Vehicle entered limp mode at highway speed (approximately 70 mph) on [XXX] freeway in Washington State, it felt like I had rear ended a vehicle it shut down so hard. This was due to mis diagnosed inspection and service repair from a local Subaru Dealer. Vehicle lost power rapidly, creating a dangerous safety situation. Root cause traced to TCV/PCV system defects previously documented by authoriz
... ed Subaru dealers. Previous Subaru authorized dealer replaced a torn PCV hose but did not diagnose the problem causing the tear. The dash warning lights were still illuminated after the diagnostic and repair when vehicle was picked up after service. I have attached a more detailed overview of the dangerous situation. The car finally lost power on the freeway while I was in the fast lane pushing 70 miles an hour. I was in the fast lane and with no power I had to cut across both lanes of traffic and get to the shoulder. I was 45 minutes from my home and it was dark, raining and the Subaru would not start. I called a mobile mechanic I found online who came out to the freeway, roadside, where I was stranded. He worked on the car for a couple of hours in the rain. He got the car to start but it would not stay running. I had the car towed to a friend's house in Vancouver, WA about 6 miles from where it had broken down. The same mobile mechanic came out and changed out one of the oil actuators and cleaned and replaced two of the OCVs then flushed the oil system and put new oil in the car. Cleared all the codes. The car starts, idles and will drive up to about 1500 rpm’s but any more than 1500 rpm’s and it dies. It's in limp mode and will move past those rpm’s. The warning lights and the PCV house splitting must have been signs of something else being wrong. That is the discussion I had with Wilsonville Subaru. They diagnosed the vehicle and let me know the PCV hose was the problem. Our 2015 Kia Sedona VIN [XXX] developed CAN Bus fault codes U0001 U0100 U0101 rendering it completely undrivable following an incomplete warranty extension repair under WTY019 by Kia of Des Moines on [XXX] RO# [XXX]. The power sliding doors have been a persistent safety concern. The incomplete warranty extension repair left the sliding door not closing properly creating a safety hazard. The vehic
... le subsequently developed CAN Bus communication faults making it completely undrivable. After 28 days at Kia of Des Moines from May 4 to June 1 2026 under RO# [XXX] the CAN Bus fault was never diagnosed or addressed. The power sliding door still does not function after two warranty extension repairs. Upon retrieval June 1 2026 the vehicle continues to exhibit active CAN Bus warnings BDS warning lights flickering humming sound and rough driving. We believe the sliding door safety issues and CAN Bus failure are directly connected and represent a safety concern for other 2015-2017 Kia Sedona owners. I am filing this complaint regarding Honda's reimbursement program associated with the piston ring warranty extension related to engine misfire issues on certain vehicles. Prior to Honda issuing the warranty extension, I paid out of pocket to have the repair completed at an authorized Honda dealership. After Honda announced the warranty extension and reimbursement program, I submitted a reimburse
... ment claim along with all required documentation and proof of payment in August 2025. My reimbursement claim has now been pending for approximately 11 months. Since submitting the claim, I have contacted Honda numerous times seeking an update. On multiple occasions, Honda representatives advised me that the reimbursement had been approved and was simply awaiting issuance of payment. However, no reimbursement has been received. Due to the lack of progress and conflicting information, I filed a complaint with my state's Attorney General's Office. In response to that complaint, Honda stated that the reimbursement claim was still under review, which directly contradicts what I had previously been told by Honda representatives. I have complied with all documentation requirements and have made repeated attempts to obtain a clear answer regarding the status of my claim. Despite this, I have not received reimbursement, a definitive status update, or a reasonable explanation for why a reimbursement claim related to a known warranty extension issue has remained unresolved for nearly a year. I am requesting review of this matter and whether Honda is properly administering and processing reimbursement claims associated with this warranty extension program within a reasonable timeframe.Read more

