1. The vehicle has experienced multiple safety-related defects and is available for inspection upon request: The Instrument Cluster Panel was subject to Safety Recall and required service. The radio system suffered an internal malfunction. The Electric Hydraulic Power Steering system was subject to a safety recall issued on June 11, 2026 and no remedy is currently available from the manufacturer.
... 2. The instrument cluster defect could prevent the driver from viewing critical safety information, like speed, odometer, and warning indicators. The Electric Hydraulic Power Steering defect poses an even greater risk, as a loss or reduction of steering assist could impair vehicle control and increase the likelihood of a crash. Additionally, the recalled steering system may overheat and cause a fire, whether the vehicle is being driven or the engine is off, creating an immediate life-threatening hazard to occupants, nearby persons, vehicles, and property. This is especially concerning as a Texas resident driving in the summer months with extreme temperatures. 3. The instrument cluster issue was addressed through recall service performed by an authorized Jeep dealership. The radio/internal failure was reported to the dealership, which acknowledged the malfunction. The power steering issue has also been recognized by the manufacturer through the issuance of a safety recall on 6/11/2026. 4. The vehicle has been inspected and serviced by authorized Jeep dealerships in connection with the instrument cluster recall and the reported radio malfunction. Additionally, the vehicle underwent multiple routine service inspections before the Power Steering recall was issued, and no steering-system defects or safety concerns were identified or disclosed. 5. No warnings or messages appeared before the failures. Radio failure caused infotainment screen to go blank while instrument cluster and power steering issues were identified by manufacturer recalls instead of vehicle warnings.Read more

Vehicle is experiencing severe, dangerous transmission malfunctions matching Ford Safety Recall 26S28 / NHTSA 26V237. Multiple times while cruising at highway speeds, the vehicle has abruptly downshifted into 2nd or 3rd gear without warning, causing violent engine revving, sudden loss of forward momentum, and severe steering wheel shaking. These incidents occurred in rainy, low-visibility environm
... ents. The vehicle is primarily driven by my wife with our [XXX] granddaughter inside. A sudden drop to 2nd gear at highway speeds in low-traction conditions creates an immediate, catastrophic risk of a high-speed rear-end collision or loss of control. On Wed [XXX], we took the truck to Hubler Ford (Shelbyville, IN). The service department officially diagnosed the vehicle and acknowledged the malfunctions are caused by Recall 26S28. However, they refused to repair it because they are "waiting on a software code" from Ford corporate, estimating a delay until mid-July 2026. They refused to provide a safe loaner, forcing us to drive an actively malfunctioning vehicle. On 6/18/26, I called Ford National Customer Service to escalate this safety issue and request an interim rental. The Ford rep contacted Hubler Ford but failed to authorize a rental. Instead, Ford corporate placed me back in a customer service loop, telling me to discuss symptoms with the dealer again, even though the dealer already officially diagnosed the defect. This loop has caused numerous 20+ min holds with no one addressing the issue. Both Ford and Hubler Ford are refusing to provide intermediate safe transportation for an officially diagnosed, severely malfunctioning safety-recall vehicle, leaving a toddler exposed to a severe highway crash hazard. We are the original owner of a 2014 Hyundai Santa Fe. The vehicle is experiencing issues from internal engine oil consumption caused by a manufacturing defect in the GDI powertrain. In 2023, while driving under normal operating conditions, the vehicle began severely jerking, shuddering and hesitating during acceleration. This hesitation and sudden drop in power creates a severe and immediate traff
... ic hazard, as the vehicle loses acceleration capabilities unpredictably while driving, merging, entering a highway or crossing intersections. I have since experience low oil volume due to this systemic oil consumption. On April 7, 2023 while I was under the 100,000 mile factory warranty (at 99,644), the issues were reported to the dealership but they found no issues. Since then the engine light come up regularly and we must add or change oil soon. On June 15, 2026 we took it back with the same issue and we have been told after checking it and filling it up we must repeat the process from 2023, drive it for 1000 miles without changing or adding oil, even if it dries up. This will create a safety hazard not only for the public but for my family. Now we are told the engine needs to be replaced but since we're out of warranty, we must pay $21,000. We were under warranty when we reported the issue and they purposedly led us out knowing the issues were happening to other Sonata and Santa Fe models. Now we are left with a dangerous vehicle which we cannot in good faith sell to any other family.Read more

COMPONENTS THAT FAILED: Multiple systems failed in a connected chain. (1) The DPF aftertreatment differential pressure sensor — a CONFIRMED manufacturing defect per Cummins recall T7186/T7187 (Jan 2026). (2) The Allison automatic transmission — catastrophic internal failure: torque converter destroyed, flexplate warped and cracked, metal fragments throughout. (3) The transmission control unit
... wiring (circuits 143/123) reversed at the factory. The DEF sensor in the same aftertreatment system also failed back in Dec 2023, 14 months before the DPF symptoms — proving a system-wide defect. All failed components and repair invoices are available for inspection upon request. SAFETY RISK: After the dealer declared repairs complete, the truck was driven under 10 miles when the engine entered SEVERE DERATE (limp) mode WITHOUT WARNING, with four alerts firing at once including aftertreatment and transmission failures. Sudden, drastic power loss on a heavy commercial truck in live traffic is extremely dangerous — it can leave the vehicle unable to maintain highway speed, risking a collision with surrounding vehicles. The dealer released the truck in this hazardous state with no warning. CONFIRMED BY MULTIPLE SHOPS: Yes. The problem was reproduced and confirmed repeatedly — by the authorized dealer, by an independent transmission shop, and at final repair. The fault returned within 30 minutes on one visit and within 10 miles on another. INSPECTED BY: The authorized dealer and an independent service center inspected the vehicle and failed components. Documentation is available. WARNING SIGNS: Yes. Warning lamps appeared from the very first visit in Feb 2025 — transmission code P071A and high soot codes 1981/1922. They persisted for NINE MONTHS without proper repair, until the transmission catastrophically failed in Nov 2025. The vehicle was out of service over 45 days.Read more

