On 06/26/26, while traveling at highway speeds, the vehicle suffered a sudden, catastrophic loss of motive power in active traffic due to a blown out/melted Cylinder 4 ignition coil. This vehicle was previously serviced under NHTSA Campaign 21V264000 (Subaru Recall WRE-21), which was intended to permanently remedy an ECM software defect that over-energizes and melts the ignition coils. Subaru of America (Case # 260630-2101598) is denying coverage, claiming that because the recall was executed 90,000 miles/5 years ago, and because the current software Calibration ID (CID) matches their post-recall database, the thermal melting is standard "wear and tear." I am filing this complaint because a safety recall remedy must be permanent and effective. The physical evidence, a melted plastic coil housing with a hole blown through it, is an engineering impossibility under normal wear and tear and proves that the original software patch failed to permanently mitigate the high-voltage overloading defect. Over a 5-year period of gradual thermal degradation, the defect reoccurred, creating the exact highway hazard the recall was mandated to prevent. I request a federal investigation into the long-term efficacy and failure rates of the WRE-21 software reflash.Read more

While driving, my vehicle's check engine light came on and the vehicle entered limp mode without warning. It would not exceed 25 MPH and was stuck in 2WD, displaying the message "reduced engine power – take vehicle to nearest dealer." ... nosed trouble codes P219C00, P160500, P030027, and P0301300 — cylinder #1 air/fuel ratio imbalance, rough idling, random misfire, and cylinder #1 misfire. Through active injector testing, the technician confirmed the cylinder #1 direct (high-pressure) fuel injector has failed, and recommended replacing all four direct injectors at a quoted cost of approximately $6,600. The dealer's inspection report also cited Lexus service bulletin L-SB-0007-21 for possible worn exhaust valve guides, a separate documented defect in this engine that produces these same misfire codes and limp mode, with a repair (cylinder head and valve replacement) that can exceed $10,000. Lexus has known about these failures in the 8AR-FTS engine since at least March 2021, when it issued L-SB-0007-21 covering 2018–2021 NX 300 vehicles. The bulletin acknowledges affected vehicles enter limp mode. Despite this, Lexus has not issued a recall, and owners whose vehicles fail shortly after the warranty period expires are left paying thousands of dollars for a known defect. My warranty coverage lapsed shortly before this failure occurred, and Lexus corporate declined meaningful assistance. They charge me $179.00 just to confirm what they said about needing a new transmission. I was devastated and confused why I wasn’t told that the day before. So a few days later I took it to another machanic to see if the could see why the lights weren’t working and he ended up putting new bulbs in and got them working but also he put it in a diagnosis machine 2 of them and couldn’t get a reading at all and said I should take it back to find out why he couldn’t get a reading. I was so frustrated at that point I just was upset! I then had a friend of the family mechanic look at it and he said it wasn’t the transmission it was a knock sensor that I needed so I ordered it. When I called him to install the knock sensor he was too busy to do it and it had already been a whole month of waiting so I reached out to a place in Cumberland RI to see if he could install the knock sensor had it towed there and he kept it for three days and said it was a transmission computer and he tried two of them and neither of them worked and charged me $150 for that I was humiliated and crying , I went there to have my car towed home seen him while waiting for the tow truck to come he said I needed a new transmission and now a motor, crying again. I am requesting that NHTSA review this incident as a potential recall exclusion problem: owners with the same engine family and serious internal failures are being denied recall coverage on a technical VIN cutoff, even though the safety consequence—loss of power and shutdown of critical safety systems while driving—is similar to the failures already recognized under Recall 23V751 and the ongoing engine defect investigation. A vehicle that loses power or stops unexpectedly at intersections creates a risk of collision and puts myself, my passengers, and other motorists in danger. This issue has occurred multiple times and has required repeated service attention. The vehicle is scheduled to be inspected again due to the ongoing powertrain failure concern. Despite significant repairs, including a complete engine replacement, the underlying issue appears to remain unresolved. Prior to the failures, the vehicle displayed warning messages related to the powertrain system. The symptoms have included reduced acceleration, hesitation, loss of power, and the vehicle stopping unexpectedly while driving. The vehicle has been inspected by Ford service providers, including the prior engine replacement performed in May 2025. My vehicle ran perfectly fine and smooth before this update was applied and no CEL lights are on. I did this update because it was required to be done by Hyundai. When I started researching why my vehicle was hesitant/jerky after the update is when I found that anyone that did this update is having these same issues, and it has been happening for a while now and Hyundai corporation has done nothing to correct this problem other than tell us to sit for at least 3 minutes or more before driving it. GM was involved in approving the engine replacement under warranty. To my knowledge, the vehicle has not been inspected by police or insurance representatives. While all of this was happening, I was passed unsafely by several angry drivers who were behind me, including one who tried to cut around me after I was able to get the car restarted, nearly clipping the front-right corner of my vehicle. I have an appointment with the dealer to have the issue investigated, but the appointment is not for another week. There was no indication of trouble prior to the first time this occurred. I had completed all recommended maintenance prior to the failure. 500 miles after that repair was completed, my transmission overheated and completely left me stranded on the side of the road. On March 5th, 2026, when I went to pick up my car, they told me the bad news that is a well known manufacturer defect within the industry regarding these vehicles. They used plastic components in the engine compartment and in the cooling system, which makes the vehicles malfunction. From their report, with pictures, they state the following repairs need to be made, "During the vehicle inspection noticed active coolant leaks are present from multiple locations. The radiator plastic tank actively leaking coolant. The water pump change over valve leaking coolant. The turbocharger coolant line has a coolant crud build up . Oil filter housing and coolant flange on the cylinder block are discolored. For proper cooling system repair recommend complete update of the cooling system. Both balance shaft end caps leaking oil. Recommend replacement during cooling system update." They quoted me $8,914.86 for this repair. That's over HALF the value of the vehicle. They said the entire system needs to be replaced because ALL of the plastic parts in Florida's hot sun will fail. Sudden coolant failures in FL = DEATH.Read more

