2018 Honda Accord EX-L with the 2.0L turbocharged engine. At approximately 113,000 miles the head gasket failed, allowing engine coolant to leak into cylinders 2 and 3. Symptoms included repeated cold-start engine misfires (diagnostic codes P0302, P0303, P0300), illuminated check-engine light, intermittent loss of power / limp mode, and gradual loss of coolant with no visible external leak. The fa
... ilure was confirmed by an independent mechanic as a blown head gasket between cylinders 2 and 3. This matches a widely reported design defect in Honda's 1.5L and 2.0L turbocharged engines (2016–2022 Accord, Civic, CR-V, Acura RDX/TLX) that is the subject of federal class-action litigation. The sudden power loss this defect can cause while driving presents a safety risk. Repair cost approximately $6,000, paid out of pocket, with no recall in effect. My 2025 Kia Telluride has experienced seven engine overheating events across three separate incidents, all under identical conditions: air conditioning on while climbing in altitude on Colorado mountain highways. Incident 3 (XXX): Driving to [XXX]. Vehicle overheated FOUR times during the trip, including in an area with no cellular reception. Occupants included an infant and a [XXX]. Had the engine failed completely, we would have been stranded with no ability to call for emergency assistance. The failure is consistent and reproducible in real-world conditions (sustained grade + AC load) but has not been reproduced by dealership diagnostics performed at the shop. No fault codes were reportedly stored. The pattern is consistent with a cooling system deficiency under high load (cooling fan circuit, condenser airflow, or thermostat), and similar complaints from other Telluride owners describe the same scenario with dealers unable to replicate. Safety concern: repeated loss of propulsion capability on steep mountain grades, risk of engine failure and stranding in remote areas without cell coverage, with young children aboard. My 2021 Jeep Wrangler 3.0L EcoDiesel experienced a failure of the engine oil cooler that allowed engine oil and engine coolant to cross contaminate. The oil cooler on this engine sits between the oil and coolant passages, so an internal failure of that component mixes the two systems. The failure first occurred in September 2025 while the vehicle was in use, and coolant intrusion into the oil syst
... em has recurred in 2026. This condition is a safety concern because it can cause engine overheating and sudden loss of engine performance or engine failure while driving. On one occasion the vehicle overheated within approximately one mile of leaving a service facility. Low oil pressure was also observed (as low as 9 psi), which risks engine damage and loss of power in traffic. The manufacturer (FCA US LLC / Stellantis) initial XXX November 7, 2025 the arbitrator ruled in my favor, finding the oil cooling system failure "was more likely a result of a Manufacturer defect" and that my modifications "did not cause the oil cooling system failure," and ordered the manufacturer to repair it. FCA's own [XXX] owner dashboard showed the Powertrain Limited Warranty active (expiration 10/07/2026 or 100,000 miles) as recently as May 18, 2026; yet by XXX a Stellantis dealership reported the vehicle under a complete vehicle restriction and refused to diagnose it, leaving a known engine defect unrepaired. I am reporting this so the potential safety defect in the EcoDiesel oil cooler and the resulting engine overheating and failure risk are on record, and because the manufacturer has not repaired a defect that an arbitrator determined to be a manufacturing defect. June 28, 2026 (90,351 miles): Audi MMI system showed oil near minimum after only 151 miles. June 30, 2026 (90,397 miles): Audi MMI showed oil at Stage 2 warning after 197 miles. June 30, 2026 (90,430 miles): Audi MMI showed oil at minimum line after 230 miles total. All readings documented with timestamped photographs from Audi's own MMI system. The authorized Audi dealer has documented repeated oil top-offs in their service system. Audi USA Customer Care was contacted on June 30, 2026 and denied goodwill repair citing no active recall and vehicle out of warranty. An open case has been filed with Audi USA. The engine lubrication system appears defective. Oil dropping to minimum in under 230 miles without adequate warning poses serious safety risk of catastrophic engine failure while driving at highway speeds. The engine oil level has been gradually dropping and has required monitoring and/or adding oil between normal service intervals. The exact source is unknown. It may involve abnormal oil consumption, an external oil leak, internal engine oil consumption, PCV/oil separator issue, turbo-related oil consumption, gasket/seal issue, or oil loss that is not leaving an obvious visible puddle. This is bei
... ng reported as a safety concern because low engine oil or abnormal oil loss can lead to low oil pressure, warning messages, engine overheating or damage, smoke, oil contacting hot engine/exhaust components, limp mode, stalling, or loss of propulsion while driving. The issue should be inspected rather than treated as normal maintenance unless a proper diagnosis confirms normal operation. No crash, fire, injury, or fatality occurred. Yes, the truck is available for inspection. 2)This issue has happened to me on five occasions
... where I was traveling over 50 MPH and the vehicle violently jerked with my wife and children in the truck with me. This defect poses an immediate, severe safety hazard due to unpredictable, sudden engine/transmission jerk while operating at highway speeds. My main priority is my family! This failure compromises my ability to safely maneuver, accelerate, or steer the heavy-duty vehicle out of active lanes of traffic, dramatically increasing the risk of a high-speed rear-end collision or crash. 3) The problem has been reproduced by Rick Hendrick Chevrolet Norfolk and R.K. Chevrolet Virginia Beach. There are service invoices confirming these reproducible jerks and subsequent engine codes thrown. 4) No, I brought this up to General Motors as a safety issue but they have brushed off my concerns. 5) The second time I had the violent jerk I had a message appear on my DTC that stated "174 mi Until 65 MPH Max Speed and Service Emission System See Owner's Manual" that was on 2/21/2026. Extra Note: The vehicle sat disabled at a dealership service center for 21 cumulative days during the initial attempt to repair this specific defect under TSB #24-NA-012. Merely 58 days after the repair, the identical crankshaft position sensor circuit/wiring failure reoccurred while driving, resulting in secondary engine failure and requiring a visit to an independent service facility for diagnostic verification and wiring replacement. Internal vehicle history confirms that prior to my truck ownership the crankshaft position sensor was replaced in December 2025 weeks before I purchased the truck.Read more

