Issue 1 - Engine had a rough idle - dealer investigation found that the "upper engine mount had collapsed completely allowing the engine to rest on the housing". This problem occurred at 52 months from new vehicle purchase at 24,165 miles (warranty is 4 years, 50,000 miles). The vehicle was never in an accident, nor drive rough. No warning lamps or other indicators occurred. An internet search has
... identified this to be a common problem. I believe that the engine provides front end stability for a frontal crash, and any issue with the retention of the engine in such a situation would constitute a safety issue. I believe that Mini has a poor design, where all the stresses and loads that the engine sees are not included in the motor mount design - a fundamental safety issue - that should be part of a recall, or at a minimum a factory covered repair item for longer than the standard warranty. Issue 2 - At the same time I brought the car to the dealer for Issue 1 above, I identified an issue with the steering - steering had a dead spot where it was unclear if steering wheel movement resulted in actual vehicle steering. The dealer investigated and found "fault is internal to the steering rack, need to replace the steering rack and lower steering shaft as the is a high failure rate item". An issue with steering, especially an issue that is classified by Mini as a HIGH FAILURE RATE ITEM, should be considered a safety issue as loss of steering/power steering can result in the inability to avoid an accident. There were no warning lights or other indicator faults. This appears to be another design, manufacturing or assembly issue. Steering should last well beyond 52 months and 24,165 miles.Read more

Honda 2018 CRV VIN [XXX]. Car was brought to Honda dealer due to smoking issue from exhaust directly after fuel pump recall. CRV head gasket was replaced for $6,000 and still smoke was coming from exhaust. Returned the vehicle to Napleto
... n Honda dealer and America Honda then claims issue is a Turbo issue and that is what is causing the smoking for another $5,500 they can swap that part too. I expect America Honda will recommend next the rear-view mirror will need to be replaced for that is how we are seeing the smoke from the exhaust. The Dealer is under direction from America Honda on what to replace and they have failed to fix the issue. Multiple websites reference that the recalled fuel pump is causing engine trouble like fuel gasket and turbo which America Honda knows for that is what they are recommend being replaced so far. If you do some discovery on this, you will also see there are several class-action lawsuits on this same issue in the US and several other countries. My 2026 Chevrolet Silverado 3500 with the 6.6L Duramax diesel has experienced three separate P2463 Diesel Particulate Filter excessive soot accumulation events at approximately 4,300, 5,500, and 10,280 miles. Each event required a Chevrolet dealer to perform a forced regeneration to restore normal operation. The most serious event occurred while towing an approximately 15,000-pound loaded dump tr
... ailer on a public county road. While climbing a hill, the truck entered reduced-power/limp mode associated with the emissions/DPF system. Power was reduced so severely that the truck could no longer continue uphill and came to a stop in the travel lane. This created a safety hazard because the road was narrow and winding, with no safe place to pull off. I had to back the truck and loaded trailer several hundred yards downhill while traffic was present behind me. Before the event, the truck displayed “Continue Driving, Cleaning Exhaust Filter.” The warning did not state that severe power reduction or loss of towing capability was imminent. I did continue operating the vehicle, but the truck was being used for normal commercial towing/work operations where immediate highway-speed driving was not practical. The issue has been confirmed by Chevrolet dealers through DTC P2463 on multiple visits. The only correction performed has been forced regeneration. I was advised that no failed component was identified and no permanent repair has been performed. The vehicle is available for inspection, and repair records are available. My concern is that the emissions-related derate strategy can cause sudden and severe loss of motive power while towing on public roads, leaving the vehicle unable to maintain forward movement or safely clear the travel lane. This has occurred repeatedly during normal work use, not as a one-time event, and the warning provided did not give a clear indication that the truck could soon become unable to tow or maintain forward movement.Read more

