I bought the 2015 ford fusion for my son as a graduation present as he headed off to college on his own. The car started misfiring upon cold start for about 5-10 seconds. Eventually the car started misfiring while driving. He frequently had to pull over on the side of the road while driving to make it to his destination. I had him take the car to a ford dealership to be inspected. To my surprise,
... Ford Parks of Wesley Chapel told me that the 2015 Ford Fusion 1.5L ecoboost has a known issue with coolant leaking into the engine causing misfiring and eventually engine failure. They offered no assistance in fixing the problem. They told me the cost to replace the engine was over 13,000.00.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The Auto Start/Stop function only works intermittently on our 2021 Honda Odyssey. Occasionally, when the Auto Start/Stop function does shut off the engine when the vehicle is at a complete stop, the engine fails to restart on its own. The vehicle is available for inspection upon request. How was
... your safety or the safety of others put at risk? This malfunction occurs intermittently and without warning when the vehicle is at a complete stop in traffic. The vehicle cannot be driven under its own power in this condition, often leaving it disabled in a traffic lane with other vehicles stuck behind and/or passing by at normal travel speeds. As such, it endangers both the occupants of my vehicle and the other vehicles in the vicinity. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. My local Honda dealership acknowledged the problem and completed a PGM-FI software update. However, this update did not correct the issue, and Honda refuses to take financial responsibility for the repairs necessary to correct the problem. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes. The following timeline details my interactions with my local Honda dealership regarding this issue: 4/13/23 - The dealership inspected the vehicle and claimed the Auto Start/Stop system was "operating as designed"; 9/6/24 - The dealership completed a PGM-FI update as directed by a Honda product update campaign; 2/12/26 - The dealership inspected the vehicle, suggested battery replacement (which tested good on 3/3/26), and if still not corrected, advised replacement of the starter at my expense. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The problem has occurred without notice since purchase on 3/11/21.Read more

On May 21st , 2026, while waiting in line to make a right turn (positioned 3rd in queue), the vehicles ahead moved forward. As I transitioned from the brake to the gas pedal to follow, the vehicle experienced an instantaneous engine stall and total electrical failure (NOT an i-Stop event). Pressing the gas caused the vehicle to slowly roll backward. When I immediately tried to reapply the brakes,
... the brake pedal became extremely stiff and nearly impossible to press down, failing to hold the vehicle effectively. The driver behind me began honking aggressively due to the imminent collision risk. I attempted to lower the windows to signal the other driver, but all electronic controls were completely dead. I had to quickly shift the gear into Park (P) and hit the start button to successfully restart the engine, which temporarily restored functions.Read more

