While driving my 2022 Honda CRV at highway speeds and at operating temperature, the steering sticks and/or pauses then releases causing the steering to jerk back in the opposite direction. This creates a major safety issue while driving on the highway and can potentially cause an accident. Additionally, when turning the vehicle left at slow speeds, we would periodically hear a clicking noise. Thes
... e problems began at the beginning of 2024 and have progressively gotten worse. Initial attempts to have Honda identify the problems failed as they could not replicate the problems at the time of diagnostics. During this time, our warranty expired. On May 20, 2026, we once again had the vehicle inspected by Pohanka Honda of Salisbury, MD for the same issues. This time the lead mechanic technician was able to replicate and pinpoint the problem. He noted the problem was the "Electrical Power Steering System Assembly and Gearbox Assembly", which contained the worm gear and worm wheel assembly. The dealership quoted us a price of $2,153.00 for parts, labor, and a full alignment, which is required during this type of repair. The service technician advised there are no current recalls to cover the cost of this repair. The Honda Corporation has suggested we apply for their “Goodwill Assistance Program”, to help cover cost for out-of-warranty repairs, but there was no guarantee that we would qualify for any monetary relief. We believe Honda Corporation is ignoring a major defect and safety issue regarding the electrical power steering assembly and gearbox, on older CRV models. This same part has been deemed as defective and a major safety issue on newer CRV models between 2023-2025 and has recalled those vehicles for repair. I believe the company should be required to recall all vehicles with the defective electrical power steering assembly and gearbox. Fortunately for us, Honda made the repair and covered the full cost through the Goodwill Assistance Program.Read more

At approximately 87,000 miles, my 2021 GLE350 (M264 2.0T engine) experienced a severe engine misfire resulting in a flashing check engine light, rough idle, engine shaking, significant loss of power, and near-stall condition. The vehicle was taken to Speedhaus Automotive (Newbury Park, CA) where all four ignition coils and spark plugs were replaced. Total repair cost was approximately $635. This f
... ailure pattern is consistent with the documented M264 exhaust valve seat wear issue acknowledged by Mercedes-Benz technicians. A Mercedes dealer service report on an identical 2021 GLE350 explicitly states: "COMMON ISSUE FOR EXHAUST VALVE SEATS TO WEAR — KNOWN ISSUE WITH 260 AND 264 MOTORS." Mercedes-Benz has extended the warranty on the M260 engine (transverse variant) for this identical failure mode but has not extended it for the longitudinally-mounted M264 despite the same defect being present. Repair costs for full cylinder head failure on the M264 are being quoted at $6,000–$13,000. I am requesting NHTSA investigate whether a safety recall is warranted for the M264 engine across all affected GLE350, GLC300, C300, and E350 vehicles.Read more

A major engineering and material design failure occurred involving internal fluid containment seals, resulting in a severe electrical safety hazard. Pressurized engine coolant breached an internal component seal and began migrating internally directly through the vehicle's engine wiring harness via capillary/siphoning action. The fluid used the sealed wiring loom as a literal pipeline, traveling
... inside the wire insulation to bypass all outer weather seals and completely flood the Engine Control Module (ECM) pin connectors. The vehicle's dealership service department documented extensive galvanic corrosion and shorting inside the primary vehicle computer pins due to this blue-green fluid pooling. Because the dealership has not yet completed the full repair, the exact originating component source of the leak (e.g., the engine coolant temperature sensor or the coolant expansion tank housing barrier) remains unconfirmed. However, the physical mechanism of the coolant migration through the sealed harness into the primary electrical architecture is fully verified. This defect presents a critical safety risk. The sudden flooding and short-circuiting of the Engine Control Module can cause an immediate and unexpected loss of engine propulsion, power steering assist, and electrical control while driving at highway speeds, creating an imminent risk of a high-speed collision.Read more

