Engine started making a loud knocking noise and it sounded like the engine was about to throw a rod. Turned engine off and called GM. Vehicle was towed to dealership and it was diagnosed with a thrust bearing failure. My truck only had 14k miles on it. To fix the bearing issue, it will need a new engine. GM does not currently have engines to replace the faulty ones so it has sat at the dealer
... ship for 3 weeks with No ETA as to when it will be fixed. This is apparently a common problem in the 2025-2026 models LZ0 3 liter diesel. They are still selling these affected vehicles currently, knowing that they may have a problem. GM needs to issue a recall and take these vehicles back.Read more

Component Failure & Availability: A GM remanufactured 2.4L engine block, installed as a replacement in July 2025, suffered a catastrophic internal failure (blown head gasket) within 11 months of installation. The vehicle and the component are currently in the owner's possession and are available for inspection upon request. Safety Risk: A blown head gasket with an internal coolant leak into Cylind
... er #3 causes immediate engine overheating, severe engine misfires, and a loss of motive power. If this defect causes the engine to stall while the vehicle is in motion, the driver suddenly loses power steering and acceleration, directly compromising the safe operation of the vehicle in traffic. Confirmation by Service Centers: The catastrophic failure was formally reproduced, pressure-tested, and confirmed by an independent service center. Inspection by Manufacturer/Others: Following the independent confirmation, the vehicle was inspected by an authorized manufacturer dealership. During this inspection, the authorized dealership executed an unauthorized teardown of the engine block and sublet the engine head to a third-party machine shop, physically altering the failed component prior to manufacturer review. Warning Lamps & Symptoms: A check engine light illuminated shortly after the engine's initial installation. The authorized dealership evaluated the vehicle, reset the diagnostic trouble codes (DTCs), and released the vehicle back onto the roadway without properly diagnosing the warranted engine block. This allowed a defective and degrading powertrain component to remain in operation until failure occurred.Read more

My 2021 Subaru Ascent (40,000 miles) suffered multiple major mechanical failures that pose ongoing safety concerns: 1. Engine Oil Leak: The vehicle developed an active engine oil leak at 20,000 miles, posing a risk of fluid contacting hot exhaust/engine components under the hood or causing low oil levels during operation. Repair time more than 35 days due to parts not available. 2. Loose Factory E

I keep getting coolant level low. I add coolant fluid into the tank and the warning message comes up again. I see no leaks under my vehicle so not sure if there’s a leak inside the tank itself or air leak somewhere but I’ve gotten that warning message about 10 or more times. Also as im driving the blind spot sensor just randomly goes off without a car or anything being next to me.

I’ve reported this problem but as I got into the repair their was far more damaged that was caused due to the vacuum pump. It took out my exhaust camshaft and sent metal through out and the timing needs replaced as well. A shop would rather total my car than what it would cost to fix. I’m attempting to fix it myself but have been without my car for months now. This is created such a hardship a
... nd the money it’s continues to take as more and more needs to be fixed or replaced as well as all the gaskets and screws that are only one time use as well as parts like my high pressured fuel line I mean this lost keeps going. I and to replace the water pump as well and all due to this vacuum pump that’s been reported and reported and nothing has been done this is outrageous that what’s caused is so much more then just a vacuum pump it can destroy your vehicle miles don’t matter the less or if you have more the pump goes out when it feels like it. Creating more issues then it should and potentially totaling out a vehicle as it’s been said mine should be. So I’m trying to fix my car to prevent losing it and it’s been one thing after another and such a nightmare. The fact that this issue continues to be ignored is unacceptable and a complete disregard to public safety due to the serious safety issue as mine I could barley get my car to stop the breaks were hard and unable to push down without basically standing on them to get into stop at only 7-8 miles per hour I was moving when the vacuum booster went out. Someone should not have to die to have this issue addressed and to be taken seriously and made right! I can’t believe nothing has been done and that it’s not required to have to address this issue with the severity it bring to one’s safety as well as public safety is beyond me. This is a major lawsuit and people’s lives and lovely hood should be taken more seriously and instead nothing is being done. It’s very concerning to think what has to happen for this to be addressed!Read more