I filed a complaint when the first engine seized. It's a miracle we got off the road safely that day. While it was at the dealership, the engine was replaced with a used engine and covered by my extended warranty from Vroom. I believe the cost was approximately $14,000. I was told the engine had around 30,000 miles on it. I've put around 20,000 miles on this engine and the same thing happened agai
... n. This time, the engine had been knocking for a few months. When it started knocking, I took it to the dealership because the new extended warranty by Hyundai was in effect and I assumed it would be covered. Immediately, Autonation Hyundai in Savannah, GA said it would not be covered because it was a used engine. I don't understand why when the engine can be traced and the mileage known by the number on the engine. On XXX, my [XXX] granddaughter took her driving test in the vehicle. About 2 hours later, the engine seized with her driving. We were on the same road and it was about the same time of day. Once again, it was a miracle we got off the road safely. I just talked to Hyundai and we'll see what they say. From the stories I've seen, they'll do anything they can to avoid covering these engines and they get away with it. The people who get hurt the most from this are the ones who can least afford it. I'm a [XXX] widow raising my 5 grandchildren by myself and believed I was getting a good vehicle, mainly due to the report by Consumer Reports magazine. Even after the first engine failed and was replaced, I still wasn't in a financial position to get another vehicle and assumed if something happened again, it would be covered under the extended warranty Hyundai was made to provide. I have fighting this issue since January and have not gotten any help or relief from Chevrolet. This issue caused a significant safety risk and has cost me $12,000. I am writing to formally report a serious safety issue involving my 2021 Chevrolet Silverado that resulted in a sudden loss of power while merging onto an interstate highway. On XXX, I brought my vehicle in for service to replace the
... upper and lower ball joints, as well as the control arms. I retrieved the vehicle from the repair shop on XXX, and departed the same day for travel to [XXX]. Later that evening, while traveling southbound on [XXX] near [XXX], I stopped at Exit XXX. Upon re-entering the highway and accelerating to merge into traffic, the vehicle began shifting through gears abnormally. While in approximately 6th gear at around 2,900 RPM, a loud banging noise occurred, followed immediately by a significant loss of engine power. At the same time, the check engine light illuminated. Due to the sudden and unexpected loss of power while actively merging into highway traffic, I was nearly rear-ended by another vehicle. I was forced to take immediate evasive action and pull onto the shoulder to avoid a collision. This incident created a highly dangerous situation not only for myself but for surrounding motorists. This failure occurred without warning and directly impacted my ability to safely operate the vehicle in a high-speed traffic environment. I believe this presents a significant safety concern that warrants investigation. Please let me know if any additional information is required. I appreciate your attention to this matter and request that this incident be formally documented and reviewed. I am a [XXX] resident. While commuting to work in [XXX], my GMC Terrain experienced a sudden, catastrophic failure of the brake vacuum pump while traveling at highway speeds. When I attempted to slow down for the exit, the brake pedal instantly became extremely stiff and nearly impossible to depress. I realized I had suffered an absolute loss of power braking assist. To avoid a high-speed collisio
... n with heavy highway traffic, I was forced to deliberately steer onto the roadside rumble strips, relying on tire friction to slow the vehicle down. This severe near-miss left me terrified and distraught. Because the shoulder was narrow, my disabled vehicle was left partially protruding into the active roadway. Passing cars sped by so close that my vehicle was violently rocking, creating an incredibly unsafe situation. I was forced to call the State Police for emergency assistance. They dispatched a trooper and a tow truck to remove me and the vehicle safely from the highway. The vehicle was towed to a mechanic in Massachusetts. The mechanic discovered that the vacuum pump had internally exploded. This explosion sent catastrophic metal shards and debris directly into the engines oil galleys and oiling system. While the vehicle has 121,776 miles, this is not a normal wear-and-tear component failure. The brake vacuum pump suffered a catastrophic internal mechanical explosion at highway speeds, completely eliminating my ability to stop the vehicle. A mechanical component failure should never cause an immediate, life-threatening loss of brake control, regardless of mileage. This is a known, systemic engineering defect that is currently the subject of active federal class-action litigation (XXX v. General Motors LLC). This defect did not just cause an immediate life-threatening braking failure, it completely contaminated the oiling system, destroyed the entire engine, and rendered the vehicle a total loss requiring a full engine replacement. [XXX] [XXX] [XXX]; [XXX] [XXX] [XXX] Date: XXX To: National Highway Traffic Safety Administration Office of Defects Investigation (ODI) Subject: Formal Safety Defect Complaint – 2017 Hyundai Sonata Theta II Engine Failure, Excessive Oil Consumption, Intake/Engine Component Failure and Safety Hazard Dear Sir or Madam: I am submitting this formal complaint and request for immediate investigat
... ion into the dangerous and ongoing mechanical failures involving my 2017 Hyundai Sonata equipped with the Theta II engine. I believe this vehicle contains a serious manufacturing and/or engineering defect that presents a substantial safety hazard to consumers. My vehicle has experienced a pattern of progressive mechanical failure beginning with abnormal and excessive oil consumption, which appears to be directly associated with malfunctioning engine airflow and intake-related components. Specifically, the lower engine airflow system and intake functionality appear defective, placing excessive strain on the intake manifold system and contributing to cascading engine failures. As a result of these failures, the vehicle has experienced: Severe and excessive oil burning/consumption Intake manifold malfunction and abnormal operating stress Suspected overheating and/or premature head gasket failure Ignition coil failure and electrical shorting/misfires Progressive engine deterioration rendering the vehicle unsafe and unreliable Increased risk of sudden engine shutdown, stalling, fire hazard, or catastrophic engine failure while operating The vehicle has become increasingly unreliable and inoperable due to these conditions. These failures raise significant concerns regarding public safety, especially given the known history of complaints and recalls associated with Hyundai Theta II engine defects. I am deeply concerned that this vehicle may reflect a broader pattern of defective manufacturing, insufficient quality control. I am filing a complaint regarding my 2021 GMC Yukon due to a serious engine failure involving the camshaft, lifters, and crankshaft. The vehicle began exhibiting engine ticking and knocking noises, rough idle, loss of power, and reduced performance. These symptoms progressively worsened and ultimately resulted in major engine damage. The vehicle was diagnosed with failed lifters and camshaft damag
... e, which required extensive repairs. In addition, the crankshaft failed, resulting in catastrophic engine damage and making the vehicle unsafe to operate. These are critical engine components that should not fail prematurely under normal driving and maintenance conditions. The most serious concern is safety. The engine's loss of power and deteriorating performance created the risk of sudden failure while driving. A vehicle that loses power in traffic, while merging, crossing intersections, or traveling at highway speeds can create a dangerous situation and increase the risk of a crash. Occupants and other motorists could be placed at risk if the engine fails without warning. My vehicle has been properly maintained and serviced. Despite regular maintenance, these major engine components failed unexpectedly. I am aware that other owners of GMC and Chevrolet vehicles equipped with similar engines have reported comparable camshaft and lifter failures, suggesting a potential defect rather than an isolated incident. The failure resulted in significant repair costs, loss of vehicle use, and concerns about the long-term reliability and safety of the vehicle. I believe these engine failures may indicate a manufacturing or design defect affecting 2021 GMC Yukon vehicles. I respectfully request that NHTSA investigate camshaft, lifter, and crankshaft failures in the 2021 GMC Yukon and determine whether a safety-related defect exists that warrants corrective action or a recall.Read more