My 2019 Mazda CX-5 (equipped with the 2.5L naturally aspirated SkyActiv-G engine with cylinder deactivation) has experienced a severe structural failure resulting in a cracked engine cylinder head. This casting defect allows pressurized motor oil to leak continuously from the cylinder head down onto the hot engine block and nearby exhaust components. This has led to the following severe safety ris
... ks: engine oil is leaking onto the exhaust manifold and creates an extreme risk of engine compartment fire when the vehicle is in motion; sudden loss of oil pressure or complete structural failure of the cylinder head can cause the engine to stall or seize unexpectedly without warning, completely disabling the vehicle in live traffic and creating an imminent crash hazard; and dropping oil levels and leaking fluids burning against the engine produce strong smoke and chemical odors that enter the cabin, which can cause driver distraction and severely limiting visibility if smoke escapes through the hood. We are also inhaling these odors. Mazda is fully aware of this structural engine defect and has detailed it internally under Technical Service Bulletin TSB #SA-031-21 (as well as related TSBs for oil seepage/leaks on these cylinder heads). Despite acknowledging the manufacturing flaw, Mazda has failed to issue a safety recall for this dangerous fire and stalling hazard, forcing consumers to drive unsafe vehicles or pay thousands of dollars out of pocket for a known defect. The vehicle currently has 62,317 miles. The crack and oil leak were officially diagnosed by the Carlsbad Mazda dealership service department on 5/26/26. I am requesting that the NHTSA launch a formal safety defect investigation into Mazda 2.5L cylinder head cracking to force a safety recall before this defect leads to a fatal fire or highway accident.Read more

Initial Breakdown: July 2024, my 2021 Kia Soul (under 40,000 miles) idled roughly and stalled while in motion. It restarted, but as a precaution, I towed it to Kia Greenbrier Chesapeake while it was operable. After diagnostics, the vehicle would no longer start. ... , I appealed the decision but was denied (Exhibit A & B). There were no recalls active for the 2021 model at this time, but there were recalls for the 2018-2020 models for identical symptoms. Third-Party Repair: I paid $3,017 out of pocket to an independent shop for a new timing assembly to make the vehicle safe and drivable again (Exhibit C). Because my daughter is disabled and requires transportation, getting repairs was imperative. A new valve kit, fresh synthetic oil, and filter service were completed by the third-party repair shop. I also returned to Kia Virginia Beach for several subsequent oil changes (Exhibit D). In April 2025, Kia Corporate issued an official safety recall for my exact vehicle (Exhibit E). Before that, in March 2025, I attempted to have the newly issued recall (25V-099) performed, but the dealer turned my vehicle away, claiming a lack of parts. Final Failure: In May 2025, the night before a rescheduled recall appointment, the car became permanently inoperable (at only 47,500 miles). Due to extreme financial hardship and health concerns, I am without my vehicle and without an option. Complaint: The Kia dealership refused to perform the safety recall remedy, verbally citing the same 2022 maintenance gaps from the previous year. The dealership violated federal recall regulations for a known defect. I was left stranded, resulting in a financial charge-off of my auto loan.Read more

This make and model Honda with a 2.4L engine have several known defects which brings potential harm and high operational and repair costs to its owners. Honda is well aware of the engine problems but refuses to issue recalls to correct the problem. Two of the major problems are: 1. due to a design error, the engine burns excessive amounts of oil, and 2. Honda installed a defective VTC Actuator i
... n this engine. Either problem can cause the engine to suddenly sieze/fail which could lead to serious injury or loss of life. I purchased a used 2014 Honda CR-V from a Honda dealer in 2022 with about 50,000 mi. After a short time owning the vehicle I realized that the engine was burning excessive oil (about 2 quarts every 2 to 3 weeks). When I discussed this issue with my local Honda dealer, they told me that according to a Honda Service Bulliten, my car was not losing oil at a rate that warranted repair. I've owned around 10 Hondas in my life and I've never had a car burn oil where I had to add ANY oil between oil changes. So, burning multiple quarts of oil a month is EXTREMELY UNUSUAL and should have been repaired by Honda! The 2nd problem, defective VTC Actuators, actually may be related to the 1st problem due to a constant fluctuation of the oil level, and lack of oil. The defective VTC Actuators lead to a stretched/worn Timing Chain which can cause engine failure at any time and any place. After experiencing a loud rattling coming from my engine on start-up for some time, I just had several dashboard lights come on. Running a scan on the vehicle it showed a P0341 code. I took my car to my local Honda Dealer where they informed my that I had to replace the stretched Timing Chain and failed/defective VTC Actuator at a cost of over $2600. I believe that fraud was committed by Honda from the day I purchased the vehicle as Honda was aware of both significant engine problems and did not inform us a unexpecting Buyers.Read more

