I have two major safety concerns for my 2021 Nissan Rogue. 1) FAILED REAR DOOR ACTUATORS ON BOTH THE DRIVER AND PASSENGER SIDE OF VEHICLE: I own a 2021 Nissan Rogue with approximately 68,000 miles. The rear driver AND passenger side door lock actuator failed, causing the rear doors to no longer lock or unlock electronically using either the key fob or the interior power door lock switch. The do
... or must now be operated manually from the inside, no one can freely open the doors from the outside. I have to crawl in the backseat to open the doors. This creates a safety concern because, in an emergency, a rear passenger may not be able to quickly exit the vehicle if the door cannot be unlocked electronically. This failure affects a rear passenger door and could delay a child's or other passenger's ability to exit the vehicle quickly in an emergency if the electronic lock cannot be operated. It also creates a vehicle security concern because the door does not lock properly. I have found multiple reports from other 2021 Nissan Rogue owners describing the same rear door lock actuator failure at similar mileage. This appears to be more than an isolated incident. The vehicle has not been involved in an accident, and the failure occurred during normal use. 2) FAILED EGR VALVE: The EGR cooler has failed internally and is allowing engine coolant into the combustion chambers. The vehicle experienced a sudden engine malfunction related to an EGR (Exhaust Gas Recirculation) system failure. The diagnostic trouble code was P0401 –and was diagnosed with an EGR cooler failure causing coolant to leak into the combustion chambers. While driving, the vehicle displayed “Engine Malfunction-Power Reduced-Service Now.” The car went into reduced power mode, significantly limiting acceleration and normal vehicle operation, creating dangerous driving conditions. A sudden reduction in engine power on a highway or in traffic presents a significant safety hazard. Please investigate a recall.Read more

This 2025 Chevrolet Tahoe has experienced three separate and recurring engine-related failures. The first failure occurred at approximately 41,350 miles and required replacement or repair of the lifters and camshaft. The vehicle remained at the dealership for approximately 15 days. The second failure occurred at approximately 54,619 miles and involved another lifter failure along with traction con

The Check Engine warning light illuminated while the vehicle was being driven, and the vehicle again experienced engine-related problems. The vehicle has now been at the dealership for more than one week, and no repair or permanent solution has been provided. I am concerned that these repeated engine failures and traction control problems create a serious safety risk, particularly because the vehicle is driven on public roads and highways. Despite previous repairs, the engine problems continue to recur. I am requesting that NHTSA investigate these repeated safety-related failures.Read more

Hello, my car was parked at home and as I turned the engine on, I noticed the red light 'oil pressure' on the dashboard. I checked the oil level and it was very low. On the same day I purchases 2 quarts of oil to keep it at a safe level. The next day I checked the dip stick again and much oil was loss. I was very concerned and finally was able to make an appointment to my mechanic shop for inspect

Vehicle is a 2022 GMC Yukon Denali with the 6.2L L87 V8, covered by NHTSA Recall 25V-274 (GM N252494001). The recall remedy was performed. The engine passed the dealer inspection and was given the higher-viscosity 0W-40 oil remedy rather than an engine replacement. On 05/26/2026, at approximately 43,000 miles, the vehicle suffered a sudden engine knock while being driven by my wife. At the same t
... ime, the dash displayed Service ESC and Service Traction Control warnings, the vehicle's speed fluctuated on its own without driver input, and the air suspension cycled up and down while the vehicle was in motion. The vehicle had to be towed to the dealer. The dealer diagnosed a failed lifter with resulting camshaft damage and repaired it under the powertrain warranty. The vehicle was out of service approximately fifteen days. Shortly after the vehicle was returned, it began displaying a "Theft Attempted" message on the dash every day. I reported this to the dealer in writing on 06/15/2026 with a photograph at 43,898 miles. The dealer stated it was unlikely to be related and did not scan or diagnose it. On 07/14/2026, approximately 560 miles after the repair, the vehicle became inoperable and had to be towed to the dealer a second time. It is currently at the dealer and has not yet been diagnosed. The unexpected loss of power, speed fluctuation, and stability-control failure at road speed present a serious crash risk. This vehicle passed the 25V-274 inspection remedy and then experienced a valvetrain failure regardless, which is directly relevant to Recall Query RQ26001 into the adequacy of that remedy.Read more

