I am filing this complaint regarding my Ford F-350, VIN [XXX], which caught fire on XXX. The truck had approximately 130,000 miles, which I consider low mileage for a properly maintained diesel Ford F-350. These trucks are known for long service life, and many Ford diesel trucks routinely remain in service for 20 years or more and exceed 250,000 miles when properly maintained. For that reason, I
... did not expect a sudden and catastrophic fire in a truck with only 130,000 miles. While driving, passing motorists alerted us the truck was on fire. The fire appeared to originate in the engine compartment and spread extremely rapidly. Within a very short period, the front of the truck was engulfed in flames and sustained catastrophic damage. Based on the speed and severity of the fire, I am concerned a failure in the engine compartment, fuel system, electrical system, or another vehicle component may have caused or contributed to the fire. This truck has been well maintained, kept clean, and responsibly cared for. I followed the manufacturer’s maintenance guidelines and did not expect a sudden engine-compartment fire. After the fire, I found reports of other F-350 fires and references to fire-related complaints and investigations. This raises serious concerns Ford may have known of a potential fire risk involving these trucks. This incident created major safety hazard not only to the occupants of the truck, but to other motorists and anyone nearby. Had passing drivers not alerted us, the outcome could have been far worse. The severity of the fire required a major emergency response, including multiple fire trucks and police. A vehicle fire of this magnitude presents an obvious risk of serious injury or death. I request NHTSA investigate Ford F-350 and vehicle fire incidents to determine whether a defect involving the fuel system, electrical system, or related parts may cause these trucks to catch fire without warning. On 6/26/26 I was informed by the dealer that he engine is on an indefinite backorder nationwide. On 6/30/26 I received a call from GM giving me a case number and getting contact info for my local dealer. I was advised that someone would contact me within 48 business hours. 7/3/26 I was contacted by my case advisor from GM. Still no idea when replacement engines might be available. I inquired about a buyback since I need a truck for work/personal life. I advised her that I had already cancelled one vacation (towing an RV...my loaner Buick Encore won't work) and would probably have to cancel another. I tried to explain that farming doesn't work well with the loaner either. I was told that another advisor would look into a buyback situation. Ford issued Customer Satisfaction Program 21N12 for certain affected vehicles due to this exact issue, but my vehicle is past the warranty so it's SOL. The failure appears to be caused by a defect in the engine design allowing coolant to enter the combustion chamber. This is clearly a widespread defect as Ford had to create their limited warranty for it. Multiple times the engine has totally shut down due to the auto idle stop system power loss. I have had the battery replaced a couple times and had the Honda dealership do the software update and I'm still having issues. My 2019 Audi A6 was diagnosed with catastrophic internal engine failure on May 9, 2026 at Audi of Newton, Newton, NJ, at 93,645 miles. The vehicle required a full engine rebuild performed by REV Auto NJ at a cost of $12,000. The failure involved metal debris found in the engine oil, consistent with the documented EA839 3.0L V6 TFSI rocker arm needle bearing failure or piston skirt failure affectin

The EA839 engine used in this vehicle has a documented history of design defects: (1) Rocker arm needle bearings in early production engines were undersized and prone to disintegration, releasing metal debris into the oil supply and destroying camshaft lobes, balance shafts, and pistons. Audi silently revised the rocker arm part number three times (06M109417H -> 06M109417L -> 0P2109417), with the final revision featuring bearings twice the size of the originals - evidence of a known design defect. No recall was issued. (2) Audi of America issued a Piston Skirt Limited Warranty Extension in 2025 to 8 years/80,000 miles for EA839 piston failures. Independent engineering analysis by 034Motorsport, backed by laboratory testing, confirms that the EA839 piston design creates failure paths even under stock, unmodified conditions. Audi of America has not issued a safety recall for this engine failure pattern despite multiple documented failures across the A6, A7, A8, S4, S5, SQ5, Q7, and Q8 platforms sharing this engine. The failure mode - metal debris circulating through the lubrication system - poses a risk of sudden, complete engine loss while driving at highway speeds, creating an unreasonable safety risk to vehicle occupants and other motorists. I am requesting that NHTSA investigate the EA839 3.0T engine for a potential safety-related defect and consider whether a recall is warranted across the affected model population. Check engine light came on with significant loss of power. Poor acceleration. Lack of power resulting in slow speed which created a safety hazard for other traffic. Error code P300478 was confirmed by dealer. This is a known issue - EGR Valve Differential Pressure Sensor. This is a known (and common) issue. ... .1 miles into the trip, near South Buffalo Drive / Summerlin Parkway, Las Vegas, NV: While making a right turn off the Summerlin Parkway exit, the vehicle suddenly shifted into