1. The engine architecture suffered a catastrophic mechanical failure. Specifically, severe, systemic internal oil consumption/vaporization starved the hydraulic timing chain tensioner of critical hydrostatic pressure. This caused the tensioner to collapse, creating slack in the steel timing chain, which then "jumped time" (skipped a tooth on the camshaft sprockets), causing the engine to instantl
... y seize and lose all motive power. The vehicle and engine are fully intact and available for inspection at Pohanka Hyundai of Fredericksburg. 2. The vehicle suffered a sudden, complete loss of motive power and engine seizure while being driven at night on an unlit, dark secondary roadway. The driver (my daughter) was left suddenly stranded without power in a highly vulnerable, pitch-black location with passing traffic, presenting an immediate collision hazard and a severe personal safety risk before emergency assistance could arrive. 3. Yes. The vehicle was towed to an authorized Hyundai dealership (Pohanka Hyundai of Fredericksburg), where service technicians officially diagnosed that the engine had "jumped timing" and suffered a complete failure of the timing system, alongside a noted lack of engine oil volume due to vaporization. 4. The vehicle has been formally inspected and diagnosed by the authorized Hyundai dealership service department under Hyundai Motor America Corporate Case #[XXX]. No police or insurance inspections were required as no collision occurred. 5. No dash lights or warnings before failure. However, for a year prior, the engine exhibited severe internal oil vaporization, requiring constant top-offs between scheduled oil changes to maintain safe levels. There were zero external fluid leaks on the engine or ground; oil burned entirely internally. This chronic volumetric oil loss directly caused the sudden drop in pressure, hydraulic tensioner collapse, and catastrophic engine seizure. What component or system failed or malfunctioned, and is it available for inspection upon request? The Engine Control Module (ECM) is malfunctioning, triggering persistent P0101 codes related to the Mass Air Flow (MAF) sensor. This specific issue is addressed in Nissan Technical Service Bulletin DE97AA, which recommends an ECM reprogramming. The component is still installed in the vehicle and is a
... vailable for inspection. ⸻ • How was your safety or the safety of others put at risk? When the vehicle is at a complete stop and I attempt to accelerate, it becomes jumpy and unresponsive, refusing to accelerate properly. This creates a serious safety hazard, especially when trying to merge into traffic or cross intersections, as I am unable to match the speed of other vehicles. This increases the risk of being rear-ended or causing an accident due to unexpectedly slow acceleration. ⸻ • Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. An independent mechanic diagnosed the issue and confirmed the code P0101. After reviewing Nissan’s official TSB DE97AA, they advised me to go to a Nissan dealership to have the ECM reprogrammed as outlined in the bulletin. ⸻ • Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by an independent mechanic, but not yet by the manufacturer. The dealership is aware of the issue but has refused to perform the repair without payment. ⸻ • Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The Check Engine Light is consistently triggered by code P0101, and the traction control warning light also comes on when the issue is active. The symptoms appeared recently and continue to recur even after the code is reset by standard non-dealership methods like unplugging the battery.Read more

I own a 2022 Nissan Rogue with approx 71,139 miles on. While driving on the freeway at 65mph, the vehicle suddenly lost power and became disabled. Prior to the failure, the vehicle did not dispaly an overheating warning, coolant warning, or other indication the engine was overheating or experiencing coolant loss. The vehicle was being driven normally and was on the way to routine maintenance and f
... uel stop when the failure occurred. An independent repair facility diagnosed coolant intrusion into the combustion chamber and determined the engine had overheated and required replacement. The vehicle's sudden loss of propulsion occurred without warning creating a safety hazard for myself and my daughter as I was forced to coast to a stop in the Gore Area of the freeway just as you entered the [XXX] from [XXX] due to the inability to maintain speed on the freeway. This was the safest area for us to park but it was terrifying as cars zoomed past us from both sides. I am concerned that the engine failed without adequate warning to the driver and that similar complaints have been noted by several others describing the same issue with the same year, make and model as my car. Multiple reports have said it occurred while on the freeway, making this a greater concern for safety. I took my car to Nissan who stated they see the coolant intrusion, but that it was not covered under recall. I asked what caused the internal leak, to which they replied that would require them to take apart the engine to examine further at my cost. I advised the issue with the car not indicating it was even out of coolant to cause it to overheat was concerning because even if I replace the engine, if the electrical failure isn't addressed I could run into the same issue and this time actually get into an accident that could cause death or serious injury. I am submitting this complaint regarding an apparent head gasket failure involving my 2022 Honda Accord Sport 1.5T equipped with Honda's 1.5-liter turbocharged engine. At approximately 143,000 miles, I began experiencing coolant loss without any visible external leak. I also noticed rough operation and shaking during startup. Concerned about the symptoms, I consulted a Honda specialist who advised
... that the vehicle is exhibiting signs consistent with a failing head gasket and will likely require head gasket replacement. This vehicle has been meticulously maintained and serviced throughout ownership. I retain maintenance records and have taken exceptional care of the vehicle. There has been no abuse, neglect, overheating event, or failure to maintain the vehicle. Through my research, I have found numerous reports from owners of Honda vehicles equipped with the 1.5-liter turbocharged engine describing similar symptoms, including coolant loss, startup misfires, rough cold starts, coolant intrusion into cylinders, and premature head gasket failures. The consistency of these reports suggests a possible design or manufacturing defect affecting the durability of the engine. A failing head gasket can lead to loss of engine power, engine damage, misfires, overheating, and potentially unsafe driving conditions. Given the volume of owner complaints involving Honda's 1.5-liter turbocharged engines, I request that NHTSA investigate whether a safety-related defect exists and whether corrective action, including a recall or service campaign, is warranted.Read more

