The issue has been confirmed by an authorized Mazda dealer, which recorded fault logs related to the headlight system. The vehicle has been inspected multiple times. Repairs performed include replacement of the headlamp wiring harness and replacement of the entire headlamp assembly, but the malfunction continues and has increased in frequency over time. The problem first appeared intermittently over a year ago, progressed to weekly, and now occurs daily. The failure occurs under normal driving conditions, including at very low speeds (approximately 5 mph) while exiting a parking lot. One documented instance occurred with the engine still cold (blue low-engine-temperature indicator illuminated), indicating no aggressive driving. Several additional occurrences happened below 35 mph with no harsh steering or acceleration. The faulty wiring jumpers are available for inspection upon request. Safety was put at risk because the turn signals would not reliably signal driver intent. I am an independent mechanic, and discovered the issue while performing other work in May 2025. No one else has investigated the vehicle or component. The high-voltage battery was replaced in April 2024, yet the system has failed again with melted wiring less than 20 months later. Despite a previous recall performed in August 2024 (Campaign 9B4 - BMS Software Update for 12V Battery Over Voltage), the electrical system remains unstable. The headlights, and tail lights also go off and are unresponsive while driving. We've also experienced the vehicle not going in reverse or drive and had to return it multiple times for this as well. These incidents are very serious safety hazards to the driver of the vehicle, other drivers and pedestrians. On several occasions we've addressed these problems with the dealership and still no resolve. However, we are paying very expensive labor fees only to return it with the same problems repeatedly and no explanation as to why the problem keeps recurring. We have receipts, photos and videos of this problem because the dealership kept stating they weren't seeing the issues we were seeing. However, after we presented the evidence and confirmation we have, they stated that they would conduct a diagnostic test and take a look, but the issues still have not been resolved after several times picking it up after they stated that they had changed the instrumental cluster. The last time after we returned it they discovered they had put the wrong part in the vehicle, however the same issues occurred with that part as well which is the reason we returned it to be looked at again. They changed it and still the same issue has repeated again. This is concerning and alarming that we were trusting them to work on the vehicle and driving the vehicle home thinking it is fixed and then discovering that they had put a wrong part in the car.Read more

During the first service visit to an authorized Jeep dealership, I was informed that the Body Control Module (BCM) was not responding, and that multiple vehicle systems showed inactive status and communication failures. The dealership stated they could not identify the root cause. Instead of repairing the issue, the system faults and communication errors were cleared/reset, the battery was replaced “as a precaution,” and the vehicle was released without a confirmed repair. The next day, the same failure occurred again while driving on the highway. The windshield wipers activated randomly, the screen displayed a red warning stripe, multiple system failure messages appeared, and the emergency braking system was disabled at highway speed. This created an extremely dangerous situation. The vehicle behaved unpredictably, critical safety systems were unavailable, and I could have been involved in a serious or fatal accident, putting my life and the safety of others at risk. Thankfully, my children were not in the vehicle at that time. Due to the severity of the malfunction, the vehicle was unsafe to operate and required towing back to the dealership. The issue remains unresolved. This appears to involve a serious defect with the Body Control Module (BCM), vehicle communication systems, and possible wiring or grounding faults, which directly affect vehicle safety and warrant investigation.Read more

