My 2016 Volvo XC90 suddenly lost power while I was driving, requiring me to pull over and have the vehicle towed. This is a documented safety issue associated with a known piston ring defect in 2013-2016 Volvo vehicles equipped with 2.0L and 2.5L engines, which was the subject of a federal class action lawsuit ([XXX] et al. v. Volvo Car USA, LLC, Case No. XXX, filed XXX. No there we NO warning lam
... ps, messages or other symptoms of the problem prior to the failure. I purchased this vehicle as a second owner from Carvana [XXX] — one month after the class action was filed. I was never notified of the defect, the litigation, or any service campaign or extended warranty coverage associated with this VIN, through Carvana nor Volvo. Since the power loss, I have spent $11,543.75 in out-of-pocket repairs (replacement engine, labor, towing). Despite a new engine being installed, the vehicle enters limp mode within one mile of driving. A Volvo-authorized dealer has quoted an additional $19,959.65 in repairs without certifying that the work will resolve the safety issue. Volvo Customer Care has declined to assist, stating their internal service campaign has concluded. I am submitting this safety complaint because sudden, unexplained power loss in a moving vehicle is a significant safety hazard, and because second owners of affected vehicles were never properly notified of the defect. I never knew nor was this disclosed to me before purchase from Carvana nor told to me by Volvo. I was in the middle of nowhere and I was not sure what to do and being a single woman, I was unsafe and scared. What ever this issue is Volvo and the dealer in South Dakota (where it is parked right now) as I had it towed on June 5th, and they don't want to help nor fix my vehicle. I need my catering business vehicle as this is a very concerning for my loss of use for over a year now for my catering part of my business. My vehicle has suffered catastrophic electrical failure due to water intrusion directly into the rear cargo compartment. Incident date: Apr 14, 2026
I am reporting repeated failures involving both the transmission and diesel emissions (DEF) system on my 2021 GMC Sierra 2500HD Denali equipped with the 6.6L Duramax diesel engine and Allison 10-speed automatic transmission. While under warranty, the truck required replacement of the aftertreatment fuel injector and later the Reductant Control Module due to DEF/emissions system failures. General
... Motors also completed recall-related transmission software updates on the vehicle. At approximately 154,833 miles, the transmission began shifting improperly, set transmission fault codes, and required complete replacement at my expense. Less than 5,000 miles later, the vehicle again experienced a "Service DEF System" warning with multiple diagnostic trouble codes. Repairs required replacement of the DEF tank, DEF heater assembly, and NOx sensor. These repeated failures involve major drivetrain and emissions components on a heavy-duty truck. DEF system failures can place the vehicle into reduced-power (derate) mode, limiting vehicle performance and creating potential safety concerns while driving or towing. I am submitting this complaint so these failures can be documented as part of NHTSA's evaluation of similar vehicles.Read more

While traveling at highway speed during rush hour commute, the vehicle lost engine power and stalled. While coasting, I tried to restart the engine several times without success. I continued to try and restart the vehicle without success. I was in an unsafe position during rush hour commute for about 50 minutes until roadside assistance arrived and loaded and towed the vehicle. There was no warning of the issue prior to the incident. 2019 Hyundai Elantra (VIN: [XXX]) suffered engine failure at 152,061 miles — abnormal knocking, loss of power, towed to Clay Cooley Hyundai of Sherman, TX. Diagnosed with crank/rod knock requiring long block replacement. This is the exact failure progression documented in NHTSA Recall 21V301 (Hyundai Recall 203): defective piston oil rings causing cylinder bore scuffing, accelerated oil consumpt
... ion, connecting rod bearing failure, and engine block damage. Coverage was denied solely due to mileage — 2,061 miles beyond the 150,000-mile threshold. I am also reporting a critical discrepancy: a VIN search on NHTSA's website shows zero open or unrepaired recalls on this vehicle, despite the following facts: 1. This is a 2019 Elantra with a 2.0L Nu MPI engine — the exact population covered by 21V301. 2. The vehicle experienced the exact failure mode 21V301 was issued to address. 3. The dealership repair order references TSB 25-01-054H (tied directly to Recall 203) and performed an ECM software update under that bulletin during this visit. It is unclear whether this VIN was improperly excluded from the recall population, or whether the recall was marked remediated based solely on the software update — despite the engine already having suffered catastrophic internal damage. Either outcome warrants investigation. I am requesting NHTSA examine whether this VIN should have been included in 21V301, and whether Hyundai's practice of applying a software update to an already-failed engine and closing the recall is consistent with the intent of this safety recall. Repair estimate: $9,245.58. Vehicle is undrivable. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While driving in normal traffic, when I come to a complete stop and keep my foot on the brake, the Auto Idle Stop system activates as designed. However, when I release the brake to continue driving, the engine intermittently fails to restart properly. Instead, it repeatedly attempts to restart, cycling on and off several times before shutting down completely. When this occurs, the vehicle becomes
... disabled in traffic and does not respond as expected. It can take multiple attempts and a significant amount of time before the engine finally restarts, leaving me stranded in intersections or in moving traffic. This creates a serious safety hazard because surrounding vehicles continue moving while my vehicle is unable to move. This problem has occurred multiple times under normal driving conditions and is not an isolated incident. During one occurrence, the vehicle failed to restart after the Auto Idle Stop system activated, leaving my vehicle disabled in traffic. As a result of this incident, my vehicle was involved in a collision that **damaged the front and back bumper**. Fortunately, no one was seriously injured, but the incident clearly demonstrates the significant safety risk associated with this defect. The vehicle is currently at an authorized Honda dealership for diagnosis and repair. I am submitting this complaint because a vehicle that unexpectedly shuts down and fails to restart after Auto Idle Stop activation presents a serious safety hazard. I am concerned that this defect could lead to additional crashes, property damage, or serious injuries if it is not corrected.Read more