While driving my 2024 Toyota Tundra Hybrid, the engine suffered a catastrophic seizure at approximately 37,974 miles. The dealership retrieved substantial metal machining debris (swarf) from inside the engine, which had contaminated the #1 main bearing. This is the exact same manufacturing defect Toyota has publicly acknowledged in multiple safety recalls for 2024 Tundras. Service Manager Dan Mor
... an at Toyota of Colorado Springs confirmed during a recorded call that his dealership has seen the identical debris-related engine failure in other hybrid Tundras. Despite this, Toyota excludes all hybrids from the recalls. Even with clear physical evidence matching the recalled defect, Toyota directed an in-house rebuild (using a new short block but many of my original contaminated components) instead of providing a full factory crate engine. I was never offered a choice in the remedy. This sudden engine failure created a serious safety hazard due to unexpected loss of power. The current repair does not fully eliminate the known root cause (manufacturing debris contamination). The vehicle was well within warranty at failure and remains so. The dealership has the debris and full repair records available for inspection. I have also submitted the debris photo, repair documents, and recorded call summaries to NCDS (Case #[XXX]) and the Colorado Attorney General’s office. No warning lights, oil pressure alerts, or dashboard notifications appeared before or during the failure. The vehicle had operated well since purchase with no prior issues of any kind. I was to cross the hwy onto the shoulder, this was a serious safety haza
... rd for myself and other motorists. Land Rover is holding me liable for $32,000 in repairs, claiming a maintenance interval overage of ~36,000 miles caused the failure. No written root cause analysis has been provided. I am disputing this for four reasons: 1. RECALL N763 / N763v3: This recall covers a camshaft carrier oil gallery defect on 2023-2024 Velar engines causing oil leaks and engine damage. My VIN falls outside the listed range, but my failure is fully consistent with this defect. I request evaluation for recall expansion. 2. OIL FILTER HOUSING TSB: A TSB for 2023 Velar Ingenium I6 3.0L engines documents oil filter housings cracking and causing high-pressure oil leaks — a second documented oil system defect matching my failure. 3. NO WARNING LIGHTS: Recall N763 states affected vehicles display an oil pressure warning before engine damage. No warning appeared on my vehicle before complete engine failure at highway speed — indicating either the warning system failed, or the failure was too sudden and structural to trigger it. Both are safety risks. 4. MANUFACTURER BAD FAITH: Land Rover attributes this failure to maintenance neglect with no written diagnostic evidence, despite a documented federal recall covering the same defect pattern on the same engine family. I request NHTSA: - Evaluate my VIN for inclusion in expanded Recall N763; - Investigate whether the warning system functioned as designed; - Investigate if Land Rover is systematically using maintenance neglect claims to deny recall-related repairs outside the current VIN range.Read more

