On May 14, 2026, while driving a 2021 Subaru Ascent (approx. 94,291 miles) on a Texas expressway at highway speed during rush hour, I experienced a sudden and significant loss of engine power. This incident occurred only 3 days and approximately 125 miles after the vehicle was returned following a major engine repair performed at an authorized Subaru retailer in Texas under active powertrain warranty. That repair was supervised by a Subaru of America District Quality Manager and Field Technical Specialist, who conducted a 110-mile validation drive prior to release. Prior to the highway failure, visible fluid leaks were documented across the intake ducting, residue on lower engine components, a sweet and burning odor after driving, and milky sludge returning to the coolant reservoir — all within 125 miles of pickup. These are consistent with ongoing oil and coolant cross-contamination, which was the original diagnosis documented in the dealership's digital service portal. The vehicle has now undergone three major engine teardowns since March 17, 2026 — over 100 days out of service — for the same underlying powertrain issue on the FA24 engine. The current repair involves cylinder heads sent to a third-party machine shop. Vehicle telematics confirm the vehicle has not been started since May 27, 2026. This report is filed to establish a federal safety record of the highway power loss incident and the recurring nature of this powertrain failure. I arrived at the soccer field and parked the car for the remainder of the day, which was several hours. It did not do that again that evening. The other thing that has happened a couple of times prior to the incident and since is, the Nissan panel display that lights up when you turn the car on turned completely off and on again several times at random while I was driving. Usually the panel stays with eliminated and plays audio or shows a map. It turned itself completely off to Black and then back on again as if the car was starting up again. On Tuesday, I took the car to Nissan because I had called the problem in over the weekend, but their service center was not open. They were unable to replicate the problem. However, on my way to Nissan, the accelerator again pressed forward independently of my actions and the car accelerated without warning. This is very scary and felt extremely dangerous. I told the technicians that Nissan that the problem appeared similar to a recall that had been in place a few years ago. But they said that unless they could replicate the problem, they weren’t able to do anything about it. I was very unsatisfied with that because it seemed like a pretty dangerous situation.Read more

A year ago while driving on the interstate, my Ford Maverick suddenly threw a "Stop Safely Now" alert. I was able to turn the ignition off and back on to safely exit the highway. ... d to go back to the dealership. They gave me a copy of a memo that they have sent to all of their shops reporting that NHTSA is investigating this problem with Hyundai and Kia vehicles. The Kia dealership is not concerned about this. The documentation for this service says last visit we did not tighten the drain plug properly and another shop confirmed that. Please check oil level, drain plug, and filter to ensure proper levels/tightness and report MISC drain plug was found loose. They observed it for two hours. They also noted that mileage coming into the shop was 61212 mileage, out of the shop was 61212. Please note they had not done previous oil change as noted in their documentation. I have a service contract with this Kia dealership where I bought the car. I stopped going there because they recommended that three of my brakes needed to be changed on March 4, 2025. In spring of 2025 I had two different mavis shops. Tell me that my brakes did not require changing. One set had 30% wear, the other set had only 20% wear. It is now June 25, 2026. My break still have not required being changed. That was not recommended Service today either.Read more

Issue 2 – Electrical System Failure The vehicle has also experienced ongoing electrical problems involving the keyless entry system, repeated 12V Battery Low warnings, inoperative interior lights, and loss of connectivity with the Volkswagen mobile app. These problems have persisted despite nine repair attempts by the dealership and remain unresolved. The vehicle has spent a significant amount of time at the dealership throughout 2025 and 2026 for these recurring defects. The repeated electrical failures raise significant safety concerns. The keyless entry system can fail to unlock or recognize the key, the low 12V battery warnings indicate an unresolved electrical issue, and the loss of interior lighting reduces visibility inside the vehicle. The repeated failures also raise concerns about the reliability of other critical electrical systems and the possibility of the vehicle failing to start or operating unpredictably. Despite six repair attempts for excessive oil consumption and nine for electrical defects, Volkswagen has been unable to permanently fix these issues. The vehicle has now been out of service for over 3 months, and we have paid for two ~100‑mile tows because Ford refused to authorize repairs or provide a loaner/rental after a second engine failure. On June 18 the car was towed to Bommarito Ford, who falsely