I am reporting a severe vehicle safety hazard involving a sudden loss of motive power and extreme thermal failure on my 2012 Hyundai Equus Signature 5.0L. ... ginal mechanical breakdown—the servicing dealer chose to only flush the lines, creating a latent fluid restriction. After approximately 18,000 miles of operation, this restriction completely choked the transmission fluid pressure loop. The extreme thermal stress and back-pressure caused the internal walls of the transmission cooler to suffer a catastrophic structural rupture. This caused the transmission to suddenly overheat and fail while driving, creating an immediate highway safety risk and a total loss of vehicle control. Furthermore, the internal rupture of the cooler walls has caused direct fluid cross-contamination ('SMOD'), forcing transmission fluid and metallic shrapnel directly into the engine's main radiator and cooling passages. This metal shrapnel is currently circulating through the engine's cooling network, creating a severe secondary hazard of catastrophic engine seizure and thermal runaway while traveling at highway speeds. The manufacturer has been notified but has formally refused in writing to dispatch a field engineer or authorize a safety inspection of this cross-contaminated powertrain network.Read more

It appears the software is designed not to store the code for intermittent issues by design. First incident documented along with the bad injector causing cylinder misfire at dealership on 5/18/26. I have video of this inspection sent by dealership to me on this date as evidence. Car taken to dealership again on 6/19/26 with same vibration and error codes. Only software update approved by Honda. 4th of July car began to shake and again provided multiple codes. By the time the dealership was open on 7/6/26 the computer showed no codes and Honda would not authorize repairs under warranty. In the video, the dealership admits to finding a serious problem and they verify it, but they claim they cannot fix it unless the computer gives them a code. We were told Honda programed the computer to not store the error codes which appeared indicating to contact the dealer. I have photos of all and they were submitted to the dealer but it is not enough for Honda to honor the warranty or to fix a known safety issue.Read more

I returned the vehicle to the dealership (Wallace Hyundai), where the service advisor personally test drove the car with me and confirmed he could feel the hesitation firsthand. Despite this, the dealership stated no issue was found and advised warming the car up for 1-3 minutes before driving as a workaround. I tested this exact recommendation, and the issue returned even after the vehicle was fully warmed and had already been driven. Hyundai Motor America's National Consumer Affairs department has stated this is "under review" but has also described the hesitation as "normal" cold-start behavior related to catalyst warm-up. This does not match what I am experiencing, since the issue also occurs when the vehicle is already warm, and their own dealership's advisor confirmed the problem exists. I have also noticed a decrease in fuel efficiency (MPG) since the software update associated with this recall. This issue directly affects the safe operation of the vehicle — specifically the ability to accelerate reliably, including in situations like merging onto highways or pulling into traffic, where a lack of power could result in a collision. ... s. Yesterday received call from dealer, stating that engine needed total replacement due to "lack of oil". Quoted repair cost estimated at approximately $8,000. Car has been regularly serviced, including oil and filter changes per handbook. I have all records for the services. The were no indications of low oil pressure or engine overheating, no warning lights, all gauge readings displayed normal ranges. This same car had a failed transmission replaced approximately 2 years ago, which was covered by an extended warranty related to a recall. There have been 2 engine failure recalls as a result of 2 class action lawsuits for Kia cars with this same engine failure up through the 2019 model years, providing new engines and/or extended warrantees, however, this does not applicable to 2020 and newer models, even though it's the same exact same engine model with the exact issue causing the failure. Engine failure in traffic condions can obviously lead to immediate safety issues. ... reason for the noise however did notice that there was metal in the oil and heard a knocking that was abnormal in the engine. After some investigation, we discovered that 2017 Honda Ridge lines were known for connecting rod, bearing failures, and many of the vehicles were under recall. We checked the VIN number of our vehicle and were told that this was not under this warranty. We called Honda Customer Care to talk to them about the fact that this problem seem to be the same problem as the service bulletin problem. We asked if they could cover the cost of the repair. They suggested we needed to have it diagnosed by a Honda dealership. We brought it to the Honda dealership who told told us that it would cost $1000 out-of-pocket to be able to diagnose this problem properly. We now have a car that potentially has a extremely dangerous and potentially catastrophic engine problem with premature deterioration and failure of the crankshaft connecting rod bearings that we are afraid to drive and we don’t have the money to fix. When doing research it suggests that there are at least 1.4 million vehicles that are under investigation for the possibility of having the problem that is addressed by the recall, and many of these vehicles are still having this engine failure and we think ours may be one of these. We have not seen any engine warning lights yet however, the mechanic suggested that it is premature crankshaft connecting rod bearing failure as evidenced by the amount of metals showing up in the oil. I was on the highway on the way to work when my car started stalling. Every time I pressed the gas to give it some power, it would not go anyway where. after getting it towed to get looked at some of the fuel injectors failed and were misfiring. Despite wearing jeans, I have suffered multiple burns over time. The most recent burn blistered and has now left permanent scarring on my leg. I have owned this motorcycle since it was new and have noticed the excessive heat issue repeatedly. I have also experienced what appears to be increasing heat over time. I believe riders should not be exposed to temperatures capable of causing burns during normal operation of a motorcycle. This condition creates a safety hazard because it can distract the rider, cause sudden pain while operating the motorcycle, and lead to loss of concentration or control. In my case, it has resulted in repeated burns and permanent scars. There was no advance warning that a catastrophic engine failure was imminent. The vehicle ultimately became inoperable and was towed to an authorized Nissan dealership. The dealership diagnosed the engine with coolant leaking into the cylinders and determined that replacement of the engine assembly, turbocharger, and catalytic converter was required at an estimated cost exceeding $10,000. My vehicle had previously experienced engine-related concerns and was included in Recall Campaign R25D1. That recall was completed, but less than 30 days later the engine failed catastrophically. Based on this sequence of events, I am concerned that the existing recall may not identify or address all engine conditions that can result in sudden loss of motive power and catastrophic engine failure. I have since learned of numerous publicly reported complaints from other owners describing similar failures involving coolant intrusion, sudden loss of power, and complete engine replacement in 2022 Nissan Rogue vehicles equipped with this engine. I respectfully request that NHTSA investigate whether these engine failures represent a broader safety defect and whether additional corrective action is necessary to protect the driving public.Read more