On November 20, 2025, my 2022 GMC Sierra 1500 AT4 equipped with the 3.0L Duramax diesel engine began experiencing intermittent engine overheating conditions. Concerned about a potential safety issue, I contacted a GMC dealership and was advised that because no diagnostic trouble codes were present, there was nothing to diagnose or repair and that I should continue driving the vehicle. Over the fol
... lowing months, the overheating condition continued intermittently. On May 24, 2026, while driving on the highway, the vehicle suddenly overheated, displayed fault code P1098, and entered reduced power ("limp mode"). The vehicle lost normal performance and acceleration while traveling in traffic. This created a significant safety hazard because the vehicle was unable to maintain normal highway speeds, increasing the risk of a rear-end collision or other traffic accident. The loss of power occurred while driving and without sufficient warning, leaving the vehicle unable to safely perform as expected in highway traffic. After researching this issue, I found numerous reports from other GMC and Chevrolet owners describing similar overheating, coolant control valve, thermal management system, and P1098-related failures. Many owners report symptoms involving engine overheating, reduced engine power, and limp mode operation while driving. I am concerned that this may indicate a broader defect involving the vehicle's cooling system and thermal management components. The vehicle exhibited overheating symptoms for approximately six months before the final failure occurred. Additionally, I understand that certain GM vehicles have been subject to service actions involving coolant control valves and thermal management system components. Given the similarity of symptoms, I am concerned that the overheating condition reported in November 2025 may be related to the same type of failure mechanism, yet my vehicle has not been included in any recall or special coverage program.Read more