The vehicle suffered a sudden, catastrophic 48V mild-hybrid electrical power supply breakdown and complete system failure while traveling at highway speeds during a cross-country move. The sudden failure forced an emergency lane change into an isolation zone just off the highway. The sudden failure left the cabin completely powerless with no operational safety systems, hazard lights, or air condit
... ioning in extreme 100+ degree heat. This roadside isolation created an immediate physical hazard and directly inflicted acute heat exhaustion and severe dehydration, requiring a subsequent emergency clinical medical evaluation on July 5 for lingering environmental exposure symptoms. Subsequent forensic diagnostics performed at an authorized secondary dealership (Mercedes-Benz of North Scottsdale) captured digital video evidence of active, high-voltage 48V electrical arcing across a completely loose, un-torqued main grounding network connection left by a prior repair facility (Mercedes-Benz of Syracuse) during a major warranty engine removal repair just 7 days prior. Secondary dealer diagnostics officially logged this internal structural failure under factory tracking as Damage Code 5482G388. The loose connection created an active, immediate thermal and fire hazard capable of igniting nearby fuel or fluid lines. The original repairing facility deflected liability, claiming the high-voltage ground studs 'loosen due to many reasons' on an asset with no prior electrical faults. This creates a severe ongoing fire, thermal, and crash risk for this model year's 48V electrical system architecture.Read more

My 2017 Kia Sorento experienced a catastrophic, sudden engine lockup on the roadway due to a known manufacturing defect involving rod bearing failure. This defect caused complete loss of motive power and stalled the vehicle on the road, creating an extreme safety hazard for myself, my passengers, and surrounding motorists.Prior to the failure, the vehicle heavily consumed its own motor oil at a da
... ngerous rate of nearly 5 quarts every 1,000 miles. I repeatedly attempted to address this with Kia, completing three separate official oil consumption and combustion tests. The vehicle failed all three tests. Despite documented failures of these critical safety components, Kia Corporate and the local dealership, court Kia, issued bad-faith warranty denials. As a direct result, I was forced to drive a dangerously unstable vehicle for 8,148 additional miles until the engine permanently and violently seized on the road. After being towed, the engine was confirmed to be completely locked up. My husband subsequently dropped the engine's oil pan and discovered a pieces of physical rod bearing debris and pieces of a snapped timing chain floating inside. The vehicle had full engine oil at the time of failure, as we were constantly topping it off to counteract the severe consumption defect.Manufacturer and Dealer Misconduct / Safety Cover-Up:The servicing dealer, Regal Kia, and Kia Corporate Case Manager Jennifer (who previously denied the vehicle after the consumption test failures) have actively colluded to falsify safety records to escape their lifetime warranty obligations under the federal engine class action settlement. The dealership deliberately withheld the vehicle's diagnostic trouble codes (specifically camshaft codes triggered when the timing chain snapped), falsely telling Kia Corporate that "no codes" existed. They fabricated a false history, claiming I stated the vehicle "stalled and the oil light flickered" prior to arrival, despite the vehicle being towed in with a pRead more

The engine failed catastrophically. White smoke out of the exhaust at 145,000 miles. Dealer checked out the car and they initially though it was a head gasket failure. After taking the engine head off, it turns out it was a 'structural defect' (verbatim words from the mechanic), which was a divot in the engine block. The head gasket was in great shape. Therefore, I needed a 'new' used engine (at $
... 9K) instead of a head gasket replacement (at $4K). A search on the internet shows that forums (driveaccord.net, facebook groups) shows that many Honda Accord owners have similar head gasket failures. I contacted Honda USA and they said there's no recall so I won't get reimbursed.Read more

On June 20, 2026, while the 2014 Hyundai Santa Fe Sport was being driven on Richardson Highway near Mile 155 in Alaska, smoke and flames developed from the engine compartment without any collision. The driver stopped and both occupants exited before the vehicle became fully engulfed. The fire created an immediate risk of serious injury or death and blocked the northbound lane. There were no known
... prior drivability problems, warning lamps, or other symptoms before the incident. The exact failed component is UNKNOWN. Potential areas requiring investigation include the engine, electrical system, ABS/HECU brake module, multi-fuse, fuel system, and other engine-compartment components. Alaska Department of Public Safety responded. Its report states that there was no obvious cause, but that the fire appeared to have originated in the engine compartment. The vehicle was towed and preserved for the manufacturer. Hyundai inspected the vehicle and confirmed that the inspection was completed on July 11, 2026. Hyundai later stated orally that it did not find an engine defect, but it has not provided the complete inspection report, testing methods, photographs, technical findings, or a written origin-and-cause determination. Hyundai has not explained whether the ABS/HECU, electrical system, multi-fuse, fuel system, or other potential fire sources were separately examined. The vehicle sustained catastrophic damage and was a total loss. Both occupants escaped, but experienced severe ongoing psychological distress after the fire. I request that NHTSA evaluate whether the fire involved an engine, electrical, ABS/HECU, fuel-system, recall-remedy, or other safety defect. The vehicle was available for Hyundai's inspection. Its current availability for further inspection is UNKNOWN. The vehicle remains preserved and is currently available for further inspection upon request.Read more