This vehicle was sold to me on July 18, 2025 with Campaign CH7 open and unperformed. The dealer's disclosure form falsely stated that I had been informed of all recalls with the vehicle. No specific mention of Campaign CH7 was ever made. Separately, Acura manufacturer records (eVRM/Techline) confirm that this vehicle's original engine was previously replaced under warranty following a seizure (De

I am requesting NHTSA investigate whether the open Campaign CH7 recall contributed to the engine failure, and whether FSL Motors' practice of selling vehicles with open recalls while falsely certifying recall disclosure warrants further review.Read more

It has happened mostly at lower speeds while decelerating or braking, but has occurred once on a highway. Should this issue happen in high traffic, or high speed, it could absolutely cause an accident. The car has been at a dealership since 21MAY over this issue. i’m unsure if the person i bought this vehicle from had the codes wiped before selling to me, but there were NO lights on dash for 3 days after minimal driving only to work and back, then all of a sudden the check engine light came on, and in the info box it would pop up saying “service stability system” - i took to o’reiley’s to see what it was for, apparently it was the ECM (electronic
... computer module) which essentially controls like everything. days go by i don’t do anything more about it and drive as usual to work, and it starts overheating, and smoking like crazy, but doesn’t say it’s hot on my dash, so i had no idea how hot it got or anything. i got to work, worked my shift, and went to put coolant in before returning home- just to find that it was full and i didn’t need to put any in. weird, right? ok well on my way home i’m going about 55mph on the highway and it suddenly just wouldn’t let me accelerate anymore and behaved as if i ran out of gas but i had over half a tank. when i came to a stop i turned the car off, and tried to turn it back on when it wouldn’t even turn over. i sat about 15 mins on the side of the highway before it actually started and i drove home. a week goes by, and now this ecm is effecting more and more- driving on the highway/streets it’s shifting really hard, immediately following that, my speedometer and rpm meter fall to “0” on the dash but the vehicle is still driving and the message pops up about the service stability system. i replaced my tire and now it’s telling me to service airbag system and the abs light is stuck on, as well as traction control and it’s just so weird. i just don’t feel safe in this car and after looking it up, apparently everyone with the make and model have very similar problems, but specifically searching by my VIN- apparently there’s no recalls and it’s just extremely frustrating and i don’t know what to do. if i fix the ECM will all these issues be resolved or will i waste $600-$800 replacing it?Read more

... vehicle was diagnosed by Faulkner Honda in Harrisburg, Pennsylvania. The dealership found DTC P0430 (Catalyst System Efficiency Below Threshold) and performed an air/fuel ratio test. According to the dealership, four of the six cylinders were running rich due to excessive fuel delivery. The dealership recommended replacement of the fuel injectors, and all six fuel injectors were replaced. The injector repair cost approximately $1,100. The dealership advised that possible causes included internal fuel injector failure or carbon buildup but could not provide a definitive explanation for the failure. Neither the dealership nor Honda could explain why my vehicle was excluded from Honda's fuel injector warranty extension despite exhibiting the same symptoms and requiring the same repair associated with that program. I was advised that the warranty extension applies only to certain VINs produced during specific manufacturing periods and facilities. However, my vehicle experienced the same failure pattern, including a rich-running condition, emissions-system warning, catalyst-efficiency diagnostic code, and replacement of the fuel injectors. While researching the issue, I reviewed NHTSA complaints involving 2019 Honda Pilot vehicles. Engine seized up on the highway going onto incoming traffic. Almost got into a car accident.
Incident date: Jun 18, 2026
The vehicle exhibits a chronic, systemic electrical failure characterized by the intermittent loss of critical lighting systems, including turn signals and brake lights. This failur5e has persisted over several years and thousands of miles, independent of battery replacements or specific usage patterns. The issue has been confirmed via multiple independent service interventions. Most notably, in
... February 2024, a third-party professional repair - which included the replacement of the Power Distribution Unit (Central Fuse Box) was performed to address these electrical faults. Despite this professional intervention, the defect recurred in May 2026. The failure of the turn signals and brake lights while in traffic resulted in a Law Enforcement stop and the issuance of a citation. Prior to total failure, the vehicle often displays erratic engine performance, including sputtering and hesitation. These symptoms were identified early in the Vehicle's history, but inquiries regarding recall campaigns for similar Makes and Models were denied by Authorized Dealerships, who cited recall registry despite exhibiting identical Mechanical and Electrical failure modes. The failure of the brake lights and turn signals poses a severe and immediate safety risk to myself, my passengers, other motorists and those on the road...as it creates an unpredictable environment were emergency stopping and lane changes cannot be communicated to the surrounding traffic. The electrical architecture of this vehicle is fundamentally flawed, as evidenced by the recurring nature of the defect despite multiple repair attempts, including the replacement of the primary fuse distribution center. The vehicle, all service records. third-party repair receipt, and relevant law enforcement citation are available for inspection upon request to support this report. The recurring loss of safety-critical signaling systems represents a systemic design defect rather than isolated wear & tear.Read more

