This incident occurred 24, June 2024. Time was approx 1745hrs and temperature was 93 and humid. Was stopped at a stop sign, attempted to make a phone call and reached to temporarily turn off the air conditioner by pushing the on/off button. When I pushed the button, it turned off the vehicle engine completely! When I was startled as to what had just happened, I pushed the air conditioner button a
... second time; It STARTED the truck's vehicle engine! I repeated this test three times in the presence of my spouse who was also in the truck. We then drove approximately one mile and at a second stop again was able to duplicate the engine turned off/on by pushing the air conditioner on off again! It was if the air conditioner on/off button was the vehicle start stop button. I did not try to duplicate this under roadway speed, fearing if the engine did in fact turn off, simply by pushing the on/off air conditioner button. It would most assuredly result in the complete loss of driver inputs for steering, brakes and control.Read more

Recalls addressed yet engine continued to have ongoing problems. As of today Nissan service said the transmission needs full replacement yet takes zero responsibility for the problem. Car and Engine has continually had problems of cutting out and lagging. Now they are denying this has any correlation to their existing engine problems. This is a major ongoing safety problem due to the fact that wh

Nissan needs to take responsibility telling an owner to replace a transmission in hopes that this will address an engine problem that has gone on for two years straight is not the answer. They need to understand the dangers of any driving situation where an engine stalls/stops. It’s amazing with this existing problem that nobody has been injured on record yet! While driving on the highway at approximately 65 mph with my child in the vehicle during rush-hour on May 21, 2026, my 2018 Honda Accord Hybrid Touring (105,746 miles) suddenly dropped speed, and began backfiring and stopped responding to the gas pedal. The check engine light flashed and the vehicle ran rough. I activated emergency flashers and drove slowly to the shoulder. Sunnyvale Honda (Sunnyv
... ale, CA) diagnosed coolant intrusion into one of the cylinders and head gasket failure. The vehicle had never overheated and showed no prior warning signs. All maintenance was performed at Honda dealerships per the maintenance minder schedule. The dealership performed a head gasket repair costing $7,500+ out of pocket. On the first highway drive after pickup, again with my children in the vehicle, the identical failure occurred: backfiring, complete loss of throttle response, vehicle unable to exceed 48 mph with the accelerator fully depressed. The repair did not resolve the defect. The vehicle is currently back at the dealership for further investigation. This failure created an immediate safety hazard on both occasions: sudden loss of speed on highway with no warning. Honda Customer Service denied goodwill assistance. I am aware of numerous similar complaints involving coolant leak and head gasket failure on 2018-2022 Accord including complaints documenting the same failure on the Accord Hybrid 2.0L engine. NHTSA should investigate this as a potential safety-related defect affecting multiple Honda engine variants.Read more

Vehicle: 2023 Ford Explorer ST VIN: [XXX] I am reporting ongoing safety-related powertrain and drivability defects that continue despite numerous repair attempts, including complete engine and transmission replacement. Since approximately June 2025, my vehicle has experienced delayed acceleration, hesitation, erratic RPM behavior, harsh shifting, loss of power, and intermittent warning lights. T
... he vehicle has also stalled, jerked during acceleration, and exhibited transmission shuddering and lurching. The problems have been repeatedly reproduced, documented, inspected, and repaired by Kendall Ford of Anchorage, Alaska, the sole servicing Ford dealership involved. Repairs have included replacement of fuel injectors, cylinder heads, a complete engine assembly, and a complete transmission assembly. Despite these repairs, the safety concerns remain. Current symptoms include: • 1-4 second delay in throttle response. • Vehicle may fail to accelerate below approximately 30 mph. • Engine RPM rises significantly without proportional vehicle acceleration. • Erratic shifting, surging, and harsh jolts. • Intermittent check engine lights, FordPass alerts, and electrical warnings. These conditions create a significant safety risk because the vehicle may fail to accelerate when entering the highway, merging into traffic, crossing traffic, making left turns, or avoiding hazards. The delayed throttle response and sudden acceleration after the delay increase the risk of a crash. Attached service records document stalling, misfires, loss of propulsion concerns, and repeated powertrain failures requiring replacement of both the engine and transmission, yet the drivability concerns continue. I believe this may involve a defect in the powertrain, transmission controls, engine management system, electronic throttle control system, or related vehicle control modules. I respectfully request that NHTSA investigate these recurring safety-related defects. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.CRead more