On July 4, 2026 in the early evening, I was driving in the HOV lane of the freeway when the engine abruptly lost power. Once I pulled to the shoulder, I restarted the vehicle and was able to drive home safely. What put my wife
... and I in great danger was the vehicle losing power without warning when driving on the freeway and having to navigate safely to the shoulder in traffic without having acceleration power. I took the vehicle into the Audi Ontario dealer on July 6 for a rearview camera recall notice and to address the engine problem. I did not drive the vehicle after the July 4 incident but the loss of power did not happen again after I restarted the engine nor when I drove it to the dealer on July 6. The dealer notified me the next day that they could not recreate the engine problem and that the engine automatically turns off after coming to a stop. I explained I had not come to a stop, that I was driving at about 80 MPH with both hands on the steering wheel. I asked about what trouble codes they found and if I could have a print out since this was a safety concern and the vehicle only has 1,500 miles on it. The loss of propulsion was not the auto turn off when coming to a stop. They kept the vehicle for further review and I have not heard back as of July 14. I called to make inquiry and they are supposed to call me back. My concern is not only for me, but also for anyone that has this vehicle year, make, and model - the next person may not be so lucky to navigate to safety if it happens to them.Read more

I am reporting a severe vehicle safety hazard and engine stalling incident on my 2013 Ford Focus caused by an un-remedied factory emissions safety defect (related to Ford Recall 26S40 / 18S32). Due to an incomplete factory software monitoring patch, the canister purge valve stuck open and allowed continuous, unchecked engine vacuum to pull straight out of the plastic fuel tank. This heavy vacuum p
... hysically collapsed and deformed the bottom of the plastic fuel tank inward by approximately 0.25 to 0.375 inches. This structural collapse compressed the internal fuel pump assembly and mechanical float arm, falsifying the dashboard fuel gauge and distance-to-empty calculations. On April 2nd, while driving on Interstate 95 at midnight, the vehicle displayed an inaccurate fuel level, ran completely dry, and suffered a sudden engine stall in high-speed traffic without warning. My family and I were trapped in a highly hazardous situation and were forced to physically push the disabled vehicle 3 miles down the highway in total darkness. We ultimately had to enlist the emergency intervention of the Florida State Patrol Road Rangers to dispatch a unit and supply fuel so we could safely clear the interstate. The vehicle has since thrown a hard diagnostic trouble code P0443, confirming a complete electrical circuit malfunction of the EVAP system. Ford's failure to successfully deploy a functional software monitor has caused severe structural damage to my fuel system and created an imminent, life-threatening safety risk to my family on public highways. I am requesting a federal record of this incident to compel a full vehicle inspection and complete fuel system component replacement by the manufacturer."Read more

Safety Hazard: Oil Pump Failure Leading to Sudden Engine Stall (BMW B58TU engines) Description of Defect: The oil pump module in the 2020 BMW M340i (G20) and many other models between 2019-2020 contains internal plastic components that are prone to thermal degradation and premature failure. These plastic parts can crack or disintegrate, causing a sudden loss of oil pressure. BMW corrected this in
... later models with metal parts but failed to issue a recall or the older models containing plastic parts. Safety Risk: A failure of the oil pump leads to insufficient lubrication of critical engine components, which can result in a sudden and complete engine seizure while the vehicle is in motion. This causes an immediate loss of propulsion and power steering assist, significantly increasing the risk of a high-speed collision or being stranded in dangerous traffic conditions. Additional Note: This is a known mechanical defect where a critical engine component (oil pump) relies on non-durable plastic materials, creating an unreasonable safety risk for drivers and passengers due to potential catastrophic engine failure without prior warning.Read more

While operating a 2023 Ram ProMaster 2500 (3.6L V6) for normal commercial use at approximately 39,129 miles, the coolant began bubbling and engine temperature spiked suddenly while driving. The vehicle was immediately parked and towed to an authorized Chrysler Jeep Dodge Ram dealership as it was completely undrivable. This created a serious safety risk as the vehicle lost drivability suddenly on p
... ublic roads during active commercial operation. Upon teardown the authorized dealer documented: coolant leaking into cylinders 3 and 4, coolant coming out of exhaust, stretched engine bolts, warped block and head, and internal coolant leak causing overheat. No external leaks from the cooling system were found. The internal component failures caused the overheating not the reverse. The vehicle was still within its active powertrain warranty at time of failure. Stellantis denied the warranty claim under TSB D-23-20 claiming an undetermined overheat caused the engine failure directly contradicting their own dealer's findings which show zero external cause. I own a 2026 Carnival Prestige SX Hybrid and having loss of propulsion problems. After much research, and absolutely no help from KIA, I now think my problem is my foot, size 15, is too large to safely drive this vehicle. My shoe intermittently depresses the brake ever so slightly while I accelerate from a stop. This triggers the Brake-Throttle Override which temporarily cuts propulsion power.
... The BTO is in all US cars but of the five vehicles (2025-26, different manufacturers) I have driven this past six months, this is the only one that has given me concern for my safety! KIA claims the vehicle is operating as expected! I claim that they have engineered the brake and acceleration pedals too close to each other creating a serious safety issue for those of us with larger foot sizes. The owners manual makes no mention of the BTO function nor how to restore propulsion once the BTO has been activated! While all US vehicles have the BTO function, they vary as to how the function performs. In KIA, once the BTO is triggered, all propulsion is suspended! The BTO was designed to prevent runaway acceleration. In this KIA, they have created a new safety problem because they failed to allow enough space for larger footed individuals room to safely operate their vehicle! KIA needs to introduce a foot size requirement for all prospective buyers or redesign the area around the accelerator and brake pedal so safety is no longer an issue!Read more