The 2.0L EcoBoost engine in my 2018 Ford Edge Titanium experienced head gasket failure due to Ford’s known cylinder channel design defect present in 2015–2019 EcoBoost 2.0 engines. Ford scored channels between cylinders in an attempt to improve cooling, which allows coolant to degrade the head gasket and mix with engine oil. Symptoms included white smoke from the exhaust, coolant loss, rough i
... dle, and a check engine light prior to failure. The coolant-oil contamination caused engine performance degradation and risk of sudden failure while driving, endangering myself and other motorists. The engine drained all coolant unbeknownst to me while driving, causing the car to overheat and causing damage to the head gasket. Car had to be pulled over and towed to a shop. Head gasket was repaired, but the issue continued and more coolant was found in the engine oil in 2026. Coolant constantly has to be replenished after driving. The failure was confirmed by an independent mechanic. The vehicle has not been inspected by Ford or NHTSA. Ford has not issued a recall for this known defect despite widespread reports from 2015–2019 Edge, Fusion, and other EcoBoost 2.0 owners experiencing identical failures. I am filing this complaint to contribute to the pattern of evidence needed to compel a formal NHTSA investigation and potential recall action.Read more

The first time I heard the engine noise was on [XXX]. I brought the vehicle to the Schaumburg Muller Acura dealership and dropped it off on May 20. On May 21, the dealership performed an initial inspection and informed me that they needed authorization to further disassemble the engine in order to determine the cause of the problem. I provided that authorization, and by May 22 the engine had been
... torn down to the point where the failure could be identified. On May 22, I spoke with my service advisor, Ken, who informed me that the connecting rod bearings were worn and that the engine would need to be replaced. I was sent photographs, a video, and an estimate for the repair totaling $12,943.83. While I was shocked by the cost, Ken advised me that the dealership would be contacting Acura to request goodwill assistance on my behalf. Because this occurred immediately before the Memorial Day weekend, several days passed before I received an update. On May 30, a different service advisor informed me that Acura had approved $8,000 in goodwill assistance. During that conversation, he asked me, “The original estimate was around $16,000, right?” I knew that was incorrect, as the estimate I had received was $12,943.83. Concerned by the discrepancy, I contacted Acura Customer Relations on June 1 and opened a case for review. Later that same day, I spoke with Ken and provided him with the case number. During that conversation, he informed me that the original estimate had been incorrect because certain parts and/or labor had allegedly been omitted. I was then told that the actual repair cost would be approximately $18,000 to $19,000 plus tax. While I can understand minor adjustments to an estimate, a difference of several hundred dollars or even $1,000 might be reasonable. However, an increase of approximately $6,000 from the original estimate is extremely concerning and raises serious questions about the accuracy of the repair assessment and cost estimate. The vehicle suffered a sudden, severe engine malfunction/misfire while in operation, presenting a serious safety hazard on the road. Following the incident, the vehicle was taken to an authorized dealership service center (Repair Order #XXX), where technicians verbally confirmed that the engine requires a complete replacement due to internal mechanical failure. The catastrophic mechanical failure
... of the engine directly caused consequential damage to the emissions system, entirely clogging the catalytic converter with unburned fuel/debris. Despite the clear link to the known, systemic defects inherent to the Theta II engine line—which is subject to active federal consent orders, safety recalls, and lifetime warranty extensions—both the dealership and Kia Corporate are refusing to honor the 8-year/80,000-mile Federal Emissions Defect Warranty. The manufacturer is attempting to use the catastrophic failure of their own defective, recalled engine line to evade their legal environmental and safety warranty obligations, leaving the consumer stranded without alternative transportation. This represents a systemic effort by the manufacturer to deflect liability for documented powertrain safety failures onto individual consumers. Post-Recall Low Oil Warning + Performance Issues on 2021 GMC Yukon Denali – Campaign 25V274000 Vehicle: 2021 GMC Yukon Denali, 6.2L V8 Recall: 25V274000 Owner: One owner since new (purchased 2021) I had recall campaign 25V274000 performed on my vehicle. This was the first low oil warning I have ever received since purchasing the SUV new in 2021. I have always followed the vehicle’s oil life mo