On 2 separate occasions (August 2025 & February 2026), the car has had a "transmission malfunction" light come on while driving on the highway around 65-70 mph. They have NOT attempted to fix it because they DO NOT have a fix. This IS NOT SAFE for my daughter to drive. This is her car and it has been in the shop since February 2026. We purchased the car from Bob King Mercedes in Wilmington, NC as a certified pre-owned in 2024. Both dealerships have been great to work with and stand by me that it is not safe for her to drive, while corporate has dismissed it and said it is safe to drive and threatened that we'll have to pay lot rental if we don't pick it up. However, that very afternoon, I was driving down the interstate, where the posted speed limit is 70mph, when my check engine light came on again and suddenly lost power, drastically reducing my speed and could not accelerate faster than 39mph. Luckily, the interstate was not busy at the time (about 10:30-11:00 a.m. Saturday morning) and there were only less than 4 cars behind me that were able to easily switch lanes and pass me. I moved over to the shoulder to a complete stop and restarted my car. My check engine light remained illuminated and was still slow to accelerate and "sluggish" but was able to (slowly) reach interstate speed (60-70mph) until I reached an acceptable exit and safely made it to my destination. I am reporting a severe safety hazard regarding an immediate, drastic increase in fuel consumption and erratic fuel range calculations following a manufacturer recall repair. My 2025 Ford Explorer ST-Line was operating perfectly until a Ford Mobile Service technician performed safety recall 26S21 (IPMA Reset). According to official Ford manufacturer specifications, this SUV has an expected drivi
... ng range of up to 501 miles on a full 17.9-gallon tank of fuel. However, immediately following this software update, the vehicle's total driving range drastically dropped. On a 100% full tank of gasoline, the dashboard's "Distance to Empty" (DTE) display now only reads 316 miles left to empty—nearly 200 miles below the manufacturer's expected capability. Furthermore, during a subsequent continuous, short 1.8-mile drive near my residence with no stop-and-go traffic, the DTE display rapidly plummeted from 15 miles down to 5 miles remaining, dropping visibly every two seconds. The vehicle is physically burning through gasoline at an accelerated, unsafe rate. This creates an immediate safety risk. If the vehicle is suffering from an uncalibrated software glitch in the powertrain/fuel modules due to the recall update, or if the procedure triggered a mechanical fault, the vehicle is at high risk of a sudden highway stall or leaving the driver stranded in live traffic due to inaccurate fuel signaling. I request that NHTSA look into whether the software installation process for recall 26S21 can corrupt engine/fuel management maps or trigger fuel delivery faults on the 2.3L EcoBoost platform.Read more

On May 19, 2026, my 2025 Chevrolet Tahoe suffered a sudden, catastrophic engine failure while traveling at 60 mph on a public roadway. I originally took retail delivery of this vehicle brand new on October 29, 2025, with only 14 miles on the odometer; at the time of the failure, the vehicle had only accumulated 10,049 total miles. The failure resulted in an immediate, complete loss of motive power
... at highway speeds, creating a severe, life-threatening safety hazard to myself, my passenger, and surrounding traffic. The vehicle was towed to an authorized GM dealership for emergency service. Upon removing the oil pan, dealership technicians discovered a high volume of metal debris and fragments, indicating a severe internal mechanical failure (likely failed engine bearings). General Motors corporate subsequently mandated secondary diagnostic teardowns. Following these tests, GM confirmed a total, non-repairable engine failure requiring a complete engine assembly replacement. This vehicle has remained entirely out of service and non-operational since the date of the incident. On June 24, 2026, with the vehicle out of service for 36 consecutive days, I issued a formal demand letter to General Motors Customer Relations, demanding that they adhere to New York State Lemon Law statutory requirements and initiate an immediate vehicle buyback. Due to this severe defect, this vehicle is now deemed entirely unsafe and unreliable for my family, as our confidence in its mechanical integrity is permanently destroyed. Furthermore, a vehicle requiring a complete engine replacement at this mileage has suffered severe diminished financial value. A catastrophic engine failure on a low-mileage, current-generation vehicle poses an immediate, critical manufacturing defect and an unacceptable risk of high-speed collisions.Read more

Since purchasing this vehicle new, the Auto Idle Stop (AIS) system on my 2017 Honda Pilot has intermittently failed during the engine restart sequence after the vehicle comes to a complete stop. When the AIS system shuts the engine off and I release the brake to resume driving, the vehicle intermittently fails to restart properly or does not restart at all, preventing the vehicle from moving as ex
... pected. In some instances, warning messages, including a collision mitigation message, has appeared on the dashboard before the car restarts. This condition has occurred repeatedly over several years and has happened at intersections, while stopped in traffic, and while stopped on highways. The inability of the vehicle to restart and move creates a safety risk because the vehicle may remain disabled in active traffic when acceleration is needed. The issue was reported to the selling and servicing Honda dealership (Cambridge, Honda in Cambridge MA) multiple times over several years. In August 2025, the vehicle received Honda's prescribed repair under Service Bulletins 23-008 and 23-009, including replacement of the starter assembly, starter relay, and battery. Despite these repairs, the same condition returned less than one year later and has occurred at least seven additional times. The vehicle stalls and sometimes displays the collision mitigation/crash warning message before restarting. In June 2026, the vehicle was inspected again by the dealership. Technicians were unable to reproduce the condition during testing and declined to perform additional repairs. The defect remains unresolved. The vehicle has already received Honda's recommended AIS repair, yet the same stalling and crash mitigation message continues to occur. The intermittent nature of the defect makes it difficult to reproduce during short dealership evaluations, but it continues to occur during normal operation and presents an ongoing safety concern.Read more