I own a 2018 Chrysler Pacifica equipped with the 3.6L Pentastar V6 engine. At approximately 81,600 miles, the vehicle developed a check engine light and a Cylinder 5 misfire (P0305). The vehicle began exhibiting hard starts, rough engine operation during cold starts, white smoke from the exhaust, and coolant loss with no obvious external leak. An independent repair facility initially diagnosed the
... vehicle and found evidence suggesting coolant intrusion into Cylinder 5. The vehicle was then inspected by a Chrysler dealership. The dealership confirmed that coolant was present inside Cylinder 5 using a borescope inspection. The dealership recommended replacement of the cylinder head gasket. The safety concern is that the failure developed with little warning other than a check engine light and intermittent rough operation. The engine misfires on startup and can run rough, which could result in reduced power, hesitation, stalling, or loss of acceleration while operating the vehicle. The vehicle also consumes coolant internally, which may lead to engine overheating or further engine damage if not detected quickly. During my research, I found numerous reports from other owners describing similar coolant intrusion, head gasket failures, and Cylinder 5 misfires in Chrysler vehicles equipped with the 3.6L Pentastar engine. Despite the apparent frequency of these complaints, I am not aware of any recall addressing this issue. In my case, the repair estimate is approximately $5,000 for head gasket replacement. I am submitting this report because coolant intrusion into a combustion cylinder can create a significant safety concern due to engine misfires, loss of performance, and the potential for engine failure while driving. I believe this issue warrants investigation to determine whether a defect exists affecting other vehicles with this engine.Read more

The vehicle experienced a coolant intrusion failure involving the 2.0L EcoBoost engine. While driving, the engine began to misfire and run rough, and the check engine light illuminated. Diagnostic testing confirmed a cylinder misfire. A dealership inspected the vehicle and confirmed that coolant was leaking internally into one or more cylinders due to engine coolant intrusion. The dealership advis
... ed that the manufacturer recommends replacing the engine short block rather than performing a head gasket repair because the underlying engine design is the cause of the failure. This condition creates a significant safety concern because the engine can suddenly lose power, run unpredictably, or stall while driving, increasing the risk of a crash, especially in traffic or at highway speeds. Continued operation also risks catastrophic engine failure. The condition has been confirmed by an authorized dealership and is available for inspection upon request. Prior to diagnosis, symptoms included rough engine operation, a check engine light, and a confirmed cylinder misfire. The coolant intrusion was verified during the dealer's inspection. Based on the dealership's findings, this appears to be an internal engine defect rather than normal wear or maintenance-related failure.Read more

Description of IncidentThe vehicle experienced a severe engine system defect and catastrophic internal mechanical failure. The primary symptom prior to the failure was excessive, rapid engine oil consumption between scheduled maintenance windows. The issue was officially confirmed by an authorized dealership service center, where diagnostic testing verified that the engine has critically low cylin
... der compression in Cylinder 1. The vehicle is currently completely incapacitated, and the disassembled engine block remains at the dealership facility available for inspection upon request. Safety Risk AssessmentA sudden and severe loss of internal engine cylinder compression creates a critical vehicle performance defect, an immediate stall risk, and a catastrophic hazard when operating on high-speed roadways. In this instance, the mechanical failure completely disabled the household’s primary, medically modified means of transport. Because the manufacturer and dealership failed to maintain or provide a structurally or layout-compliant alternative vehicle, a permanently disabled occupant was forced into a tight, non-accommodated cabin geometry. This directly induced severe physical trauma, muscle spasms, and an immediate loss of lower extremity neurological sensation while operating the replacement vehicle. The combination of a catastrophic powertrain failure and a total refusal by the dealer and manufacturer to provide an accessible replacement layout has completely stripped a vulnerable consumer of independent mobility and created a severe, ongoing physical safety hazard.Read more

I respectfully request that the Florida Department of Highway Safety and Motor Vehicles investigate Rick Case Honda for a repair I believe was performed negligently and the dealership's refusal to accept responsibility. On December 2, 2025, Rick Case Honda performed a cylinder head replacement on my 2023 Honda Civic before selling it at auction. I later purchased the vehicle on March 16, 2026, fro
... m Newton Nissan South, which had acquired it through the auction. Several months later, the engine developed an oil leak. Newton Nissan South diagnosed the leak at the front crankshaft seal using UV dye and referred me to Honda of Murfreesboro for warranty repair. During disassembly, Honda of Murfreesboro discovered a broken timing chain tensioner retaining bolt inside the engine. I met with the Service Director and a Honda North America Field Technical Specialist, who showed me the failed parts, including the broken bolt, broken tensioner, wear marks on the timing cover, and grooves in the engine block consistent with prolonged movement before the bolt failed. I was advised that this bolt requires only 7-9 ft-lbs of torque and that improper installation can cause delayed failure. Honda of Murfreesboro advised that the last repair involving removal of this component was Rick Case Honda's December 2, 2025 repair. Since then, no repairs have been performed in that area of the engine, only routine maintenance and diagnosis of the oil leak. Rick Case Honda has denied responsibility based primarily on the passage of time and changes in ownership, despite being the last dealership to perform this engine repair. I respectfully request that your office investigate whether Rick Case Honda failed to perform the repair according to Honda specifications and improperly refused responsibility for the resulting engine damage. I am seeking repair of my vehicle and any other relief deemed appropriate.Read more
