CRITICAL SAFETY INCIDENT / LIFE-THREATENING VEHICLE FIRE HAZARD. On January 2, 2026, I purchased a 2020 Chevrolet Trax. Within less than five months, while driving in active traffic, the vehicle suffered a sudden, catastrophic powertrain failure. The vehicle completely lost all acceleration, leaving me trapped and stranded in a highly dangerous roadway position. Immediately following the power fai
... lure, the engine bay filled with heavy smoke and began severely overheating. The vehicle came within moments of bursting into a full thermal engine fire. This vehicle was a rolling death trap that almost killed me on the road. No one is taking this life-threatening safety failure seriously. The selling dealership refused to address the known hazard, forcing a predatory repossession that destroyed my financial life while leaving a severely defective, fire-prone vehicle completely unaccounted for. I need an immediate federal investigation into this vehicle before this defect causes a fatal fire for another driver.Read more

I am reporting a severe drivability and engine failure issue with my vehicle. The vehicle has been exclusively maintained by authorized Mercedes-Benz service centers. Immediately following a scheduled 60,000-mile (Schedule B/C) maintenance, the Check Engine Light began cycling on and off. The vehicle simultaneously developed severe drivability symptoms: the engine ran extremely rough, and accelera
... tion became erratic and "jumpy." This unpredictable power delivery and hesitation poses a significant safety hazard when accelerating or merging in traffic. I immediately returned the vehicle to the service center to report the issue. The dealership has now retained my vehicle for over 20 days and diagnosed that the cylinder head requires complete replacement. Despite this catastrophic failure occurring immediately after their maintenance procedures, the dealership is holding me financially responsible for the repair costs because the vehicle is past the 4-year/50,000-mile warranty. My vehicle remains immobilized at the dealership."Read more

The engine in my 2022 GMC Yukon XL equipped with the 6.2L L87 V8 experienced a sudden loss of power while traveling at interstate speed. The vehicle had previously received the GM L87 engine recall remedy. Despite the recall being completed, the engine failed, leaving me and my family in a dangerous situation. The vehicle is currently at a GMC dealership and is available for inspection upon reques
... t. The sudden loss of engine power on a busy interstate created an immediate safety hazard. Traffic was traveling at highway speeds, and the vehicle rapidly lost the ability to maintain speed, increasing the risk of being struck from behind or being involved in a serious collision. My family was in the vehicle at the time, making the incident especially concerning. The GMC dealership has confirmed that the engine failure is related to the L87 engine issue and advised that the repairs will be covered under the applicable warranty/recall-related coverage. Additionally, after the original recall service was performed, an expanded recall was added to my VIN, further indicating that the original remedy may not have fully addressed the underlying defect. The vehicle is currently being inspected by the authorized GMC dealership. Prior to the engine failure, there were no warning signs indicating an imminent catastrophic failure. The recall service had already been completed, leading me to believe the vehicle was safe to operate. Since December 2020, our Suburban has been in for seven extended, documented dealership repair events at Guaranty Chevrolet and Simpson Chevrolet of Garden Grove, totaling 192 cumulative days out of service and $47,560.86 in documented repair invoices — approximately 71% of what we paid for the vehicle. Those repairs have included a complete transmission replacement (58,861 miles), a recurrence
... of the same active-lifter defect only ten days later, an ignition coil failure, fuel injector and air conditioning failures, an oil pump failure, and, most seriously, a complete engine replacement (66,854 miles) after the dealership documented metal in the oil and filter and engine end play. The situation has continued to worsen. We picked up the vehicle on XXX following the complete engine replacement. Only six days later, on July 6, 2026, the check-engine light returned and we brought the vehicle back to the dealership. We were initially told the issue was a sensor; the following day, we were informed that the replacement engine — which the dealership had disclosed to us at the time was a refurbished unit — was itself defective and would need to be replaced a second time. As of this writing, that repair remains open. In other words, the remedy provided for this vehicle's most serious failure did not resolve the problem, and we are once again without our vehicle. This pattern of failures has caused real, ongoing hardship for our family. We specifically purchased a full-size, top-trim Suburban for its seating and cargo capacity: we have two teenagers at home who depend on us for transportation to school and activities, we regularly care for grandchildren, and our [XXX] daughter has Complex Regional Pain Syndrome (CRPS), a debilitating condition that often requires her to use a wheelchair. The Suburban's cargo space is what allows us to transport her together with her wheelchair. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I own a 2026 Mazda CX-90 hybrid with approximately 13,000 miles. The vehicle began making a grinding noise while driving, which has progressively worsened over time. It has since developed a hard knocking noise accompanied by noticeable vibration when the vehicle shifts gears. This knocking occurs during normal in-town driving. I have taken the vehicle to the selling/authorized Mazda dealership o
... n two occasions — June 2, 2026 and July 14, 2026. On both visits, the dealer was able to reproduce and confirm the noise and knocking. The dealer stated they are aware other customers have reported the same issue, but that Mazda considers the noise/knocking acceptable and declined to perform a repair. No warning lights or diagnostic trouble codes have appeared at any point; the only symptoms are audible and physical. The vehicle is under both factory warranty and a purchased service/maintenance agreement. It has only been inspected by the selling dealership; no inspection has been performed directly by Mazda North America, law enforcement, or an insurance representative. I am concerned about my safety and the safety of others because a worsening, unexplained drivetrain issue that produces hard knocking and vibration during routine driving suggests a mechanical fault that could progress to sudden failure, and the manufacturer has declined to investigate or repair it further despite the symptom escalating across two documented visits. The vehicle remains available for inspection upon request.Read more