This is not the first time I have heard of this happening and after researching, it appears it is more common than not. I was driving eastbound in my 2012 Kia Sportage on I-82 just past Benton City MP97 at about 11AM on Monday June 22, 2026, when my engine knocked twice, the red check engine light came on, and I lost power. I pulled over off the road next to a concrete barrier that was beside a rock roadcut. My flashers would not operate, I put it into park, turned off the ignition and immediately noticed the smell
... of burning oil or rubber. I had 2 half gallon containers of water on my front seat, so I popped the switch, lifted the hood, used the prop rod to hold the hood up as I started dowsing the flames that were popping up on the passenger side of my engine compartment near the firewall. After using both half gallons, I ran to the rear of the vehicle, opened the hatchback, and grabbed a gallon spray bottle of water and brought that back around to the front of the vehicle. By this point the prop rod holding the hood was bending and the flames were popping back up, I pumped the sprayer and directed a stream of water with my left hand and used my right to hold up the hood as the prop rod was not working. After the fire was completely out, I returned to the rear of the vehicle, donned my reflective vest, put a portable flasher on my vehicle, and at this point a Benton County fireman in a pickup arrived - he was called by a motorist that saw me pulling over with smoke trailing my vehicle. The fireman was followed by a large firetruck and crew. They propped up the hood with a bar and inspected the motor to verify the fire was completely extinguished. They took my information and left. A Washington State Patrol arrived, assessed the scene and offered assistance, but I already had called my wife so he left me to wait for her. He told me I had 24 hours to get the car off the interstate. I had the car towed to my driveway by A&E towing at about 2:30PM. I just realized this morning that I ought to report it to you. My 2025 Honda Odyssey has experienced an intermittent failure of the Auto Idle Stop system on four separate occasions. When I release the brake pedal to proceed after the light turns green, the engine fails to restart automatically. Instead, the dashboard lights illuminate and flash, and the vehicle displ
... ays the message "Press brake and START button." Pressing the START button once does not restart the engine. I have had to repeatedly press or hold the START button before the vehicle eventually restarts. During these events, the vehicle is completely disabled in active traffic and cannot be driven until it restarts. This has occurred four times, including while stopped at busy multi-lane intersections. The vehicle remained stationary after the traffic signal turned green while surrounding traffic expected it to move. This created a safety hazard because the vehicle unexpectedly became immobilized in an active traffic lane. I took the vehicle to an authorized Honda dealership. The service advisor acknowledged they are aware of similar complaints from other 2025 Odyssey owners but indicated Honda has not provided a definitive repair or service procedure. A diagnostic appointment has been scheduled. I have also found numerous reports from other 2025 Honda Odyssey owners describing the same Auto Idle Stop failure. In many of those cases, dealerships ultimately replaced the starter under warranty after diagnosis. The consistency of these reports suggests this may not be an isolated incident. I am submitting this complaint because this is a safety issue. A vehicle that unexpectedly cannot move when traffic begins to flow creates a risk of a rear-end or other collision.Read more