About one week later, the low oil warning came on. I had the vehicle towed to the dealership and did not drive it after the warning. Since the recall service, I have also experienced performance issues including noticeable stuttering that was really evident before I towed it in. The dealership initially agreed to a consumption test but later acted like it was not needed, said everything was fine, and were dismissive — blaming me for not following their sticker. The vehicle has an unresolved electrical/propulsion failure involving sudden shutdowns while driving and CAN C bus/lost-communication faults. Since the high-voltage battery was replaced through a recall-related repair, the vehicle has had repeated shutdown/loss-of-communication events, occurring dozens of times. In the most serious incidents, while driving at speed, the dash flickered, windshield
... wipers activated unexpectedly, headlights turned off, turn signals would not work, and steering locked or became difficult to control. The vehicle then completely shut off while still moving. After shutdown, the hazard lights would not work. One documented incident occurred on the interstate at about 70 mph. This puts occupants and others at risk because the vehicle can suddenly lose motive power and critical electrical/control functions at the same time, including lighting, signals, hazards, instrument display, and steering/control assistance. The vehicle has been inspected multiple times by a Chrysler dealership. Dealer records documented multiple modules losing communication on the CAN C bus. The dealer replaced the shifter module as a possible contributor, but the problem continued. The dealer later corrected loose pin tension at a communication/star connector, but the vehicle continued to shut down and later scan reports still showed widespread active/stored CAN and lost-communication faults across powertrain, hybrid control, body control, braking/ABS, steering, and shifter-related systems. The vehicle is available for inspection upon request. The problem has been confirmed by dealer scan reports and repair records, but the root cause remains unresolved. The manufacturer has been contacted through customer care and repair/scan documentation was submitted. The exact root cause is UNKNOWN, but dealer findings indicate an unresolved major electrical/CAN communication problem possibly involving the harness, connector network, or communication system.Read more

My 2022 Volkswagen Tiguan (VIN: [XXX] ) is experiencing excessive engine oil consumption. The vehicle requires oil replenishment every less than 1,000 miles, which is far beyond what any reasonable consumer would expect from a properly functioning engine. I reported this issue to the Volkswagen dealership while the vehicle was still under the manufacturer’s warranty. The dealership stated that t
... his level of oil consumption is “normal” and only added oil without performing any diagnostic testing or repairs. No repair order documenting a defect was issued, and no corrective action was taken. This problem is consistent with the known defect in the EA888 2.0L TSI engine, specifically the defective piston rings that fail to create a proper seal, allowing oil to leak into the combustion chamber and burn off during operation. A class action lawsuit has been filed against Volkswagen regarding this exact defect in 2022–2023 Tiguan models. The excessive oil consumption poses a serious safety risk. If oil levels drop critically low without warning, the engine could suffer catastrophic failure, stall unexpectedly, or overheat while driving — all of which significantly increase the risk of a crash. Despite this being a warranty-covered vehicle and a known manufacturing defect, Volkswagen and its authorized dealership have refused to repair the issue or acknowledge it as a defect. I am requesting that NHTSA open a formal investigation into this matter and consider a safety recall for all affected 2022–2023 Volkswagen Tiguan vehicles. The vehicle's original engine suffered severe oil consumption and a blown head gasket, which contaminated the exhaust system. While operating the vehicle, I began to hear a metallic grinding noise originating from the center of the engine compartment. The sound was clearly audible within the cabin. Approximately one mile before the vehicle ultimately failed, the engine began to malfunction. As I slowed to a stoplight, the vehicle started to shake before shutting off abruptly and entering a coasting state. All electrical
