The vehicle's engine had previously been replaced once under Kia's Theta II engine recall/warranty program approximately five years prior to this incident. The failure occurred at approximately 198,000 miles. This is the second engine failure on this vehicle, both consistent with the known Theta II engine defect involving premature bearing wear and connecting rod failure that has been the subject of prior NHTSA recalls and a related class action settlement. The failure created a risk of sudden loss of engine power while driving, which can pose a safety hazard in traffic. The vehicle was inspected by the authorized Kia dealership, which confirmed the hole in the engine block and the diagnostic codes noted above. The engine and related repair estimate are available for inspection upon request. A warranty claim for repair coverage was submitted and denied by the manufacturer, citing insufficient maintenance documentation, despite this being the vehicle's second occurrence of the same underlying defect.Read more

We were left immobilized in an active traffic lane while vehicles approached and passed us at approximately 55 mph. We had difficulty getting the vehicle restarted and moving again. This presents a serious rear-end collision risk because approaching drivers expect traffic to move when the light changes. Four children regularly ride in this vehicle, and we do not believe it is safe to continue driving. We contacted the Honda dealership again after the battery replacement failed to correct the problem. We were told that unless technicians could duplicate the issue, there was nothing further they could do. Because the failure is intermittent, we cannot predict when it will occur, but it has happened approximately 10 times and has continued after Honda’s attempted repair. We also opened a case with American Honda Motor Co., Inc. The dealer diagnosed the vehicle with a leaking A/C condenser and a leaking heat exchanger pipe and recommended replacement of both components. Honda declined goodwill assistance because the vehicle is outside the original warranty period. I later learned that Honda previously issued an extended warranty for A/C condenser failures on 2018–2020 Accord models due to a known condenser defect. My vehicle exhibits the same symptoms of gradual refrigerant loss and declining cooling performance, despite being a 2021 model year. In researching this issue, I found multiple reports from other 2021 Accord and 2021 Accord Hybrid owners describing similar A/C condenser failures and refrigerant leaks. I am submitting this complaint because I believe there may be a broader pattern affecting 2021 Accord vehicles that warrants review. This is not something that happens every time so it isn’t entirely predictable but it is safe to say at the very least 1in10 times when the conditions are as mentioned above. 2.Recent Near-Miss Incident: This "delay-then-lunge" defect recently almost resulted in a pedestrian collision at an intersection when the vehicle hesitated during a turn and suddenly lurched forward into the crosswalk. The unpredictable nature of this acceleration profile makes controlled low-speed maneuvering extremely difficult and unsafe. 3.I brought this safety concern to my authorized Mercedes-Benz dealership service department. The service advisor and technicians verified the issue and confirmed that this lag is a widespread, inherent design issue across the CLE 300 line—reproducible in other identical customer models and dealership loaner vehicles. According to the service team, the delay stems from factory programming coordinating the 48V mild-hybrid system, drive-by-wire throttle mapping, and Eco Start/Stop function in default Comfort mode. Because the vehicle automatically reverts to default Comfort mode every time the engine is started, drivers are continuously exposed to this safety risk unless manually overridden on every single drive. The dealership indicated there is currently no official factory software recall or bulletin available to fix the programming. 4. The vehicle ultimately began losing power about 2 hours after replacing the battery, causing all the warning lights on the dash to turn on, the main center console display lost power and then I lost power steering. All of this happened while I was on the interstate, putting myself and my family in serious danger. I was able to pull the car off an off ramp and park it safely. I called a tow service to take it to the nearest Hyundai. The dealership verified the alternator was bad, but later found that the tensioner assembly had also failed and would have caused serious damage if they had not caught it. The vehicle has since been repaired under warranty, but I researched and found this is a common failed item. The vehicle only has 33,253 miles on it, and is barely 2 years old. ... ime, we became aware of the coolant intrusion issue with these Ford Escape engines due to design flaw. The service personnel found no antifreeze intrusion into the cylinders but noted that the spark plugs had been changed from OEM to another type (before the expected time for a tuneup) and they were all fouled out, indicating that a potential problem existed. The spark plugs were replaced with OEM plugs and the car ran fine for a time. The P0303 (Cylinder 3 Misfire) Check Engine code returned on April 13. Took the vehicle to a different Ford Dealership for repair. Provided previous maintenance information and extended warranty information to Service Manager. The inspection diagnosis revealed engine failure due to antifreeze intrusion into the cylinders requiring an engine replacement with cost of $10,875.00. The extended warranty company denied the repair claim due to the faulty design of the engine block by Ford. Although Ford had a Customer Satisfaction Program in place for the repair of this specific issue (21N12) which provided financial compensation for those affected by the antifreeze intrusion issue resulting in engine failure, this program ended on November 30, 2022. This left us no option but to replace the engine on a vehicle we purchased only a few months before.Read more

Also noticed that sometimes shifting on a flat surface 65 miles an hour engine will speed up to almost 2000 RPM and when it goes to shift back, I get a clunk or sometimes a shutter normal cruising is about 1500 RPM at 65 mile an hour. I didn’t notice towing an empty trailer about 2600 pounds shattering seemed worse. With no Trailer I sometimes get erratic shifting where RPM goes 1500 to 2000 to 2500 and then back down again then back up again then it can shift all the way back to 1500 RPM still maintaining 65 mph. Also another problem may be connected to transmission, but I have pulled out into traffic pushed on the pedal and it feels like the truck has no power push on the pedal a little bit more still no power and all of a sudden it’s like everything kicks in RPMs, increase rapidly and truck accelerates extremely fast, throwing things off the dash moving things on the floorboard. Sometimes I see that a downshift occurs and RPMs increase other times I see RPM staying right at the 2000 mark and it still occurs almost got rear ended a couple of times. Check engine light has been on one dealership said they pulled over 40 codes. Still they all claim can’t duplicate problem. My trailblazer will not shift properly into drive from park or reverse. GM calls this a "loss of forward motion" or "delayed forward engagement" in their service bulletin TSB-24-NA-009. It is a safety issue because you never know when or how long the vehicle will take to engage and allow forward movement. This causes issues when leaving a parking spot or a driveway, especially on busy roads.
... If the car will not move forward, my family is stuck in the road until it finally engages into gear and allows forward movement. When this occurs, the vehicle engine will shutter and not allow any forward movement. I usually have to move the drive selector back to park and back to drive again. This creates an immediate and unreasonable safety risk because the vehicle will not move for several seconds while I am sitting in traffic. This generates a P2714 code. I had the car looked at by a GM dealer as well as an independent transmission repair shop and both indicate it will likely need a new transmission. GM is fully aware of the defect and has issued a service bulletin, TSB-24-NA-009 to instruct dealers how to diagnose and repair the problem. The bulletin proves a widespread engineering or manufacturing flaw exists. However, GM is treating this as a maintenance issue rather than the severe safety hazard that it is. It is my understanding, GM has since abandoned this transmission on the newer models, further indicating that GM knew these transmissions were not designed well. I am requesting the NHTSA office cross-reference my complaint with GM TSB-24-NA-009 and upgrade the component defect to a mandatory, manufacturer-funded safety recall before it causes a collision or fatal injury.