... ing, the engine began running extremely rough with severe vibration and shaking. The Check Engine Light began flashing, and Service StabiliTrak and Service Traction Control warnings appeared. A few minutes after I stopped the engine, it would not restart. I found no measurable engine oil on the dipstick. When the engine was started again, a large amount of blue smoke was emitted from the exhaust, while the engine continued running extremely rough. GM's official MyCadillac application displayed critical warnings for the Engine & Transmission System, Emissions System, Transmission System, and Variable Valve Timing (VVT) System. GM ultimately determined that the engine required complete replacement under warranty. Despite completion of GM Safety Recall N252494000 (NHTSA Campaign No. 25V-274), the engine required complete replacement only approximately 2,347 miles later. The replacement engine remains on backorder. I understand that NHTSA has opened Recall Query RQ26001 regarding post-remedy L87 engine failures. Because my vehicle experienced a post-remedy engine failure only approximately 2,347 miles after completion of the recall repair, I respectfully request that this complaint be included as part of Recall Query RQ26001. In November 2025, after the brake pedal became hard, an independent repair facility inspected the vehicle and determined the brake problem was caused by a failing engine vacuum pump. They advised that the vehicle be repaired by a Chevrolet dealership. By January 2026, the brake condition had worsened to the point that the vehicle was no longer safe to drive and had to be towed. A Chevrolet dealership later confirmed that the engine vacuum pump had seized and damaged the camshaft, requiring replacement of both components. The vehicle remains inoperable. The failed vacuum pump caused loss of brake assist, resulting in a hard brake pedal, reduced braking ability, and an increased risk of a crash. I believe this represents a serious safety defect because failure of the vacuum pump significantly impairs the driver’s ability to stop the vehicle safely. I understand GM has issued service information regarding vacuum pump failures on this engine, and I am submitting this complaint because I believe other drivers may experience the same dangerous brake failure.Read more

On 17FEB2026 I took my vehicle into my Chevy/GMC Dealership to request an oil consumption test at ~35, 089mi. On 30JUN2026 I returned to the dealership for my 2000mi check-in, at ~37,800mi, and was informed that my vehicle had failed the consumption test. With over 600,000 vehicles recalled since 2024 in the 6.2L category, my vehicle/VIN has never been associated with a recall. Despite having minimal miles on this engine, I am now financially penalized for repairs because I had just exceeded the initial Powertrain Warranty and my extended warranty would not cover a pre-existing condition. GM’s 6.2L engines have pre-existing issues from the factory and represents a danger to consumers and the driving public because of these failures.Read more