... systems powered down and the vehicle would not restart. Shortly thereafter, heavy smoke began to emit both inside and outside the cabin, visibly coming from the hood and through the air vents. There was also a distinct burning odor. I attempted to restart the vehicle multiple times without success. The dashboard displayed the message: “conditions not suitable to shift.” Due to the vehicle becoming disabled on a curve and posing a safety hazard, I contacted the Sheriff’s Office for assistance to prevent a potential collision. Upon their arrival, it was noted that the engine compartment and hood area were exceptionally hot, particularly where the smoke was originating. The dealership has since confirmed they have also been unable to restart the vehicle. 2021 Ford F-150 Lariat, 5.0L Coyote V8, purchased new, now 65k miles. Two interrelated safety defects: OIL CONSUMPTION: On 3/21/2026 at 62k miles, I discovered the engine was critically low on oil. Only 3 quarts drained during a full oil change, against a rated 8.8 quart capacity. The engine had been running on one third of required oil. The onboard oil life monitor showed 9% remaining and gave NO
... low-oil warning, no warning lights, no alerts. No external leak. On 5/22/2026, Koons Ford Woodbridge formally confirmed excessive oil consumption after a documented oil consumption test. Ford TSB 19-2365 acknowledges this exact defect on the 5.0L Coyote for 2018-2020 model years, attributing it to high intake manifold vacuum during deceleration fuel shut-off pulling oil into the combustion chamber. The 2021 engine is mechanically identical. TRANSMISSION: Hard and slipping gear shifts reported to Ford within months of purchase in 2021. Ford issued multiple software updates over the years to mitigate the behavior, most recently late 2025. Fix was temporary; hard/slipping shifts returned. CATASTROPHIC FAILURE: While awaiting Ford HQ authorization on repairs, the vehicle suffered complete drivetrain failure on Friday night. Engine lost power and transmission failed entirely. I was near home and coasted safely off the road. Had this occurred at highway speed during my daily commute, the result could have been catastrophic. Vehicle towed to Koons Ford Woodbridge. SAFETY CONCERN: This is my only vehicle, used for daily highway commuting. The oil consumption defect provides no warning before engine damage. The transmission has degraded despite years of Ford software patches. The vehicle has demonstrated total drivetrain failure capability with no driver warning. The 2018 F150 I purchased started out a good vehicle like most vehicle do. The problems then began to come one after the other. Routine oil changes on the vehicle at the dealership with no hiccups turned into “Your motor is low on oil”. The tech tried to find a leak. The motor wasn’t leaking ANYTHING, so naturally the tech told me the motor was burning the missing oil. The tech told me “th
... ey all burn alittle oil” and topped it back off. I had another “incident” where I was driving on the highway a few weeks later and my engine light came on, and I lost oil pressure. The red “low oil pressure” message came on the dash and I promptly limped my truck to the dealership once again. The tech diagnosed the truck and found a plugged PCV valve. It was changed for a brand new one from the dealer, followed by another oil change well under the recommended interval naturally, due to the motor burning almost all of the oil. The truck still burns oil, and the issue seems to be impossible to remedy. The issue that followed the engine burning oil is the 10R80 Transmission installed from the ford manufacturer. Once again everything seemed fine when I purchased the truck. After driving it for 20-30k miles, the transmission has developed a flare shift between the 3rd-4th gears. The transmission is getting more and more hesitant to upshift and maintain and gain speed. This is considerably noticeable when attempting to get on the highway. This is unacceptable. It puts me in a very vulnerable spot because it doesn’t happen all the time. So I have to be EXTREMELY aware and prepared for the truck to become inoperable while merging into the flow of traffic. On two occasions now, while merging onto the highway with traffic, I have had to pull over to the shoulder of the road and put my hazards on to allow traffic to go around me due to my 2018 F150 with the 5.0/10R80 combination NOT increasing speed, and reving close to 5k RPMs. THESE ISSUES MUST BE ADDRESSED.Read more