The vehicle is undriveable and the required recall repair cannot be completed because the replacement engine is on backorder with no ETA.Read more

In December 2021 My 2016 Odyssey experienced serious engine breakdown and could not be driven due to deterioration of piston ring functions in cylinders 1-3. That malfunction caused serious engine problems. It was diagnosed and repaired by Riverside Honda, [XXX] on 12/28/2021. The total cost paid by me for that repair, and for no other unrelated work, was $4598.10. In July 2025 I received from Hon
... da a notice of a recall for this same specific issue. I was informed that I may be eligible for reimbursement since the repair work was already done. I filed the required documents. Almost one year later, on XXX I received a check from American Honda Motor Co in the amount of only $2352.32. I called Customer Service and was given a case number. I caseworker informed me that the reimbursement amount was accurate in that other work not related to the issue was not reimbursable. I stated my disagreement that the work order reflected all work was related to the issue and that the full reimbursement was justified. I asked the case manager if there were any appeal options available. She said quite emphatically the there are none. She made no mention of the NHTSA or any other options that I requested. I request that NHTSA intervene and investigate as to whether or not Honda acted appropriately in reimbursing for only half of the repair cost. I request that I be paid the entire amount, less what has already been paid me. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On 7/9/2026, my 2022 VW Taos was brought to the dealer for being severely underpowered, check engine lights on, and a strong raw fuel smell inside the vehicle. On 7/10/2026, the dealer stated the vehicle was ready, claiming they replaced a turbo charge pipe O-ring. The service advisor verbally stated the vehicle had been successfully test-driven for 5 miles on streets and the highway. However, off
... icial paperwork only stated "cleared faults verified no leaks in system, vehicle is now operating as designed." Immediately upon driving home on the highway, as the vehicle reached 70–75 mph under load, a loud whistling noise and a sudden, dangerous loss of propulsion occurred simultaneously. The vehicle was returned to the dealer on 7/10/2026. On 7/13/2026, the vehicle was picked up after a second repair where the dealer admitted the plastic charge pipe (05E-145-673-Q) was actually fractured and had completely blown off the turbocharger during my highway drive, generating fault code P227900 (Intake Air System Leak). Upon reviewing the paperwork from both visits, I discovered the odometer had only increased by exactly 1 mile during the first service visit. The dealer completely failed to road-test the vehicle under actual highway boost loads before certifying it as safe, which directly caused a catastrophic structural engine failure and an emergency safety hazard at 75 mph.Read more

I am submitting supplemental evidence regarding my 2021 GMC Sierra (VIN [XXX]) for Recall Query RQ26001, Campaign 25V-274. Despite multiple repair attempts, my vehicle remains unsafe and non-compliant with GM Service Bulletin N252494002. The recall remedy was performed deficiently. [XXX] Northeast Buick GMC installed a remanufactured short block instead of the required complete engine, reused the
... original seized-engine oil pan (which continues to leak), and failed to address critical electrical damage. Documentation proves they deleted Labor Operation 14BUZ (Engine Electrical) after their technician noted a blown mega fuse. A second dealer, Penske Chevrolet, subsequently deleted Labor Operation 77CVZ01 from my repair order, demonstrating a systemic pattern of ignoring recall-required repairs. Electrical damage has cascaded, resulting in a burnt fuse block and active faults P0615, P0617, P26E4, P26E6, U0623, and U0284. More critically, within 24 miles of my last service, the vehicle developed a new, severe safety failure: the "Service Brake Pad Monitor" system failed, accompanied by a red "Service System" dashboard alert and a noticeably spongy brake pedal. These brakes were recently serviced, yet this failure introduces a new risk of crash not present prior to the recall "remedy." GM Executive Resolution Case XXX has provided no resolution, reimbursement, or safety assurance, ignoring all deadlines since XXX. I possess extensive documentation, including service orders showing deleted labor operations and diagnostic logs. I request this update be integrated into the investigation of RQ26001, as the current remedy is demonstrably ineffective and has exacerbated safety hazards. The manufacturer’s refusal to remediate these issues, despite my compliance, leaves me with an unsafe vehicle and significant financial loss. I am prepared to provide all supporting records to substantiate this complaint. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