I have asked the dealer to investigate and they claim no error codes and the battery is ok. I suspect either a starter, brake pedal issues, or software update is needed. Ford Motor Company (Case No. 2:20-cv-01796) claim that Ford was aware of this defect in their vehicles dating back to 2010. I purchased my 2018 Lincoln MKZ back in 2019.Read more

At approximately 130,000 miles, vehicle developed and internal coolant leak caused by a failed Exhaust Heat Recovery System (EHRS) in the center muffler assembly, matching TSB 23-EM-009H - a known manufacturing defect documented by Hyundai. Coolant has been slowly leaking internally into the exhaust system. The authorized Hyundai dealership initially failed to diagnose the internal leak and told m
... e there was no leak and wanted to send me home. After pushing back, the internal leak was confirmed. If not caught, this defect could result in engine overheating or complete engine failure without warning. Hyundai Motor America has denied goodwill coverage despite this being a documented manufacturing defect.Read more

Engine Auto off/ on when stopped for signal light. Routinely, when stopped at a light, the engine will fail to turn back on and the car will go off but will not turn itself completely off. This leads to a delay for the engine clicks into neutral and in order to restart, I must turn the vehicle completely off and then restart. This issue occurs when I release the brake to begin my acceleration afte

When I come to a full stop and the Idle Stop feature engages, sometimes the engine will not restart. I get a message saying to press the button to start the engine, but it does not start after doing that either, so I have to shut the car off and restart it. There are a bunch of lights that flash when this happens.

I am filing a safety complaint regarding my Hyundai vehicle after experiencing a sudden engine failure that appears consistent with known rod bearing and engine defects affecting certain Hyundai engines. While driving, my vehicle began making a knocking noise and shortly thereafter suffered catastrophic engine failure. The engine lost power and became inoperable, requiring the vehicle to be towed
... . This failure occurred without any prior indication that the engine was about to fail. My concern is that this type of engine failure creates a significant safety hazard because it can occur unexpectedly while driving, potentially causing a loss of power in traffic and increasing the risk of a crash. The dealership inspected the vehicle and engine but did not report excessive sludge buildup. I have provided available maintenance records showing that oil changes were performed during my ownership. Despite this, there appears to be disagreement regarding the cause of the failure. I am aware of reports involving rod bearing and engine failures in similar Hyundai vehicles and believe NHTSA should investigate whether this failure is related to a broader manufacturing or design defect. The cost of replacing the engine is substantial, and consumers may be placed at risk if these failures occur while vehicles are being operated on public roads. I am requesting that NHTSA review this issue and determine whether additional investigation, recalls, or other corrective actions are warranted. 1. The vehicle caught fire during normal operation. It is a 2021 Jeep Wrangler (VIN [XXX]) subject to FCA US Recall 21D / NHTSA Campaign 26V363000, which identifies the electric hydraulic power steering (EHPS) pump and its wiring as a fire source that can overheat and ignite. The exact failed component has not yet been confirmed by diagnostic, but the fire is consistent with the defect described i
... n that recall. The vehicle is intact and available for inspection — it was towed to Waxahatchie Jeep, an authorized Stellantis dealer in Waxahachie, TX, on June 19, 2026 and remains in their custody under repair order [XXX]. 2. The vehicle caught fire while being driven on the highway between Waxahachie and Ennis, Texas, with two occupants returning from a Boy Scout trip, my husband, the driver, and our [XXX] son. The driver was forced to take evasive action, steering off the roadway into a service station to get the occupants clear and stop the fire from spreading further. This created a serious risk of burns, of a collision during the evasive maneuver, and of harm to the occupants and other motorists. 3. Has the problem been reproduced…? No. No diagnostic has been performed. The vehicle has been in the authorized dealer’s custody since June 19, 2026 ([XXX]), but the manufacturer has not completed an inspection or diagnostic. The underlying defect is the subject of an active recall (21D / 26V363000) affecting more than one million 2021–2025 Jeep Wrangler and Gladiator vehicles, for which the manufacturer has stated no remedy is yet available. 4. The vehicle was towed by AAA to Waxahatchie Jeep on June 19, 2026 and has remained there since. It has not yet been formally inspected or diagnosed by the manufacturer. 5. There were no warning lamps or messages prior to the fire. The first indication of a problem was smoke from the engine area at which point the driver immediately pulled off the road and saw a plume of smoke coming from the exhaust.
