concerns that repairs may not have been properly completed by a certified facility. This raises concern that the airbag system may have been defective, improperly repaired, or compromised prior to the accident. I was not informed of any airbag or safety-related defects at the time of purchase from HGreg. I am submitting this complaint due to concerns of a potential safety defect involving failure of the airbag system during a serious collision.Read more

There are two issues with the headlights as it relates to the instrument panel and controls. First, the symbol on the instrument panel is the same for both headlights and running lights. This means at dusk, I cannot know whether my auto-headlights have turned or if I need to override the auto-feature to turn them on manually. Second, the headlight controls are buried in a menu, hidden behind the
... steering wheel. The "quick menu" is inconvenient, but the fact that it is hidden bedding the steering wheel means that I have to lean forward and around to fiddle with it, which has led to me drifting into another lane as I am distracted. From the driver's perspective, the "quick menus" are all hidden behind the wheel, and the control panel is just far enough out of reach that the driver must adjust and lean forward to reach for any controls not on the steering column.Read more

On November 28, 2025, my 2025 Volvo EX90 displayed a headlight warning message on the dashboard and the headlights failed to function properly while parked and after starting the engine. The defect temporarily resolved but recurred on December 5, 2025. A headlight defect that is not immediately apparent to the driver poses a serious safety risk before or during nighttime driving. The symptoms are
... consistent with NHTSA Safety Recall No. 25V-071 (Volvo internal reference R10298), which covers 2025 EX90 vehicles and involves unexpected headlight malfunction. I request that NHTSA investigate whether my vehicle's defect is related to Recall No. 25V-071 and whether the recall remedy has been properly implemented. If unrelated to Recall No. 25V-071, I request that NHTSA investigate this as a separate unreported safety defect. Other 2025 EX90 owners may be experiencing the same issue without reporting it. Volvo has contacted me directly on multiple occasions to schedule a central computer replacement on my vehicle. I understand this replacement is being offered to all 2025 EX90 owners. I request that NHTSA investigate whether this widespread hardware replacement is related to the safety defect I experienced and whether it should have been conducted as a formal safety recall. When brought in for repair on December 13, 2025, the service department advised parts were on indefinite backorder with no estimated delivery date. The vehicle remained at the service center for 47 days while Volvo was unable to supply the necessary parts. I request that NHTSA evaluate whether this represents an adequate response to a safety-critical defect.Read more

Vehicle is subject to two active safety recalls, both listed as “remedy not available.” One recall involves a potential engine defect related to manufacturing debris, which may result in engine damage and sudden loss of motive power. Manufacturer recall documentation indicates this condition may increase the risk of a stall, particularly under sustained load such as towing or highway operatio
... n. A second recall involves reverse lamp failure, which reduces rear visibility and may increase the risk of a crash when backing. No failure event has occurred. However, both safety defects remain unresolved, and no repair or timeline for remedy has been provided. The vehicle is used for towing and hauling, which requires sustained engine load. Due to the risk of engine stall under load and the absence of manufacturer guidance regarding safe operation, use of the vehicle for towing and long-distance travel has been limited. These conditions have been confirmed through manufacturer recall notices. The vehicle has not been repaired because a remedy is not yet available. No warning lights or symptoms have been observed. The concern is based on the nature of the defects described in the recall and the lack of available repair or operational guidance.Read more

Tesla designs a single piece headlight with no serviceable parts. This includes headlights, high beams, and namely daytime running lights. All of these components are on an LED strip that overheats and un-solders due to this high heat, which causes premature failure. Some owners have reported as little as 3 months of life before failing. The daytime running lights are essential for safety driving
... in low light situations. These daytime running lights are seemingly built to fail. The lights dim then eventually no longer work. Tesla's solution is to have the customer purchase a brand new headlight at $2,500. My vehicle is 2018 Tesla s with 43,400 miles and always kept in carnage. Tesla should either goodwill swap out these headlights for a non-built to fail component, or offer a repair for a quality LED strip with proper heat distribution. Without daytime running lights safety is a major concern on foggy, rainy, and low light situations. I have attached an image showing how the failure begins on top then slowly goes all the way down to the side of the headlight which fails later,.Read more

Component: Exterior Lighting / Headlamps Vehicle: 2021 Genesis GV80 VIN: [XXX] Mileage at Failure: 77,000 Complaint: The front left headlamp on my 2021 Genesis GV80 failed completely after visible moisture and condensation accumulated inside the lamp housing for approximately one week. Genesis Customer Care initially informed me that if the dealer confirmed the failure was consistent with the TSB, the repair would be covered under goodwill. Incident date: Mar 12, 2026
I was driving at night when I got a headlight system error message on my instrument panel and I noticed my driver side headlight was not on. A few seconds later the light came back on and a minute or so later it went off again and came back on and I got the error message again. It came back on and has worked pretty much since but I have seen it flickering at night on a few occasions. The next da
... y when it was light out I looked at both headlights and noticed a lot I mean a lot of condensation inside the headlight assembly. I know some fogging and some light condensation is normal on these LED lights but the amount inside the driver side headlight is not normal. And I was lucky enough to have the error and light go off and on in a fairly well lit area so thankfully I was not in an accident or driving on road that’s not well lit. There’s clearly a defect in the sealing of the headlight that’s allowing that much water in. And not having a headlight working because of an error caused by water that can disable my visibility especially on the driver side to due to the headlight not being sealed properly. There is not physical damage like a crack or anything in the housing of the headlight. And since the whole headlight needs to be replaced since it’s LED that’s $2200 before labor. And there didn’t appear to be anything wrong with the headlight no condensation when I looked at it on the car lot when I bought it on 3-4-26. I tried to have my Nissan dealer repair/replace it using the Endurance Apex Unlimited warranty I bought when I purchased the car but they won’t cover it due to a faulty seal. I’ve seen a couple post about the same side headlight on others 2019 Maxima’s with the same issue so this doesn’t appear to be a one off. And I would think headlights going out from moisture and condensation causing errors to the electric contacts controlling the headlights would be a serious safety issue that could potentially cause bodily harm or even death from an accident.Read more