I was travelling on the highway at approximately 72 miles an hour in the left lane trying to pass a vehicle in the right lane. while accelerating my truck was like the brakes were slammed on and all kinds of lights starting flashing. This included the check engine light, low oil pressure light and lane assist lights. The truck shifted into Neutral and I could not place back into drive. I swerv
... ed and almost lost control of the vehicle and nearly missed colliding with the safety rails on the left of the road and then with the car on the right side. I then pulled onto the shoulder after nearly being rear ended by another vehicle approaching on the left. The vehicle would not react to the gas pedal. I pulled onto the shoulder and turned the vehicle off. Tried to restart but again nothing but mutliple lights and warnings. I checked my app for Chevrolet and it stated that the vehicle needed engine and transmission system service. Variable valve timing system needs serviced. Ingition system needed serviced. The onstar diagnostic alert indicated P0016 was detected. I opened the hood to inspect and the coolant overflow was spewing coolant. I checked the oil level at that point and it was just a little below full. I had taken this vehicle into the dealership two other times stating that the vehicle had stalled before on the highway but not to this degree. The other two times it stalled when accelerating it would allow me to pull over and restart. But both of those times I never had any flashing lights or diagnostic codes. I explained this to the dealership and how unsafe it was. Both of those times almost ended in a vehicle accident with other vehicles around me. The dealership said they could not diagnose any issues unless the truck created a diagnostic code. These issues have been ongoing with these vehicles for a long time and the dealership just did not want to deal with them. I had the vehicle go thru the recall diagnostic tests. It did not fix themRead more

It is commonly referred to as the Chevy thump and studder and a known mfg defect in which GM refuses to pay for repairs causing consumers an extreme hardship! This is an ongoing issue affecting thousands of vehicles and someone is going to die! There is no negligence of operator of vehicle, it is a manufacturing defect that affects thousands of GM vehicles and they are doing NOTHING about it except expecting customers to keep incurring cost to try to remedy a repair at our own expense which is unfair! On [XXX], my 2018 Kia Soul (VIN [XXX] ) displayed DTC P1326 with a blinking check engine light and went into limp mode. Ray’s Automotive documented P1326 in writing, added oil, and cleared the light only to allow safe transport. At Greenway Kia (Avenues), they charged $199 and refused to create a Vehicle Diagnosis Number (VDN) or perform the Bearing Clearance Test (BCT) unless they could reprodu

This directly contradicts Kia TSB PI2107Y/Z (Rev 1, 11/07/2024), which states: “A Vehicle Diagnosis Number (VDN) must be created with or without DTC P1326, after scanning for DTCs, prior to performing PI2107Y/Z.” Ray’s found low oil, but my next scheduled oil change was still 300 miles away. I refused to accept the vehicle back because limp mode is unsafe for my daughter (primary driver). The dealer recently told me to pick it up anyway. Kia Corporate opened Case #[XXX] after 2 separate 2-hour Kia recorded calls on June 1 and June 3, 2026, but continues to refuse the required diagnostic. This is a known manufacturing defect covered under Kia Product Improvement Campaign PI2107 and the class-action engine settlement. The refusal creates a serious safety risk for my daughter and family. Requested Action: Require Kia to follow the published TSB (VDN creation and Bearing Clearance Test with or without active code) and provide a rental vehicle.