To Whom It May Concern, I am filing a formal safety complaint regarding my 2021 Kia Seltos (VIN: [XXX]). My vehicle was previously serviced by Kia in connection with a recall in 2021. Despite the recall-related repair being completed, the issue has not been resolved. My check engine light continues to illuminate, and my vehicle has repeatedly stalled and completely shut off while stopped at red
... lights and intersections. This creates a serious safety hazard. When a vehicle unexpectedly loses power in traffic, it places the driver, passengers, and surrounding motorists at risk of an accident, injury, or worse. I understand that there have been reports, recalls, investigations, and consumer complaints involving certain Kia vehicles with engine-related issues, including concerns regarding piston oil rings, excessive oil consumption, engine damage, and engine failure. While I am not a mechanic and cannot state with certainty that this is the cause of my vehicle’s problems, I believe my vehicle may be experiencing a similar issue based on the symptoms it is displaying. My concern is that the ongoing engine warning light and repeated stalling events may be related to an underlying engine defect that was either not corrected by the previous recall repair or has developed despite that repair. Given the known concerns surrounding certain Kia engines and piston oil ring-related issues, I believe my vehicle should be thoroughly reviewed to determine whether it qualifies for coverage under any applicable recalls, warranty extensions, service campaigns, or defect-related repair programs. I have been informed that my vehicle is not covered; however, I respectfully request that Kia reevaluate my VIN and investigate whether the stalling condition is connected to any known engine-related defects or recall conditions. I am requesting that: • Kia provide towing assistance if the vehicle is unsafe to operate. • Kia perform a complete diagnostic inspection at no cost. While traveling on US Highway 95 between Lewiston and Riggins, Idaho, a 2024 Toyota Tundra TRD Pro with approximately 18,000 miles experienced a sudden loss of engine power while descending a mountain grade with multiple curves. The driver reported hearing abnormal engine noises immediately before the loss of power. At the same time, the brake pedal became extremely hard to depress and the vehicle
... did not decelerate as expected, requiring substantially increased braking effort. The vehicle was carrying family members and was traveling on a winding downhill section of highway adjacent to steep terrain. Due to the loss of power and increased braking effort, the occupants feared the vehicle would be unable to stop safely. Fortunately, a roadside pullout area was available, and the driver was able to steer the vehicle into the pullout and bring it to a stop without a collision. The vehicle was towed to an authorized Toyota dealership for inspection. The dealership diagnosed the vehicle with a catastrophic engine failure requiring engine replacement. The failed engine remains in the possession of the dealership and should be available for inspection. Approximately five days before this incident, the vehicle had undergone routine maintenance, including an oil change and inspection at a Toyota dealership. The vehicle had also previously been presented for brake pulsation concerns and other drivability-related complaints. The vehicle was returned to service and represented as operating normally. No collision occurred and no physical injuries were reported; however, the incident created a significant safety risk because the loss of engine power occurred while descending a mountain highway and the driver experienced substantially increased braking effort while attempting to stop the vehicle.Read more

I am submitting this complaint regarding a cylinder head failure on my Mazda CX-5. The vehicle has approximately 53,000 miles and has been properly maintained throughout its service life. The vehicle developed a crack in the cylinder head, which was subsequently confirmed by an authorized Mazda dealership. Mazda has previously issued a Technical Service Bulletin (TSB) addressing cylinder head cra
... cking on certain vehicles; however, my vehicle's VIN is not included in the covered range despite experiencing the same failure described in the TSB. The dealership initially provided a repair estimate of $8,789 plus tax to replace the cylinder head. Following further discussions, the dealership reduced the repair cost to $3,903 plus tax. They recognize that this crack is not part of normal wear and tear but due to the VIN number not being included in the recall are unable to provide full coverage. A cylinder head crack is a major engine failure that should not occur on a well-maintained vehicle with only 53,000 miles. I am concerned that vehicles outside the VIN range identified in Mazda's TSB may be experiencing the same defect and that the scope of the issue may be broader than currently recognized. The vehicle was purchased used from the same Mazda dealership that diagnosed the failure and is currently performing the repair evaluation. I respectfully request that NHTSA review this issue to determine whether additional Mazda CX-5 vehicles may be affected by the same cylinder head defect despite not being included in the existing TSB.Read more

I am reporting a confirmed safety defect in my 2017 Ford Escape involving engine coolant intrusion into the cylinder bores. This is a sudden, random manufacturing defect that occurs without warning. I have exercised full due diligence as an owner regarding upkeep and maintenance on my vehicle. I now have a formal, written diagnosis from a Ford Service Center confirming this internal failure (consi

The verified diagnosis proves my vehicle is currently a high-risk hazard that could lose power without warning on high-speed roadways. Ford’s Customer Satisfaction Program 21N12 is an insufficient response to a defect that causes vehicles to become disabled and vulnerable in traffic. I am requesting that NHTSA escalate this to a mandatory safety recall based on the documented, spontaneous nature of this failure despite my proven maintenance history.