After I lodged a complaint, the General Manager suggested I seek service elsewhere. This recommendation was made after I responded to an email stating that, following the completion of repairs to the headliner, third brake light, and brakes/rotors, I would no longer be utilizing Crain's services. The headliner sustained water damage due to improper installation by your technician, and fingerprints were also evident. I firmly believe in transparent communication regarding service progress, which was notably absent in this instance. Both the General Manager and Service Manager deferred responsibility, instructing me to contact GM directly, only for GM to inform me that direct contact was inappropriate. This resulted in a continuous cycle of unaddressed concerns with the service manager. Currently, I am operating an $80,000 vehicle that has incurred damage from a technician and exhibits a ticking engine. Furthermore, the driver's side mirror, which was previously replaced due to shaking at highway speeds, is now exhibiting the same issue, despite a technician's assertion that this is normal. Yes. The vehicle was taken immediately to an independent service center following the incident and has remained there due to the severity of the malfunction. The service center confirmed abnormal electrical behavior and initiated further diagnostic evaluation. 4. The vehicle has been inspected by an independent service center. The manufacturer has not yet conducted a direct inspection. No police inspection was performed, as no collision occurred. 5. No warning symptoms were observed prior to the failure. The malfunction occurred suddenly and without warning on August 8. This issue affects multiple systems simultaneously, including: •Reverse (backup) lights •Tail lights •Backup camera License plate lighting The reverse lights and backup camera sometimes do not function at night, and the tail lights may flicker or go out. Safety Incident / Near Miss: At night, while backing up, I nearly struck a pedestrian because: •I could not clearly see behind the vehicle due to the backup camera and reverse lights not functioning, and •The pedestrian did not realize the vehicle was backing up because the reverse lights were not illuminated. No injury occurred, but this was a serious near-miss involving a pedestrian at night. Additional Information: I have since learned that Hyundai has issued a Technical Service Bulletin (TSB) for 2015–2016 Genesis vehicles related to damage or failure of the trunk wiring harness, which can cause loss of rear lighting and backup camera function. My vehicle’s symptoms match the conditions described in the bulletin. This issue appears to be a design-related wiring failure caused by normal trunk operation, not misuse or modification. ... ect described in NHTSA Recall 21V-035 related to the MCU/eMMC failure. I reported this safety issue to Tesla Service and provided photo evidence of the screen rebooting while driving. Tesla has refused to address this as a recall or safety repair. Tesla claims the recall was previously completed under a prior owner but refuses to provide any documentation proving that the recall repair was actually performed, citing prior owner privacy. Tesla has further stated that diagnosis of this active safety defect would be customer-paid due to the basic warranty being expired and has indicated that the only path forward is a paid infotainment upgrade. Tesla is effectively requiring payment to diagnose and resolve a known safety defect. As the safety defect is actively occurring while driving and Tesla has marked the recall as complete without providing proof or correcting the issue, I am requesting NHTSA review whether Tesla is improperly closing out recalls without ensuring the defect has been remedied and whether charging customers to diagnose known safety defects is appropriate. After entering my VIN into NHTSA’s recall lookup, NHTSA shows Recall 21V-035 (MCU/eMMC failure) as Recall Incomplete for my vehicle. This directly contradicts Tesla’s claim that the recall was previously completed. The contact stated that force was used to maneuver the steering wheel. The contact was able to coast to the side of the road. There was an unknown message displayed on the instrument panel. The vehicle failed to restart. The vehicle was towed back to the dealer, where it was diagnosed, and determined that an unknown cable was not attached when the engine was replaced. The dealer tested the battery, and the battery failed the test. The battery was replaced. The contact stated that the infotainment screen displayed the message "Passenger Side Window Open" while the passenger side window was closed. The contact stated that when the passenger’s side window was rolled up, the window independently rolled down a few inches. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. Useless at 45 MPH turn on, to many back roads where you need them are under 45 mhp. 2.) high beams go off when they sense the oncoming traffic lights, but alomost 100% of the time they fail to realize oncoming traffic terribly blinding oncoming traffic. 3.) ... ssue diagnosed and is still not repaired and TSB states for dealerships not to do more work to fix as this is a computer program issue and not a component issue, nonetheless the camera is still not functioning. The second area of failure is the transmission shift module. This module determines when the vehicle has been shifted out of park into R, N or D. The module has been replaced 2 times in 55,000 miles. When the module goes bad the vehicle gives a warning that the ability to shift will be lost. Finally the last safety issue that has been largely reported is the failure of the Telematics Onstar Module which runs the onboard communications for Onstar, Maps, GPS, and other systems that use the GPS signal. This means that any hands free technology tied to these systems no longer work leading to legal issues in most states that have hands free requirements. GM is charging around $1,000 to replace the Onstar module and it is taking 6 months or longer to even access the part if at all. Vehicle has also had running light/headlight, indicator light go out with no way to change a bulb, This requires an entire LED assembly, this issue is intermittent and may reset after the vehicle has been fully powered down, but creates a safety issue while driving with lights and blinker being in operable.Read more

I took my truck to the dealership and they told me I would have to get an appointment for them to take digital images of the headlamps to send to Ford Motor. I left the dealership the second time with only one headlamp working. 24 hours later, Ford Motor told the dealership they believed it was caused by an outside collision and would not be covered by the bumper to bumper warranty. The dealership offered to replace at cost and suggested I contact the State Attorney General’s office and try to mediate Ford’s decision. I made an appointment to replace both headlamps after the weekend at their earliest opening. I have kept the faulty headlamps. I believe Ford Motor is trying to mitigate cost of a faulty product at the expense of public safety. The States Attorney General attempted to mediate. However, a Consumer Affairs Legal Analyst from Ford wrote that the warranty was denied because the headlamp was damaged in a car accident. It was not.Read more