The vehicle suddenly lost power while traveling approximately 70 mph on the highway, making it difficult to maintain speed and safely keep up with traffic. This created a risk of being struck by other vehicles or causing an accident. The problem has since recurred during normal driving. The problem has been reproduced multiple times. A diagnostic scanner consistently identified code P1326. The knock sensor was replaced and the engine oil was replenished, but the same symptoms returned after several trips. The vehicle has not yet been inspected by a Hyundai dealership. The vehicle has been inspected by the owner and a family mechanic who replaced the knock sensor and identified an engine oil leak. It has not yet been inspected by Hyundai, law enforcement, or an insurance representative. There were no warning lights or messages indicating low engine oil or an oil leak before the failure occurred. I had the engine oil changed approximately one month before the incident. While driving on the highway at approximately 70 mph, the vehicle suddenly lost power, the check engine light began flashing, and the engine started making a loud knocking noise. Vehicle Safety Complaint My Kia experienced an ICCU (Integrated Charging Control Unit) failure and was at the dealership for approximately two months while awaiting repairs. The ICCU and related components were replaced, and I was told the vehicle had been repaired. However, immediately after I received the vehicle back, the “Check Electric Vehicle System” warning illuminated again. Since th
... at time, the vehicle has continued to experience serious electrical issues. The vehicle has unexpectedly shut off while stopped at red lights and has failed to restart on multiple occasions, leaving me stranded. This creates a significant safety hazard because the vehicle can lose the ability to move while in traffic, placing me and other motorists at risk. I have returned the vehicle to the dealership twice since the ICCU replacement, but the problem has not been resolved. The dealership has been unable to identify or permanently repair the defect. I have been without my vehicle for nearly three months this calendar year due to this issue. More importantly, I no longer feel safe driving the vehicle because I cannot trust that it will remain operational in traffic. I am submitting this complaint because I believe this represents a serious safety defect involving the vehicle’s electrical system and propulsion system. I request that NHTSA investigate this issue to determine whether other Kia owners are experiencing similar failures and whether additional action is warranted to protect consumers.Read more

I drive a leased 2024 Porsche Macan that has been serviced exclusively by Porsche of [XXX]. In May 2026, I brought the vehicle in because the engine was revving/racing while I was driving locally at slow to normal speeds. I also reported that the vehicle initiated sudden collision-avoidance braking when no vehicles or obstacles were present. I was told no problem could be found or replicated. On
... the morning of XXX while driving about 25–30 mph, the engine again began revving loudly and continued even while I was stopped at a traffic light. This was the same issue I had reported in May. At the next light, I shifted into Park and then back into Drive; the revving temporarily subsided. I stopped briefly at a drive-through pharmacy and then drove directly home. As I pulled into my driveway with my foot on the brake, the vehicle suddenly accelerated sharply on its own and struck my home, causing significant damage to the house and vehicle. Smoke came from the front of the vehicle. Because I live in an attached home and my neighbors were inside, I attempted to back the vehicle a few feet away. With my foot on the brake, I shifted into Reverse, and the vehicle again accelerated sharply, traveling backward at high speed into the street. I steered away from a parked car and a brick wall, pulled the emergency brake, and shut off the vehicle. I exited and called 911. Police arrived within minutes, photographed the scene, and arranged for the vehicle to be towed because it was blocking the street. This vehicle has had unexplained problems repeatedly, yet I was told they could not be replicated. No one was physically injured, but this incident could easily have killed or seriously injured me, a pedestrian, another driver, or a child. I live next to a park and school. I believe this vehicle presents a serious safety risk and should not be returned to service without a complete investigation. My 2025 Jeep Grand Cherokee 4xe suffered a catastrophic internal engine failure and set DTC P2338 ("Cylinder 3 Above Knock/Combustion Vibration Sensor Threshold"). Although my VIN is not included in Safety Recall 78C (NHTSA 25V-766), my vehicle experienced the same failure mode, symptoms, and diagnostic code associated with the recall. Stellantis' STAR Center reviewed the case, confirmed internal
... engine damage, and recommended complete engine replacement. While driving at highway speed, the vehicle suddenly lost power and entered limp mode. I was able to safely exit the highway, but this defect created a serious safety risk and could have resulted in a crash. Stellantis has already expanded the recall population after identifying additional affected vehicles. My vehicle was built during the same production period, experienced the identical failure, and has a manufacturer-confirmed engine replacement recommendation. Chrysler's XC2 program provides unlimited years/unlimited mileage engine coverage for this defect, which is linked to engine manufacturing issues. Based on the identical failure characteristics and confirmed diagnosis, I believe my vehicle is affected by the same defect and should be included in Recall 78C and the associated XC2 warranty coverage. I am submitting this complaint to request that NHTSA investigate whether additional vehicles outside the current recall population are experiencing the same defect and whether Recall 78C should be expanded to include affected vehicles such as mine.Read more