neutral with no prior warning. A check engine light and "Gearbox fault detected" alert appeared on the dashboard. The vehicle coasted downhill and my worker was able to steer it to the shoulder. Once stopped, the gearshift would not engage any gear other than neutral, and the electronic parking brake also failed to engage. My worker kept his foot on the brake pedal continuously and did not turn off the engine, fearing the vehicle could not be safely towed otherwise. A tow was requested at 10:09 AM and arrived at 11:01 AM; at that point the parking brake functioned again, and the vehicle was winched onto the tow truck. The vehicle was dropped off at Land Rover Las Vegas at 11:41 AM. Upon repair pickup on July 7, 2026, the service advisor stated the issue was software-related — the vehicle's computer had taken the transmission offline. The rod knock was confirmed by the dealership's technician on the date of the diagnostic visit. The diagnosis noted the engine was low on oil and recommended an oil change, but no repair was performed. XXX “my car was perfect, didn’t know why it was even there and trying to find something wrong with it was like chasing unicorns.” I said please look again and on XXX the engine seized up as it was taken on a drive by said advisor. Via dealership, they took pictures of manifold, but Hyundai denied my replacing my engine per the recall. I called Hyundai to request an appeal and received a case number/case manager ( Madge case # XXX) on XXX. I then submitted maintenance records which showed at least one oil change at each requested interval and still denied on XXX. I continued to call both parties: dealership & case manager but got no reply. I received my case manager’s direct line at Hyundai on XXX via [XXX] who said no notes were on file and case manager would return call in 1-3 days and he has never returned my calls, nor has the service manager who said my car was in perfect driving condition. Today is July 8 and I am still waiting for a written letter or phone call explaining the reason for my denial. Stopped in heavy traffic in a four lane highway. Auto start/stop shut the engine down as expected. Traffic ahead cleared. I took my foot off of the brake to step on the gas. The engine didn't start up, and the radio and all lights inside went blank. Traffic behind me approached at about 45 mph. and were swerving around to avoid hitting my dead vehicle from the rear. ... othing. Again. Nothing. Several times more with my foot on the brake, or off the brake. Nothing. I put it in park and kept trying the start button. Finally it started, the radio and dash display came on, and from then on it operated as usual. Scared the hell out of us. There was no warning that anything in the car was malfunctioning before this happened. Took it to the dealer. They looked it over, kept it overnight. Told me the next day that the battery can get drained too much if the air/cond is on max and the start/stop feature doesn't like that. They used a machine to drain the battery and recharge it and they tell me now that it should be good. I don't believe one has to be aware not to use the air on max while the start/stop is enabled. I'm afraid to drive the car now! Our lives were in danger.Read more

In addition, I am extremely dissatisfied with the customer service and conduct of an employee named Khern. He explicitly identified himself to me as a manager; however, after speaking with other staff members, I discovered he is not a member of management and works at the service intake desk. Furthermore, when I requested to speak with the actual service manager or another member of leadership to discuss my concerns, Khern actively refused to contact them or allow me to escalate the matter. As a customer, I expect honesty, transparency, and the ability to speak with management when a situation cannot be resolved at the front desk. Instead, I was misled, ignored, and left without assistance while my vehicle sat unrepaired for weeks. I request the following actions in response to this letter: A written explanation as to why my vehicle remained at the dealership for four weeks without any diagnostic progress or work being performed. A detailed, line-item justification for the approximately $13,000 diagnostic estimate presented to me before repairs would begin. An explanation regarding why frontline staff denied my explicit requests to speak with a dealership manager. I am the owner of a 2019 Mazda CX-5 with approximately 50,000 miles. I am experiencing an active engine oil leak accompanied by a burning smell and visible smoke from the engine compartment. ... fire hazard. Despite this, Mazda has extended warranty coverage only to turbo-equipped models (Program CSP11), leaving non-turbo owners with the identical defect unprotected. At only 50,000 miles, this is a premature failure caused by a manufacturing defect — not normal wear and tear. Oil leaking onto a hot exhaust manifold constitutes a serious fire risk to vehicle occupants and the public. I am requesting that NHTSA investigate this pattern of failure across 2018–2021 Mazda CX-5 non-turbo models and consider whether a formal recall is warranted. I have contacted Mazda North American Operations directly requesting goodwill warranty coverage and have a case number on file. The front timing chain cover on the engine was leaking at 53k miles. This is a 2GR-FKS engine. We have had the car for 19k miles, and bought it as a Certified Pre-Owned. We're looking at spending over 10% of the purchase price of the car on a repair, after barely having it for three years.