accused us of “destroying the car” and removing the catalytic converter, despite written confirmation from Pettus Ford that THEY removed it during diagnosis. Bommarito refused recall work, claimed the car was “out of warranty,” and ordered us to remove it from their lot or face storage fees. Ford Corporate opened cases [XXX] and [XXX]. The second rep was rude, accusatory, did not return calls, and repeated false claims. Ford denied coverage using a 12k‑mile parts warranty, even though this is a federal safety recall. Ford Corporate also misidentified the owner and listed my vehicle incorrectly as a “Ford Edge.” After notifying Ford Executive Office, Tier 2, the Missouri Attorney General, and legal counsel, Pettus Ford’s access to Ford’s system was suddenly restored. Pettus Ford continued to help, but Ford still refused to authorize the repair. We have now towed the car back to Pettus Ford in Fredericktown, MO. We have been without a vehicle for nearly 4 months and have spent significant time and money on towing. Ford only began to respond after the Missouri Attorney General complaint and after Ford Corporate threatened legal action instead of providing assistance. The dealer diagnosed a punctured radiator caused by a rock or similar road debris that allegedly passed through the front grille and struck the radiator. The dealer confirmed that coolant loss from the puncture triggered the vehicle's protective reduced-power mode. This failure created a significant safety risk. A sudden loss of power while traveling at highway speed increases the risk of rear-end collisions and other accidents, particularly in heavy traffic where drivers may not expect a vehicle to rapidly decelerate. There were no warning lights, messages, leaks, drivability concerns, or other symptoms before the incident. All warnings appeared suddenly at the time of failure. Toyota has declined warranty coverage, classifying the damage as a road hazard. However, I believe this may indicate a design defect. I have identified numerous reports from other GH owners describing nearly identical failures involving rocks penetrating the radiator through the front grille, often at relatively low mileage. These reports suggest the radiator may be inadequately protected from ordinary roadway debris. My concern is not simply radiator damage, but that a small piece of road debris encountered during normal driving can disable a critical component and cause the vehicle to enter a severe reduced-power mode at highway speed. It receives routine maintenance and after my last oil change my mechanic (not from Kia) said that they found metal shavings in the oil and that there was only 1 qt of oil. First, I took the vehicle to Auto Zone to run diagnostics that showed cylinder 2 and 3 were misfiring. Then took the vehicle to Meineke where they ran diagnostics that stated cylinder 2 and 3 were misfiring and then they switched the cylinders to see where the issue was occurring they also added coolant since the level was low. I then was advised to drive the vehicle and come back if the engine light turned on. The engine light appeared and observed shaking of the vehicle so I returned to Meineke. They then ran further testing and came to the conclusion that there was a coolant intrusion issue (loss of coolant) and startup misfiring and I was advised to take it to a dealership where certified people could do more testing. I took the vehicle to a Ford dealership and they came to the same conclusion that the issue was coolant intrusion of the engine and that the whole engine would need replacing with symptoms of startup misfires and coolant loss. I then took it to another Ford dealership for another opinion and they also came to the same conclusion where they observed startup misifires, coolant loss, and white smoke. The warning signs were the check engine light and shaking of the vehicle. After observing that, I didn't drive it until the following morning to get it checked out.Read more

Because this was a serious diagnosis, I then took the vehicle to another reputable automotive repair shop for a second opinion. That shop also advised that the engine needed to be replaced due to the coolant intrusion issue. This creates a safety concern because coolant intrusion can lead to misfires, engine damage, overheating, loss of power, stalling, or engine failure while the vehicle is being driven. I was more than 250 miles from home and had to avoid driving the vehicle because of the risk of further engine damage or possible failure on the road. The vehicle is available for inspection upon request. The issue was inspected by the dealership and by an independent automotive repair shop. No crash, fire, injury, or death occurred. ... ases the risk of a crash. Furthermore, the blocked exhaust gases generate extreme temperatures under the hood, presenting a distinct vehicle fire hazard. I have proactively maintained this vehicle and had all previous safety campaigns addressed, including a complete engine replacement and the required engine control software updates. Despite these recalls being fully completed, the vehicle has still suffered this severe exhaust system failure. The Check Engine light was on, and diagnostic scanning shows fault code P2096. A professional dealer assessment confirmed a "collapsing manifold catalytic converter," "contamination of honeycomb on front