On July 23, 2026, my 2023 Audi Q5 suddenly lost power while I was driving on the highway, with no prior symptoms before the incident. The vehicle began stuttering, entered limp mode, displayed multiple drive-system malfunction warnings, and developed a loud knocking sound from the engine area. The loss of power created a serious safety risk because the vehicle could no longer accelerate or operate
... normally in traffic. I pulled over as soon as it was safe, shut the vehicle off, and had it towed. The vehicle had approximately 74,945 miles at the time of the incident. An independent auto repair shop performed a diagnostic inspection and documented fault codes P030300 for a cylinder 3 misfire, P130A00 indicating cylinder 3 fuel injection had been disabled to protect the catalytic converter, and P013B00 involving the rear oxygen sensor. The technician reported severe connecting-rod knock from inside the engine block and found a large amount of metallic debris in the engine oil. The shop concluded that the engine had suffered catastrophic internal failure and required replacement. The symptoms and findings closely resemble the connecting-rod failure condition described in Audi recall 13i5 / NHTSA recall 22V753, although Audi advised that my VIN is not currently included. The vehicle is now at an authorized Audi dealership for further diagnosis. I am reporting this because the sudden loss of power and internal engine failure occurred while the vehicle was being driven and could have resulted in a crash. The incident occurred during the evening and the next morning I drove the vehicle to Herb Chambers Honda of Seekonk, the dealer who has performed every service on the vehicle, including recalls. Of note is that the service team at the Honda dealer, specifically Malainne Costa, has been exceptional in addressing this issue. American Honda was consulted to determine the best plan for resolution. I understand that American Honda is trying to work through necessary adjustments with the new 2025 model year. One month after the incident, my vehicle remains at the dealer. I learned that the two cylinder heads in the engine block on my vehicle need to be replaced and those parts are still on backorder. Another engine component system may require replacement due to calcium buildup on the intake valves. I was assured that my driving habits were not a factor in this incident. My concern is that this incident occurred at a time when I was in a safe location and was able to await the arrival of AAA without worry about my personal safety. I bought the brand new vehicle in November, 2024 and it is obvious that I drive a significant amount of miles per year. I worry about the safety risk of other drivers having a similar experience in a less than optimal location especially since there was no indication of a problem prior to this critical incident. The charging system and alternator failed due to a known safety recall involving an oil leak from the front cylinder bank cover gasket. The oil leaked onto the alternator, causing the charging system to become inoperative and resulting in the engine stalling. The vehicle is currently at the Hyundai dealership and is available for inspection upon request. The engine stalled unexpectedly without any
... warning, creating a serious safety hazard and increasing the risk of a crash. The failure occurred because the recalled defect caused the charging system to fail. The vehicle is currently at the Hyundai dealership, where the failure has been confirmed. The vehicle has been inspected by the Hyundai dealership. Hyundai has replaced the alternator under the applicable safety recall but has declined to cover the engine failure under the recall warranty. Based on the circumstances of this incident, I believe the engine failure was directly caused by the recalled defect. According to the recall, oil leaking from the front cylinder bank cover gasket can drip onto the alternator, causing the charging system to become inoperative and resulting in an engine stall. My vehicle experienced this exact sequence of events. There were no warning lights or prior indications of a problem before the engine stalled unexpectedly. Although Hyundai acknowledged the recall condition by replacing the alternator, it has denied responsibility for the resulting engine damage, despite the engine failure occurring immediately after the recalled oil leak caused the charging system to fail. I am requesting that Hyundai honor the intent of the recall warranty by covering all damage directly resulting from the recalled defect, including the engine failure.Read more

This is a head gasket failure allowing coolant to enter the cylinders — a failure mode widely reported on this engine — and it occurred at low mileage, creating a risk of engine misfire, stalling, and loss of power. No crash, fire, or injury has occurred. The dealer's repair order documenting these findings is available on request. I have reported this to the Honda corporate, and they are not responding and stalling the delivery or any update on the case. It has been 3 weeks, and it is causing prolonged inconvience and safety issue to me and my familyRead more