My 2016 GMC Terrain 2.4L Ecotec first stalled while idling at a red light in Raleigh, NC. I restarted it and it sounded different, so I drove it home. The next morning it would not start. The failure occurred at approximately 69,939 miles with no meaningful warning. The dealer’s diagnostic report states there were no current relevant codes and no relevant codes in history. The dealer documented
... major timing-system failure: timing chain tensioner failure, timing chain guide failure, a timing chain guide bolt pulled out of the cylinder head, aluminum contamination in the cylinder head/oiling system, and engine replacement recommended. The vehicle was found unsafe to operate. This vehicle regularly transported my family, including my children. If the stall/no-start failure had occurred at highway speed, while merging, or in heavier traffic, it could have created a serious crash risk. The oil was maintained outside the dealership network and had been serviced before failure. I also topped off the oil shortly before this occurred. The 2.4L Ecotec engine family is known for oil-consumption issues, yet this vehicle gave me no direct warning that oil consumption had reached a dangerous level. A reasonable owner would not know to check oil every day when there were no relevant stored warning codes. Other NHTSA complaints involving 2016 GMC Terrain vehicles describe similar patterns: oil consumption, no warning lights, sudden stalling or no-start conditions, timing-chain failure, and engine replacement recommendations. I am filing because this appears to be an ongoing safety risk involving oil consumption without adequate warning, timing-chain/timing-guide failure, internal contamination, and sudden engine failure. I request NHTSA review this issue for investigation or recall expansion before this failure mode causes a preventable serious crash, injury, or fatality. ... 23 Bronco Sport, with a DO NOT DRIVE ADVISORY due to a suspension failure, was issued. These issues are incredibly unsafe and put drivers at a very serious risk of injury or death. I attempted to work with a local Ford dealership in hopes they would buy the vehicle back however they REFUSED unless I traded it in for another vehicle on their lot. Otherwise, I would have had to PAY FORD $2,000 to make up the difference in “value” (I disputed the value as I had one KBB report and they had a “different” one) and my loan payoff. I stated several times I cannot put thousands of dollars down to purchase a new vehicle at this time; however, I do not want to drive this unsafe hazardous vehicle for another day. They would not work with me at all nor did they offer to fix it or even look at the vehicle. Under NO circumstances should an owner have to pay Ford for their negligence and poor manufacturing, especially with a recent DO NOT DRIVE ADVISORY issued, no matter how much the owner owes on their loan. This is 100% a FORD issue, not a user issue, not wear and tear issue. These recalls are wildly unsafe and defective issues that lie within FORD that we as owners, should not bear the burden for. How is FORD being held accountable and at what point will they start buying these unsafe and terrible cars back without penalizing the owners? The car is available for inspection upon request. Safety was put at risk due to the cars potential to break down, or catch fire, due to high volumes of oil consumption and leakage from the piston rings. This was part of a recall campaign (#SC336 PISTON OIL RING), where we had two authorized Kia dealers inspect and claim the car "passed" their test. Recently, we had a catastrophic engine failure du
... e to the piston rings, where we feel the fix was insufficient given the failure point (piston rings). There were no warnings or symptoms prior to failure, when driving at a low speed the car stalled and the oil pressure warning light came on briefly. After that, we parked the car immediately and had it towed for inspection. Our concern is the fix was allegedly implemented in July 2025, but there is no note in our maintenance record that the fix was even implemented.Read more

To Whom It May Concern: I am submitting this complaint to report ongoing safety concerns involving my 2013 Hyundai Sonata. I purchased this vehicle in 2022 through financing and paid it off in 2025. Since acquiring the vehicle, I have experienced persistent mechanical issues that I believe may pose a safety risk to myself, my child, and others on the road. The most concerning issue involves the
... braking system. I have had the brakes replaced and serviced multiple times, yet the brakes begin grinding again within a very short period, including an instance where the grinding returned approximately one month after replacement. Despite spending money on repairs, the problem continues to reoccur. I am concerned that there may be an underlying defect affecting the braking system that is not being properly identified or corrected through normal maintenance. In addition, the vehicle consumes an excessive amount of oil, requiring frequent monitoring and replacement between oil changes. I am concerned that this issue could potentially lead to engine failure or other dangerous mechanical problems while the vehicle is being operated. As a single mother, I rely on this vehicle daily for transportation to and from work and for caring for my family. I have experienced significant financial hardship attempting to keep the vehicle operational and safe. Despite my efforts to maintain the vehicle, these issues continue to occur. I respectfully request that the National Highway Traffic Safety Administration review this complaint and determine whether these issues may be related to known defects, recalls, investigations, or patterns reported by other owners of 2013 Hyundai Sonata vehicles. I am willing to provide maintenance records, repair invoices, mileage information, and any additional documentation that may assist in evaluating this matter. Thank you for your attention to this complaint and for your commitment to vehicle safety.Read more

I returned home from shopping at local store 15 miles away. Arrived home around 11:30pm on June 3. Unloaded groceries and sat down. Around 11:45pm-11:50pm heard yelling from outside that our car was on fire. My husband ran out and and seen flames coming from under hood of car. I called 911 and within a couple mins middle of hood was completely starting to melt and components were spraying fluid an
... d popping pieces of metal and plastic were thrown everywhere. A neighbor proceeded to use 2 fire extinguishers to try to control blaze which did not seem to calm flames. Fire department arrived and sprayed down side of house and back of our other car which was parked In front of our BMW. Do to the extreme heat we could not get into other car to move it as it was parked by closed garage doors. Myself, our 2 daughter's and visiting friend were forced from the house due to concerns that the house might catch fire. We watched as the cars whole front engine area burned completely.During the trip to the store there were no warning signs on vehicle stating anything wrong. The fire marshal did a report on the fire and it was towed to local tow yard. At this time it is available for inspection. Preceding fire since it was unexplained to us we looked into recalls of the BMW and found multiple claims and reports for recall. Looking up VIN on NHSTA' s website VIN came back as "0" unrepaired recalls. We purchased the vehicle from a private dealer who never mentioned if vehicle ever had any recalls or had been fixed. And our search before purchase said there were no recalls on the vehicle. Due to the extreme heat our driveway was burned and our vehicle in front received heat damage to whole back of car. The fire was so intense that the engine area all the way to the back seat is burned. Thank goodness me and my daughter were not driving the vehicle at the time this happened as the engine fire burned quickly. This fire was not caused by arson or any factors under our control.Read more