of rear catalytic converter," and severe "contamination on the O2 sensor." An oxygen sensor replaced in December 2025 has already been destroyed by the upstream ceramic debris. Abrasive ceramic dust from the melting manifold structure is actively being drawn back into the engine cylinders, threatening to destroy the newly replaced engine. This failure mirrors the exact physical mechanics and safety risks outlined in Kia's previous safety campaign SC176 (NHTSA Campaign 19V-120). Despite experiencing the identical defect and safety threat on a fully updated vehicle, my VIN was excluded from that specific campaign, and Kia corporate has flatly denied goodwill assistance. I am filing this complaint because this known safety defect poses an active threat to vehicle control and occupant safety. They stated this VIN did not fall under the recall and there was nothing they would do. Kia was called directly and also stated no assistance would be offered. Took it to local Honda service shop (Performance Kings Honda at 9131 Fields Ertel Rd, Cincinnati, OH 45249), diagnosed as needing fuel injectors replaced (running too rich), and got the fuel injector repair. Got the car back within a week, problem now solved. This is an issue for the NHTSA because it is a known issue on this make and model of car by owners and Honda. See the following: Honda Service Bulletins 21-007, 22-002, 23-042. Fuel injectors are an expensive repair, and they're known to be faulty on these vehicles by the manufacturer. Without correction, other major systems in the vehicle will fail, with issues such as engine misfire, overheating, catalytic converter failure, and catastrophic damage to the engine. Honda already knows this is an issue on the 2016 and 2017 models. The warranty extension they already issued for these years must be expanded or replicated to cover this issue which has persisted in the 2018-2020 models. First, within the first 5 minutes of engine startup, and most pronounced on every cold engine startup, the throttle delivery is highly erratic, causing uncommanded low-speed surging. In a specific incident in a parking lot following a cold start, I accelerated to approximately 15 MPH and completely removed my foot from the accelerator pedal to coast. Instead of slowing down, the vehicle unexpectedly lunged and pulled ahead forcefully against my intent. Second, even after reaching full operating temperature, the vehicle completely loses safe and predictable engine braking at greater speeds; when lifting off the gas pedal, engine RPMs aggressively drop, causing the vehicle to coast freely as if in neutral. Re-applying the accelerator causes a violent "phantom shifting" downshift and a hard transmission lockup. My safety and the safety of others are put at risk every time the vehicle is driven. The low-speed throttle surging creates an ongoing risk of hitting pedestrians or cross-traffic, while the permanent loss of predictable engine braking drastically extends stopping distances. I asked if it was the timing belt and he said "No, unfortunately, there is nothing you could've done to prevent this". ... t abnormal oil consumption. We reported these symptoms repeatedly during routine service visits. In March 2026, the CR‑V was brought to the Honda dealer for a routine oil change. The vehicle arrived running normally. After the service, the dealer advised that the vehicle would no longer start and was undriveable. Their diagnosis was a failed VTC actuator, a stretched/jumped timing chain, and internal engine damage requiring timing‑system repair, cylinder head/valve work, and possibly pistons and rings due to oil‑consumption‑related wear. The failure occurred without any warning lights or messages immediately prior to the event. The only prior symptom was the recurring cold‑start rattle that had been present for years and was previously addressed once by replacing the VTC actuator. The dealer has confirmed the failure and provided written estimates; the vehicle remains disabled. This pattern—cold‑start rattle, VTC actuator failure, timing‑chain stretch/jump, and internal engine damage—appears consistent with a known defect in the VTC actuator/timing system on this generation of Honda CR‑V. While my vehicle was in neutral and moving through an automatic car wash on a track, the Automatic Emergency Braking (AEB) system malfunctioned. My vehicle shifted itself into park and shut down. This sudden engagement caused the vehicle behind me to collide with the rear of my vehicle. As a result, the driver of the following vehicle has filed a collision claim against my insurance policy. My vehicle has been taken to the dealership 6 times for this AEB-related issue, along with other ongoing concerns, on the following dates: 09/11/25, 10/07/25, 11/12/25, 01/19/26, 03/09/26, and as of 6/05/26 is back at the dealership. The vehicle will unexpectedly shift out of gear at low speeds, requiring me to stop and place it back into gear before I can continue driving. The vehicle is currently at the dealership as of 06/05/26. Despite several repair attempts, the problems have not been resolved. I am extremely concerned that the AEB system will continue to malfunction causing a more serious collision and injury. The dealership was able to reproduce the problem in September; however, they have not been able to reproduce it again since that time.