I’m also being told this time around that, it was only a temporary fix which I was never told the first two times that they fixed it. 2023 JEEP Wrangler Sahara 4xe has been recalled several times for battery potential fire risk issues due to faulty hi voltage battery design. Each time, a software patch was applied to provide a 'fix'. With my engineering background, I am very sensitive to machinery and its behavior. I have noticed with each successive update, the power output of the traction battery has been capped or limited to
... restrict maximum current draw from the battery to prevent over heating. With the latest 68C recall, there is now a clear safety issue. Power is restricted so much, pulling on a highway is dangerous. As I accelerate, battery output climbs to 50%, the engine comes on and there is a significant pause before acceleration can resume safely. Under battery only acceleration, the car bogs down with loss of power. The 4xe no longer resembles the vehicle I purchased. Jeep needs to do a physical remedy to restore original specs or buy back the car, 100% of purchase price. This is a clear safety issue. On May 19th 2026 around noon time, while we (me and my wife passenger) were driving my 2022 Subaru Outback Touring (2.5L, non-turbo) in active congested traffic on our way to airport, the engine shut off as expected by the Auto Start-Stop system when I came to a stop. After shifting to Park and pressing the ignition button multiple times, the engine eventually restarted. There was no prior history of stalling, no check engine light, and the battery and vehicle have been maintained per Subaru's schedule. This is a critical safety defect: a vehicle that disables its own engine in traffic and fails to restart on demand puts the driver, passengers, and surrounding traffic at immediate risk of rear-end collision. The vehicle experienced engine failure due to coolant intrusion into the engine cylinders, requiring complete engine replacement. The vehicle is available for inspection upon request. Prior to diagnosis, the vehicle exhibited engine misfires, rough operation, and noticeable “jumping” or jerking when starting the engine and when shifting into reverse. Warning indicator lights were also present
... . These symptoms indicated unstable engine operation and raised concern for potential sudden loss of power or engine failure while operating the vehicle, which could create a safety risk in traffic. The condition was first diagnosed by an independent repair facility and later confirmed by an authorized Ford dealership. The dealership confirmed coolant intrusion into the engine and determined that complete engine replacement was required. The manufacturer, through its authorized dealership, inspected the vehicle and confirmed the diagnosis. The vehicle is currently not safely operable due to the confirmed engine failure. Based on my understanding, Ford has previously acknowledged coolant intrusion concerns affecting certain 1.5L EcoBoost engines through Customer Satisfaction Programs. Although this specific vehicle was not included in that program, the dealership confirmed the same coolant intrusion failure.Read more

Since the 68C recall from Jeep, my vehicle has not been functioning properly. The 68C recall was issued because the high-voltage battery cells have a defect that causes them to short-circuit and catch fire. To avoid replacing hundreds of thousands of expensive batteries all at once, Jeep implemented a "fix" via a software flash. The new software "fix" heavily restricts how much power can be drawn

If you push the pedal enough to draw more than roughly 40–50% power—or if the battery temperature hits a strictly lowered threshold—the software abruptly chokes the electric motor and fires up the ICE engine to compensate. Because the electric motor just lost its punch, the computer frantically fires up the gasoline engine to make up the difference. Because there is an inherent delay while the gas engine starts up and matches the transmission speed, it engages with a sudden, frustrating lurch or surge. This is just one of the many problems i have found since jeeps "fix" has been implemented. Basically Jeep elected to reduce the capacity of the battery to avoid costly replacement of the real issue with the battery. This has resulted in a driving safety concern with the surging and hesitation. Also now I can no longer drive exclusively in the electric mode; electric mode is basically non-existent.Read more

I have a 2024 Jeep Wrangler 4xe plug in hybrid. When I first got the vehicle I could drive in the electric mode without kicking over to the gasoline mode. The manufacturer issued a recall for the battery due to fire danger. I had an update 68c performed by the dealer to address the recall issue. Upon getting my Jeep back I can no longer drive in the electric mode without the Jeep surging into and
... out of the gas mode. This happens at different times, slow speeds or fast speeds. The jeep does hesitate or bogs down prior to engaging the gas mode. After takinThe 68C recall was issued because the high-voltage battery cells have a defect (separator damage) that causes them to short-circuit and catch fire. To avoid replacing hundreds of thousands of expensive batteries all at once, Stellantis implemented a "fix" via a software flash. Here is exactly what that software is doing to your driving experience: Aggressive Battery Throttling: The new software heavily restricts how much power can be drawn from the high-voltage battery to keep it from getting warm. If you push the pedal enough to draw more than roughly 40–50% power—or if the battery temperature hits a strictly lowered threshold—the software abruptly chokes the electric motor. The "Bogging" Sensation: When the software chokes the battery, the vehicle experiences a sudden drop in power. This is the hesitation or "bogging down" you feel. The Violent Engine Surge: Because the electric motor just lost its punch, the computer frantically fires up the gasoline engine to make up the difference. Because there is an inherent delay while the gas engine starts up and matches the transmission speed, it engages with a sudden, frustrating lurch or surge. Because this clunky, unrefined behavior was intentionally coded into the 68C update by corporate engineers to limit fire risks, your local dealer's computer diagnostics read "No Errors Found" and "Operating as Designed."Read more

