Despite being aware of this widespread failure, Volkswagen continues to install the same defective headlight assemblies. Continuing to sell vehicles with a known defective and safety-critical component is unacceptable. Headlights are not a luxury feature—they are a basic safety requirement. There needs to be an immediate recall or manufacturer-covered repair for these headlights. It is only the low beams, but every couple months, the passenger side goes out. Then within 3 weeks to 2 months later the driver's side goes out. This has been happening since a month after I bought the car in 2021. This does not matter if they are changed individually when one of them goes out or if they are both changed at the same time. Occasionally, they will both just go out at the same time.

Dealer said this is a common issue and requires module replacement to prevent requiring a whole headlight assembly as BMW does not sell the fiber optic cable separately. During the two years I have owned the vehicle, the FRM has required four separate reflashes due to recurring malfunctions. The failure causes loss of proper power window operation, including the passenger window reversing or rolling down unexpectedly, and loss or malfunction of other
... body electrical functions controlled by the FRM. Headlamps will not turn off, resulting in a dead battery and electrical/intermittent dash light surges. The FRM failed again after a software reflash, which is a documented trigger for FRM internal memory corruption. MINI/BMW issued and Extended Limited Warranty for FRM failure, acknowledging this defect; however, I received the official notice after the warranty period had already expired, leaving no opportunity to repair the defect while covered. This failure affects safe operation of the vehicle, including the inability to properly secure windows and maintain normal electrical functionality. The issue is not due to wear or misuse, but an internal electronic defect recognized by the manufacturer. The repeated nature of the failure and reliance on reflashing demonstrate the defect was never fully resolved. I am reporting this issue to document the repeated failures, late manufacturer notification, and ongoing safety-related electrical system issues. I purchased the vehicle with 86,000 miles. It now has 100,000, well below the criteria for the LW. In the 14,000 miles and two years I have owned and driven it, it has required four FRM reflashings. Had I known about the Extended Limited Warranty, I would have had BMW repair it under said warranty. I am reporting an unresolved safety defect involving the 12.3-inch instrument panel display on my Toyota RAV4 Prime, which is the subject of NHTSA Recall 25V595 (Manufacturer Recall 25TB08). The defect affects the LED digital display that provides critical driver information, including vehicle speed, warning indicators, and operating status. Initially, the display would intermittently go blank at
... startup and sometimes return during the drive. However, the condition has progressively worsened. The instrument panel display has now been completely blank and unresponsive for over three consecutive days and has not returned at all, including after multiple drive cycles. As a result, the vehicle cannot display critical operating information or malfunction indicators. This issue was first reported to an authorized Toyota dealer in August, prior to the recall being issued. At that time, I was informed that I would be responsible for replacing the entire display/instrument system at my own expense. On September 11, 2025, Toyota issued Recall 25V595 acknowledging that the instrument panel display may be blank at startup and stating that operating the vehicle in this condition reduces the ability to see malfunction indicators and could increase the risk of a crash or injury. The vehicle was most recently serviced on December 19, at which time the dealer confirmed that the issue is related to the recall and that no remedy is currently available. The recall remains incomplete, and Toyota has not provided a repair, interim solution, loaner vehicle, or timeline for resolution. Because the instrument panel display is completely nonfunctional, the vehicle cannot be safely operated. The lack of a remedy for a safety-related defect affecting a core vehicle safety system has exceeded a reasonable timeframe. I am requesting NHTSA review and intervention.Read more

I have been out of town caring for my mother who had cancer and recently passed on 12/18/25. I was not previously aware of the recall. From my reading online, due to this being a safety recall, it should still be eligible for recall repairs up to 15 years of the car's age. The dealership also told me I could just put on my brights manually. This blinds other drivers on the road and then they put on their brights driving towards me, which decreases my ability to see while driving. the Toyota engagement center dropped the ball after 3 months of trying to report how dangerous this situation is. I am now reaching out to your department because of serious safety issues. I did not buy a car to be placed in a dangerous situation where I can't see to drive at night up.