My 2016 Honda Pilot Touring repeatedly stalls and shuts off while driving, including at lower speeds in traffic. ... of a crash and of being stranded in a travel lane. The failed component is the fuel delivery system (fuel pump / fuel system pressure). The condition has been diagnosed repeatedly with trouble code P0087 (Fuel Rail/System Pressure — Too Low). It was first documented on July 18, 2025 at 144,303 miles by an independent service center, which diagnosed a failing fuel pump. On March 13, 2026 a Honda dealer again retrieved code P0087 and diagnosed a failing fuel pump. In April 2026, a second Honda dealer could not reproduce the code, replaced the fuel injectors instead, and the stalling returned within about 670 miles. In May 2026 that same dealer documented the problem as a "reoccurring issue" and now recommends replacing the in-tank fuel pump. The warning symptom prior to failure was the check-engine/malfunction-indicator lamp, which set code P0087; the stalling followed. The problem has been confirmed and reproduced by both an independent service center and a Honda dealer, and the vehicle is still owned and available for inspection upon request. No crash, fire, or injury has occurred. This stalling/loss-of-power failure mode is the same one described in NHTSA recall 23V-858, in which the in-tank fuel pump can fail and cause the engine to stall while driving. This vehicle is exhibiting exactly that failure mode but is not currently included in that recall population. I am reporting it so the agency is aware, and I request that this vehicle be considered for inclusion in recall 23V-858.Read more

The issue has been brought to two separate authorized Honda dealerships within a 30-day window. The first dealership diagnosed a failed battery and replaced it under warranty, but the stalling recurred. The second dealership held the vehicle for five days for diagnostic testing but stated they could not replicate the defect, returning it with no repairs made. Immediately upon picking the vehicle up from the second dealer, it suffered another starting failure in a parking lot. I could feel the car start to lose power and handle differently than normal. I immediately pulled to the shoulder and had the car towed to the dealership. My vehicle has over 100k miles and it isn’t even 6 years old. I have had older Hondas, acuras and Toyotas with similar mileage or more and no issues like this. If so many drivers are having blown head gaskets Honda needs to be looked into for this and have a recall to repair their defect. The service adviser told me it was nothing I had done to my vehicle to cause this. I get oil changes when the maintenance light comes on and I have completed scheduled maintenance on my vehicle. Hyundai Motor America (case #42847594) has refused repair, citing a "branded title"; the only brand on the title is a "Not Actual Mileage" odometer disclosure, not a salvage or rebuilt brand. NHTSA guidance is that title branding is not a valid basis for denying a recall remedy. The defect poses a risk of sudden engine failure and stalling while driving. No crash, fire, or injury. ... ng Failure), and P0921 (Gear Shift Forward Actuator Circuit Range/Performance). A Hyundai dealer diagnosed the failure as requiring transmission replacement at approximately 93,000 miles. This is the same 7-speed DCT and the same failure mode addressed by NHTSA Campaign 16V628000 (Hyundai Recall 149) and Hyundai Service Campaign T1B, which covered 2016–2017 Tucson vehicles but excluded the 2018 model year despite using the same transmission. Hyundai has acknowledged this defect in 2018 vehicles through TSB 21-AT-014H and TSB 22-AT-007H, which prescribe a TCU software update, but issued these as service bulletins rather than a recall, so owners were never notified. When I requested the TSB software update, the Hyundai dealer refused to perform it. This defect creates a serious safety hazard: the vehicle can drop out of gear or fail to accelerate without warning while in traffic, increasing the risk of a crash — the identical hazard NHTSA cited in granting recall 16V628000. I request that NHTSA review whether the existing recall scope should be expanded to include 2018 Tucson vehicles.Read more

This 2026 Ford Explorer has stopped, stalled, hesitated and jerks after fuel fill-ups. It stopped twice for myself in busy intersections. The dealer has had it for weeks and when driving the vehicle back to me, stalled and hesitated for Ford Service dept. twice. They told me I could not have my car back for safety reasons.

Over time this progressed to recurring, confirmed misfires on cylinders 1, 2, 3, and 5 (diagnostic codes P0300, P0301, P0302, P0303, P0305) and a mixture control fault (P219B). The check engine light has intermittently stayed illuminated for months at a time, then gone off for extended periods while the same fault codes remain stored and confirmed by a diagnostic scanner — currently the light is off but the codes persist. The vehicle frequently runs rough enough that the engine must be turned off and restarted to temporarily return to closer-to-normal operation. Noticeable vibration is felt during much of this pattern. Spark plugs, ignition coils, oxygen sensors, and the PCV valve have all been inspected/replaced and ruled out as the cause on multiple occasions over more than a year, isolating the cause to the cylinder head itself. I feel anxious when driving on the highway that the car will just stop, though I think that is unlikely given how this happens. We have gotten into the habit of turning off the automatic idle stop feature when we drive and that gets around it, but you have to shut this off every time you start the car, so we forget sometimes. No warning lights before the failures occur nor after.