Engine failure , has been confirmed by a shop. It is available for inspection upon request. No previous problems or warning lights. Stalled and put myself and my children at risk of serious injury.
Incident date: Nov 11, 2025
I have a problem with parking, the warning comes on,”Shift Into Park” knowing that I’m in park, so it’s not registering. Have to move the shifter a couple of times before it comes off. My check engine light stays on and then comes off a week or two later and then comes back on and no code comes up, so I’ll clear it or let it clear itself.

#1. This vehicle uses a lot of oil !!! It burns through it like crazy I have to check the oil every other day. #2. When I come to a stop while driving and give it gas to take off IT WONT GO !!!!! It hesitates and when it does go it LUNGES FORWARD!! This is a huge manufacturer problem not a buyer problem. The engine is junk.! 3. I’ve called numerous places to get help. This car needs to be reca
... lled. This is a huge safety violation. It doesn’t wanna go when you give it gas and when it does, it lunges forward that can cause a huge accident on the highways. This car was made in China. It is junk. I’ve called to get help with this issue and no one wants to help. Nobody returns my calls and I’ll Hyundai wants to do is it even more money out of me when this isn’t my fault the car is junk. For safety concerns I would like someone to call me at [XXX]. This car needs to be recalled and my daughter has one. It’s doing the same thing except hers is a lot worse. Hers is further advance where it’s actually stalling in the middle of the road while driving huge safety violation. Incident Description: On [XXX], I took delivery of a brand-new 2026 Harley-Davidson Road Glide from a certified franchise dealership. Within 15 to 20 minutes of departing the dealership, under normal riding conditions on a well-maintained roadway, the motorcycle suffered a sudden, catastrophic mechanical failure. I observed a large cloud of white smoke trailing heavily from the motorcycle. I immed
... iately pulled over to the shoulder and shut the engine down. Upon inspection, the entire right side of the motorcycle was saturated in motor oil. The oil was actively ejecting from the air cleaner housing and rapidly accumulating on the ground in a large puddle. Safety Hazard & Impact: The volume of motor oil ejected from the air cleaner assembly created an immediate, severe traction hazard. Because the air cleaner is situated on the right side of the motorcycle directly upwind of the rear tire path, the atomized and pooling oil thoroughly saturated the right side of the chassis. Continued operation would have inevitably coated the contact patch of the rear tire, introducing an incredibly high probability of an unrecoverable rear-wheel slide (low-side crash), leading to catastrophic vehicle damage and severe bodily injury or death. Resolution & Dealer Action: The motorcycle was recovered by the dealership. Due to the immediate mechanical safety failure and discrepancies with delivery mileage, I demanded the bill of sale be rescinded and my financial assets/trade-in vehicle returned. The dealership reluctantly complied and unwound the deal, but they completely refused to provide a mechanical explanation or diagnostic report for why a brand-new vehicle suffered this severe oil-expulsion failure. The front exhaust pipe on my 2022 Subaru Ascent cracked at approximately 77,800 miles. The component is available for inspection. Prior to diagnosis, I noticed a rattle/fluttering noise coming from the undercarriage during acceleration. The problem was confirmed by an authorized Subaru dealership. Subaru has issued TSB 09-106-23 acknowledging this is a known design defect affecting 2019-2023 Ascen
... ts, and has redesigned the part to correct the issue. Despite being within the 8 year/80,000 mile federal emissions warranty period, the dealership denied warranty coverage, claiming coverage only applies in California and Maryland. This is incorrect - the federal emissions warranty applies nationwide. The dealership quoted over $1,000 out of pocket for a known manufacturing defect on a 4 year old vehicle. The cracked exhaust pipe poses a safety risk as exhaust fumes can enter the vehicle cabin. I am requesting NHTSA to investigate whether Subaru is improperly denying federal emissions warranty claims on affected vehicles outside of California and Maryland.Read more