My 2019 Mazda CX-9 (VIN [XXX]) suffered catastrophic cylinder head failure on the SKYACTIV-G 2.5T engine at approximately 124,000 miles. The failure is consistent with Mazda TSB 01-013/21 (and successors 01-007/22, 01-010/24), in which Mazda acknowledged that cylinder heads on this engine can crack at the stud bolt hole and exhaust manifold flange due to manifold deformation and residual productio
... n stress. The first warning came in mid-May 2026, when the engine temperature warning illuminated during normal driving. An independent mechanic replaced the thermostat. The warning returned within a week, and the thermostat was replaced a second time with a higher-quality part. Four to five days later, the warning returned alongside more severe symptoms: the vehicle began jerking while in motion, a strong coolant-burning smell developed, and smoke was visible from the exhaust pipe upon shutdown. The vehicle became undriveable, had to be left at the failure location, and was towed. Safety risk: The jerking compromised vehicle control in traffic. Upon shutdown, smoke and burning coolant odor created fire and fume-exposure risks. Had this failure occurred at highway speeds or in heavy traffic, the sudden loss of engine power and erratic behavior could have caused a serious collision. Family members were in the vehicle during several escalating events. Confirmation: The vehicle was towed to an authorized Mazda dealership, where Mazda-certified technicians confirmed in writing that it is exhibiting the exact defect described in TSB 01-013/21 and that complete engine replacement is required. The diagnosis is formally documented and has been communicated to Mazda's warranty team. Mazda Customer Experience has been engaged since mid-May 2026 and acknowledged the issue. Summary of issue The vehicle developed an engine/emission warning light and failed mechanically without any crash or impact. The car drove fine until about one week ago. I have driven the vehicle normally and performed regular maintenance. Events and observations A warning light appeared indicating engine and emission problems. I took the vehicle to two separate mechanics: Mechanic A diagnosed e
... ngine damage and catalytic converter issues. Mechanic B diagnosed engine damage and a possible fuel pump problem. I am not a mechanic and cannot independently interpret technical findings, but both inspections indicate serious engine-related failures. The vehicle has approximately 80,000 miles and was purchased for more than $20,000 less than two years ago. No collisions, impacts, or abuse occurred that could explain the failure. I believe this is a potential manufacturing defect. I contacted a Honda dealer about the problem; they stated it is not a manufacturing defect and declined to accept responsibility for repair. I believe the dealer is avoiding responsibility. Requested action I request NHTSA open an investigation (or add my report to any existing investigation) into this vehicle’s engine and related components to determine whether a safety defect or noncompliance exists. I request guidance on next steps and that this complaint be considered in any recall evaluation.Read more

The condition is repeatable and occurs during normal driving. The battery warning lamp and low voltage condition were observed and documented. This failure creates a potential safety concern because the vehicle can lose charging capability while operating on public roads.Read more