Incident description: On three occasion I have had the same catastrophic Engine failure putting myself and my family along with the general public in harms way. Starting March of 2022 while driving at approximately 60 MPH on the West Shore Expressway my truck experienced a sudden and total loss of motor power. My engine stalled out and I lost my ability to safety steer and stop my vehicle. This a
... ll happened during rush hour while driving in the left lane of traffic. This sudden loss of power forced me to maneuver the vehicle to the right shoulder of the expressway across the right lane of traffic. As there was no shoulder to pull over on the left side of the highway. This created an immediate crash risk for myself and other drivers. Upon inspection at the authorized GMC dealership where I purchased the truck new, a certified mechanic determined that the engine experienced a complete failure requiring replacement or rebuilding of the engine. I paid to have the engine rebuilt at the dealership. The second instance was similar to the above incident. Once again I paid for the repair. The third instance was more dangerous as when my trucks engine failed it almost caused an accident. As of today my truck is still sitting in the dealership for over 6 months and no one from the dealership or manufacturer will give me an answer on what has to be done now. This patter of sudden repeated engine failure clearly indicates a systemic safety defect that extends beyond normal wear and tear. I urge the NHTSA to open a defect investigation into the 2018 GMC Sierra 1500 engine systems.Read more

The engine oil pump failed internally. The vehicle is equipped with a factory-installed variable-displacement oil pump that utilizes an internal thermoplastic adjusting ring. Due to heat cycles and mechanical stress, this plastic component degrades, becomes brittle, and catastrophically shatters or cracks. The first symptom appeared when the vehicle became entirely unable to complete an electronic
... engine oil level measurement via the iDrive system, consistently freezing and aborting the test at approximately 15% to 20%. This defect directly compromises road safety. A sudden drop in oil pressure causes severe internal engine friction, leading to immediate engine seizure or a total loss of motive power without adequate warning. Experiencing a sudden engine stall while driving at highway speeds places the driver, passengers, and surrounding motorists at extreme risk of a high-speed rear-end collision. Furthermore, a total engine shutdown disables power-assist steering and braking, making the vehicle incredibly difficult to control and maneuver to safety. Catastrophic mechanical failure from oil starvation also presents a severe under-hood fire hazard if oil leaks onto hot exhaust components. The problem has been officially reproduced and confirmed. The vehicle was brought to Bavarian Werkstatte, where certified technicians diagnosed the issue as an internal structural failure of the variable-displacement oil pump's plastic adjusting ring. The component is currently available for inspection upon request. BMW is fully aware of this defective design. They updated this exact component in mid-2020 to an all-metal oil pump to prevent plastic fragmentation. Despite knowing that the 2019 thermoplastic rings degrade, become brittle, and shatter under normal operating heat cycles, the manufacturer has failed to issue a safety recall in the United States, leaving drivers exposed to dangerous sudden engine failure.Read more

I have a ford edge 2016, which I can see online that many vehicles like mine are having the same problems. I am having a severe coolant leak into the engine. It trembles when I start it or when I am at a red light, white smoke leaks out of my exhaust when I first start my car after adding coolant. I am constantly adding coolant, mostly every other day because it burns it so fast. My vehicle aler

Fuel injector randomly went out, I dont have the HPFP recall done on my car. Caused a massive amount of power loss while I was going down the road. Now hyundai wants to pretend like the HPFP had nothing to do with this. I keep being told its an emission part and isnt covered under powertrain warranty. Recall in question is Extension Z04 (TSB 25-FL-002H).

I have had 2 different instances where my vehicle stops in the middle of a busy intersection or traffic and have no warning what so ever. This is a know issue on 2021 models. The exhaust bracket is breaking away from the muffler/catalytic converter. The weld is breaking causing the exhaust to break away and both pipes must be replaced resulting in a 1500 repair. Why if it bad in later years is there no maintenance bulletin or mandatory fix for the 2020 models? It was repaired and fixed by McClusky Chevrolet in Cincinnati OH.

Following a coolant rapid mate replacement performed by an authorized Tesla service center, the vehicle experienced a complete shutdown on a public road the same evening it was returned. The high-voltage cooling system was not properly purged of air following the service. This caused low coolant flow to the rear drive unit during a 63-mile drive in record heat. The fault cascade included a HIGH VO
... LTAGE ISOLATION FAULT (BMS_f123), drive inverter warning (DI_w126), and multiple battery management warnings. The instrument cluster displayed a red emergency warning: "Vehicle shutting down — PULL OVER SAFELY." The vehicle required towing. The failure was confirmed by the service center's own follow-up invoice, which documented "trapped air pockets in the coolant system following the most recent service" as the cause. The corrective repair was performed at $0 as a Goodwill service — the service center's internal acknowledgment of liability. The service advisor made a written text admission of causation the following day, then retracted it in a recorded phone call. Both are preserved. No post-repair Isolation Resistance test results were provided. The HV isolation fault may indicate latent thermal damage to HV insulation that could cause a safety failure at higher mileage outside any remaining warranty. The vehicle is available for inspection upon request.Read more

There was a recall-SC209A for a faulty piston ring. This vehicle “passed” prior to my purchase. I was unaware that my vehicle was part of the recall- when the VIN is entered in the NTSB website, no active recalls show. Starting in 10/24, the vehicle began having problems accelerating, and when I brought to my local mechanic it was found to be nearly empty on oil despite a recent oil change.
... I have since experienced continuing issues with low oil and oil consumption. The latest incident was in XXX on a trip to [XXX] . The car seized in the highway, and was brought to a local Kia dealer. They diagnosed a bad transmission, and it was replaced. Despite this, the car has continued to burn oil. I have brought the car to my local Kia in Schaumburg where it was purchased. It was today that I found out it was part of the recall. I asked for an oil consumption test, and I was told this would be at a $200 cost to me. I am worried for my families safety taking this car on the road- the oil light doesn’t come on when it is low. Kia hasn’t offered to retest the car, and I feel at significant risk continuing to drive it. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