My vehicle is at the Genesis dealership with what they even consider a common problem. Road debris putting a hole in the unprotected Turbo Intercooler Radiator. This is a safety issue because the loss of power in the vehicle once you lose your turbo. The engine light came on and started making noise. We drove it to a mechanic shop, and they said the code stated camshaft and other component issues, but the engine ended up failing completely and was rebuilt. On June 3, 2026, my 2019 Ford Edge (2.0L EcoBoost) experienced a sudden loss of motive power and stalled in the middle of a busy, high-traffic intersection. Before stalling, the vehicle emitted thick white smoke from the exhaust. This was a life-threatening situation as I was stranded in moving traffic. The vehicle was towed to Mike Burkart Ford, where it was diagnosed with an internal EGR cooler

The 6.2 liter engine has been recalled. The problem is if you pass inspection you are now required to use a higher grade of oil. This results in an additional cost of about $50 per change. The reimbursement of the difference should have been covered by the recall as this was not an intended cost when I purchased the truck.

My 2019 Hyundai Santa Fe 2.4L (VIN: [XXX]) is experiencing a severe safety defect involving rapid engine oil starvation. The vehicle recently failed an official oil consumption test conducted by Fred Beans Hyundai of Abington, where it was documented that the engine consumed 1 quart of oil in just 881 miles (PA Request #XXX). This vehicle is equipped with the Theta II engine, which currently has
... active warranty extensions (TXXC and TXXI) specifically covering connecting rod bearing failure. Despite the documented severe oil loss—which is the primary known cause of connecting rod bearing wear and sudden engine seizure—Hyundai Corporate (Case #XXX) is refusing to evaluate the vehicle under these warranties. They have improperly classified my claim as a denied "goodwill" request and are refusing to perform a connecting rod bearing clearance test to determine if the bearings have been damaged by this starvation. Because Hyundai refuses to perform the required diagnostic safety protocols, the vehicle must be driven in an oil-starved condition. This poses an imminent and severe safety risk of sudden engine seizure, stalling, and complete loss of motive power at highway speeds without warning, which could result in a catastrophic crash. The manufacturer is ignoring their own failure thresholds and putting driver safety at risk. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am filing a formal complaint against BMW Seattle for violating federal recall remedy mandates under 49 U.S.C. Chapter 301. The vehicle was delivered for mandatory safety recall 23V-707 after an on-road mechanical shear of the covered VANOS bolts. The dealer has created illegal commercial barriers and failed to fully execute the federal remedy. First, the service department issued a written estim
... ate for $4,405.67, claiming the CAS module and locks failed and required replacement to resolve a no-start condition. I challenged the repair on technical grounds and authorized a new battery and valid key only, the dealer admitted in writing that the electrical sync restored seamlessly and the engine started. They attempted to extract thousands for a functioning module before verifying the recall. Second, the VANOS bolts sheared resulting in certain valve-piston contact. The dealer claimed the engine passed a compression check but refused to provide raw ISTA diagnostic logs to verify this safety threshold. Instead, they demanded an additional out-of-pocket fee of $669.60 to re-perform a test they claimed to have completed, creating a commercial barrier to basic remedy verification. Third, the dealer demanded $2,136.69 to replace a compromised valve cover and PCV valve which functioned without issue pre-failure. Valve cover removal is required for vanos repair and requiring consumer expense for mandatory safety repair is legally prohibited. Indeed, Under BMW SIB 11 09 23, collateral damage must be routed through corporate TSARA, not the consumer. Fourth, the steel VANOS bolt heads sheared completely off. The official remedy mandates dropping the oil pan to locate and remove these fragments to prevent total oil pressure loss. Despite my repeated requests for verification of removal, nothing was provided, the car was inoperable, and I was advised that the recall repair was complete and the vehicle was in outdoor fenced lot awaiting arrangements for removal.Read more