I Purchased the vehicle on May 29, 2026 and it stated the 6.2 liter engine recall had been fixed and that I have extended special coverage for the exact issue of the recall until 150008 miles on this vehicle. On July 14, 2026 the vehicle was being driven and stalled out at a traffic light and restarted, yet within a quarter mile it stalled again at the next traffic light and became completely disa
... bled, it would not even allow a shift into nuetral, so I had it towed less than 100 feet into the Walmart on Loudon Road in Concord, NH 03301 parking lot. On July 23, 2026 I was able to get the batter charged and attach a code scanner which showed one code (P0016). Pursuant to Recall N252494001 (which had allegedly been repaired, likely with the bandaid fix of thicker oil) any 6.2 liter GM truck with the loss of propulsion needs to be plugged into a scanner and if code P0016 is present, engine replacement is needed. Specifically noted to the recall is Special coverage N252494003 which provides the coverage to 150,008 miles on this vehicle is directly related to the recall. At the time of loss of propulsion the vehicle had 99,195 miles present on the odometer. The following day July 24, 2026 I had the vehicle towed to Banks Chevrolet GMC in Concord NH, after the vehicle arrived, I spoke with the service department, provided them with the recall paperwork and printout of the special coverage uner the vin as well as a photo of the odometer and my scanner displayining one code P0016. They then informed me it would take them until August 24, 2026 to "diagnose" the vehicle. The recall is clear it just needs a code scan if disabled. I have contacted GM and informed them of the need for a loaner as the vehicle is a vehicle utilized for business. I have since been told they will try to look at it earlier and no loaner will be offered but I "may" be able to be reimbusred $44.00 a day for a rental. There are no rentals for that price that can tow over 9500 pounds. It is unnacceptable.Read more

When driving on highway, the engine light came on and the car suddenly slowed down causing me concern about traffic around me still going at highway speed. For several months before the failure, the Auto Idle Stop (engine stop-start) system became progressively slower to restart the engine after stopping. Each restart sounded sluggish, as though the starter was dragging, and the vehicle would remain stopped for several extra seconds before moving. This occurred repeatedly at traffic lights and other stops creating a safety concern as drivers behind m
... e expected my vehicle to begin moving when traffic started. The unexpected delay increased the risk of being rear-ended or causing confusion in traffic. The failure eventually became complete. While stopped on a two-lane road waiting for another vehicle to turn left into a driveway, the engine shut off as expected with the Auto Idle Stop system engaged but did not restart when I released the brake. Instead, the vehicle entered what appeared to be a neutral state and could not be driven. I was unable to restart the vehicle for several minutes, leaving it disabled in the travel lane while other vehicles had to drive around it. This created a safety hazard because the vehicle was stranded in an active roadway. The vehicle had not previously displayed any warning messages related to this condition. Looking back, the progressively slower and dragging engine restarts appear to have been warning signs before the complete failure to restart. The vehicle is scheduled for inspection the Honda dealership this week. The failed components should be available for inspection if requested.Read more

The vehicle experienced a sudden, severe mechanical engine condition resulting in a violent rough idle, loss of motive power, and a flashing Check Engine light with hard misfire codes on Cylinders 4, 5, and 6 (the entire driver-side engine bank). Additionally, the vehicle has been losing engine coolant internally with no external leaks or puddles on the ground, indicating fluid is entering the com
... bustion chambers. These symptoms exactly mirror the mechanical pattern documented in Ford Safety Recall 24S55 (NHTSA Campaign Number 24V635) involving brittle engine intake valves fracturing. This vehicle has an August 2022 manufacture date and sits just outside the manufacturer's current restricted recall window, indicating that the defective component batch extends significantly past the reported dates. Component Availability: The engine is intact and available for federal inspection. Safety Risk: The sudden loss of engine power, hard stalling, and severe misfires while operating the vehicle creates an immediate risk of being rear-ended by traffic or stranded on a high-speed roadway without motive power. Dealer Confirmation/Inspection: These symptoms occurred recently at roughly 58,300 miles. The vehicle is currently being towed to an authorized dealership for diagnostic confirmation, relative compression testing, and cooling system pressure testing to officially verify the root cause. It has not been inspected by police or insurance representatives. Warning Symptoms: Prior to the sudden rough idle, the vehicle exhibited a low coolant level in the reservoir. There were no dashboard warning lamps or performance symptoms prior to the sudden flashing Check Engine light and immediate hard misfires.Read more