I am submitting this complaint to document what I believe is a recurring defect involving the air conditioning compressor on my 2013 Acura ILX Hybrid. Vehicle history: * 2013: Vehicle purchased new. * 2019: Original A/C compressor failed and required replacement. * 2026: Replacement A/C compressor has failed again. This means the vehicle is now on its third A/C compressor since new. Based on my
... experience, each compressor has lasted only about 4–6 years before failing. The first compressor replacement, performed by an Acura dealer in 2019, cost approximately $5,700. I have now been quoted approximately $3,600 by an independent repair facility and approximately $5,200 by an Acura dealer for the current repair. In addition to the extremely high repair cost, I have found that many independent A/C repair shops are reluctant to perform this repair because of the complexity and expense of replacing the compressor on this hybrid model. Replacement options are also very limited because this compressor is essentially an Acura OEM-specific component, with few, if any, widely available equivalent alternatives. The current failure began with a rattling/chattering noise whenever the A/C was engaged. Eventually, the compressor load became severe enough that the accessory belt was thrown off when the A/C engaged. An independent mechanic inspected the vehicle, ruled out the belt tensioner, and concluded that the compressor is the source of the problem. While compressors are wear items, replacing the compressor three times over the life of the vehicle—with each unit lasting only about five years—appears to be an unusually short service life, especially considering the exceptionally high cost of repair. In my opinion, this suggests a potential design or durability issue with the compressor used in the 2013 Acura ILX Hybrid. I am requesting that NHTSA document this complaint and review whether other owners of the same vehicle have experienced similar repeated compressor failurRead more

I was never notified that this campaign needed to be completed, and I believe that failure to receive notice contributed to Hyundai’s denial of my engine replacement claim. My car was at only 55,000 miles, there was a weird shuddering and my RPMs were very jumpy. It almost felt like my car was going to stall out. I took it in to the ford dealership and I had to get my Torque Converter replaced. I had no dash lights on or any other problems. I got it replaced, but my car still feels shaky while at low speeds. Apparently it’s a very common problem in this car.

I was parking at the dentist office in their parking lot going slow, turned into a parking space and thought i was slowly braking but my car lunged forward and hit a tree. I would normally be minimally braking, so I'm not sure what happened but it was sudden. I could have hit the gas pedal but i would have pressed it a lot I think for the sudden lunge to happen.

The engine warning light came on and Autozone indicated that it involved the evap cannister purge valve. As I was trying to figure out which part to order over a couple of days, the car stopped in the middle of an intersection while my wife was driving. It would not restart. We had the car towed and spent several hundred dollars on a repair. I don't understand how a faulty purge valve should cause

I didn’t hear back from them until about a month later (July 3rd) and wh

About a week and a half later, they said it did not fix the issue and started to become more of a problem to where they will need an “engine technician” to see if I need a new engine due to the “crank shaft thrust bearing”. The advisor said he will call me on Wednesday, July 15th to update me on what I will need. I requested a Loaner vehicle since I dropped it off and they said no because the dealer does not provide that and would have to get approval from GM to grant me the loaner vehicle bast off the diagnostic.Read more

The car misfires every now and then. For last two years (I bought it used two yers ago odometer=105k now 113k), it has been a repeated problem. A few times it lost all power and stopped completely. Once on highway. Once I had to get it towed. Another occasion it restarted after 15 minutes break as if nothing had gone wrong. This is very scary - I have reduced my driving and try to avoid highways
... as much as possible. It gives a code of P0302. It happens irrespective of hot or cold, speed or outside temperature. The mechanic repaired it - by changing injector coil, spark plugs, once even added injector cleaning fluid to see if the problem would go away. Plus top up the coolant. In the most recent mechanic trip - he tested for coolant leakage on head gasket and confirmed that is the problem. He also informed me that it is a widely known problem with this model (2017 Honda CRV 1.5L) although I wasn't aware of that when I bought it from a used car dealer, basically relying on Honda's reputation.Read more
